Basal cell carcinoma stem cells exhibit osteogenic and chondrogenic differentiation potential

AB  - Specific cell subpopulations identified as cancer stem cells (CSCs) can be found in basal cell carcinoma (BCC).
Generally, CSCs have a marked trans-differentiation potential that could potentially be used in differentiation therapies.
However, there are no studies regarding BCC CSCs multipotency. The aim of the study was to analyze the characteristic of
CSCs of BCC with emphasis on their differentiation potential upon specific induction. Specific staining and cell
morphology were used for differentiation confirmation, along with the expression analysis of osteogenic (ALP, BSP,
Runx2, OCN, BMP2), chondrogenic (COL1 and COL2A1), adipogenic (PPAR-γ) and neurogenic (Nestin and MAP2)
markers. BCC CSCs differentiated into osteogenic and chondrogenic lineages, as judged by staining and high
expression of specific markers (from 2-to 92-fold higher upon induction). Concomitantly with differentiation, the
levels of cancer stem cell markers decreased in the cultures. Adipo-differentiation and neuro-differentiation were
unsuccessful. In conclusion, BCC CSCs exhibit the capacity to trans-differentiate, a characteristic that may potentially
be useful in the development of new strategies for the treatment of aggressive BCCs.

AB  - Background/Aims Management of maxillofacial trauma in the geriatric population poses a great challenge due to anatomical variations and medical comorbidities. The aim of this study was to analyze the management variables, timing, and outcomes of facial fractures in elderly patients (aged 70 years or more) at several European departments of oral and maxillofacial surgery. Materials and Methods This study was based on a systematic computer-assisted database that allowed the recording of data from all geriatric patients with facial fractures from the involved maxillofacial surgical units across Europe between 2013 and 2017. Results A total of 1334 patients were included in the study: 665 patients underwent closed or open surgical treatment. A significant association (P  lt  .005) was found between the presence of concomitant injuries and a prolonged time between hospital admission and treatment. The absence of indications to treatment was associated with comorbidities and an older age (P  lt  .000005). Conclusions Elderly patients require specific attention and multidisciplinary collaboration in the diagnosis and sequencing of trauma treatment. A prudent attitude may be kept in selected cases, especially when severe comorbidities are associated and function is not impaired.

AB  - Introduction Management of body fractures in patients with edentulous atrophic mandibles represents a challenging task due to patient's age, medical comorbidities, poor bone quality, and vascularity, as well as reduced contact area between the fracture ends. The aim of the study was to assess the demographic and clinical variables, the surgical technique, and outcomes of unilateral body fractures of the edentulous atrophic mandible managed at several European departments of oral and maxillofacial surgery. Methods This study is based on a systematic computer-assisted database that allowed the recording of data of all patients with fractures of the atrophic edentulous mandible from the involved maxillofacial surgical units across Europe between 2008 and 2017. The following data were recorded for each patient: gender, age, comorbidities, etiology, synchronous body injuries, degree of atrophy of the mandible according to Luhr classification, type of surgical approach and fixation, length of hospitalization, and presence and type of complications. Results A total of 43 patients were included in the study: 17 patients' mandibles were classified as class I according to Luhr, 15 as class II, and 11 as class III. All patients underwent open reduction and internal fixation by extraoral approach in 25 patients, intraoral in 15 patients, and mixed in 3 patients. A single 2.0 miniplate was used in 16 patients, followed by a single 2.4 reconstruction plate in 13 patients, by two 2.0 miniplates, and three 2.0 miniplates. Outcome was considered to be satisfying in 30 patients, with no complications. Complications were observed in 13 cases. Conclusions Treatment of unilateral body fractures of the edentulous mandible must still be based on the type of fracture, degree of atrophy, experience of the surgeon, and patients' preference. An adequate stability can be obtained by different plating techniques that have to be appropriately tailored to every single specific patient.

AB  - Background/Aims Management of maxillofacial trauma in the geriatric population poses a great challenge due to anatomical variations and medical comorbidities. The aim of this study was to analyze the management variables, timing, and outcomes of facial fractures in elderly patients (aged 70 years or more) at several European departments of oral and maxillofacial surgery. Materials and Methods This study was based on a systematic computer-assisted database that allowed the recording of data from all geriatric patients with facial fractures from the involved maxillofacial surgical units across Europe between 2013 and 2017. Results A total of 1334 patients were included in the study: 665 patients underwent closed or open surgical treatment. A significant association (P  lt  .005) was found between the presence of concomitant injuries and a prolonged time between hospital admission and treatment. The absence of indications to treatment was associated with comorbidities and an older age (P  lt  .000005). Conclusions Elderly patients require specific attention and multidisciplinary collaboration in the diagnosis and sequencing of trauma treatment. A prudent attitude may be kept in selected cases, especially when severe comorbidities are associated and function is not impaired.

AB  - Introduction: Treatment of condylar fractures in patients with atrophic edentulous mandibles is a peculiar field that has been little considered in the literature. The aim of the study was to assess the demographic and clinical variables as well as management and outcome of mandibular condylar fractures in edentulous patients with atrophic mandibles that were treated at several European departments of oral and maxillofacial surgery. Methods: The data of all patients with fractures of the atrophic edentulous mandible from the involved maxillofacial surgical units across Europe between January 1, 2008, and December 31, 2017. Only patients that were diagnosed with condylar fractures of the edentulous atrophic mandible were included. Results: A total of 52 patients met the inclusion criteria and were included in the study: 79% of patients reported one or more comorbidities. Thirty-four unilateral neck or subcondylar fractures, 9 bilateral neck or subcondylar condylar fractures, 7 unilateral head condylar fractures, and 2 bilateral head condylar fractures were diagnosed. No treatment was performed in 37 cases, whereas in 4 patients a closed treatment was decided, and 11 patients underwent open reduction and internal fixation. Outcome was considered to be satisfying in 48 patients, with no complications. Conclusions: The golden rule still remains that the diagnosis of a subcondylar or neck fracture in an edentulous patient should constitute an indication for open reduction and internal fixation. However, an appropriate choice of management options has to be individualized on a case by case basis, also depending on the patient consent.

AB  - Introduction: The progressive aging of European population seems to determine a change in the epidemiology, incidence and etiology of maxillofacial fractures with an increase in the frequency of old patients sustaining craniofacial trauma. The objective of the present study was to assess the demographic variables, causes, and patterns of facial fractures in elderly population (with 70 years or more). Materials and methods: The data from all geriatric patients (70 years or more) with facial fractures between January 1, 2013, and December 31, 2017, were collected. The following data were recorded for each patient: gender, age, voluptuary habits, comorbidities, etiology, site of facial fractures, synchronous body injuries, Facial Injury Severity Score (FISS). Results: A total of 1334 patients (599 male and 735 female patients) were included in the study. Mean age was 79.3 years, and 66% of patients reported one or more comorbidities. The most frequent cause of injury was fall and zygomatic fractures were the most frequently observed injuries. Falls were associated with a low FISS value (P  lt  .005). Concomitant injuries were observed in 27.3% of patients. Falls were associated with the absence of concomitant injuries. The ninth decade (P  lt  .05) and a high FISS score (P  lt  .005) were associated with concomitant body injuries too. Conclusions: This study confirms the role of falls in the epidemiology of facial trauma in the elderly, but also highlights the frequency of involvement of females, and the high frequency of zygomatic fractures.

AB  - Introduction: The objective of the present study was to assess the demographic variables, causes, and patterns of edentulous atrophic fractures of the mandible managed at several European departments of oral and maxillofacial surgery. The results of this multicenter collaboration over a 10-year period are presented. Methods: The data of all patients with fractures of the atrophic edentulous mandible from the involved maxillofacial surgical units across Europe between January 1, 2008, and December 31, 2017 were recorded: gender; age; voluptuary habits; comorbidities; etiology; fracture sites; synchronous body injuries; atrophy of the mandible according to Luhr classification; eventual type of treatment; timing of the eventual surgery; length of hospital stay. Results: A total of 197 patients (86 male and 111 female patients) with 285 mandibular fractures were included in the study. Mean age of the study populationwas 75 years. Statistically significant associations were found between Luhr classes I - II and condylar fractures on one hand (p  lt .0005), and between Luhr class III and body and parasymphyseal fractures on the other hand (p  lt .05). Finally, 135 patients underwent open reduction and internal fixation, 56 patients did not undergo any intervention, and 6 patients underwent closed reduction. No statistically significant association was observed between treatment, timing of treatment, comorbidities, and concomitant injuries. Conclusions: The management of edentulous atrophic mandibular fractures remains challenging. Treatment decisions should continue to be based on the clinician's previous experience and on the degree of bone resorption in edentulous mandible in relation to fracture subsites.

To investigate the prevalence of p16(INK4) (a), p14(ARF), tumor protein p53 (TP53), and human telomerase reverse transcriptase (hTERT) promoter hypermethylation in mucoepidermoid carcinomas (MECs) and search for a possible association between methylation status and clinicopathological parameters. Study design. DNA extracted from 35 formalin-fixed and paraffin-embedded MEC samples and 10 normal salivary gland (NSG) tissue samples was analyzed for the presence of promoter hypermethylation using methylation-specific polymerase chain reaction testing. Results. The percentages of gene hypermethylation in MECs versus NSGs were the following: p14: 100% versus 20% (P  lt  .001); p16: 60% versus 20% (P = .035); hTERT: 54.3% versus 20% (P = .078); and TP53: 31.4% versus 30% (P = .981). Multiple sites were found to be methylated in 86% of MECs compared with 10% in NSGs (P  lt  .001). TP53 and hTERT were more often methylated in lower clinical stages (P = .033 and P = .005, respectively). Conclusions. Hypermethylation of p14 appears to be an important event in the development of mucoepidermoid carcinoma. High frequency of gene hypermethylation and high incidence of methylation at multiple sites point to the importance of epigenetic phenomena in the pathogenesis of MECs, although with modest impact on clinical parameters.

AB  - Purpose: Adenoid cystic carcinoma (ACC) is one of the most common malignant salivary gland tumors. It is characterized by a high rate of recurrence, perineural invasion and development of distant metastases many years after removal of the primary tumor. Disorders of the induction of apoptosis and its cascade reactions where caspases are involved may be significant in the pathogenesis of this tumor. Methods: The immunohistochemical expression of caspase 9 and caspase 3 was analyzed by tissue microarray (TMA) in 50 cases of ACC in relation with different clinicopathological parameters (gender, age, localization, histological type and overall survival). Results: Caspase 9 was expressed in the cytoplasm and nuclei of ACC tumor cells with varying degrees of staining intensity (1+, 6%; 2+, 54%, 3+, 40%). Comparison of caspase 9 expression in tumor cells with clinicopathological parameters (gender, age, localization, histological type and overall survival) showed no statistically significant difference except that the expression was more pronounced in females. Caspase 3 was expressed in the cytoplasm of tumor cells with varying degrees of staining intensity (1+, 22%; 2+, 36%; 3+, 42%). No correlation between the expression of caspase 3 and clinicopathological parameters was noticed. Conclusions: The expression of caspases 9 and 3 in ACC of the salivary glands can contribute in the better characterization of molecules involved in apoptosis of tumor cells.

Materials and Methods: Genomic DNA from paraffin-embedded samples of 50 pleomorphic adenomas and 10 carcinomas ex pleomorphic adenoma was subjected to methylation specific polymerase chain reaction for hypermethylation analyses and real time polymerase chain reaction for the relative telomere length calculations. Results: Promoter hypermethylation of the two genes was a very frequent event in both neoplasms between 60% and 90% of samples were hypermethylated - but without significant difference between the groups. The mean relative telomere length in the pleomorphic adenoma group was significantly increased in comparison to the control group (P = 0.00), and significantly decreased in comparison to the carcinoma group (P = 0.05). Telomeres were also longer in myxoid and cellular histological subtypes of adenomas than in the classic type (P = 0.044 and P = 0.018, respectively). Longer telomeres were more frequent in tumors with hypermethylated p14(ARF) alleles (P = 0.013). Conclusion: Promoter hypermethylations seems to be an important mechanism of p16(INK4a) and Pl4(ARF) inactivation in parotid gland tumors. Telomeric lengthening appears to be involved in the pathogenesis of both benign and malignant tumors of the parotid glands.

AB  - Background: Genetic studies of salivary gland neoplasms including pleomorphic adenomas (PA) were mainly focused on chromosomal changes and several specific patterns of chromosome translocations have been described. Molecular changes involved in tumour pathogenesis are less well characterized, and data regarding genetic polymorphisms as risk factors for PA development are almost inexistent. Aim: The aim of this thesis was to investigate the putative role of several polymorphisms in genes involved in apoptosis (survivin), cell cycle control and apoptosis (TP53), and extracellular matrix degradation and remodeling (matrix metalloproteinases - MMP-9) as potential susceptibility factors and prognostic factors in Serbian patients with PA. Materials and Methods: DNA was extracted from 74 formalin fixed, paraffin embedded samples of PA that were surgically removed at the Clinic of Maxillofacial Surgery, School of Dental Medicine, University of Belgrade. Control specimens of DNA were extracted from buccal swabs of healthy donors, sex and age matched with the patients group. Polymorphisms analysis of single nucleotide polymorphisms (SNPs) in the promoter region of the survivin gene, at positions -241 (a C/T substitution) and -31 (a G/C substitution), in the promoter region (-1562 C/T) of the MMP-9 gene, and Pro47Ser in the TP53 gene, was performed by PCR-RFLP followed by electrophoresis. AB  - Uvod : Genetičke studije neoplazmi pljuvačnih žlezda, uključujući i pleomorfni adenom (PA) su uglavnom bile fokusirane na hromozomske aberacije, i nekoliko specifičnih obrazaca hromozomskih translokacija je do sada opisano kao ključni događaji u nastanku PA. Molekularne promene uključene u patogenezu ovih tumora znatno su manje izučavane i okarakterisane, a podaci o genetičkim polimorfizmima kao faktorima rizika za razvoj PA ili kao prognostičkim faktorima, gotovo da su nepostojeći . Cilj: Cilj ovog rada bio je da se ispita potencijalna ulogu nekoliko polimorfizama u genima koji su uključeni u apoptozu i kontrolu ćelijskog ciklusa (survivin, TP53) i degradaciju i remodeliranje ekstracelularnog matriksa (matriksna metaloproteinaza 9 - MMP-9), kao potencijalnih faktora susceptibilnosti za nastanak pleomorfnom adenoma i kao prognostičkih markera u populaciji Srbije. AB  - Background: Genetic studies of salivary gland neoplasms were mainly focused on chromosomal changes, and some specific patterns of chromosome translocations have been described. However, molecular alterations, in particular the role of HER-2/H-ras/c-myc signalling cascade in pleomorphic adenoma pathogenesis (PA), are less well characterized. In addition, data on single nucleotide polymorphisms (SNPs) as potential susceptibility factors for PA development are also quite scarce. Methods: Mutational analyses were performed by means of real-time PCR (HER-2 and c-myc amplification analysis), PCR-SSCP and sequencing (H-ras point mutation detection). Polymorphisms analysis was performed by PCR-RFLP (survivin and MMP-9 genes). Results: Amplification of HER-2 and c-myc has been found in 13% and 9% of PA cases respectively. Point mutations in H-ras codons 12/13 have been detected in 17% of PAs. No correlation could be established between these alterations and clinical characteristics of PAs, whereas they might play a role in a subset of malignant salivary gland tumours. As for survivin -31 G/C polymorphism, C allele carriers had a 4-fold decrease of the risk of developing PA (p=0.05). Carriers of the variant allele T of the -1562C/T SNP in MMP-9 gene had a 4-fold increase of the risk of developing PA (p lt 0.001). Conclusions: A longer follow-up of PA patients harbouring mutations could uncover a prognostic role of HER-2 and c-myc amplification as predictors of adenoma transformation into carcinoma. Both survivin and MMP-9 promoter polymorphisms represent susceptibility factors for the development of PAs in the Serbian population.
AB  - Uvod: U ispitivanjima mehanizama nastanka tumora pljuvačnih žlezda uglavnom dominiraju citogenetičke studije, pa su tako detektovane i opisane različite hromozomske translokacije sa specifičnim obrascem javljanja. Međutim, molekularne promene u ovim tumorima i dalje su relativno slabo poznate, a pogotovo je malo podataka o potencijalnom značaju signalnog puta HER-2/H-ras/c-myc u razvoju i progresiji pleomorfnih adenoma (PA). Takođe, retki su i podaci 0 polimorfizmima pojedinačnog nukleotida (SNP) kao faktora predispozicije za nastanak PA. Metode: Analize somatskih mutacija urađene su metodama real-time PCR (analiza amplifikacije HER-2 i c-myc), PCR-SSCP i sekvenciranja (detekcija tačkastih mutacija u H-ras). Ana I iza polimorfizama pojedinačnih nukleotida (SNP) vršena je prime- nom metode PCR-RFLP (u genima za survivin i MMP-9). Rezultati: Amplifikacija gena HER-2 detektovana je u 13%, c-myc u 9% a tačkaste mutacije u kodonima 12/13 H-ras gena u 17% uzoraka. Nije ustanovljena veza između ovih promena i kliničkih odlika adenoma. Na malom uzorku karcinoma, pokazano je da je amplifikacija HER-2 povezana sa recidivima tumora. Nosioci C alela u -31G/C SNP gena za survivin imaju četiri puta manji rizik od nastanka PA (p=0,05), dok nosioci varijantnog alela T kod -1562 C/T SNP u MMP-9 genu imaju četiri puta veći rizik da obole od PA u odnosu na kontrolu (p lt 0,001). Zaključak: Dužim praćenjem pacijenata sa PA mogla bi da se ustanovi prognostička uloga HER-2 i c-myc amplifikacija kao indikatora za transformaciju adenoma u karcinom. Polimorfizmi u promotorima gena za survivin i MMP-9 predstavljaju modulatore rizika za razvoj pleomorfnih adenoma u srpskoj populaciji.

Epidemiological, Clinical, and Forensic Aspects of Chainsaw, Circular Saw, and Grinding Saw Injuries in the Maxillofacial Region

AB  - The aim of this article was the epidemiological, clinical, and forensic evaluation of the chainsaw, circular saw, and grinding saw maxillofacial injuries. A retrospective analysis of the medical records at the Clinic of Maxillofacial Surgery, School of Dentistry, University of Belgrade, was performed. A total of 133 patients were treated during a period of 19 years (1989-2008). Grinding saw injuries were more frequent (62%) in comparison to chainsaw and circular saw injuries that were present in 23% and 15% of patients, respectively. All injured individuals were men, and most of them were aged 31 to 40 years. Accident by self injuring was the only mechanism of all the analyzed injuries. There were no suicide or homicide attempts. Isolated injuries of the facial soft tissues, which were mainly lacerations, were present in most patients. Less frequently, soft tissues injuries were compounded with bone fractures of the face and teeth injuries. Surgical debridement, revision, and suturing were performed in all patients where only soft tissues were injured. Patients with compound injuries of the soft tissues, facial bones, and teeth were treated according to the common surgical protocol for the type of the injuries. Most often, these injuries were accidental without fatalities. The number of these injuries increased in the recent years owing to the "do-it-yourselfers" for home hobbies. To avoid these types of injuries, users should be carefully instructed, and attention should be paid to the use of accurately guarded saws and appropriate safety equipment.

Positional changes of the lower and upper anterior teeth after surgical correction of mandibular prognathism

AB  - Background/Aim. To establish the influence of surgical corrections of mandibular prognathism upon the position of the lower and upper anterior teeth (incisors). Methods. The changes in position of the lower and upper anterior teeth (incisors) after the surgical correction of mandibular prognathism were analyzed by means of x-ray craniometry in 183 patients (female: n = 110, male: n = 73) in which the correction had been made in accordance with the principles of sagital osteotomy of the mandibular ramus. There were 4 angular and 2 linear parameters determined in the pre- and postoperative tele-xray-pictures. The changes of these parameters were tested by means of the parametric statistic tests. Results. The performed surgical procedures did not cause statistically significant changes in the angular parameters. The changes of both linear parameters were thought highly statistically significant. Conclusion. Under the influence of the performed surgical procedures no changes were found in the position of anterior teeth (incisors) in relation to SN and the mandibular plane. They were actually moved along with the medium fragment of the lower jaw and that fragment was moved linearly backwards along the occlusal plane with a practically insignificant rotation in the cranial direction.
AB  - Uvod/Cilj. Utvrditi uticaj hirurške korekcije mandibularnog prognatizma na položaj donjih i gornjih sekutića. Metode. Promene položaja donjih i gornjih sekutića posle hirurške korekcije mandibularnog prognatizma analizirane su rendgenkraniometrijski kod 183 osobe (ž = 110; m = 73) kod kojih je korekcija izvršena po principima sagitalnih osteotomija ramusa mandibule. Na preoperativnim i postoperativnim telerendgenogramima određivana su četiri angularna i dva linearna parametra. Promene tih parametara testirane su pomoću parametrijskih statističkih testova. Rezultati. Sprovedene hirurške procedure nisu izazvale statistički značajne promene angularnih parametara. Promene oba linearna parametra bile su visoko statistički značajne. Zaključak. Pod uticajem sprovedenih hirurških korektivnih procedura ne dolazi do promene položaja sekutića u odnosu na SN i mandibularnu ravan, već se oni pomeraju zajedno sa središnjim fragmentom donje vilice. Taj fragment se kreće translatorno unazad duž okluzalne ravni, sa praktično zanemarljivom rotacijom u kranijalnom pravcu.

Changes of the upper lip after surgical correction of mandibular prognathism

AB  - The aim of this study was to establish and estimate the effects of surgical correction of mandibular prognathism on the height and position of the upper lip. The changes of the upper lip were analyzed on 115 patients between 16 and 30 year of age (71 females and 44 males) who were subject to surgical correction of mandibular prognathism using the sagittal split ramus osteotomy. The lateral cephalometric radiographs were taken of each patient before and six months after the surgical procedure. The soft tissue cephalometric analysis included the measurements of six linear and two angular parameters. The obtained data were statistically processed and evaluated. Statistical analysis revealed the significant changes in two linear cephalometric parameters after surgical procedure - the incisal step and the length of the upper lip. The changes in the length and position of the upper lip after surgical correction of mandibular prognathism by means of sagittal split ramus osteotomy seem to be minor and clinically insignificant. Only the length of the upper lip was significantly changed. It became longer. The upper lip advancement amounted approximately 15% of the complete mandibular set-back during operation.
AB  - Cilj istraživanja je bio da utvrdi i proceni efekte hirurške korekcije mandibularnog prognatizma na dužinu i položaj gornje usne. Promene gornje usne su kefalometrijski analizirane kod 115 osoba (f=71; m=44) kod kojih je mandibularni prognatizam korigovan metodom sagitalnih osteotomija grane donje vilice. Svakom pacijentu su načinjena po dva profilna telerendgenska snimka glave, jedan pre i jedan nakon hirurške intervencije. Kefalometrijska analiza mekog profila lica je obavljena merenjem i poređenjem šest linearnih i dva angularna parametra. Dobijene vrednosti su statistički obrađene i evoluirane. Primenjene hirurške korektivne procedure rezultirale su statistički značajnim promenama kod dva linearna parametra (incizalna stepenica i dužina gornje usne). Promene gornje usne posle hirurške korekcije mandibularnog prognatizma pomoću sagitalnih klizajućih osteotomija grane donje vilice su minimalne i gotovo neznačajne. Značajno se menja jedino dužina gornje usne - postaje duža. Gornja usna se pomera za 15% ukupnog pomeranja struktura donje vilice.
