Gene polymorphisms in odontogenic keratocysts and ameloblastomas: A systematic review

The aim of this systematic review was to critically analyze available data on gene polymorphisms in odontogenic keratocysts (OKC) and ameloblastomas, including their possible relationship with clinical and histological features of these lesions.

Materials and Methods
A comprehensive search of Web of Science Scopus, PubMed, Cochrane Central Register of Controlled Trials and EMBASE was conducted using relevant key terms and supplemented by a gray literature search. Quality assessment of included studies was performed using criteria from the Strengthening the Reporting of Genetic Association (STREGA) statement.

Results
Ten studies were included in the final review. Survivin ‐31G/C, interleukin IL‐1α ‐889 C/T, p53 codon 72 G/C, tumor necrosis factor TNF‐α (−308G>A) and its receptor TNF‐R1 (36A>G), glioma‐associated oncogene homolog 1 rs2228224 and matrix metalloproteinase 2 rs243865 gene polymorphisms were reported to be associated with OKC. For ameloblastomas, p53 codon 72 G/C, X‐ray repair cross‐complementing protein 1—codons 194 and 399 and matrix metalloproteinase 9 rs3918242 gene polymorphisms were identified as risk factors. It was not possible to establish a relationship between specific polymorphisms and clinical and histological features of investigated lesions.

Conclusions
Several gene polymorphisms might be considered as a risk factor for the development of these lesions. Future studies should investigate whether these polymorphisms might be used to identify patients with increased risk of recurrence or aggressive disease.

This study aimed to perform a more precise estimation of the association between tumor necrosis factor-alpha (TNF-α) – 308 G/A single nucleotide polymorphism (SNP) and the risk of development of AP and its phenotypes based on all available published studies.
Methods
The study was carried out according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines and is registered in PROSPERO (CRD42020176190). The literature search was conducted via: Clarivate Analytics’ Scopus, PubMed, Cochrane Central Register of Controlled Trials and China National Knowledge Infrastructure databases, from inception to December 2020 with no language restrictions. Two reviewers were involved independently in study selection, data extraction and appraising the studies that were included. The quality of included studies was evaluated using the Strengthening the Reporting of Genetic Association (STREGA) and the Grading of Recommendations Assessment, Development and Evaluation (GRADE) system. Frequencies of genotypes and alleles of TNF-alpha (G>A 308, rs1800629) gene, with 95% Odds ratio was used.
Results
Four studies met the inclusion criteria with moderate risk of bias. This study revealed no significant association between TNF-α – 308 G/A SNP and AP, and the risk of AP development. Moreover, there was no significant association between genotype or allele frequency distribution and clinical manifestations (acute versus chronic) of AP. The certainty of evidence per GRADE was very low.
Conclusions
Due to very low certainty of evidence, whether there is an association between TNF-α – 308 G/A SNP and AP, warrants further well-designed multi-centric studies to adjudicate a better understanding of the role of genetic factors in the etiopathogenesis of AP.

To critically evaluate the reporting quality of a random sample of clinical trials published in Endodontics against the PRIRATE 2020 checklist and to analyse the association between the quality of reported trials and a variety of parameters.

Methodology
Fifty randomized clinical trials relating to Endodontics were randomly selected from the PubMed database from 2015 to 2019 and evaluated by two independent reviewers. For each trial, a score of ‘1’ was awarded when it fully reported each item in the PRIRATE guidelines whereas a score of ‘0’ was awarded when an item was not reported; when the item was reported inadequately a score of ‘0.5’ was awarded. For the items that were not relevant to the trial, ‘Not Applicable (NA)’ was given. Based on the interquartile range of the overall scores received, trials were categorized into ‘Low’ (0–58.4%), ‘Moderate’ (58.5–72.8%) and ‘High’ (72.9–100%) quality. The associations between characteristics and quality of clinical trials were investigated. Descriptive statistics, frequency analysis and percentage analyses were used to describe the data. To determine the significance of categorical data, the chi‐square test was used. The probability value 0.05 was considered as the level of significance.

Results
Based on the overall scores, 13 (26%), 25(50%) and 12 (24%) of the reports of clinical trials were categorized as ‘High’, ‘Moderate’ and ‘Low’ quality, respectively. Three items (1b, 6d, 11e) were adequately reported in all manuscripts whilst two items (5k, 5m) were scored ‘NA’ in all the reports. The reports published from Europe had a significantly greater percentage of ‘High’‐quality scores, compared to Asia, Middle East, North America and South America (P = 0.0002). The ‘High’‐quality reports were published significantly more often in impact factor journals (P = 0.045). Reports of clinical trials published in journals that adhered to the CONSORT guidelines had significantly more ‘High’ scores compared to those that did not (P = 0.008). Clinical trials with protocols registered a priori had a significantly greater percentage of ‘High’ scores compared to the trials that were not registered in advance (P = 0.003). No significant difference occurred between the quality of clinical trials and the number of authors, journal (Endodontic specialty vs. Non‐Endodontic specialty) or year of publication.

Conclusions
Reports of randomized clinical trials published in the speciality of Endodontics had a substantial number of deficiencies. To create high‐quality reports of clinical trials, authors should comply with the PRIRATE 2020 guidelines.

PRIASE 2021 guidelines for reporting animal studies in Endodontology: explanation and elaboration

AB  - Laws and ethics require that before conducting human clinical trials, a new material, device or drug may have to undergo testing in animals in order to minimize health risks to humans, unless suitable supporting grandfather data already exist. The Preferred Reporting Items for Animal Studies in Endodontology (PRIASE) 2021 guidelines were developed exclusively for the specialty of Endodontology by integrating and adapting the ARRIVE (Animals in Research: Reporting In Vivo Experiments) guidelines and the Clinical and Laboratory Images in Publications (CLIP) principles using a validated consensus‐based methodology. Implementation of the PRIASE 2021 guidelines will reduce potential sources of bias and thus improve the quality, accuracy, reproducibility, completeness and transparency of reports describing animal studies in Endodontology. The PRIASE 2021 guidelines consist of a checklist with 11 domains and 43 individual items and a flowchart. The aim of the current document is to provide an explanation for each item in the PRIASE 2021 checklist and flowchart and is supplemented with examples from the literature in order for readers to understand their significance and to provide usage guidance. A link to the PRIASE 2021 explanation and elaboration document and PRIASE 2021 checklist and flowchart is available on the Preferred Reporting Items for study Designs in Endodontology (PRIDE) website (http://pride‐endodonticguidelines.org/priase/).

A bibliometric analysis of the dental scientific literature on COVID-19

AB  - Objectives
The rapid production of a large volume of literature during the early phase of the COVID-19 outbreak created a substantial burden for clinicians and scientists. Therefore, this manuscript aims to identify and describe the scientific literature addressing COVID-19 from a dental research perspective, in terms of the manuscript origin, research domain, study type, and level of evidence (LoE).

Materials and methods
Data were retrieved from Web of Science, Scopus, and PubMed. A descriptive analysis of bibliographic data, collaboration network, and keyword co-occurrence analysis were performed. Articles were further classified according to the field of interest, main research question, type of study, and LoE.

Results
The present study identified 296 dental scientific COVID-19 original papers, published in 89 journals, and co-authored by 1331 individuals affiliated with 429 institutions from 53 countries. Although 81.4% were single-country papers, extensive collaboration among the institutions of single countries (Italian, British, and Brazilian institutions) was observed. Conclusion

Materials and methods
Data were retrieved from Web of Science, Scopus, and PubMed. A descriptive analysis of bibliographic data, collaboration network, and keyword co-occurrence analysis were performed. Articles were further classified according to the field of interest, main research question, type of study, and LoE.

Results
The present study identified 296 dental scientific COVID-19 original papers, published in 89 journals, and co-authored by 1331 individuals affiliated with 429 institutions from 53 countries. Although 81.4% were single-country papers, extensive collaboration among the institutions of single countries (Italian, British, and Brazilian institutions) was observed. Key Study Factor The authors conducted a systematic review and meta-analysis of case-control and cross-sectional studies on the associations of human cytomegalovirus (HCMV) with periodontitis, and with apical periodontitis of endodontic origin. Studies included populations consisted of periodontal and endodontic patients positive for the presence of HCMV in examined samples compared with patients with negative HCMV result. Main Outcome Measure The main outcome measure was the prevalence of HCMV infection analyzed by different laboratory methods (ie, variants of polymerase chain reaction [PCR] methods [single, nested, multiplex, hot starts, real-time], and loop-mediated isothermal amplification) in periodontitis and apical periodontitis samples compared with healthy controls. In addition, the authors performed quality assessment analyses of the included studies based on their risk of publication bias. Main Results The pooled analysis of 25 studies with 2385 subjects (1324 with periodontitis and 1061 without periodontitis) revealed significantly increased odds for periodontitis when subgingival HCMV was detected (odds ratio [OR] = 5.31; 95% confidence interval [CI] = 3.15-8.97; P < .00001). Heterogeneity test was 1.00, and the test for overall effect was Z = 6.26, which indicates a significant heterogeneity (I-2 67%; P < .00001). In contrast, results from 5 included studies (n = 317, 219 periapical lesions and 98 healthy control samples) revealed no significant odds for apical periodontitis when HCMV was detected (OR = 3.65; 95% CI = 0.49-27.1; P = .21). Heterogeneity test was 3.94, and the test for overall effect was Z = 1.26 (I-2 83%; P < .00001). The quality of the included studies was assessed by analyzing their risk of bias using the US National Institute of Health National Heart, Lung and Blood Institute quality assessment tool for case-control, cohort, and cross-sectional studies. Of 25 studies with periodontitis samples, 18 were considered to be of fair quality, 7 were of poor quality, and only one study was appraised as good quality. Of 6 studies with apical periodontitis samples, 2 were considered to be of fair quality and 4 were of poor quality. Significant publication bias was observed among the 25 included studies related to periodontitis (Egger's test t = 7.0479, df = 23, P < .0001). Conclusions HCMV is significantly more common in patients with periodontitis than in healthy control samples. However, there was no significant difference in the occurrence of HCMV in patients with apical periodontitis compared with healthy control samples.

AB  - Background: The existence of an association between cardiovascular diseases (CVDs) and apical periodontitis (AP) remains unclear because results obtained from previous clinical studies and reviews are inconsistent or inconclusive.

Objective: To conduct an umbrella review to determine whether there is an association between CVDs and the prevalence of AP in adults.

Methods: The protocol of the review was registered in the PROSPERO database (CRD42020185753). The literature search was conducted using the following electronic databases: Clarivate Analytics' Web of Science Scopus, PubMed and Cochrane Database of Systematic Reviews, from inception to May, 2020, with no language restrictions. Systematic reviews with or without meta-analysis that evaluated the association between CVDs and AP were included. Other types of studies, including narrative reviews, were excluded. Two reviewers independently performed a literature search, data extraction and quality assessment of included studies. Any disagreements or doubts were resolved by a third reviewer. The quality of the reviews was assessed using the AMSTAR 2 tool (A measurement tool to assess systematic reviews), with 16 items. A final categorization of the systematic reviews classified each as of 'high', 'moderate', 'low' or 'critically low' quality.

Results: Four systematic reviews were included in the current review. Three reviews were graded by AMSTAR 2 as 'moderate' quality, whereas one review was graded as 'critically low' quality.

Discussion: Only one systematic review included a meta-analysis. Substantial heterogeneity amongst the primary studies included within each systematic review was notable in preventing a pooled analysis.

Conclusions: From the limited 'moderate' to 'critically low' quality evidence available, the current umbrella review concluded that a weak association exists between CVDs and AP. In the future, well-designed, longitudinal clinical studies with long-term follow-up are required.

 This study aimed to summarize data on apical periodontitis (AP) and nonsurgical root canal treatment (NSRCT) prevalence and risk factors related to age, gender, and quality of restorative and endodontic treatment in the general population from cross-sectional studies published between 2012 and 2020.
 
 Methods
 An electronic search was performed in the following databases: Web of Science, Scopus, and PubMed. The conducted literature search covered studies published between 2012 and 2020, without restrictions on language. The STROBE and NOS tools were used for quality assessment of the included studies.
 
 Results
 Sixteen articles were included in the review. In total, 200,041 teeth were examined. On average, 6.3% of teeth had AP, and 7.4% had NSRCT. Forty-one percent of RCT teeth had AP, and 3.5% of untreated teeth had AP. Female patients were less prone to AP in endodontically treated teeth only, compared with male patients (P < .001). Variable stratification of age subgroups among included studies prevented us from conducting a meta-analysis. An increase in AP frequency was found in teeth with inadequate restorative and endodontic treatment (P < .001 and P < .001, respectively). Because of high heterogeneity, these results should be taken with caution.
 
 Conclusions
 There is an increased AP prevalence in the adult general population compared with data from 2012 (6.3% versus 5.4%) in both endodontically treated (41.3% versus 35.9%) and untreated teeth (3.5% versus 2.1%). In addition, AP developed less frequently in female patients with endodontically treated teeth and in teeth with inadequate compared with adequate restorative and endodontic treatment.

This study aimed to summarize data on apical periodontitis (AP) and nonsurgical root canal treatment (NSRCT) prevalence and risk factors related to age, gender, and quality of restorative and endodontic treatment in the general population from cross-sectional studies published between 2012 and 2020.

Methods
An electronic search was performed in the following databases: Web of Science, Scopus, and PubMed. The conducted literature search covered studies published between 2012 and 2020, without restrictions on language. The STROBE and NOS tools were used for quality assessment of the included studies.

Results
Sixteen articles were included in the review. In total, 200,041 teeth were examined. On average, 6.3% of teeth had AP, and 7.4% had NSRCT. Forty-one percent of RCT teeth had AP, and 3.5% of untreated teeth had AP. Female patients were less prone to AP in endodontically treated teeth only, compared with male patients (P < .001). Variable stratification of age subgroups among included studies prevented us from conducting a meta-analysis. An increase in AP frequency was found in teeth with inadequate restorative and endodontic treatment (P < .001 and P < .001, respectively). Because of high heterogeneity, these results should be taken with caution.

Conclusions
There is an increased AP prevalence in the adult general population compared with data from 2012 (6.3% versus 5.4%) in both endodontically treated (41.3% versus 35.9%) and untreated teeth (3.5% versus 2.1%). In addition, AP developed less frequently in female patients with endodontically treated teeth and in teeth with inadequate compared with adequate restorative and endodontic treatment.

Additionally, the potential correlation between investigated molecules in periapical lesions was analyzed. Materials and methods: Sixty-four apical periodontitis lesions were obtained subsequent to standard apicoectomy procedure. The presence of EBV was determined using nested PCR. Based on the presence of EBV all periapical lesions were divided into two groups, 29 EBV positive and 35 EBV negative lesions. A reverse transcriptase real-time PCR was used to determine mRNA levels of Notch2, Jagged1, RANKL, OPG, TNF-?, IL-1? and IL-6. Results: Significantly higher mRNA levels of Notch2, Jagged1, RANKL and IL-1? were observed in EBV positive compared to EBV negative lesions. Significant positive correlation was present between Notch2 and Jagged1, Jagged1 and RANKL, and IL-? and TNF-? in EBV positive periapical lesions. Conclusions: Notch signalling pathway may be involved in alveolar bone resorption in apical periodontitis lesions infected by EBV.

AB  - Aim To investigate the possible association between TNF alpha (-308 G/A) and IL-1 beta (-511 C/T) single nucleotide polymorphisms (SNPs) and GSTT and GSTM deletion polymorphisms and risk of apical periodontitis (AP) development, and determine the association of different genotypes with the presence of herpesviral infection in AP. Methodology The study included 120 periapical lesions and 200 control samples. Gene polymorphism analysis was performed using either polymerase chain reaction (PCR) or PCR/ restriction fragment length polymorphism (RFLP). Relative gene expression of TNF-alpha and IL-1 beta was analysed using reverse transcriptase - real-time PCR. The presence of Epstein-Barr virus (EBV) and human cytomegalovirus (HCMV) was assessed by nested PCR. Chi-square and Fisher's exact tests and logistic regression analyses were done for polymorphisms, whilst Mann-Whitney U-test was performed for the expression analysis. The expected frequency of variants was analysed by the Hardy-Weinberg equilibrium test. Results TNF-alpha (-308 G/A) SNP increased AP susceptibility for heterozygous (odds ratio (OR) = 1.72, 95% confidence interval (CI) = 1.06-2.80, P = 0.027) and homozygous (OR = 8.55, 95% CI = 1.77-41.36, P  lt  0.001) carriers of the variant A allele. On the other hand, IL-1 beta (-511 C/T) polymorphism exerted a protective effect both in heterozygotes (OR = 0.540, 95% CI = 0.332-0.880, P = 0.013) and homozygotes (OR = 0.114, 95% CI = 0.026-0.501, P  lt  0.001). In addition, GSTM1 and GSTT1 null genotypes separately, as well as concomitantly, were associated with an increased risk for AP development (P  lt  0.001). The null GSTT1 genotype increased approximately twice the risk of Epstein-Barr infection (EBV) in AP (OR = 2.17, 95% CI = 1-4.71, P = 0.048), whilst TNF-alpha SNP decreased it, both in heterozygotes (OR = 0.20, 95% CI = 0.08-0.48, P  lt  0.001) and AA homozygotes (OR = 0.07, 95% CI = 0.01-0.37, P = 0.001). Conclusions GSTM and GSTT deletion polymorphisms, as well as TNF alpha (-308 G/A) SNP, are associated with increased risk, whereas IL-1 beta (-511 C/T) polymorphism decreases the risk of AP development. GSTT and TNF alpha polymorphisms also appear to modulate the risk of EBV infection in Serbian patients with AP.

Diabetes Mellitus May Negatively Influence the Outcome of Conventional Nonsurgical Root Canal Treatment

Subjects or Study Selection

The umbrella review was developed according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines and registered a priori in the PROSPERO database (CRD42019141684). The literature search was conducted using multiple electronic databases (PubMed, EBSCOhost, Cochrane, and Scopus electronic databases) from inception to July 2019. Additional searches were performed using citation mining but without gray literature. This review included systematic reviews with or without meta-analysis, published only in English, and evaluating any root canal treatment outcomes comparing diabetic and nondiabetic patients. All other types of studies (eg, case reports, clinical studies, laboratory studies, animal studies, and narrative reviews) were excluded. Two reviewers independently screened titles and abstracts to identify relevant articles, and a third senior reviewer was consulted in cases of disagreement and doubt.

Key Study Factor

This umbrella review included 4 systematic reviews with a total of 30 primary studies (case-control, cross-sectional, and prospective longitudinal) that investigated the outcome of root canal treatment in diabetic compared with nondiabetic patients. All included reviews were published between 2016 and 2019. In addition, all included reviews performed quality assessment of the primary included studies, while only two conducted a meta-analysis.

Main Outcome Measure

The main outcome measure was the prevalence of persistent apical periodontitis, measured radiographically, associated with the clinical outcome of root filled teeth in diabetic compared with nondiabetic patients. A quality assessment of included systematic reviews was completed.

Main Results

Two out of 4 systematic reviews reported that diabetic patients had a greater prevalence of periapical radiolucencies associated with root filled teeth on radiographs. One of the 4 systematic reviews revealed a relationship between diabetes and increased frequency of loss of root filled teeth. In contrast, one systematic review concluded that there was no overall effect of diabetes on root canal treatment outcome. The quality of each individual systematic review included in the current umbrella review was appraised using "A MeaSurement Tool to Assess systematic Reviews" (AMSTAR) and categorized as "medium". Of the two previous meta-analyses carried out, one reported high heterogeneity of included studies while the other reported low heterogeneity. A meta-analysis was not conducted in this umbrella review.

Conclusions

Diabetes mellitus is associated with the outcome of conventional nonsurgical root canal treatment and should be considered as an important preoperative prognostic factor; however, high-quality clinical studies with improved control of confounding factors and longer follow-up are required to improve the quality of the primary studies in this area.

Subjects or Study Selection

The umbrella review was developed according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines and registered a priori in the PROSPERO database (CRD42019141684). The literature search was conducted using multiple electronic databases (PubMed, EBSCOhost, Cochrane, and Scopus electronic databases) from inception to July 2019. Additional searches were performed using citation mining but without gray literature. This review included systematic reviews with or without meta-analysis, published only in English, and evaluating any root canal treatment outcomes comparing diabetic and nondiabetic patients. All other types of studies (eg, case reports, clinical studies, laboratory studies, animal studies, and narrative reviews) were excluded. Two reviewers independently screened titles and abstracts to identify relevant articles, and a third senior reviewer was consulted in cases of disagreement and doubt.

Key Study Factor

This umbrella review included 4 systematic reviews with a total of 30 primary studies (case-control, cross-sectional, and prospective longitudinal) that investigated the outcome of root canal treatment in diabetic compared with nondiabetic patients. All included reviews were published between 2016 and 2019. In addition, all included reviews performed quality assessment of the primary included studies, while only two conducted a meta-analysis.

Main Outcome Measure

The main outcome measure was the prevalence of persistent apical periodontitis, measured radiographically, associated with the clinical outcome of root filled teeth in diabetic compared with nondiabetic patients. A quality assessment of included systematic reviews was completed.

Main Results

Two out of 4 systematic reviews reported that diabetic patients had a greater prevalence of periapical radiolucencies associated with root filled teeth on radiographs. One of the 4 systematic reviews revealed a relationship between diabetes and increased frequency of loss of root filled teeth. In contrast, one systematic review concluded that there was no overall effect of diabetes on root canal treatment outcome. The quality of each individual systematic review included in the current umbrella review was appraised using "A MeaSurement Tool to Assess systematic Reviews" (AMSTAR) and categorized as "medium". Of the two previous meta-analyses carried out, one reported high heterogeneity of included studies while the other reported low heterogeneity. A meta-analysis was not conducted in this umbrella review.

Conclusions

Diabetes mellitus is associated with the outcome of conventional nonsurgical root canal treatment and should be considered as an important preoperative prognostic factor; however, high-quality clinical studies with improved control of confounding factors and longer follow-up are required to improve the quality of the primary studies in this area.",

AB  - Apical periodontitis represents a chronic inflammatory process within periapical tissues, mostly caused by etiological agents of endodontic origin. Progressive bone resorption in the periapical region represents the hallmark of apical periodontitis and occurs as the consequence of interplay between polymicrobial infections and host response. The Notch signaling pathway is an evolutionary conserved cell-signaling system that plays an important role in a variety of cell functions including proliferation, differentiation and apoptosis. In recent years its involvement in bone homeostasis has attracted a significant consideration. We hypothesized that Notch signaling pathway, which has a complex interplay with proinflammatory cytokines and bone resorption regulators, contributes to alveolar bone resorption via increased Notch receptors on immune cell surface and stimulates Notch receptor intracellular domain (NICD) translocation into the nucleus. The potential benefit of medications aimed to down-regulate these pathways in apical periodontitis treatment remains to be assessed.

This study aimed to analyze the expression of Notch signaling molecules (Notch2, Jagged1, and Hey1) and proinflammatory cytokines (tumor necrosis factor alpha [TNF-alpha], interleukin [IL]-1 beta, and IL-6) in human apical periodontitis lesions with different receptor activator of nuclear factor kappa B ligand (RANKL)/osteo-protegerin (OPG) ratios and determine their potential correlation. Methods: The study group consisted of 50 periapical lesions collected in conjunction with apicoectomy. The relative gene expression of the investigated molecules (Notch2, Jagged1, Hey1, RANKL, OPG, TNF-alpha, IL-1 beta, and IL-6) in all tissue samples was analyzed using reverse transcriptase real-time polymerase chain reaction. The Student t test, Mann-Whitney U test and Spearman correlation were used for statistical analysis. Results: Based on the RANKUOPG ratio, periapical lesions were either RANKL predominant (RANKL > OPG, n = 33) or OPG predominant (RANKL  lt  OPG, n = 17). Symptomatic lesions occurred more frequently in RANKL-predominant compared with OPG-predominant lesions (24 vs 7, P=.029). Notch2,Jagged1, Hey1, and TNF-alpha were significantly overexpressed in lesions with predominant RANKL compared with lesions with predominant OPG (P =.001, P =.001, P =.027, and P =.016, respectively). Significant correlations were observed between the investigated genes in periapical lesions. Conclusions: Notch signaling appeared to be activated in periapical inflammation. An increase in Notch2, Jagged1, Hey1, and TNF-alpha expression in RANKL-predominant periapical lesions corroborates their joined involvement in extensive periapical bone resorption.

This review describes the most recent findings on herpesviral infections and offers current concepts of herpesviral role in the pathogenesis of periapical periodontitis.

Recent Findings
Thirty articles reported data on herpesviral infection in periapical periodontitis. Epstein-Barr virus and human cytomegalovirus are the most frequently detected herpesviruses in periapical samples. The main hypothesis postulates a bidirectional herpesviral-bacterial relationship in the etiopathogenesis of periapical periodontitis. A high heterogeneity of herpesviruses incidence was registered within the studies, in part, due to various methodological approaches used in laboratory testing, different inclusion criteria, study design, seroprevalence of herpesviruses, and sociodemographic characteristics of investigated populations.

Summary
Herpesviruses have been shown to potentially impair local host defense in periapical tissue. Although it has been demonstrated that endodontic pathogenic bacteria are able to reactivate herpesviruses, further, in vitro studies should provide more data on herpesviruses as a factor in the pathogenesis of the periapical pathoses. It is, therefore, necessary to investigate potential benefits of antiviral therapy in well-designed controlled longitudinal studies.

AB  - ObjectivesThis study aimed to analyse Epstein-Barr virus (EBV) and human cytomegalovirus (HCMV) viral loads in symptomatic and asymptomatic apical periodontitis lesions, to determine levels of TNF-, IL-1 and IL-6 in these lesions and to investigate a possible correlation between herpesviral copy numbers and levels of proinflammatory cytokines. Materials and MethodsA total of 100 samples of apical periodontitis were subjected to HCMV and EBV copy numbers analysis by nested polymerase chain reaction (PCR) and TaqMan real-time PCR. The concentrations of TNF-, IL-1 and IL-6 were determined by ELISA method. SPSS software was used for statistical analysis. ResultsThere were no significant differences in the occurrence of EBV and HCMV between symptomatic and asymptomatic periapical lesions (p=.686, p=.879, respectively). Only 12 of 74 EBV (16.2%) and four of 54 HCMV (13.5%) nested PCR-positive samples showed increased viral copy numbers above the limit of 125 copies/ml. There was no significant correlation between the levels of analysed proinflammatory cytokines and herpesviral copy numbers in our sample. ConclusionThe observed low viral loads point to a relatively rare occurrence of active EBV and HCMV infection in our sample. Latent herpesviral infection does not enhance the production of investigated proinflammatory cytokines.

abstract = "ObjectivesThis study aimed to analyse Epstein-Barr virus (EBV) and human cytomegalovirus (HCMV) viral loads in symptomatic and asymptomatic apical periodontitis lesions, to determine levels of TNF-, IL-1 and IL-6 in these lesions and to investigate a possible correlation between herpesviral copy numbers and levels of proinflammatory cytokines. Materials and MethodsA total of 100 samples of apical periodontitis were subjected to HCMV and EBV copy numbers analysis by nested polymerase chain reaction (PCR) and TaqMan real-time PCR. The concentrations of TNF-, IL-1 and IL-6 were determined by ELISA method. SPSS software was used for statistical analysis. ResultsThere were no significant differences in the occurrence of EBV and HCMV between symptomatic and asymptomatic periapical lesions (p=.686, p=.879, respectively). Only 12 of 74 EBV (16.2%) and four of 54 HCMV (13.5%) nested PCR-positive samples showed increased viral copy numbers above the limit of 125 copies/ml. There was no significant correlation between the levels of analysed proinflammatory cytokines and herpesviral copy numbers in our sample. ConclusionThe observed low viral loads point to a relatively rare occurrence of active EBV and HCMV infection in our sample. Latent herpesviral infection does not enhance the production of investigated proinflammatory cytokines.",

MethodologyThe experimental group consisted of 30 EBV-positive and 30 EBV-negative periapical lesions collected in conjunction with apicoectomy. The pulp tissues of 20 impacted third molars were used as healthy controls. The qualitative and quantitative analysis of EBV was performed by nested and real-time polymerase chain reaction (PCR), respectively. The levels of RANKL and OPG were analysed by reverse transcriptase real-time PCR. The levels of 8-OHdG and GSSG were determined by enzyme-linked immunosorbent assay (ELISA). Chronic low grade inflammation in aging process as a link on a chain of obesity - related vascular disorders

AB  - The pathogenesis of obesity-related vascular disorders has not been fully elucidated. The fundamental role of inflammation in aging process is now widely recognized, particularly for atherosclerotic disease which begins before birth. The number of obese individuals worldwide has reached two billion, leading to an explosion of obesity-related vascular disorders associated with increased morbidity and mortality. Obesity, as a chronic low grade inflammatory process, is important risk factor for metabolic and cardiovascular disease. Despite a well-known genetic component, this risk appears to originate from several abnormalities in adipose tissue function associated with a chronic inflammatory state. In particular, obesity as the most common nutritional disorder in industrialized countries, is closely related to impaired endothelial function, a well-known marker of preatherosclerotic disease. These conditions disrupt vascular homeostasis by causing an imbalance between the nitric oxide pathway and the endothelin-1 system, with impaired insulin- stimulated endothelium-dependent vasodilation. Having in mind the growing population of overweight and obese people worldwide, along with an increasingly aging population, understanding the pathophysiology of obesity on cardiovascular system is essential. The mechanisms linking obesity-related vascular disorders and low grade inflammation in aging process are the focus of this paper.
AB  - Patogeneza vaskularnih poremećaja koji su povezani sa gojaznošću nije u potpunosti razjašnjena. Danas je poznata fundamentalna uloga inflamacije u procesu starenja, naročito kod aterosklerozne bolesti, koja počinje pre rođenja. Broj gojaznih osoba širom sveta je dostigao cifru dva biliona i doveo do “eksplozije” vaskularnih poremećaja koji su povezani sa gojaznošću i povećanim morbiditetom i mortalitetom. Gojaznost, kao hronični niskostepeni inflamacijski proces, je važan faktor rizika za nastanak metaboličkih i kardiovaskularnih bolesti. Pored dobro proučene genetske komponente, izgleda da je za ovaj rizik zaslužno i nekoliko poremećaja funkcije masnog tkiva koji se dovode u vezu sa stanjem hronične inflamacije. U tom smislu prednjači gojaznost, pošto je kao najčešći poremećaj ishrane u razvijenim zemljama blisko povezana sa endotelnom disfunkcijom, koja predstavlja uveliko poznat marker preaterosklerozne bolesti. Takvi uslovi narušavaju vaskularnu homeostazu, dovodeći do dizbalansa između metaboličkog puta azot monoksida i sistema endotelina-1, što je praćeno otežanim odvijanjem insulinom podstaknute vazodilatacije. Imajući u vidu globalni zajednički porast broja kako osoba sa viškom kilograma i gojaznih osoba, tako i starije populacije, razumevanje patogeneze gojaznosti i njenog uticaja na kardiovaskularni sistem je od esencijalnog značaja. Mehanizmi koji čine sponu između vaskularnih poremećaja povezanih sa gojaznošću i niskostepene inflamacije u procesu starenja nalaze se u fokusu ovog rada.

AB  - Introduction: Varicella zoster virus (VZV) and subsequent herpes zoster (HZ) infection have been proposed as a causative agent of periapical pathoses and root resorption. This review aimed to identify, synthesize, and present a critical analysis of the available data on the association among VZV, subsequent HZ infection, and the development of periapical pathoses and root resorption and to analyze the level of evidence of available studies. Methods: The literature search covered MEDLINE, Science Citation Index Expanded, and Scopus. A qualitative critical appraisal of the included articles was performed. Results: The electronic database search. yielded 66 hits from PubMed, 73 hits from Web of Science, and 107 from Scopus: Seven case reports and 3 cross-sectional studies were included in the final review. When summarized, in 7 patients with a history of a previous HZ attack and with no other apparent cause, -23 teeth were diagnosed with apical periodontitis, 8 teeth with internal and 1 tooth with external root resorption. The cross-sectional studies investigated the presence of VZV DNA in samples of acute apical abscess. The VZV DNA was found only in 2 of 65 samples. Conclusions: All studies included in this systematic review.had a low level of evidence (4 and 5). Still, the potential role of VZV in the etiopathogenesis of periapical pathoses and root resorption cannot be ruled out. Future investigations should be directed toward the analysis of VZV pathologic effects on pulp blood vessels, which might cause local ischemia and tissue necrosis.

AB  - The aim of this study was to assess the presence of herpesviruses and periodontopathic bacteria and to establish their potential association with pericoronitis. Fifty samples obtained with paper points (30 from pericoronitis and 20 controls) were subjected to polymerase chain reaction (PCR) analysis. A single-stage and nested PCR assays were used to detect herpesviruses: human cytomegalovirus (HCMV) and Epstein-Barr virus (EBV) and six periodontopathic anaerobic bacteria: Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis, Prevotella intermedia, Parvimonas micra, Treponema denticola, and Tannarella forsythia. Pericoronitis samples harbored HCMV and EBV at significantly higher rates than the control group (70 vs. 40 % and 46.7 vs. 15 %, P = 0.035, P = 0.021, respectively). P. micra and T. forsythia (66.7 vs. 0 %, and 40 vs. 10 %, P = 0.001, P = 0.021, respectively) were significantly more common in pericoronitis compared to the control group. Multivariate logistic regression analysis showed that the presence of T. forsythia was associated with pericoronitis development (OR 7.3, 95 % CI, 1.2-43.2, P = 0.028). The occurrence of HCVM and EBV extends our previous knowledge on microbiota in pericoronitis. These PCR-based findings demonstrated that bacterial and viral DNA occurred concomitantly in pericoronitis samples. T. forsythia appeared to be significantly associated with pericoronitis development in the examined sample. Herpesviral-bacterial co-infections might exacerbate the progression of pericoronitis.

AB  - Periapical lesions are characterized by chronic inflammation and destruction of tooth-supporting tissues in the periapical area of non-vital teeth. Previous studies confirmed that polymicrobial infection of endodontic origin is essential for its development. At the beginning of 21st century herpesviruses have been detected in tissue samples of periapical lesions. It has been hypothesized that concomitant herpesviral infection may contribute to the pathogenesis of apical periodontitis in the same way as it has been proposed for marginal periodontitis. Up to now, 20 studies investigated the occurrence of herpesviral infection in apical periodontitis. Their results indicate that Epstein-Barr virus (EBV) and Human cytomegalovirus (HCMV) are the most frequently detected species of Herpesviridae family in apical periodontitis. Previous studies showed that herpesviruses occurred more frequently in symptomatic and large compared to asymptomatic and small periapical lesions. However, their exact role in the pathogenesis of apical periodontitis has not been fully elucidated. Although it is considered that a great diversity of viral strains may influence the course and severity of disease there are no scientific data about the occurrence of different EBV and HCMV genotypes in apical periodontitis samples. Furthermore, there is a lack of data about the relationship between levels of pro-inflammatory cytokines and herpesviral copy number in the pathogenesis of apical periodontitis. AIMS. This study aimed to detect the occurrence of HCMV and EBV, to assess their copy number and the distribution of their genotypes in periapical lesions and healthy control tissues. AB  - Periapikalne lezije karakteriše hronična zapaljenska reakcija koja uzrokuje razgradnju alveolarnog koštanog tkiva u predelu vrha korena avitalnih zuba. Dosadašnja istraživanja pokazuju da su bakterijski mikroorganizmi i njihovi štetni produkti najčešći uzročnici nastanka zapaljenske reakcije u apeksnom parodoncijumu. Početkom 21. veka dokazano je prisustvo herpesvirusa u tkivu periapikalnih lezija i postavljena je hipoteza da pomenuti virusi učestvuju u procesu njihovog nastanka po modelu kojim doprinose razgradnji alveolarnog koštanog tkiva, kao i u marginalnom parodontitisu. Do danas je publikovano 20 istraživanja koja su ispitivala učestalost herpesvirusne infekcije u tkivu periapikalnih lezija. Rezultati istraživanja ukazuju da su Epštajn-Bar virus (EBV) i Humani citomegalovirus (HCMV) najčešće detektovani pripadnici porodice Herpesviridae u periapikalnim lezijama. Takođe, nedovoljno je ispitan odnos povećanog broja genskih kopija herpesvirusa i nivoa proinflamatornih citokina u patogenezi periapikalnih lezija. CILJ ISTRAŽIVANJA. Ovo istraživanje je imalo za cilj da odredi učestalost prisustva, broja genskih kopija i vrste genotipova EBV i HCMV u tkivu periapikalnih lezija i kontrolnog pulpnog tkiva, kao i da uporedi dobijeni nalaz među periapikalnim lezijama sa različitim kliničkim, radiološkim i patohistološkim karakteristikama. Takođe, cilj ovog istraživanja bio je i da se odredi vrednost proinflamatornih citokina (TNF-α, IL-1β i IL-6) u tkivu periapikalnih lezija i kontrolnog pulpnog tkiva, te da se dobijene vrednosti uporede sa brojem genskih kopija EBV i HCMV u ispitivanom uzorku. Comparative clinical evaluation of two different techniques of local anaesthesia in the posterior mandible using 4% articaine with 1:100,000 adrenaline

AB  - Background/Aim. Local infiltration anaesthesia (LIA) is significantly simpler compared to the inferior alveolar nerve block (IAB) and less unpleasant for patients. However, it is not efficient if used in posterior region of the mandible, at least with traditional local anaesthetics. The aim of this study was to compare anaesthetic efficacy of two techniques the LIA in the posterior segment of the mandible, and the IAB, using 4% articaine with 1 : 100,000 adrenaline and to note possible changes in haemodynamic parameters caused by these two techniques. Methods. Sixty pre-informed patients were divided into two study groups. Both groups received 1.8 mL of the same anaesthetic solution, 4% articaine with adrenaline 1 : 100,000, with two different techniques of local anaesthesia. The first group received the LIA in projection of root apex of the first lower molar; the second group received the IAB. The examined parameters were: changes in tooth sensitivity after 5 and 30 minutes in relation to the value recorded before administering the anaesthetics, onset of anaesthesia, width of anaesthetic field, and duration of anaesthesia. Also, the impact of the applied techniques on cardiovascular parameters was noticed. Results. The LIA group had a statistically significant decrease in sensitivity 5 minutes after application of the local anaesthetic. The decreasing trend continued between 5 and 30 minutes, although without statistical significance. There was no statistically significant difference in sensitivity changes between two groups for the first molar and the first and second premolars. However, there was a statistically significant difference in duration of local anaesthesia in favour of the IAB, while the width of anaesthetic fields was significantly higher after the LIA. Significant changes in hemodynamic parameters were not recorded within the two groups. Conclusion. The effect of the LIA on tooth sensitivity of premolars and first molar is quite satisfactory. The IAB was more effective for canine and second molar. None of the tested techniques had any significant effect on the cardiovascular parameters.
AB  - Uvod/Cilj. Lokalna infiltraciona anestezija (LIA) mnogo je jednostavnija u poređenju sa mandibularnom alveolarnom blok anestezijom (MA), a i prijatnija je za pacijente. Međutim, ona nije efikasna ukoliko se daje u posteriorni deo mandibule korišćenjem tradicionalnih lokalnih anestetika. Cilj istraživanja bio je da se uporedi efikasnost dve tehnike anestezije, LIA u bočnom segmentu mandibule, i mandibularne anestezije primenom 4% artikaina sa 1 : 100 000 adrenalina, i uoče eventualne promene hemodinamskih parametara koje ove tehnike mogu da prouzrokuju. Metode. Šezdeset prethodno informisanih pacijenata bilo je podeljeno u dve studijske grupe. Obe grupe su dobile 1,8 mL istog anestetika, 4% artikain sa adrenalinom 1 : 100 000, primenom dve tehnike anestezije. Prva grupa ispitanika primila je lokalnu infiltracionu anesteziju u projekciji vrhova korenova prvog donjeg molara. Druga grupa ispitanika primila je mandibularnu anesteziju. Ispitivani parametri bili su: promene u osetljivosti zuba nakon pet i 30 minuta u odnosu na vrednosti zabeležene pre davanja anestetika, početak dejstva anestezije, širina anestetičkog polja i trajanje anestezije. Takođe, zabeležen je i uticaj primenjenih tehnika na kardiovaskularne parametre. Rezultati. Grupa ispitanika LIA imala je statistički značajno smanjenje osetljivosti zuba pet minuta nakon primene LIA. Trend smanjenja postojao je i dalje, između pet i 30 minuta, iako bez statističke značajnosti. Nije bilo statistički značajne razlike u promenama osetljivosti između dve grupe na prvom molaru i prvom i drugom premolaru. Postojala je statistički značajna razlika u trajanju anestezije u korist MA, dok je širina anestetičkog polja bila značajno veća u LIA grupi. Klinički značajne promene hemodinamskih parametara nisu zabeležene ni u jednoj studijskoj grupi. Zaključak. Učinak LIA bio je zadovoljavajući kada su u pitanju premolari i prvi molar. Za anesteziju očnjaka i drugog molara uspešnija je bila MA. Nijedna od ispitivanih tehnika nije imala značajan uticaj na kardiovaskularne parametare.

Obe grupe su dobile 1,8 mL istog anestetika, 4% artikain sa adrenalinom 1 : 100 000, primenom dve tehnike anestezije. Prva grupa ispitanika primila je lokalnu infiltracionu anesteziju u projekciji vrhova korenova prvog donjeg molara. Druga grupa ispitanika primila je mandibularnu anesteziju. Ispitivani parametri bili su: promene u osetljivosti zuba nakon pet i 30 minuta u odnosu na vrednosti zabeležene pre davanja anestetika, početak dejstva anestezije, širina anestetičkog polja i trajanje anestezije. Takođe, zabeležen je i uticaj primenjenih tehnika na kardiovaskularne parametre. Rezultati. Grupa ispitanika LIA imala je statistički značajno smanjenje osetljivosti zuba pet minuta nakon primene LIA. Trend smanjenja postojao je i dalje, između pet i 30 minuta, iako bez statističke značajnosti. Nije bilo statistički značajne razlike u promenama osetljivosti između dve grupe na prvom molaru i prvom i drugom premolaru. Postojala je statistički značajna razlika u trajanju anestezije u korist MA, dok je širina anestetičkog polja bila značajno veća u LIA grupi. Klinički značajne promene hemodinamskih parametara nisu zabeležene ni u jednoj studijskoj grupi. Zaključak. Učinak LIA bio je zadovoljavajući kada su u pitanju premolari i prvi molar. Za anesteziju očnjaka i drugog molara uspešnija je bila MA. Nijedna od ispitivanih tehnika nije imala značajan uticaj na kardiovaskularne parametare.",

The characteristics of craniofacial and cervicovertebral morphology in different genetic syndromes - a literature review and three case reports

AB  - Introduction: Patients with genetic syndromes were characterized by variety of skeletal craniofacial and cervicovertebral morphology.Skeletal anomalies are recognized concomitants of the various genetic syndromes.The aim of the study was to review the current literature on this topic and to present the characteristics of craniofacial and cervicovertebralmorphology and subsequent anomalies in three patients with Crouzon syndrome, Treacher Collins syndrome and cleidocranial dysplasia.

Materials and methods: A comprehensive electronic search was performed using PubMed via Medline, Web of Science and SCOPUS. A manual search involved references form articles retrieved for possible inclusion. There were no restrictions as to date of publication, study design or language. The search, evaluation of relevant articles, and their critical appraisal were performed by two independent judges. Discrepancies between reviewers were resolved through a consensus with a third party.

Case reports: Additionally, this paper presents a radiographic analysis of craniofacial and cervicovertebral morphology in patients with cleidocranial dysplasia, Crouzon, and Treacher Collins syndromes. The most characteristic findings of cervicovertebral morphology were the presence of cervical spine fusions in all three patients. The intervertebral fusions in patients with Crouzon and Treacher Collins syndromes have been characterized with “block vertebrae”. Cervicovertebral complex of the patient with cleidocranialdysplasia is characterized by delayed mineralization of vertebral bodies (C1–C7).

Results: Although craniofacial and cervicovertebral anomalies in presented syndromes have different phenotype expression, the vastmajority of cases are caused by mutations in specific, syndrome-related genes (FGFR2, FGFR3, RUNX2, TCOF1, POLR1C,POLR1D). Craniofacial anomalies, that include changes in development of hard and soft tissues, were considered as traditional concomitant of presented syndromes. Apart from these changes, cervicovertebral region could also be affected. Recent reports show different changes in vertebral structure (delayed mineralization) and unphysiological relations (cervical spine fusions).

Conclusion: The limitation of cervical range of motion resulting from these anomalies may have clinical significance on multidisciplinary management approach in these patients. Recent progress in dentistry resulted in better diagnostic and therapeutic options andoutcomes for individuals with genetic syndromes.
