Characterisation of NiTi Orthodontic Archwires Surface after the Simulation of Mechanical Loading in CACO2-2 Cell Culture

AB  - Nickel-titanium (NiTi) orthodontic archwires are crucial in the initial stages of orthodontic therapy when the movement of teeth and deflection of the archwire are the largest. Their great mechanical properties come with their main disadvantage-the leakage of nickel. Various in vitro studies measured nickel leakage from archwires that were only immersed in the medium with little or minimal simulation of all stress and deflection forces that affect them. This study aims to overcome that by simulating deflection forces that those archwires are exposed to inside the mouth of a patient. NiTi orthodontic archwires were immersed in CACO2-2 cell culture medium and then immediately loaded while using a simulator of multiaxial stress for 24 h. After the experiment, the surface of the NiTi orthodontic archwires were analysed while using scanning electron microscopy (SEM) and auger electron spectroscopy (AES). The observations showed significant microstructural and compositional changes within the first 51 nm thickness of the archwire surface. Furthermore, the released nickel and titanium concentrations in the CACO2-2 cell culture medium were measured while using Inductively Coupled Plasma Mass Spectroscopy (ICP-MS). It was found out that the level of released nickel ions was 1.310 mu g/L, which can be assigned as statistically significant results. These data represent the first mention of the already detectable release of Ni ions after 24 h during the simulation of mechanical loading in the CACO2-2 cell culture medium, which is important for clinical orthodontic praxis.

Growth hormone positive effects on craniofacial complex in Turner syndrome

AB  - Objective: Turner syndrome occurs in phenotypic females with complete or partial absence of X chromosome. The leading symptom is short stature, while numerous but mild stigmata manifest in the craniofacial region. These patients are commonly treated with growth hormone to improve their final height. The aim of this study was to assess the influence of long-term growth hormone therapy on craniofacial morphology in Turner syndrome patients. Design: In this cross-sectional study cephalometric analysis was performed on 13 lateral cephalograms of patients with 45,X karyotype and the average age of 17.3 years, who have received growth hormone for at least two years. The control group consisted of 13 Turner syndrome patients naive to growth hormone treatment, matched to study group by age and karyotype. Sixteen linear and angular measurements were obtained from standard lateral cephalograms. Standard deviation scores were calculated in order to evaluate influence of growth hormone therapy on craniofacial components. Results: In Turner syndrome patients treated with growth hormone most of linear measurements were significantly larger compared to untreated patients. Growth hormone therapy mainly influenced posterior face height, mandibular ramus height, total mandibular length, anterior face height and maxillary length. While the increase in linear measurements was evident, angular measurements and facial height ratio did not show statistically significant difference. Acromegalic features were not found. Conclusions: Long-term growth hormone therapy has positive influence on craniofacial development in Turner syndrome patients, with the greatest impact on posterior facial height and mandibular ramus. However, it could not compensate X chromosome deficiency and normalize craniofacial features.

Differences in cytocompatibility, dynamics of the oxide layers' formation, and nickel release between superelastic and thermo-activated nickel-titanium archwires

AB  - Superelastic (SE) and thermo-activated (TA) nickel-titanium (NiTi) archwires are used in everyday orthodontic practice, based on their acceptable biocompatibility and well-defined shape memory properties. However, the differences in their surface microstructure and cytotoxicity have not been clearly defined, and the standard cytotoxicity tests are too robust to detect small differences in the cytotoxicity of these alloys, all of which can lead to unexpected adverse reactions in some patients. Therefore, we tested the hypothesis that the differences in manufacture and microstructure of commercially available SE and TA archwires may influence their biocompatibility. The archwires were studied as-received and after conditioning for 24 h or 35 days in a cell culture medium under static conditions. All of the tested archwires, including their conditioned medium (CM), were non-cytotoxic for L929 cells, but Rematitan SE (both as received and conditioned) induced the apoptosis of rat thymocytes in a direct contact. In contrast, TruFlex SE and Equire TA increased the proliferation of thymocytes. The cytotoxic effect of Rematitan SE correlated with the higher release of Ni ions in CM, higher concentration of surface Ni and an increased oxygen layer thickness after the conditioning. In conclusion, the apoptosis assay on rat thymocytes, in contrast to the less sensitive standard assay on L929 cells, revealed that Rematitan SE was less cytocompatible compared to other archwires and the effect was most probably associated with a higher exposition of the cells to Ni on the surface of the archwire, due to the formation of unstable oxide layer.

The correlation between pain perception among patients with six different orthodontic archwires and the degree of dental crowding

AB  - Introduction Forces generated in orthodontic treatment with fixed appliances create tension and compression zones in the periodontal ligament resulting in a painful experience for patients. In the first phase of orthodontic treatment, when leveling of teeth is needed, nickel-titanium (NiTi) archwires can be completely engaged in brackets, even in the cases of extreme crowding, exerting small forces. There is a great individual variation in the pain perception related to the application of orthodontic forces. Objective The aim of this study was to investigate the pain perception among patients with dental crowding after insertion of six different NiTi orthodontic archwires as a part of fixed appliances in the first stage of orthodontic treatment. Methods The study was conducted on a sample of 189 orthodontic patients receiving one of six different either superelastic or heat activated NiTi archwires, in the first phase of orthodontic treatment. Pain perception was evaluated in groups of patients with different degree of crowding. The modified McGill Pain Questionnaire with Visual Analogue Scale was used to evaluate the quality and intensity of pain. Statistical analysis was performed using simple descriptive statistics, and Pearson's chi-square test with statistical significance of p lt 0.05. Results Majority of patients reported pain as discomfort or pressure of moderate intensity caused by chewing or biting, started within 12 hours, carried on for 3-4 days, and decreased over time without self-medication. Conclusion No correlation was found between pain perception among patients with different types of NiTi archwires and the degree of crowding.
AB  - Uvod Ortodontske sile kojima se tokom terapije fiksnim aparatima pomeraju zubi dovode do istezanja i kompresije parodontalnih vlakana, te pojave bola kod pacijenata. U prvoj fazi ortodontskog lečenja, kada se zubi nivelišu, žice od legure nikl-titanijuma proizvode relativno malu silu i mogu se potpuno ligirati u slotove bravica, čak i u slučajevima velike teskobe. Postoje veoma velike individualne varijacije u percepciji bola prilikom primene ortodontskih sila. Cilj rada Cilj istraživanja je bio da se ispita percepcija bola kod osoba s teskobom u zubnim nizovima nakon ligiranja šest različitih ortodontskih žica u sklopu prve faze terapije fiksnim aparatima. Metode rada Studija je urađena na uzorku od 189 pacijenata kojima je u okviru ortodontskog lečenja fiksnim aparatima bila ligirana jedna od šest različitih superelastičnih ili termo žica od legure nikl-titanijuma. Percepcija bola je procenjivana kod osoba s različitom izraženošću teskobe. Korišćen je modifikovani Makgilov (McGill) upitnik za bol s Vizuelnom analognom skalom radi opisivanja kvaliteta i intenziteta bola. Statistička obrada podataka je obuhvatila deskriptivnu statističku analizu i primenu h2-testa sa statističkom značajnošću od p lt 0,05. Rezultati Najveći broj ispitanika je opisao bol kao neprijatnost ili pritisak srednjeg intenziteta izazvan žvakanjem ili dodirom, koji je počinjao do 12 sati od ligiranja žice, trajao je tri-četiri dana i smanjivao se bez primene lekova. Zaključak Nije utvrđena statistički značajna razlika u percepciji bola kod osoba s različitom izraženošću teskobe zubnih nizova kojima su bile ligirane superelastične i termo žice od legure nikl-titanijuma.

AB  - Class III malocclusion is orthodontic anomaly where mandibular arch is in mesial position to maxillary arch. Reasons for Class III malocclusion can be found in mandibular prognathism, maxillar retrognathism or combination of these two. In most cases of mandibular prognathism, it is necessary to postpone the treatment until the growth ceases. However, if certain conditions are accomplished it is possible to start early treatment of class III malocclusion to improve occlusal relations and provide more favorable environment for future growth. The aim of this study was to present treatment of two patients with Class III maloclussion in early mixed dentition, using two different appliances: Delaire mask and Frankel functional regulator type 3. The treatment with Delaire mask resulted in rotation of maxilla downward and forward due to the angle of extraoral part of the mask to the occlusal plane that was modified to be almost 45°. At the end of the treatment facial esthetics was significantly improved. Moving maxilla forward resulted in straight profile, whereas moving maxilla downward lead to coordination of upper, middle and lower facial third. The result of the treatment in patient who used Frankel functional regulator was correction of anterior crossbite by premaxilla development and incisors protrusion. Significant differences in SNA, SNB and ANB angle values at the beginning and at the end of the treatment were not found suggesting that most changes were dental but not skeletal.
AB  - Skeletna malokluzija III klase je nesklad u međusobnom odnosu gornje i donje vilice, jer je donja vilica postavljena mezijalnije u odnosu na gornju. Uzrok nastanka skeletne malokluzije III klase može biti mandibularni prognatizam, maksilarni retrognatizam ili kombinacija ova dva uzroka. U slučaju isuviše razvijene donje vilice najčešće je potrebno sačekati završetak rasta, kako bi se primenilo konačno lečenje. Međutim, ako su ispunjeni određeni uslovi, moguće je primeniti ranu terapiju III klase, da bi se poboljšali okluzalni odnosi i obezbedila dobra osnova za dalji rast. Cilj ovog rada bio je da se prikaže terapijski efekat dva slučaja malokluzije III klase u ranom uzrastu pomoću različitih ortodontskih aparata: Delerove maske i Frenklovog regulatora funkcije tip 3. Kod pacijentkinje koja je tokom lečenja nosila Delerovu masku gornja vilica je zarotirana unapred i nadole, jer je ugao delovanja sile modifikovan da bude skoro 45 stepeni. Na kraju lečenja postignut je znatno bolji izgled lica. Pomeranjem gornje vilice unapred postignut je prav profil, dok je pomeranje nadole dovelo do usklađivanja visine srednje trećine lica sa gornjom i donjom. Kod pacijenta kod kojeg je tokom lečenja primenjena terapija Frenklovim regulatorom funkcije tip 3 do korekcije obrnutog preklopa sekutića došlo je kombinacijom razvijanja premaksile i protruzije sekutića. Značajne promene u vrednostima uglova SNA, SNB i ANB nisu zabeležene na kraju terapije u odnosu na početak, što bi ukazivalo na to da su postignute promene uglavnom dentalne, a ne skeletne.

AB  - The present study assessed changes of craniofacial complex in Turner syndrome (TS) patients treated with growth hormone (GH) during development. The objective was to examine the growth rate and pattern of craniofacial structures and to establish effects of GH on craniofacial development. The study population consisted of 15 TS patients treated with GH aged 5-18.5 years (13.3 +/- 4.4) and corresponding control group of 45 females aged 6.8-18.7 (11.4 +/- 2.6). According to the stage of cervical vertebral maturation, subjects were categorized into pre-growth (5 TS and 15 controls) and growth (10 TS and 30 controls) subgroups. The cephalometric analysis comprised angular and linear variables, measured on lateral cephalometric radiographs. The mandibular corpus/anterior cranial base ratio increased significantly only in controls during development. In growth period, ramus/corpus ratio was significantly larger in TS group. SNA and SNB angles were significantly smaller in TS growth subgroup compared to corresponding controls. Among other variables, no statistically significant differences were revealed. In TS patients treated with GH, growth capacities of cranial base and maxilla are adequate which can be attributed to GH treatment. Shape of mandible is altered due to decreased growth of corpus and overdeveloped ramus. Both maxillary and mandibular retrognathism are becoming more expressed during development. Favorable influence of GH on craniofacial complex growth rate and altered growth pattern revealed in this study should be considered while planning both orthodontic treatment and retention.

AB  - This study was addressed to use knowledge about the orthodontic system with numerical simulation of the finite elements method. For the first time we simulated the stresses on the orthodontic system and, in this manner, calculated the orthodontic force on the tooth. A 3D orthodontic model or orthodontic system was designed resembling moderate crowding in the dental arch with all supporting structures. CATIA V5 computer software was used to set up a model for the orthodontic system and ABAQUS was used for simulation of the stresses on the orthodontic system. Our attention was focused on the stresses on the tooth lateral incisor and its periodontal ligament. The results of the numerical simulation showed complex stresses on the tooth lateral incisor and its periodontal ligament. In this paper there is presented a calculation of the orthodontic force acting on the tooth lateral incisor due to the orthodontic wire. This orthodontic force was calculated from the stresses on the bracket. The calculated orthodontic force was in the area which is considered as the optimal orthodontic force for movement of the tooth.

Reliability of computerized cephalometric outcome predictions of mandibular set-back surgery

AB  - Introduction. A successful treatment outcome in dentofacial deformity patients commonly requires combined orthodontic-surgical therapy. This enables us to overcome functional, aesthetic and psychological problems. Since most patients state aesthetics as the primary motive for seeking therapy, cephalometric predictions of treatment outcome have become the essential part of treatment planning, especially in combined orthodontic-surgical cases. Objective. The aim of this study was to evaluate the validity and reliability of computerized orthognathic surgery outcome predictions generated using the Nemotec Dental Studio NX 2005 software. Methods. The sample of the study consisted of 31 patients diagnosed with mandibular prognathism who were surgically treated at the Hospital for Maxillofacial Surgery in Belgrade. Investigation was done on lateral cephalograms made before and after surgical treatment. Cephalograms were digitized and analyzed using computer software. According to measurements made on superimposed pre- and postsurgical cephalograms, the patients were retreated within the software and the predictions were assessed by measuring seven angular and three linear parameters. Prediction measurements were then compared with the actual outcome. Results. Results showed statistically significant changes between posttreatment and predicted values for parameters referring to lower lip and mentolabial sulcus position. Conclusion. Computerized cephalometric predictions for hard-tissue structures in the sagittal and vertical planes, as well as the VTO parameters, generated using the Nemotec Dental Studio NX 2005 software are reliable, while lower lip and mentolabial sulcus position predictions are not reliable enough.
AB  - Uvod. Radi postizanja što boljih krajnjih rezultata, osobe sa dentofacijalnim deformitetima najčešće se leče kombinovanom ortodontsko-hirurškom terapijom. Na taj način rešavaju se funkcionalni, estetski i psihološki problemi. Kako najveći broj pacijenata navodi estetsku promenu kao primarni motiv lečenja, kefalometrijsko predviđanje ishoda je važan deo planiranja terapije, posebno kod složenih slučajeva. Cilj rada. Cilj rada je bio da se proceni preciznost i pouzdanost predviđanja ishoda hirurške korekcije mandibularnog prognatizma razvijenih u okviru kompjuterskog softvera Nemotec Dental Studio NX 2005. Metode rada. Ispitan je 31 pacijent s mandibularnim prognatizmom koji je operisan na Klinici za maksilofacijalnu hirurgiju Stomatološkog fakulteta Univerziteta u Beogradu. Istraživanje je izvedeno na profilnim teleradiogramima snimljenim pre i posle hirurške intervencije. Nakon digitalizacije snimaka izvršena je kompjuterizovana kefalometrijska analiza. Na osnovu merenja određenih horizontalnih i vertikalnih rastojanja na preoperacionim i postoperacionim radiogramima, urađena je simulacija hirurških korekcija i na njima izmerena vrednost sedam angularnih parametara i tri linearna parametra, čija je vrednost upoređena s vrednostima na postoperacionim radiogramima. Rezultati. Ustanovljene su statistički značajne razlike između postoperacionih i predviđenih vrednosti za parametre koji govore o položaju donje usne i mentolabijalnog sulkusa. Zaključak. Kompjuterizovana kefalometrijska predviđanja položaja čvrstotkivnih struktura u sagitalnoj i vertikalnoj ravni, kao i VTO parametara, dobijena u okviru korišćenog programa su pouzdana, dok su predviđanja položaja donje usne i mentolabijalnog sulkusa nedovoljno pouzdana.

Metode rada. Ispitan je 31 pacijent s mandibularnim prognatizmom koji je operisan na Klinici za maksilofacijalnu hirurgiju Stomatološkog fakulteta Univerziteta u Beogradu. Istraživanje je izvedeno na profilnim teleradiogramima snimljenim pre i posle hirurške intervencije. Nakon digitalizacije snimaka izvršena je kompjuterizovana kefalometrijska analiza. Na osnovu merenja određenih horizontalnih i vertikalnih rastojanja na preoperacionim i postoperacionim radiogramima, urađena je simulacija hirurških korekcija i na njima izmerena vrednost sedam angularnih parametara i tri linearna parametra, čija je vrednost upoređena s vrednostima na postoperacionim radiogramima. Rezultati. Ustanovljene su statistički značajne razlike između postoperacionih i predviđenih vrednosti za parametre koji govore o položaju donje usne i mentolabijalnog sulkusa. Zaključak. Kompjuterizovana kefalometrijska predviđanja položaja čvrstotkivnih struktura u sagitalnoj i vertikalnoj ravni, kao i VTO parametara, dobijena u okviru korišćenog programa su pouzdana, dok su predviđanja položaja donje usne i mentolabijalnog sulkusa nedovoljno pouzdana.",

Evaluation of agreement and correlation of three occlusal indices in an assessment of orthodontic treatment need

The aim of this study was to determine correlations and agreement among three occlusal indices: the Index of Orthodontic Treatment Need (IOTN), the Peer Assessment Rating Index (PAR) and the Index of Complexity, Outcome and Need (ICON) in evaluating orthodontic treatment need. Methods. A total eighty study models of patients referred to the Department of Orthodontics, School of Dentistry, Belgrade, were assessed in this retrospective study. Malocclusions of various types and severity in the permanent dentition were included. Results. The Aesthetic and the Dental Health Component of IOTN determined orthodontic treatment need in 25% and 51% of the patients, respectively. PAR determined orthodontic treatment need in 59% and ICON in 53% of patients. The Aesthetic Component of IOTN and ICON had the highest correlation (Spearman's correlation coefficient 0.95, p  lt  0.01). Correlations between indices were 0.44 to 0.61 with statistical significance (p  lt  0.01). The agreement between indices, calculated using Kappa statistics, was 0.22 to 0.63. Conclusion. The most critical in malocclusion assessment was PAR. The Aesthetic Component of IOTN and ICON correlated highly (p  lt  0.01). Correlations between other pairs of indices were moderate (p  lt  0.01). The Aesthetic Component of IOTN and ICON had substantial agreement, whereas agreement between other indices was fair or moderate. ICON could replace PAR and IOTN. Application of occlusal indices enables objective evaluation of orthodontic treatment need and easier determination of the treatment priorities.
AB  - Uvod/Cilj. Okluzalni indeksi su kvantitativni dijagnostički pokazatelji težine malokluzije, potrebe za ortodontskim lečenjem, složenosti i rezultata lečenja. Cilj rada bio je da se utvrde korelacije i saglasnost između tri okluzalna indeksa: Index of Orthodontic Treatment Need (IOTN), Peer Assessment Rating Index (PAR) i Index of Complexity, Outcome and Need (ICON) u pogledu određivanja potrebe za ortodontskim lečenjem. Primena okluzalnih indeksa omogućava objektivnu procenu potrebe za ortodontskim lečenjem i olakšava određivanje prioriteta u lečenju.

Dentaoalveolar changes in young adult patients with Class II/1 malocclusion treated with the Herbst appliance and an activator

AB  - Background/Aim. Functional appliances can be used effectively in the treatment of skeletal Class II/1 malocclusions. The best treatment results are obtained during active period of facial growth when skeletal, as well as dentoalveolar, changes occur. In comparison with removable functional appliances, such as activator, that are effective only during adolescent period of growth, the Herbst fixed appliance is also successful at the end of the growth period. It also offers a shorter treatment time and a patient compliance is not necessary. The aim of this study was to analyze and compare dentoalveolar changes in the group of young adult patients with Class II/1 malocclusion treated with the Herbst appliance and an activator. Methods. The sample for this study consisted of 50 patients of both sexes, 14-21 years of age with Class II/1 malocclusion. For estimating the effect of functional appliances used, the following cephalometrics parameters were determined: inclination of the upper and lower incisors, interincisal angle, antero-posterior molars relationships, overjet and overbite. The results obtained were statistically tested. Results. The cephalometric findings after the treatment indicated retroinclination of upper incisors (average value of 9°) and proclination of lower incisors (average value of 7°), mostly expressed in the patients treated by Herbst appliance (p  lt  0.001). Increased overjet and distocclusion were completely corrected in the group of patients treated with the Herbst appliance, while the correction of malocclusion in the activator group was only partially accomplished. No changes in the overbite were noticed at the end of the treatment in both groups. Conclusion. The results of this study revealed that the Herbst appliance is more effective in the treatment of Class II/1 malocclusion in young adults in comparison with the activator.
AB  - Uvod/Cilj. Funkcionalni aparati uspešno se koriste u terapiji malokluzija klase II/1. Najbolji rezultati postižu se terapijom u periodu pubertetskog skoka rasta, kada su moguća najveća skeletna i dentoalveolarna pomeranja. U poređenju sa terapijom pokretnim funkcionalnim aparatima, kao što je aktivator prema Andresen-u, terapija Herbst-ovim fiksnim aparatom (Herbst aparat), uspešna je i nakon pubertetskog skoka rasta, vreme terapije je kraće, a bolesnike ne treba motivisati za saradnju jer je aparat fiksiran na zubima. Cilj istraživnja bio je da se kod bolesnika mlađeg odraslog uzrasta sa malokluzijom klase II odeljenja 1 uporede efekti fiksnog funkcionalnog Herbst aparata i pokretnog aktivatora na dentoalveolarne strukture. Metode. Ispitivanje je obuhvatilo 50 bolesnika sa malokluzijom klase II/1, oba pola, uzrasta 14-21 godine. Efekti primenjenih funkcionalnih aparata, aktivatora prema Andresen-u i Herbst aparata, procenjivani su na osnovu parametara merenih na profilnim telerendgenskim snimcima, nagiba gornjih i donjih sekutića, interincizalnog ugla, okluzije po Angle-u, incizalnog stepenika i incizalnog preklopa. Dobijeni rezultati obrađeni su statistički. Rezultati. Tokom terapije nastupile su statistički visokoznačajne promene u obe grupe: oralno naginjanje gornjih i vestibularno naginjanje donjih sekutića. Promene su bile izraženije u grupi bolesnika lečenih Herbst aparatom (p  lt  0,001). Nagib gornjih sekutića promenjen je prosečno oko 9°, a donjih oko 7°. Odnos molara i incizalni razmak su u potpunosti korigovani na kraju lečenja u grupi lečenoj Herbst aparatom, dok su isti parametri u grupi sa aktivatorom bili nepotpuno korigovani. Nijedan od primenjenih aparata, na završetku lečenja, nije izazvao promene u vertikalnom preklopu sekutića. Zaključak. Poređenje terapijskih efekata Herbst aparata i aktivatora kod bolesnika uzrasta 14-21 godinu pokazalo je da je Herbst aparat efikasniji u korekciji dentoalveolarne malokluzije klase II odeljenja 1 nego aktivator.

Orthodontic treatment of nongrowing patient with class II division 2 malocclusion by Herbst appliance

AB  - Background. Inheritance is most casual etiological factor of Class II division 2 malocclusion. This kind of malocclusion is very difficult for treatment specially in older patients. Case report. In the female patient, 20 years old, at the beginning of the treatment at the School of Dentistry in Belgrade, lateral cephalogram showed skeletal and dentoalveolar Class II division 2 malocclusion. She was in the Herbst treatment for 8 months and 12 months more with a fixed multibracket appliance. The measurements were performed on lateral cephalograms before and after the treatment: ii, is, mi, ms, Pg and ss. The distance from these points to occlusal perpendicular line (Olp) were measured and compared from cephalogram before to cephalogram after the treatment. Temporomandibular joint (TMJ) tomograms were compared from before and after the treatment by superimposition. Correction was found in molar and incisor relation, overjet and overbite. There were found sagital skeletal changes and soft tissue profile improvement. Conclusion. Herbst appliance is effective in the treatment of Class II malocclusions, even in adult patients. Dental and skeletal changes as a result of Herbst treatment could be good choice instead of camouflage orthodontics or surgical decision.
AB  - Uvod. Malokluzije klase II odeljenja 2 uglavnom su nasledne etiologije. Pripadaju komplikovanijim slučajevima za ortodontsku terapiju, naročito kod starijih bolesnika. Prikaz bolesnika. Kod bolesnice stare 20 godina, na početku terapije koja je sprovedena na Stomatološkom fakultetu u Beogradu, na profilnom telerendgen snimku dijagnostikovana je skeletna i dentoalveolarna nepravilnost klase II odeljenja 2. Terapija Herbst-aparatom trajala je 8 meseci i još 12 meseci fiksnim aparatom. Na profilnom telerendgen snimku pre i posle terapije merena su rastojanja između okluzalne perpendikularne linije OLP i: najmezijalnije tačke aproksimalne površine gornjeg prvog molara (ms), najmezijalnije tačke aproksimalne površine donjeg prvog molara (mi), incizalne ivice gornjeg sekutića (is), incizalne ivice donjeg sekutića (ii), najudaljenije tačke prednje strane maksile (ss) i najisturenije tačke profila brade (Pg). Upoređene su vrednosti sa snimaka pre i posle terapije. Profilni snimci temporomandibularnog zgloba (TMZ) pre i posle terapije superponirani su i potom analizirani. Primenom Herbst aparata korigovan je odnos molara i odnos inciziva, kao i incizalni stepenik i incizalni preklop. Uočene su sagitalne skeletne promene i poboljšanje izgleda mekotkivnog profila bolesnika. Zaključak. Herbst aparat je efikasan u terapiji malokluzije klase II, čak i kod starijih bolesnika. Dentalne i skeletne promene kao rezultat terapije Herbst aparatom mogu ukazati na dobar izbor umesto terapije ortodontskom kamuflažom ili ortognatskom hirurgijom.

abstract = "Background. Inheritance is most casual etiological factor of Class II division 2 malocclusion. This kind of malocclusion is very difficult for treatment specially in older patients. Case report. In the female patient, 20 years old, at the beginning of the treatment at the School of Dentistry in Belgrade, lateral cephalogram showed skeletal and dentoalveolar Class II division 2 malocclusion. She was in the Herbst treatment for 8 months and 12 months more with a fixed multibracket appliance. The measurements were performed on lateral cephalograms before and after the treatment: ii, is, mi, ms, Pg and ss. The distance from these points to occlusal perpendicular line (Olp) were measured and compared from cephalogram before to cephalogram after the treatment. Temporomandibular joint (TMJ) tomograms were compared from before and after the treatment by superimposition. Correction was found in molar and incisor relation, overjet and overbite. There were found sagital skeletal changes and soft tissue profile improvement. Conclusion. Herbst appliance is effective in the treatment of Class II malocclusions, even in adult patients. Dental and skeletal changes as a result of Herbst treatment could be good choice instead of camouflage orthodontics or surgical decision., Uvod. Malokluzije klase II odeljenja 2 uglavnom su nasledne etiologije. Pripadaju komplikovanijim slučajevima za ortodontsku terapiju, naročito kod starijih bolesnika. Prikaz bolesnika. Kod bolesnice stare 20 godina, na početku terapije koja je sprovedena na Stomatološkom fakultetu u Beogradu, na profilnom telerendgen snimku dijagnostikovana je skeletna i dentoalveolarna nepravilnost klase II odeljenja 2. Terapija Herbst-aparatom trajala je 8 meseci i još 12 meseci fiksnim aparatom. Application of occlusal indices in orthodontic practice

AB  - Introduction. Occlusal indices were designed to improve diagnostic criteria and to enable an objective assessment of malocclusion severity. The aim of this study was to present the most frequently used occlusal indices in orthodontic practice and to determine their reliability when applied to dental models. Material and Methods. Three occlusal indices were selected for analysis: the Index of Orthodontic Treatment Need (IOTN), the Peer Assessment Rating Index (PAR) and the Index of Complexity, Outcome and Need (ICON). Twenty dental models of patients referred to Department of Orthodontics, School of Dentistry, Belgrade, with malocclusions of different type and severity were used in this study. All dental models were measured by three afore mentioned indices twice, two months apart, in order to determine intraexaminer reliability. Results. The results showed that three indices had good reliability. Weighted Kappa was calculated for IOTN (0.72 and 0.79 for the aesthetic and the dental health component, respectively) and root mean square error was calculated for PAR and ICON (2.1 and 4.5, respectively). There was no statistically significant difference in scores between two measurements of these two indices (p lt 0.01). Conclusion. Application of occlusal indices enables orthodontists to determine priorities in patient care, planning of orthodontic service, monitoring and promotion of standards. Occlusal indices are reliable diagnostic criteria.
AB  - Uvod. Okluzivni indeksi su uvedeni u praksu sa ciljem da se poboljšaju dijagnostički kriterijumi i omogući objektivna procena težine malokluzije. Cilj ovog rada je bio da se predstave najčešće korišćeni okluzivni indeksi u ortodontskoj praksi i utvrdi pouzdanost njihove primene na studijskim modelima. Materijal i metode rada. Za analizu su odabrana tri okluzivna indeksa: Index of Orthodontic Treatment Need (IOTN), Peer Assessment Rating (PAR) i Index of Complexity, Outcome and Need (ICON). U istraživanju je korišćeno 20 studijskih modela pacijenata Klinike za ortopediju vilica Stomatološkog fakulteta u Beogradu s malokluzijama različitog tipa i težine u stalnoj denticiji. Svi modeli su ocenjeni sa tri pomenuta indeksa dva puta u razmaku od dva meseca radi utvrđivanja pouzdanosti. Rezultati. Rezultati analize su pokazali da je pouzdanost sva tri indeksa bila dobra. Za IOTN pouzdanost je izražena koeficijentom kapa (0,72 za estetski i 0,79 zdravstveni deo indeksa), dok je za PAR i ICON izračunat koren srednje kvadratne greške (2,1 i 4,5). Nije bilo statistički značajne razlike u broju bodova između dva merenja ova dva indeksa (p lt 0,01). Zaključak. Primena okluzivnih indeksa omogućava ortodontima određivanje prioriteta u zbrinjavanju pacijenata, planiranje ortodontske službe, pridržavanje i promociju standarda. Okluzivni indeksi su pouzdani dijagnostički kriterijumi koji mogu značajno uticati na plan lečenja malokluzija.

Bond strength of orthodontic adhesives

AB  - The objective of this study was to evaluate and compare bond strength of four adhesives used in orthodontics for bonding brackets to tooth enamel. The adhesives used in this investigation were resin-reinforced glass-ionomer cement (Fuji Ortho LC-GC Corporation, Japan), light cured composite resin adhesives (ConTec LC-Dentaurum, Germany and Enlight Bonding system-Ormco, USA) and dual cured composite resin adhesive (ConTec Duo-Dent aurum, Germany). A sample of 80 extracted human premolars was divided into four groups of 20 teeth which were etched with 37% phosphoric acid and bonded in dry field to enamel of buccal and lingual surfaces of the teeth with the same adhesive in one group. The debonding force was produced using universal Instron testing machine with cross head speed of 1mm/min and shear bond strength was measured. Statistical analysis used in this study included: Kolmogorov-Smirnov test i Shapiro-Wilk test, Mann Whitney test, Kruskal Wallis chi-squared test. Even though, all four adhesives showed sufficient bond strength for orthodontic bonding, ConTec LC and ConTec Duo displayed the superior bonding properties comparing to Enlight and Fuji Ortho LC. .

Craniofacial complex specificities in five men with sex reversal syndrome

AB  - In order to clarify the role of sex chromosome constitution in craniofacial growth control, we compared craniofacial morphology of men with sex reversal syndrome (46, XX) with the morphology of normal men, Klinefelter's syndrome and normal women, using cephalometric measurements. Due to physical resemblance between men with 46, XX karyotype and men with 47, XXY karyotype, cephalometric analysis comprised parametres which had already been found to be specific for Klinefelter's syndrome, i.e. cranial base length and flexion, maxillary and mandibular base length, jaw position in relation to cranial base and sagittal jaw relationship. Linear measurements showed reduction of about 10% in maxillary base length in 46, XX men in relation to normal men. Mandibular base in men with sex reversal syndrome was also shortened for about 10% in relation to both normal men and Klinefelter's syndrome. Cranial base flexion in men with Klinefelter's syndrome and in men with sex reversal syndrome showed similarity. The basal angle was found to be more acute, for about 4, in comparison to individuals with normal karyotype. Unlike in men with Klinefelter's syndrome, mandibular and maxillary prognathism were not registered in men with sex reversal syndrome.

Correlation of dental and chronological maturity in girls and boys aged 7 to 14 years

AB  - The aim of the study was to assess correlation level of dental and chronological maturity related to gender in a group of children of our population. Material and method: Total number of 320 girls and boys, 7 to 14 years old, participated in the study. Dental age assessed by Demirjian's method. Statistical data were analyzed by mean value and SD for dental maturity in age groups of 7, 8, 9, 10, 11, 12, 13 and 14 years for girls and boys, respectively. Calculating the equation of linear regression and the determination and correlation ratio assessed correlation level of dental and chronological age. Results and conclusion: According to the results obtained, it can be concluded that mean values of dental age are coinciding with the chronological maturity in all 8 age groups which is confirmed by high interdependence of dental and chronological maturity (78.6% for boys and 79.6% for girls).
AB  - Cilj ovog istraživanja bio je da se na uzorku dece naše populacije utvrdi međuzavisnost dentalne od hronološke zrelosti u odnosu na pol ispitanika. Materijal i metod: Ovom studijom je obuhvaćeno 320 devojčica i dečaka uzrasta od 7-14 godina. Dentalna starost je određena metodom po Demirijanu. Dobijeni rezultati su statistički obrađeni uz izračunavanje prosečne vrednosti i standardne devijacije za dentalnu zrelost za uzrast 7,8,9,10,11,12,13 i 14 godina, i to posebno za devojčice i za dečake. Ispitivana je zavisnost dentalne i hronološke starosti, kod devojčica i dečaka izračunavanjem jednačine linearne regresije, i određivanjem koeficijenta determinacije i korelacije. Rezultati i zaključak: Na osnovu dobijenih rezultata može se zaključiti da prosečne vrednosti dentalne starosti ne odstupaju mnogo u odnosu na hronološku zrelost kod svih osam grupa i da postoji visoka zavisnost dentalne i hronološke zrelosti (za dečake 78,6%, a za devojčice 79,6%).

abstract = "The aim of the study was to assess correlation level of dental and chronological maturity related to gender in a group of children of our population. Material and method: Total number of 320 girls and boys, 7 to 14 years old, participated in the study. Dental age assessed by Demirjian's method. Statistical data were analyzed by mean value and SD for dental maturity in age groups of 7, 8, 9, 10, 11, 12, 13 and 14 years for girls and boys, respectively. Calculating the equation of linear regression and the determination and correlation ratio assessed correlation level of dental and chronological age. Results and conclusion: According to the results obtained, it can be concluded that mean values of dental age are coinciding with the chronological maturity in all 8 age groups which is confirmed by high interdependence of dental and chronological maturity (78.6% for boys and 79.6% for girls)., Cilj ovog istraživanja bio je da se na uzorku dece naše populacije utvrdi međuzavisnost dentalne od hronološke zrelosti u odnosu na pol ispitanika. Materijal i metod: Ovom studijom je obuhvaćeno 320 devojčica i dečaka uzrasta od 7-14 godina. Dentalna starost je određena metodom po Demirijanu. Dobijeni rezultati su statistički obrađeni uz izračunavanje prosečne vrednosti i standardne devijacije za dentalnu zrelost za uzrast 7,8,9,10,11,12,13 i 14 godina, i to posebno za devojčice i za dečake. Ispitivana je zavisnost dentalne i hronološke starosti, kod devojčica i dečaka izračunavanjem jednačine linearne regresije, i određivanjem koeficijenta determinacije i korelacije. Rezultati i zaključak: Na osnovu dobijenih rezultata može se zaključiti da prosečne vrednosti dentalne starosti ne odstupaju mnogo u odnosu na hronološku zrelost kod svih osam grupa i da postoji visoka zavisnost dentalne i hronološke zrelosti (za dečake 78,6%, a za devojčice 79,6%).",

AB  - The aim of this study was to examine the efects of Delair`s mask on facial skeleton in patients with Class III antero-posterior jaw relationship. Two groups of children were evaluated. The first group of 34 children with Class III was treated with Delair`s mask. The second group of 46 children with Class III did not receive any orthodontic treatment and was presented as a control group. In the first group pretreatment and post treatment radiographic tracing was done while second tracing in the control group was done a year after diagnostic radiographic tracing. Analysis of the skeletal facial profile was done measuring SNA, SNB and ANB angles. Rotation of the maxillary base (NS/SpP), rotation of the mandibular base (NS/MP) and vertical relationship between jaws (SpP/MP) were also measured. The results were statisticaly analysed using T test, descriptive statistics and Wilcoxon`s test. The results of this study showed changes in facial skeleton in children treated with Delair`s mask compared to the control group. In the first group of children SNA and ANB angles were increased and the SNB angle was decreased.
AB  - Cilj ovoga rada je bio da se odrede promene u facijalnoj morfologiji koje nastaju kod osoba sa III skeletnom klasom lečenih Deler-om maskom. U istraživanje su uključene dve grupe dece. Prvu je sačinjavalo 34 deteta sa malokluzijom III skeletne klase, kod kojih je terapija sprovedena Delair-ovom facijalnom maskom. Drugu, kontrolnu, grupu ispitanika činilo je 46-oro dece sa anomalijom III skeletne klase koji nisu bili ortodontski tretirani. Renden kefalometrijski snimci su snimani na početku i kraju terapije u prvoj grupi, dok je u drugoj grupi drugi snimak urađen posle pauze od godinu dana. Analizirani su sledeći parametri: SNA - ugao maksilarnog prognatizma, SNB - ugao mandibularnog prognatizma. ANB - sagitalni međuvilični ugao, NS/SpP - nagib maksile prema prednjoj kranijalnoj bazi, NS/MP - nagib mandibule prema prednjoj kranijalnoj bazi i SpP/MP - vertikalni međuvilični ugao. U toku statističke obrade dobijenih podataka korišćene su metode deskriptivne statistike, kao i studentov t-test i Wilcoxon-ov test. Utvrđeno je da tokom lečenja malokluzija III skeletne klase uz primenu Delairove facijalne maske dolazi do povećanja ugla maksilarnog prognatizma, smanjenja ugla mandibularnog prognatizma i povećanja sagitalnog skeletnog međuviličnog ugla.

AB  - The aim of this paper was to describe a treatment of female patient, 13 years of age, with dentoalveolar class II division 1. malocclusion duc to persistency of both upper deciduous canines. The treatment was performed by using fixed orthodontics appliances (Ricketts .018" prescription) in both jaws, with straight wire technique. The appliances were removed after 15 moths' treatment, and retention period lasted 12 months.
AB  - U radu je prikazana terapija pacijenta sa lažnom hiperodoncijom očnjaka, sprovedena fiksnim aparatom u gornjoj i donjoj vilici, primenom tehnike pravog bola . Posle ekstrakcije perzistentnih mlečnih očnjaka, izvršena je nivelacija zuba, distalizacija stalnih očnjaka i retrakcija frontalnih zuba. Terapija je trajala 15 meseci, uz retencioni period od 12 meseci.

AB  - The aim of this investigation was to assess the craniofacial morphology of five male individuals with 46,XX chromosomal constitution, and to compare them with normal male individuals (46,XY) and normal female participates (46,XX). The investigation was carried out on the lateral cephalomestic radiographs of five male individuals with sex reverse syndrome (46,XX chromosomal constitution), 62 normal female (46,XX) and 31 normal male (46,XY chromosomal constitution) participants. The following angular variables were measured: SNA, SNB, ANB, SN/SpP, SN/MP and SpP/MP. For each variable the mean value and standard deviation were calculated. The Student t-test was used to compare the result obtained between the group of patients with sex reverse syndrome and group of normal male, as well as of normal female participants. The male individuals with 46, XX chromosomal constitution showed slight bimaxillary retrognathism, and class III sagittal jaw relationship. Due to decreased inclination of the upper jaw to the anterior cranial base, the angle of vertical jaw relationship slightly increased in the group of patients with sex reverse syndrome, compared to normal male and female participants.
AB  - Cilj ovog istraživanja bio je da se ispita kraniofacijalna morfologija muškaraca sa 46,XX hromozomskom konstitucijom i uporedi sa morfologijom normalnih žena (46,XX) i muškaraca (46,XY). Istraživanje je sprovedeno na profilnim kefalometrijskim snimcima tri grupe ispitanika. Prvu grupu sačinjavalo je pet muškaraca sa hromozomskom konstitucijom 46,XX. Drugu grupu činila su 62 kefalometrijska snimka normalnih žena sa hromozomskom konstitucijom 46,XX, a treću 31 snimak normalnih muškaraca sa hromozomskom konstitucijom 46,XY. Na kefalometrijskim snimcima su mereni sledeći angularni parametri: SNA, SNB, ANB, SN/SpP, SN/MP i SpP/MP. Za sve analizirane parametre izračunata je prosečna vrednost i standardna devijacija, a t-testom je ispitivano postojanje statistički značajnih razlika između tri analizirane grupe ispitanika. Istraživanjem je utvrđeno da se 46,XX muškarci odlikuju blagim bimaksilarnim retrognatizmom i skeletnim sagitalnim odnosom III klase. Blago povećanje vertikalnog međuviličnog ugla kod 46,XX muškaraca posledica je manje inklinacije gornje vilice u odnosu na osnovnu ravan prednje kranijalne baze.

Mesiodistal size of deciduous teeth in subjects with unilateral cleft lip and palate

AB  - The aim of this study was to investigate whether there was an association between presence of a cleft and reduced size of deciduous tooth on the cleft side. The largest mesiodistal dimension of deciduous teeth was measured by a sliding caliper. The size of the same teeth on the side of the cleft and on the non-cleft side was compared. The results of the investigation were statistically analyzed. A difference between the genders was found. In the sample of boys the lateral deciduous incisor and the second deciduous molars on the cleft side were smaller than those on the non-cleft side, while in girls the central and lateral deciduous incisors were smaller on the cleft side than on the non-cleft side. It was concluded that the presence of a unilateral cleft lip and palate was associated with smaller tooth size.

AB  - In a group of 15 women with 45,X chromosome constitution, mandibular growth type was investigated by using both linear and angular measurements. The sum of the saddle, articular and genial angle, lower genial angle and y-axis was significantly greater. In addition the posterio-anterior facial height ratio in women with Turner's syndrome was significantly smaller than in the controls (61 women with 46,XX chromosome constitution), indicating a tendency to backward and downward growth changes in the mandible, caused by an X chromosome deficiency.

AB  - Lateral cephalometric radiographs were used to study sagittal jaw relationships in 22 cases with Klinefelter's syndrome. The following angles were measured: SNA, SNB and ANB. Depending on the size of SNA and SNB angles, all cases investigated were classified in the following groups: bimaxillary prognathism, bimaxillary orthognathism or retrognathism; maxillary prognathism combined with mandibular orthognathism; maxillary orthognathism combined with mandibular prognathism or retrognathism; maxillary retrognathism combined with mandibular prognathism, orthognathism, or retrognathism. The mean value of the ANB angle of the whole group was -0.20 degrees indicating the skeletal class III jaw relationship. Out of 22 cases investigated, 45.4% showed bimaxillary prognathism, and 27.3% had a combination of the maxillary orthognathism and mandibular prognathism. 27.3% of all cases investigated belonged to the remaining groups.

AB  - Lateral cephalometric radiographs of 31 cases with unilateral clefts of the lip and palate, 31 cases with complete bilateral clefts of the lip and palate as well as 31 cases with skeletal class I relationship were used for this study. The following angular and linear measurements were analysed: NSBa, SBaN, BaSN, S-N, S-Ba and N-Ba. The groups were compared with each other for all measurements studied. There was no significant difference between these groups for NSBa and SBaN angles. The mean value of the BaSN angle was significantly lower in the group with complete unilateral clefts of the lip and palate than in the remaining two groups. The mean values for linear measurements investigated were significantly lower in the group with unilateral clefts than in the remaining two groups.

AB  - The aim of this longitudinal study was to cephalometrically assess growth changes of the craniofacial system in five cases with completed unilateral cleft of lip and palate, aged 8-11 years. Two types of study were performed: a) superimposition of cephalometric tracings of the lateral skull radiographs in two growth stages; b) metric analysis of SNA, SNB, NSAr, SArGo, ArGoMe angles, as well as S-Go:N-Me percentages. On the basis of the results obtained from this study, it was found posterior growth rotation in 4 out of 5 cases, bimaxillary rethrognathism, and a decrease of the SNA and an increase of the SNB angles during the growth period investigated.
