Clinical and microbiological effects of the initial periodontal therapy

AB  - Introduction Periodontitis is a destructive inflammatory disease of the tooth-supporting tissues, primarily caused by Gram-negative microorganisms. Thus, the primary objective of cause-related initial periodontal therapy is disruption and removal of the subgingival biofilm. Objective The aim of this study was to evaluate the clinical and microbiological effects of the initial therapy in patients diagnosed with chronic periodontitis. Methods Forty patients with chronic periodontitis were included in the study. As a part of the clinical assessment undertaken prior to the initial therapy, as well as one month and three months post-therapy, plaque index, gingival index, papilla bleeding index, probing pocket depth and clinical attachment level were recorded. Microbiological testing was performed prior to the initial therapy and three months after therapy. Polymerase chain reaction assays were used to determine the presence of Porphyromonas gingivalis, Tannerella forsythensis, Prevotella intermedia and Aggregatibacter actinomycetemcomitans. Results All clinical parameters were significantly reduced after therapy. The prevalence of Aggregatibacter actinomycetemcomitans was reduced by 22.5%, which was a statistically significant decrease compared to the baseline. The prevalence of Porphyromonas gingivalis, Tannerella forsythensis and Prevotella intermedia tended to decrease after therapy; however, the difference did not reach statistical significance. Conclusion The results of the present study demonstrated the beneficial effects of the initial periodontal therapy on both the clinical and microbiological parameters.
AB  - Uvod Parodontopatija je inflamatorno, destruktivno oboljenje potpornog aparata zuba uzrokovano uglavnom Gram-negativnim mikroorganizmima. Dezorganizacija i uklanjanje subgingivalnog biofilma je primarni cilj kauzalne terapije parodontopatije. Cilj ovog rada bio je da se procene klinički i mikrobiološki efekti kauzalne terapije kod osoba s parodontopatijom. Metode rada U studiju je uključeno 40 ispitanika s hroničnom parodontopatijom kod kojih je primenjena kauzalna terapija. Klinički parametri (plak-indeks, gingivalni indeks, indeks krvarenja iz papile, dubina parodontalnih džepova i nivo pripojnog epitela) beleženi su na početku istraživanja, kao i mesec dana i tri meseca posle terapije, dok su uzorci subgingivalnog dentalnog plaka analizirani na početku i tri meseca posle terapije. Za izolovanje Porphyromonas gingivalis, Tannerella forsythensis, Prevotella intermedia i Aggregatibacter actinomycetemcomitans korišćena je multipleks tehnika reakcije lančanog umnožavanja DNK (engl. polymerase chain reaction - PCR). Rezultati Rezultati istraživanja pokazuju statistički značajno smanjenje svih ispitivanih kliničkih parametara posle terapije. Prevalencija A. actinomycetemcomitans statistički je značajno smanjena za 22,5% posle lečenja. Prevalencija P. gingivalis, T. forsythensis i P. intermedia takođe je smanjena nakon terapije, ali to smanjenje nije bilo statistički značajno. Zaključak Rezultati ove studije pokazuju da se kauzalnom terapijom parodontopatije postižu znatna klinička i mikrobiološka poboljšanja. .

Influence of gender, age and number of prostheses to the adaptation to a complete denture

AB  - Objectives: In reconstructive prosthetic dentistry adaptation refers to a process of adjusting to a complete denture and the reactions happening during the process. The prosthodontist has the task to make the period of adaptation as tolerable and short as possible for the patient. The aim of this paper was to research into the possible influence of gender, age and the number of complete dentures on the process of adaptation within edentulous patients, observed and measured against the number of follow-up appointments. Methods: The research included the total of 139 edentulous patients, of both genders, from 29 to 87 years of age. Patients who came to the Dentistry Clinic of Vojvodina had complete dentures done as a part of prosthetic rehabilitation. Gathered information has been processed by using the. chi(2) test with the level of statistical significance p lt 0.01. Results: After the prosthetic rehabilitation almost half of the patients (49.6%) have never appeared for the follow-up appointment. Closer analysis of the relation between the number of appointments and the gender of patients did not determine any significant difference in statistics regarding the process of adaptation of complete denture. The analysis of the data indicates that there is no significant statistical connection between the age of patients and the number of appointments Conclusions: The gender and age of patients, as well as the information if the patient has one or two complete dentures, do not have any influence on the adaptation of edentulous patients to complete dentures. Having in mind the constraints of this study, the number of follow-up appointments may be used as one of the indicators of the process of adaptation to complete dentures.

AB  - Long term use of intraoral optical scanning methods in the production of fixed dental restorations is one of the recognizable characteristics of Cerec® 3D CAD/CAM system. The aim of this study was to determine the degree of precision of ceramic crowns made by the technique of intraoral optical Cerec®3D scanning system. Material and methods The crowns prepared for the experimental group were scanned by the intraoral optical Cerec® 3D scanning system. In the control group, the porcelain-fused-to-metal crowns were made according to the conventional methodology. The crown accuracy was examined by measuring the size of the marginal gap between the crown and the marginal adaptation zone using the scanning microscope. Discussion The results showed that there was a difference in the accuracy between crowns of the experimental (50.27±31.50 μ) and the control group (132.92±53.21 μ). Conclusion The crowns made using the methods with intraoral optical scanning Cerec 3D system showed a high and clinically acceptable degree of accuracy.
AB  - Dugogodišnja primena metoda intraoralnog optičkog skeniranja u postupku izrade fiksnih zubnih nadoknada jedna je od osobenosti po kojoj je prepoznatljiv sistem Cerec®3D CAD/CAM. Cilj rada je bio da se utvrdi stepen preciznosti keramičkih kruna izrađenih primenom tehnike intraoralnog optičkog skeniranja sistema Cerec®3D. Ispitivanje je sprovedeno u okviru eksperimentalne grupe kruna u čijoj je izradi primenjena tehnika intraoralnog optičkog skeniranja sistema Cerec®3D. Metalokeramičke krune izrađene konvencionalnom metodologijom bile su kontrolna grupa. Preciznost kruna ispitivana je merenjem veličine marginalnog zjapa između ruba krune i demarkacije preparacije pomoću skenirajućeg elektronskog mikroskopa. Rezultati istraživanja pokazuju da postoji razlika u preciznosti kruna eksperimentalne (50,27±31,50μ) i kontrolne grupe (132,92+53,21μ). Krune izrađene primenom metoda intraoralnog optičkog skeniranja sistema Cerec 3D pokazuju visok i klinički prihvatljiv stepen preciznosti.

Prevalence of oral herpes simplex virus reactivation in cancer patients: a comparison of different techniques of viral detection

AB  - Oral reactivation of latent Herpes simplex virus (HSV) infection may easily occur in cancer patients. Virus reactivation can cause oral mucosa damage, worsen already existing lesions caused by stomatotoxic effect of cancer therapy and, whether symptomatic or asymptomatic, ample spreading and promote viral transmission. Polymerase chain reaction (PCR), cell-culture and direct immunofluorescence have been used to determine the frequency of oral HSV reactivation in 60 patients undergoing chemotherapy for different malignancies. By means of PCR, the presence of viral DNA was detected in 71.7% of patients prior to chemotherapy and in 85.0% after chemotherapy. 33.3% of patients before and 40.0% after chemotherapy were viral-culture positive, while 3.3% of patients before and 11.7% after chemotherapy were positive as shown by direct immunofluorescence. No significant difference in HSV-1 reactivation was found before and after chemotherapy. In addition, no significant difference was found when comparing HSV-1 reactivation in patients with and without mucositis. HSV-2 was not detected in any of the patients. Reactivation of latent HSV is exceptionally frequent in cancer patients. The results of this study suggest that virus reactivation occurs independently of cancer chemotherapy. The potential role of HSV reactivation in oral mucosa damage remains unclear.

Cephalometric indicators of the vertical dimension of occlusion

AB  - The aim of this investigation was to establish precise indicators of the vertical dimension of occlusion (VDO) which could be used as objective parameters in prosthodontic treatment providing exact control of the reconstructed vertical dimension of occlusion, early detection of errors and correction of the vertical dimension of occlusion during complete denture manufacturing. A total of 60 lateral cephalometric radiographs of subjects with natural dentition and class I skeletal jaw relationship, of Serbian nationality from the region of Vojvodina, were included in the investigation. Thirty subjects were males, and thirty females, their age range was 20 to 29 years. Cephalometric analysis was performed by using "Dr. Ceph" computer software (FYI Technologies, GA, USA). By evaluation of the craniofacial complex in subjects with natural dentition, horizontal dento/acial developmental growth was established in the examined sample and it was based on the relationship between the posterior and anterior total facial height (S-Go:N-Me, female (X) over bar = 68.96%, male(X) over bar = 72.8%) and statistically significant differences were found between the sexes in almost all linear dimensions and in the relation between the posterior and the anterior total facial height. Highly significant differences (p  lt  0.001) between the sexes were found in regard to anterior total facial height (N-Me, male lt (X)over bar> = 120.75mm, female(X) over bar = 112.72mm) and the anterior lower facial height (ANS-Me, male(X) over bar = 67.87mm, female(X) over bar =61.50mm) (p  lt  0.05). The proportions of the upper and lower anterior facial heights (N-ANS.-ANS-Me, male lt (X)over bar>=79.36%, female(X) over bar =80.6301o) were within normal values in both sexes. Facial harmony existed both in men and women. Cephalometric study of the subjects with complete natural dentition included in this study provided important and specific parameters for optimal reconstruction of vertical dimension of occlusion in the treatment of edentulous patients.
