Neurolytic effects of ampicillin on the rat infraorbital nerve

AB  - PURPOSE: The aim of this study was to investigate the histomorphological changes of the infraorbital nerve of rats treated with ampicillin. MATERIALS AND METHODS: The infraorbital nerve was approached through the infraorbital foramen, and 0.01 ml of ampicillin dissolved in distilled water was injected taking care not to damage the nerve. Saline solution was used in control animals. Nerves were dissected and after routine histology processing analysed by light microscopy. RESULTS: Cross-section of the nerve treated with ampicillin showed damaged axons with disintegration of heavily myelinated fibres, while thinly myelinated fibres remain unaffected. In the saline group, no damage was observed. The signs of regeneration of the damaged infraorbital nerves were detected on the fourth postoperative week. CONCLUSION: Ampicillin can cause peripheral nerve damage when injected perineurally. J Oral Pathol Med (2012) 41: 268-271

AB  - Purpose: This study was intended to investigate, through resonance frequency analysis (RFA), the stability of implants placed in an early loading protocol via the osteotome sinus floor elevation technique in the posterior maxilla. All implants featured a sandblasted/acid-etched active surface. Materials and Methods: An early loading protocol was considered for patients in whom implants with a sandblasted, large-grit/acid-etched active surface (SLActive) were placed in the posterior maxilla by the osteotome sinus floor elevation technique. Implant stability, which was measured by RFA at surgery and at weekly intervals for the following 6 weeks, was used as the most significant inclusion criterion. At the end of the stability observation period, only implants with a stability quotient of 65 or higher were loaded. Implants were followed for 2 years. Results: Twenty-seven patients received 42 implants, 40 of which were subjected to an early loading protocol 6 weeks after placement. Two years after loading, all 40 implants were surviving, without clinically or radiographically detectible complications. Conclusion: An early loading protocol can be used for SLActive implants placed in the posterior maxilla via the osteotome sinus floor elevation technique if their stability is confirmed by RFA. Anatomical study of the pterygopalatine fossa pertinent to the maxillary nerve block at the foramen rotundum

AB  - The anatomy of the pterygopalatine fossa pertinent to the technique of maxillary nerve block at the foramen rotundum was investigated and the ability of inexperienced surgeons to apply the required angles of the injection needle to the sagittal plane in a clinical environment. In 85 dried human skulls the volume, length, width and depth of 159 intact pterygopalatine fossae were measured. The frequency of reaching the sphenopalatine foramen using a 20 G spinal needle advanced from the frontozygomatic angle through the pterygomaxillary fissure was determined. 49 oral surgery postgraduates aligned the injection needle with angles of 60 degrees and 80 degrees to the sagittal plane of a volunteer's head. The dimensions of the pterygopalatine fossa were inconsistent; volume (0.1-1 cm(3)), width (1-9 mm) and depth (6-22 mm) showed the greatest variations. An enlarged sphenoidal process and a narrow pterygomaxillary fissure ( lt 2 mm) were found in 15% and 8%, respectively. The sphenopalatine foramen was reached successfully in 75%. Postgraduates in oral surgery were highly accurate in the assessment of the 60 and 80 angles to the sagittal plane. A previously described technique of blocking the maxillary nerve at the foramen rotundum was adjusted and recommendations given to overcome anatomical obstacles.

AB  - Background: Presence of the maxillary sinus and low bone density in this area often could create a problem for prosthetic rehabilitation with dental implants. Sinus floor augmentation technique can successfully increase dimensions of the posterior maxilla for implant placement. Objective: To assess quality of newly formed bone for implant insertion after augmentation of the floor of the maxillary sinus using Digora for Windows computer programme. Materials and Methods: 30 patients with indications for sinus lift procedure were involved in this clinical study. Bone density was analyzed by Digora for Windows computer programme. Results: 16 patients completed this clinical study with preoperative and postoperative orthopantomographs. Conclusion: Cases with sufficient density and bone volume in the posterior maxilla require sinus lift technique with adequate bone graft for implant insertion. This is confirmed by pre- and post-operative analysis of radiographic images in Digora for Windows programme.

Evidence that ampicillin and streptomycin can suppress autotomic behavior in rats

AB  - Autotomy is a phenomenon of deafferentation of the rat limb by transection of the sciatic and saphenous nerves, inducing an automutilation of the limb as a response to painful stimuli generated from the amputation neuroma. Autotomy has been proposed as an experimental model for neuralgia pain. The present study was designed to investigate the influence of streptomycin, ampicillin and chloramphenicol applied either in the vicinity of the nerve stump or intramuscularly. The course of autotomy was followed during 8 weeks using the scoring system of Wall et al. (1979). It was shown that streptomycin and ampicillin can suppress autotomy when applied topically whereas systemic use of the investigated antibiotics was efficient only when streptomycin was injected.
AB  - Eksperiment je izveden na 60 Albino Wistar pacova kod kojih je izvršeno presecanje n. ischiadicus-a i n. saphenus-a što je za posledicu imalo pojavu fenomena autotomije. Ovaj fenomen predstavlja eksperimentalni model bola, a ispoljava se samoodgrizanjem delova denervisanog stopala eksperimentalne životinje. Većina autora smatra da supstance i lekovi koji sprečavaju autotomiju utiču i na supresiju bola. Imajući ovo u vidu, cilj naših istraživanja bio je da ispitivane antibiotike primenimo na eksperimentalnom modelu posle ispoljavanja fenomena autotomije, kako bi utvrdili njihovo dejstvo na supresiju ovog fenomena. U isto vreme moguće je razmatrati i eventualno korišćenje ovih lekova u terapiji bola. Rezultati naših istraživanja pokazuju da streptomicin primenjen lokalno i intramuskularno sprečava autotomno ponašanje kod pacova, dok ampicilin sprečava razvoj ovog fenomena primenjen lokalno. Mehanizam delovanja ovih antibiotika još uvek nije u potpunosti razjašnjen, ali to ne umanjuje njihovu eventualnu primenu u terapiji hroničnog bola.

Assessment of available bone in the region of the mental foramen for insertion of dental implants

AB  - The aim of this study was to investigate the quantity of the residual bone above the mental foramen/canal relevant to insertion of dental implants. Measurements were performed on 39 dried mandibles that were sectioned perpendicular through the mental foramen. The thickness of bone was measured from the alveolar crest to the mental foramen and the mental canal, as well as from the mental canal to the lingual cortex. Mean distance between the alveolar crest and the mental foramen was 14.79 mm, ranging from 5.3 mm to 21.2 mm. The bony height measured between the alveolar crest and the mental canal was 17.34 mm, ranging from 6.67 mm to 24.43 mm. The mean difference was 2.99 mm, ranging from 0.5 to 6.33 mm. Bone thickness measured between the mental canal and the lingual cortex was 5.43 mm, ranging from 2.43 mm to 8.97 mm. It is concluded that the distance between the alveolar crest and the mental foramen can be used as a reliable guide for the length of an implant to be inserted in the region of the mental foramen. Such an implant can be 2 mm longer than this distance.

AB  - The aim of this study was to improve the injection technique when blocking the mandibular nerve at the oval foramen by using consistent landmarks. The lower lateral orbital angle (LLOA) and the junction of the external acoustic canal and the mastoid process (AM) proved to be reliable landmarks. Distances between landmarks for the localization of the puncture point (PP), cranial indices as well as the angle of the injection needle to the sagittal plane were calculated on 22 human dried skulls. The PP was found to lie exactly in the middle between the LLOA and AM. The angle of the injection needle to the sagittal plane was 71.86degrees. No correlation was found between the cranial index and the angulation of the needle or the distances for the localization of the PP. From this study a modified puncture technique was developed.

AB  - Kazanjian's vestibuloplasty was modified in order to eliminate secondary epithelization of the labial donor site of the mucosal flap. The crestally based mucosal flap was stretched and subperiosteal stay sutures placed at the bottom of the future sulcus leaving a 5 mm wide free mucosal margin. This mucosal strip was advanced and its periphery sutured to the free edge of the labial incision. This technique created U- shaped bottom of the sulcus and the scar that was placed out of denture borders. By using this modification satisfactory results were achieved in 15 of 16 patients.

In both patients, globadonts replacing molars, normal incisors and missing canines and premolars were found together with a high frequency hearing loss.
