AB  - Specific cell subpopulations identified as cancer stem cells (CSCs) can be found in basal cell carcinoma (BCC).
Generally, CSCs have a marked trans-differentiation potential that could potentially be used in differentiation therapies.
However, there are no studies regarding BCC CSCs multipotency. The aim of the study was to analyze the characteristic of
CSCs of BCC with emphasis on their differentiation potential upon specific induction. Specific staining and cell
morphology were used for differentiation confirmation, along with the expression analysis of osteogenic (ALP, BSP,
Runx2, OCN, BMP2), chondrogenic (COL1 and COL2A1), adipogenic (PPAR-γ) and neurogenic (Nestin and MAP2)
markers. BCC CSCs differentiated into osteogenic and chondrogenic lineages, as judged by staining and high
expression of specific markers (from 2-to 92-fold higher upon induction). Concomitantly with differentiation, the
levels of cancer stem cell markers decreased in the cultures. Adipo-differentiation and neuro-differentiation were
unsuccessful. In conclusion, BCC CSCs exhibit the capacity to trans-differentiate, a characteristic that may potentially
be useful in the development of new strategies for the treatment of aggressive BCCs.

Medication-Related Osteonecrosis of the Jaws: Two Center Retrospective Cohort Studies

AB  - This retrospective cohort study aims to describe characteristics of patients with MRONJ, to identify factors associated with MRONJ development, and to examine variables associated with favourable outcome. Totally 32 patients were followed and observed: 21 females and 11 males, in the age range 35-84 in the period from 2009 to 2018. Clinical, radiological examination (Orthopantomograph and CBCT) and biopsy were performed in order to achieve diagnosis. Demographic and clinical variables were taken into consideration: sex, age, primary disease, medication type, mode of delivery, anatomic location, drug treatment duration, timing of tooth extraction, chemotherapy, presence of bone metastasis, aetiology of MRONJ, disease stage, and treatment modality. MRONJ developed under osteoporosis and malignant disease in 11 and 21 patients, respectively. MRONJ development was triggered by tooth extraction or trauma in 30 out of 32 cases, whereas the two patients developed MRONJ spontaneously. Stages I, II, and III were confirmed in 5 (16%), 18 (58%), and 9 (28%) patients, respectively. Mandible was affected in 23 (72%) patients. MRONJ was treated in our department by conservative and surgical modality. In this study we found that 65% of all patients were classified in the cured/improvement group and 35% in the stable/progression group. The female gender, osteoporosis as primary disease, oral regime intake, shorter period on BPs, earlier stage of disease, and specific anatomic localisation (frontal and premolar maxilla) were factors associated with better response to therapy and favourable clinical outcome. Comprehensive treatment protocol and further randomized studies are necessary for further improvements.

Predictive Value of Opto-magnetic Imaging Spectroscopy in Discriminating Oral Squamous Cell Carcinoma from Non-tumor Tissue in Surgical Margins

AB  - PurposeNegative surgical margins status is of extreme importance for better prognosis and lower recurrence rate in patients undergoing surgical treatment for oral squamous cell carcinoma (OSCC). Standard histopathology is time consuming and current optical procedures come with various limitations. We examined the potential of a new imaging technique-opto-magnetic imaging spectroscopy (OMIS) in discriminating tumor (OSCC) from adjacent non-tumor oral cavity tissue.MethodsA total of 46 samples from 21 patients were included in this study. Samples were taken from a fresh surgical specimen of primary OSCC. Digital imaging of samples was performed within 60 min of resection using OMIS device. Images were processed through a specifically designed convolution algorithm based on light-matter interaction. As a result a convolution spectrum was generated. Predictive value of spectral data was assessed using correlation tests and Naive Bayes classification model.ResultsThere was no significant correlation in distribution of characteristic peaks between tumor and non-tumor tissue (P>0.05). Tumor tissue showed more magnetic activity compared to non-tumor tissue. Naive Bayes classifier with kernel density estimation discriminated tumor from non-tumor tissue with the accuracy of 82.61%, 86.96% sensitivity, 78.26% specificity and the AUC value of 0.917.ConclusionOMIS seems to be a promising optical method for ex vivo characterization of OSCC and non-tumor tissue. Further investigation is necessary to determine how tissue type and level of pathological transformation impact OMIS results. Ultimately, this could aid surgeons in using this method as an in vivo indicator for surgical resection with safe margins.

AB  - Purpose: Fractures of the zygomaticomaxillary complex (ZMC) are common injuries that may lead to loss of an aesthetically pleasing appearance and functional impairment. The aim of this study was to analyze the demographics, causes, characteristics, and outcomes of zygomatic fractures managed at several European departments of oral and maxillofacial surgery. Materials and methods: This study is based on a multicenter systematic database that allowed the recording of all patients with ZMC fractures between 1 January 2013 and 31 December 2017. The following data were recorded: gender, age, personal medical history, etiology, side of zygomatic fracture, classification of ZMC fracture, associated maxillofacial fractures, symptoms at diagnosis, type of performed treatment, and sequelae/complications. Results: A total of 1406 patients (1172 males, 234 females) were included in the study. Statistically significant correlations were found between assault-related ZMC fractures and the A3 class (p  lt  .0000005) and between Infraorbital Nerve (ION) anesthesia and B class (p  lt  .00000005). Conclusion: The most frequent cause of ZMC fractures was assault, followed by falls. The most frequently involved decade of age was between 20 and 29 years. The decision and type of surgical treatment of ZMC fractures depends on several issues that need to be considered on a case by case basis.

AB  - Objectives. The purpose of this European multicenter prospective study was to obtain more precise information about the demographic characteristics and etiologic/epidemiologic patterns of motor vehicle accidents (MVA)-related maxillofacial fractures. Study Design. Of the 3260 patients with maxillofacial fractures admitted within the study period, 326 traumas were caused by MVAs with a male/female ratio of 2.2:1. Results. The maximum incidence was found in Zagreb (Croatia) (18%) and the minimum in Bergen (Norway) (0%). The most frequent mechanisms were car accidents, with 177 cases, followed by motorcycle accidents. The most frequently observed fracture involved the mandible, with 199 fractures, followed by maxillo-zygomatic-orbital (MZO) fractures. Conclusions. In all the 3 groups (car, motorcycle, and pedestrian), mandibular and MZO fractures were the 2 most frequently observed fractures, with some variations. The importance of analyzing MVA-related facial injuries and their features and characteristics should be stressed.

AB  - Purpose: Recent evidence suggests that small subpopulations of stem-like cells are accountable for tumour initiation, progression and metastasis. Until now, studies were focused exclusively on the characterization of these cell populations within the tumour itself, while tumour margins were neglected, although it is known that the histological and molecular status of tumour margins may play a significant role in the course of the disease. Therefore, the aims of the study were to isolate cells from oral squamous cell carcinomas and their respective margins, to characterize these cells using specific markers, to assess their self-renewal potential and determine their chemoresistance. Methods: Cell cultures were obtained from 12 tissue specimens (6 tumours and 6 margins). Total RNA was extracted and gene expression analysis was done by real-time PCR (RTPCR). Flow cytometry, immunocytometry, sphere formation and MTT assays were also applied. Results: With minor differences, cells originating from both tumours and tumour margins showed the presence of stem cell markers CD133, Nanog, Sox2, CD44, and Oct4, had the capacity to form spheroids and showed chemoresistance. Conclusions: Subpopulations of margin cells appeared to have sternness properties which might raise the question of re-evaluation of optimal surgical management.

AB  - BackgroundUnderstanding the pathogenesis of basal cell carcinoma (BCC) and identifying the cells responsible for propagation and recurrence are crucial for the development of new treatment strategies. The aim of this study was to characterize the cells isolated from BCC and its margin. MethodsPrimary cultures were established from 10 BCCs, their respective close resection margins (3mm) and 10 control tissues. Stem cell markers analysis was carried out by real-time PCR and/or flow cytometry. Spheroid formation and MTT assays were also performed. ResultsReal-time PCR showed a higher expression of embryonic (Oct4, Sox2 and Nanog) and mesenchymal (CD44 and CD73) stem cell markers in tumors compared to margins and controls (P lt 0.05). Bmi-1 and GPR49 were also upregulated in tumors in comparison with margins. Both tumor and margin cells, but not normal, had the capacity to form spheroids. During passages, the number of spheres increased, while the diameter decreased. Tumor cells showed higher chemo-resistance compared to margin and control cells. ConclusionsBasal cell carcinomas expressed stem cell markers, pointing to the existence of a cancer cell side population with stemness characteristics. Margin also appeared to harbour a small number of cancer-initiating cells.

AB  - This study examined the potential of a new porous calcium hydroxyapatite scaffold covered with poly (lactide-co-glycolide) (PLGA) as a bone substitute, identifying its advantages over Geistlich Bio-Oss (R), considered the gold standard, in in vivo biofunctionality investigations. Structural and morphological properties of the new scaffold were analyzed by scanning electron and atomic force microscopy. The biofunctionality assays were performed on New Zealand white rabbits using new scaffold for filling full-thickness defects of critical size. The evaluated parameters were: the presence of macrophages, giant cells, monoocytes, plasma cells, granulocytes, neo-angiogenesis, fibroplasia, and the percentage of mineralization. Parallel biofunctionality assays were performed using Geistlich Bio-Oss (R). The appearance of bone defects 12 weeks after the new scaffold implantation showed the presence of a small number of typical immune response cells. Furthermore, significantly reduced number of capillary buds, low intensity of fibroplasia and high degree of mineralization in a lamellar pattern indicated that the inflammation process has been almost completely overcome and that the new bone formed was in the final phase of remodeling. All biofunctionality assays proved the new scaffold's suitability as a bone substitute for applications in maxillofacial surgery. It showed numerous biological advantages over Geistlich Bio-Oss (R) which was reflected mainly as a lower number of giant cells surrounding implanted material and higher degree of mineralization in new formed bone.

AB  - A synthetic bone substitute based on calcium hydroxyapatite (CHA) and poly(lactic-co-glycolic) acid (PLGA), described in this paper, was synthesized to fulfill specific requirements like biodegradability, satisfying mechanical properties, optimal porosity and nanotopology, osteoinductive and osteoconductive properties, and so forth. Structural and morphological properties of the new scaffold were analyzed by micro computed tomography and scanning electron microscopy, while its physicochemical properties were investigated by X-ray diffraction and infrared spectroscopy. In vivo biological investigations of the synthesized scaffold were conducted over the cutaneous irritation and biofunctionality assays on rabbits and the test of acute systemic toxicity on mice. The results showed that the scaffold is not irritant and that it does not exhibit any symptoms of acute toxicity. Biofunctionality assays which include evaluation of the presence of various cells of immune response, the presence of neoangiogenesis, percentage of mineralization of newly formed bone, and fibroplasia in the tissue indicated that the new scaffold is suitable for the application in maxillofacial and dental surgery as a bone substitute. Also, it showed significant advantages over commercial product Geistlich Bio-Oss(R) from the aspect of some parameters of immunological response.

AB  - Introduction: Biodegradable porous scaffolds are found to be very promising bone substitutes, acting as a temporary physical support to guide new tissue regeneration, until the entire scaffold is totally degraded and replaced by the new tissue. Objective The aim of this study was to investigate cytotoxicity of a synthesized calcium hydroxyapatite-based scaffold, named ALBO-OS, with high porosity and optimal topology. Methods The ALBO-OS scaffold was synthesized by the method of polymer foam template. The analysis of pore geometry and scaffold walls' topography was made by scanning electron microscope (SEM). The biological investigations assumed the examinations of ALBO-OS cytotoxicity to mouse L929 fibroblasts, using 3-(4,5-Dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromidefor (MTT) and lactate dehydrogenase (LDH) tests and inverse phase microscopy. Results The SEM analysis showed high porosity with fair pore distribution and interesting morphology from the biological standpoint. The biological investigations showed that the material is not cytotoxic to L929 cells. Comparison of ALBO-OS with Bio-Oss, as the global gold standard as a bone substitute, showed similar results in MTT test, while LDH test showed significantly higher rate of cell multiplication with ALBO-OS. Conclusion The scaffold design from the aspect of pore size, distribution, and topology seems to be very convenient for cell adhesion and occupation, which makes it a promising material as a bone substitute. The results of biological assays proved that ALBO-OS is not cytotoxic for L929 fibroblasts. In comparison with Bio-Oss, similar or even better results were obtained.
AB  - Uvod: Porozni biodegradabilni nosači su se pokazali kao dobri zamenici kosti jer deluju kao privremena fizička potpora za usmerenu regeneraciju tkiva dok se čitav nosač potpuno ne razgradi i zameni novim tkivom. Cilj rada Cilj ovog rada je bio da se ispita citotoksičnost nosača tkiva na bazi kalcijum-hidroksiapatita, visoke poroznosti i optimalne topologije, nazvanog ALBO-OS. Metode rada Zamenik kosti ALBO-OS je sintetisan metodom matrice napravljene od polimerne pene. Geometrija pora i zidova nosača analizirane su pomoću skenirajuće elektronske mikroskopije (SEM). Biološka istraživanja izvedena su ispitivanjem citotoksičnosti ALBO-OS-a na mišjim fibroblastima L929 pomoću MTT i LDH testova i fazno kontrastne mikroskopije. Rezultati SEM analiza je pokazala veliku i ravnomernu poroznost i zanimljivu morfologiju sa biološkog stanovišta. Biološka istraživanja pokazala su da materijal nije citotoksičan. Poređenjem materijala ALBO-OS i Bio-Oss, koji je, globalno, zlatni standard među zamenicima kosti, dobijeni su slični rezultati na MTT testu, dok su rezulati LDH testa pokazali značajno veći broj ćelijskih deoba u kontaktu sa ALBO-OS-om. Zaključak Dizajn nosača sa stanovišta raspodele veličine pora i topologije je veoma pogodan za adheziju i naseljavanje ćelija, zbog čega ima veliki potencijal kao zamenik kosti. Rezultati bioloških testova su pokazali da ALBO-OS nije citotoksičan za L929 fibroblaste. U poređenju sa materijalom Bio-Oss, dobijeni su slični ili bolji rezultati.

Changes in miR-221/222 Levels in Invasive and In Situ Carcinomas of the Breast: Differences in Association with Estrogen Receptor and TIMP3 Expression Levels

AB  - Breast cancer (BC) is a heterogeneous group of diseases that still represents a major cause of death in the female population. MicroRNAs (miRNAs, miRs), such as miR-221 and miR-222, have been shown to be involved in BC pathology by acting via its target genes such as tissue inhibitor of metalloproteinase 3 (TIMP3). The main goals of this study were to find differences in miR-221/222 levels of expression in BC groups based on invasiveness, and to investigate the association with estrogen receptor (ER), TIMP3 messenger RNA (mRNA) levels, and clinicopathological characteristics of patients and tumors. In this study, we measured levels of miR-221/222 in 63 breast tissue samples by quantitative reverse transcription-polymerase chain reaction (qRT-PCR) using TaqMan(A (R)) technology and immunohistochemistry. miR-221/222 levels varied significantly across groups based on invasiveness (P  lt  0.001). In in situ tumors, miR-221 and miR-222 were negatively associated with ER (P = 0.001, r = -0.714, and P = 0.013, r = -0.585, respectively). In invasive breast carcinomas associated with non-invasive tumors, miR-222 was inversely associated with ER (P = 0.039, r = -0.620). Pure invasive BCs showed a positive correlation of miR-221 and miR-222 with TIMP3 mRNA levels (P = 0.008, r = 0.508, and P = 0.010, r = 0.497, respectively). An increase in miR-221/222 might be an important event for in situ carcinoma formation, and miR-221/222 may be important molecules that highlight potential differences between invasive breast carcinomas associated with non-invasive and pure invasive BCs.

Radiographic cephalometry analysis of condylar position after bimaxillary osteotomy in patients with mandibular prognathism

AB  - Background/Aim. Postoperative condylar position is a substantial concern in surgical correction of mandibular prognathism. Orthognathic surgery may change condylar position and this is considered a contributing factor for early skeletal relapse and the induction of temporomandibular disorders. The purpose of this study was to evaluate changes in condylar position, and to correlate angular skeletal measurements following bimaxillary surgery. Methods. On profile teleradiographs of 21 patients with mandibular angular and linear parametres, the changes in condylar position, were measured during preoperative orthodontic treatment and 6 months after the surgical treatment. Results. A statistically significant difference in values between the groups was found. The most distal point on the head of condyle point (DI) moved backward for 1.38 mm (p = 0.02), and the point of center of collum mandibulae point (DC) moved backward for 1.52 mm (p = 0.007). The amount of upward movement of the point DI was 1.62 mm (p = 0.04). Conclusion. In the patients with mandibular prognathism, the condyles tend to migrate upward and forward six months after bimaxillary surgery.
AB  - Uvod/Cilj. Postoperativna pozicija kondila je značajna za hiruršku korekciju mandibularnog prognatizma. Ortognatska hirurgija može da promeni poziciju kondila, a to može biti jedan od faktora koji doprinosi ranom skeletnom recidivu i pojavi temporomandibularnih disfunkcija. Zbog toga je cilj ove studije bio da proceni promene pozicije kondila kao i da ne korelišu promene pozicije kondila sa angularnim skeletnim promenama nakon bimaksilarne hirurgije. Metode. Na telerendgenskim snimcima 21 bolesnika sa mandibularnim prognatizmom mereni su angularni i linearni parametri koji opisuju promene u položaju kondila, pre ortodontske pripreme i šest meseci nakon hirurške korekcije. Rezultati. Ustanovljena je statistička značajnost razlika u vrednosti parametara između grupa. Tačka DI - najdistalnija tačka na glavi kondila, pomerila se unazad 1,38 mm (p = 0,02), a tačka DC - tačka koja označava centar collum mandibulae, pomerila se, takođe, unazad za 1,52 mm (p = 0,007). Vrednost pomeranja tačke DI naviše bila je 1,62 mm (p = 0,04). Zaključak. Kod bolesnika sa mandibularnim prognatizmom, kondili su težili da migriraju unapred i naviše šest meseci nakon bimaksilarne operacije.

Zbog toga je cilj ove studije bio da proceni promene pozicije kondila kao i da ne korelišu promene pozicije kondila sa angularnim skeletnim promenama nakon bimaksilarne hirurgije. Metode. Na telerendgenskim snimcima 21 bolesnika sa mandibularnim prognatizmom mereni su angularni i linearni parametri koji opisuju promene u položaju kondila, pre ortodontske pripreme i šest meseci nakon hirurške korekcije. Rezultati. Ustanovljena je statistička značajnost razlika u vrednosti parametara između grupa. Tačka DI - najdistalnija tačka na glavi kondila, pomerila se unazad 1,38 mm (p = 0,02), a tačka DC - tačka koja označava centar collum mandibulae, pomerila se, takođe, unazad za 1,52 mm (p = 0,007). Vrednost pomeranja tačke DI naviše bila je 1,62 mm (p = 0,04). Zaključak. Kod bolesnika sa mandibularnim prognatizmom, kondili su težili da migriraju unapred i naviše šest meseci nakon bimaksilarne operacije.",

AB  - The purpose of this study was to analyse the demographics, causes and characteristics of maxillofacial fractures managed at several European departments of oral and maxillofacial surgery over one year. The following data were recorded: gender, age, aetiology, site of facial fractures, facial injury severity score, timing of intervention, length of hospital stay. Data for a total of 3396 patients (2655 males and 741 females) with 4155 fractures were recorded. The mean age differed from country to country, ranging between 29.9 and 43.9 years. Overall, the most frequent cause of injury was assault, which accounted for the injuries of 1309 patients; assaults and falls alternated as the most important aetiological factor in the various centres. The most frequently observed fracture involved the mandible with 1743 fractures, followed by orbital-zygomatic-maxillary (OZM) fractures. Condylar fractures were the most commonly observed mandibular fracture. The results of the EURMAT collaboration confirm the changing trend in maxillofacial trauma epidemiology in Europe, with trauma cases caused by assaults and falls now outnumbering those due to road traffic accidents. The progressive ageing of the European population, in addition to strict road and work legislation may have been responsible for this change. Men are still the most frequent victims of maxillofacial injuries.

The aim of this study is to present and discuss the demographic characteristics and patterns of assault-related maxillofacial fractures as reported by a European multicenter prospective study. Study Design. Demographic and injury data were recorded for each patient who was a victim of an assault. Results. Assaults represented the most frequent etiology of maxillofacial trauma with an overall rate of 39% and the values ranging between 60.8% (Kiev, Ukraine) and 15.4% (Bergen, Norway). The most frequent mechanisms of assault-related maxillofacial fractures were fists in 730 cases, followed by kicks and fists. The most frequently observed fracture involved the mandible (814 fractures), followed by orbito-zygomatic-maxillary complex fractures and orbital fractures. Conclusions. Our data confirmed the strong possibility that patients with maxillofacial fractures may be victims of physical aggression. The crucial role of alcohol in assault-related fractures was also confirmed by our study.

AB  - Objective. The aim of this study is to present and discuss the results of a European multicentre prospective study about pediatric maxillofacial trauma epidemiology during a year. Study Design. The following data were recorded: gender, age, etiology, site of fracture, date of injury. Of the 3396 patients with maxillofacial fractures admitted within the study period, 114 (3.3%) were children aged 15 years and younger, with a male/female ratio of 2.6:1. Mean age was 10.9 years. Most patients (63%) were aged 11-15 years. Results. The most frequent cause of injury was fall (36 patients). Sport injuries and assaults were almost limited to the oldest group, whereas falls were more uniformly distributed in the 3 groups. The most frequently observed fracture involved the mandible with 47 fractures. In particular, 18 condylar fractures were recorded, followed by 12 body fractures. Conclusions. Falls can be acknowledged as the most important cause of facial trauma during the first years of life. The high incidence of sport accidents after 10 years may be a reason to increase the use of mouthguards and other protective equipment. Finally, the mandible (and in particular the condyle) was confirmed as the most frequent fracture site.

Of the 3396 patients with maxillofacial fractures admitted within the study period, 114 (3.3%) were children aged 15 years and younger, with a male/female ratio of 2.6:1. Mean age was 10.9 years. Most patients (63%) were aged 11-15 years. Results. The most frequent cause of injury was fall (36 patients). Sport injuries and assaults were almost limited to the oldest group, whereas falls were more uniformly distributed in the 3 groups. The most frequently observed fracture involved the mandible with 47 fractures. In particular, 18 condylar fractures were recorded, followed by 12 body fractures. Conclusions. Falls can be acknowledged as the most important cause of facial trauma during the first years of life. The high incidence of sport accidents after 10 years may be a reason to increase the use of mouthguards and other protective equipment. Finally, the mandible (and in particular the condyle) was confirmed as the most frequent fracture site.",

The structure and crystallinity of the obtained powder were investigated by X-ray diffraction, TEM and HRTEM. Malignant ameloblastoma metastasis to the neck: Radiological and pathohistological dilemma

AB  - Introduction. Ameloblastomas are odontogenic epithelial, locally invasive tumors of slow growth and mostly of benign behavior. Their frequency is low (they account for 1% of all head and neck tumors and about 11% of tumors of dental origin). Malignant variations of ameloblastoma are malignant ameloblastoma and ameloblastic carcinoma. They constitute less than 1% of all ameloblastomas. We presented a case of malignant ameloblastoma of the mandible with neck metastasis. Case report. A patient, aged 72, presented with the following symptoms: pain in the lower jaw, swelling in the left submandibular area and difficult mouth opening. The patient was admitted to the Department of Oral and Maxillofacial Surgery, Clinical Center of Montenegro, two months after he had noticed the symptoms. Panoramic radiography (OPG) showed that both jaws were partially toothless with terminal stage of periodontitis of the remaining teeth. Also, OPG showed sharply limited semicircular defect in the retromolar region and along the front edge of the mandible rami. Conventional histopathologic examination of the neck masses showed malignant ameloblastoma which contained central fields of squamous differentiation. Immunoreactivity of several markers was determined using immunohistochemical analyses. After these diagnostic methods a definite histopathology diagnosis was made: Ameloblastoma metastaticum in textus fibroadiposus regio colli (typus acanthomatosus). Conclusion. It is not possible to distinguish conventional, ie intraosseous, ameloblastoma from malignant ameloblastoma according to histopathologic features. It is necessary to pay special attention, especially in elderly patients, and to carry out further clinical, radiological and pathohistological diagnostic procedures, such as immunohistochemical analysis. A timely and correct diagnosis and treatment of malignant ameloblastoma require a multidisciplinary approach.
AB  - Uvod. Ameloblastomi su odontogeni epitelni tumori, lokalno invazivni, sporog rasta, i u većini slučajeva pokazuju benigno ponašanje. Veoma su rijetki. Njihova učestalost je 1% u grupi tumora glave i vrata, kao i 11% kod tumora koji potiču od zubnih tkiva. Maligne varijante ameloblastoma su maligni ameloblastom i ameloblastički karcinom. Oni čine manje od 1% svih ameloblastoma. U radu je prikazan bolesnik sa malignim ameloblastomom donje vilice sa metastazom na vratu. Prikaz bolesnika. Prve subjektivne tegobe bolesnika, starog 72 godine, manifestovale su se kao bolovi u predelu donje vilice, otok u podviličnom predelu sa leve strane i otežano otvaranje usta. Bolesnik je primljen u Odeljenje oralne i maksilofacijalne hirurgije Kliničkog centra Crne Gore dva meseca nakon što je primetio prve tegobe. Ortopantomografski snimak pokazao je suptotalnu bezubost obeju vilica sa terminalnim stadijumom parodontopatije na preostalim zubima. U retromolarnoj regiji i duž prednje ivice ramusa donje vilice, uočen je jasno ograničen polukružni defekt. Biopsija promene na vratu pokazala je metastazu malignog ameloblastoma, sa prisutnim centralnim poljima skvamozne diferencijacije. Imunohistohemijskom analizom određivana je imunoreaktivnost više markera. Nakon ovih dijagnostičkih metoda postavljena je definitivna patohistološka dijagnoza: Ameloblastoma metastaticum in textus fibroadiposus regio colli (typus acanthomatosus). Zaključak. Na osnovu histopatološkog nalaza nije moguće razlikovati konvencionalni, tj. intraosealni, ameloblastom od malignog ameloblastoma. Zbog toga je potrebno obratiti posebnu pažnju, naro čito kod bolesnika starijeg životnog doba, i sprovesti sve dodatne kliničke, radiološke i histopatološke, ali i imunohistohemijske dijagnostičke procedure. Za postavljanje blagovremene i tačne dijagnoze, kao i sprovođenje adekvatnog terapijskog tretmana malignog ameloblastoma, neophodan je multidisciplinarni pristup.

Self assembly of biomimetic hydroxyapatite on the surface of different polymer thin films

AB  - The role of the process of designing a scaffold in bone tissue engineering is to provide optimal conditions for new bone tissue growth. The primary concern of such engineering is to create an adequate nanotopology of the scaffold inner walls, which can initiate the growth and activity of bone cells. Here, we present a completely new scaffold designing process based on a biomimetic approach in order to improve the nanostructure of pore walls of previously-made calcium hydroxyapatite (CHA) porous scaffolds. CHA porous scaffolds were covered with different polymer thin films (alginate, cellulose and PLGA) and exposed to simulated body fluid (SBF) for 42 days. SBF induced in situ formation of "bone-like" apatite phases on the surface of CHA/polymer composites. Fourier Transformed Infrared (FTIR) spectroscopy showed that the biomimetically-assembled phase is CHA of slightly shifted stoichiometry. X-ray diffraction confirmed that CHA is self-assembled on the surface of all investigated thin films. The calculation of crystallite sizes showed small differences in the degree of crystallinity between different samples. Scanning electron microscopy revealed a dominant blow-ball morphology of CHA particles (size 1 - 5 mu m) with nano-sized branches on their surfaces.

AB  - Since the beginning of this century the bone tissue engineering has developed into one of the essential research fields in reconstructive medicine, as an alternative approach compared with the conventional bone transplants. The main objective of this kind of therapy is to repair, regenerate and reconstruct the damaged or disease-stricken tissue. Tissue engineering is based on scaffolds which are of various biomaterials capable of participating in the process of bone tissue regeneration. A great number of materials has been researched and analyzed so as to be used as a scaffold for cell seeding. Therefore, the scaffold would enable those cells to grow, proliferate and penetrate the bone defects, whereupon the new tissue would take over the structure and function of the damaged tissue. Hydroxyapatites and other calcium phosphate materials were widely used as bone replacements for more than two decades. However, the poor mechanical features of porous hydroxyapatites led to their limited use. In case when they are combined into the compound materials together with polymer/biopolymer thin films, the porous hydroxyapatites acquire optimal characteristics required by scaffolds, which can offer excellent conditions for cell infiltration, growth and activity which is necessary in bone tissue engineering. The main objective of the doctoral dissertation was to carry out the characterization, examine the biocompatibility and biofunctionality of porous hydroxyapatite created by the modified hydrothermal method (pHAP), porous hydroxyapatite created by the modified hydrothermal method in combination with PLGA (Phap + PLGA) and porous hydroxyapatite created by the modified hydrothermal method in combination with metformin (pHAP + metformin). The research included the characterization of the tested materials by means of Fourier transform infrared spectroscopy (FTIR), X-ray distraction method (XRD), atomic forse microscopy method (AFM), scanning electorn microscopy analysis(SEM) and BET method. Biocompatibility was examined by MTT test (indirect contact of the tested materials with the cell culture L 929 fibroblasts), LDH test (direct contact of the tested materilas with the cell culture L929 fribroblasts) and test of local effects on skin in rabbits. Biofunctionality of the examined materials was researched by their implatation into defects of critical size of 6mm in the region of parietal bone in rabbits. After 12 weeks the animals were sacrifised, and tissue samples were prepared for pathohistological analysis. By means of the optical microscopy the following parametres were determined both quantitatively and qualitatively: the size of the defect, the presence of giant cells, the presence of neoangiogenesis, the presence of basophils, the presence of nonspecific inflammation in the tissue, the appearance of the newly created bone, the presence of fibroplasia in the tissue and the percentage of mineralization...
AB  - Tkivno inženjerstvo koštanog tkiva se u ovom veku razvilo u jedno od glavnih polja istraživanja u regenerativnoj medicini. Ono predstavlja alternativni pristup u odnosu na konvencionalne koštane transplantate. Osnovni cilj ove vrste terapije je da popravi, regeneriše i rekonstruiše oštećeno ili bolešću zahvaćeno tkivo. Osnovu tkivnog inženjerstva čine ćelijski nosači koji predstavljaju razne vrste biomaterijala sposobnih da učestvuju u regeneraciji koštanog tkiva. Veliki broj materijala je istraživan i analiziran sa ciljem da se koristi kao ćelijski nosač na koji bi mogle da se zasejavaju ćelije. Ćelijski nosač koji bi tim ćelijama omogućio rast, proliferaciju i njihovo unošenje u defekte kosti nakon čega bi novostvoreno tkivo preuzimalo strukturu i funkciju obolelog dela tkiva. Hidroksiapatiti i drugi kalcijum fosfatni materijali imali su široku upotrebu kao koštani zamenici više od dve decenije. Zbog loših mehaničkih osobina poroznih hidroksiapatita došlo je do ograničenja njihove primene. U slučaju kada se kombinuju u kompozitne materijale sa odgovarajućim polimer/biopolimer tankim filmovima, porozni hidroksiapatiti dobijaju optimalne osobine potrebne ćelijskim nosačima, koje mogu pružiti odlične uslove za infiltraciju, rast i aktivnost ćelija neophodnih u tkivnom inženjerstvu kosti. Osnovni cilj doktorske disertacije bio je izvršiti karakterizaciju, ispitati biokompatibilnost i biofunkcionalnost poroznog hidroksiapatita dobijenog modifikovanom hidrotermalnom metodom(pHAP), poroznog hidroksiapatita dobijenog modifikovanom hidrotermalnom metodom u kombinaciji sa PLGA (pHAP+PLGA) i poroznog hidroksiapatita dobijenog modifikovanom hidrotermalnom metodom u kombinaciji sa metforminom (pHAP+metformin). Correlation between disease progression and histopathologic criterions of the lip squamous cell carcinoma

AB  - The most common malignancy of the lip is squamous cell carcinoma (SCC). In our population, according to epidemiological data, almost a half of all (45%) SCC of oral mucous tissue spreads over the lower and upper lip. The aim of this study was to estimate prognostic importance of histopathologic characteristics - histologic grade, nuclear grade and tumor size in relation to the appearance of lymph node metastases and relapse in SCC of the lip. Methods. In the retrospective- prospective study 70 cases of lower and upper lip SCC were analyzed. They were diagnosed from 2002 to 2006 in the Clinic of Maxillofacial Surgery, Clinical Center of Montenegro. The data about localization of the carcinomas, histopathologic characteristics and lymph node status were taken from medical files of the patients. The patients were followed up in a 3-year period and the disease relapse or/and metastatic disease appearance were registereds. Results. There was statistically significant difference in tumor size among the patients with and without disease relapse (p = 0.027). Logistic regression analysis showed that the tumor size is a statistically significant factor (R = 0.186; p = 0.011) for the appearance of regional lymph node metastases. Relative risk [exp (B)] for the appearance of regional lymph node metastases in relation to tumor size was 2.807. Conclusion. Histologic and nuclear grade of lip SCC are not prognostic factors for the appearance of the disease relapse and regional lymph node metastases. Tumor size is a predictive factor of the relapse appearance, as well as for lymph node metastases appearance. In clinical practice, tumor size is a factor that classifies patients with lip SCC into the groups of higher and smaller risk of relapse appearance and for lymph node metastases appearance. Our results suggest that, risk for lymph node metastases appearance increases 2.8 times with increasing of the tumor size over 2 cm in diameter.
AB  - Uvod/Cilj. Planocelularni karcinom (PCC) je najčešće maligno oboljenje usne. Prema epidemiološkim podacima, u našoj populaciji skoro polovina (45%) PCC sluzokože usne duplje zahvata i donju i gornju usnu. Cilj ovog rada bio je utvrđivanje prognostičkog značaja histopatoloških karakteristika PCC usne: histološkog gradusa, nuklearnog gradusa i veličine tumora za pojavu recidiva bolesti i metastaza u regionalnim limfnim čvorovima. Metode. U retrospektivno-prospektivnoj studiji analizirano je 70 bolesnika sa PCC donje i gornje usne, dijagnostikovanim u periodu od 2002. do 2006. godine u Klinici za maksilofacijalnu hirurgiju Kliničkog centra Crne Gore. Podaci o lokalizaciji neoplazmi, životnom dobu i polu bolesnika, kao i o histopatološkim karakteristikama neoplazmi uzeti su iz uputnika za histopatološki pregled. Tokom tri godine, na redovnim postoperativnim kliničkim kontrolama, registrovana je eventualna pojava recidiva i/ili metastatske bolesti. Rezultati. Veličina tumora se statistički značajno razlikovala između ispitanika sa i bez recidiva bolesti (p = 0,027). Logističkom regresionom analizom utvrđeno je da je veličina tumora bila statistički značajan faktor (p = 0,011, R = 0,186) za pojavu metastaza u regionalnim limfnim čvorovima. Relativni rizik [exp (B)] od pojave metastaze u regionalnim limfnim čvorovima u odnosu na veličinu tumora iznosio je 2,807. Zaključak. Veličina tumora predstavlja prognostički faktor za pojavu recidiva i regionalnih metastaza PCC usne, tj. faktor koji omogućava podelu bolesnika sa karcinomom usne u grupu sa većim, odnosno manjim rizikom od pojave recidiva i/ili metastatske bolesti. Prema našim rezultatima, sa povećanjem veličine PCC usne iznad 2 cm rizik od pojave regionalnih metastaza povećava se 2,8 puta.

The frequency and outcome of lip cancer in Serbian population

AB  - Aim: Lip cancer is one of the most common cancers of the head and neck region. Among all lip malignancies, squamous cell carcinoma is most common, whereas basal cell carcinoma accounts for only 1% of all lip carcinomas. The aim of this paper was to evaluate the frequency of lip lesions among Serbian population and to consider its outcome. Material and Method: The analysis encompassed 479 patients who were hospitalized at the Clinic of Maxillofacial Surgery (Faculty of Dentistry, University of Belgrade) during the period 1989-2005 for the treatment of lip lesions. The following parameters were analyzed: sex, age, size and localization of the lesion, duration of hospitalization, TNM classification, histopathologic analysis, type of surgical procedure, other possible treatment, and a recurrence rate. Results: There were majority of males (78%). Average age of our patients was 63.4 years. Lesions were most often localized in the lower lip (88%), and less frequently in the upper lip (8%) or at the lip angle (4%). Average size of the lesion was 24.3 mm. Data confirmed that the 'W' excision was the most frequently used surgical procedure, and for bigger lesions, surgical reconstruction was performed by the technique suggested by Karapandzic. Histopathologic analysis confirmed the most frequent incidence of the squamous cell carcinoma. Conclusion: Squamous cell carcinoma of the lip generally has a favourable prognosis. Early detection is very important for its successful treatment.

Currently, surgery and/or radiotherapy are considered the standards of care for SCC of the lip. Materials and Methods: We retrospectively analyzed medical records of patients diagnosed with SCC of the lip at the Clinic for Maxillofacial surgery at University of Belgrade (Belgrade, Serbia) during a period between 1991 and 2000. Results: A total of 223 patients with SCC of the lip were diagnosed and treated during a 10-year period. The overall male-to-female ratio was 5 to 0. The most frequently affected site was the lower lip (92.8%,). Mean diameter of the tumor was 30.1 mm (range, 5 to 80 full Neck lymph node metastases at presentation was detected in 26.5% of patients, who subsequently underwent neck dissection. We observed a linear trend in the association between the size of the tumor and the clinical stage of the neck (chi(2) = 15.1; df = 1; P  lt  .0001). Different surgical techniques were used for reconstruction of the lip defect after tumor removal. After a median follow-up of 56 months (range, 15 to 78 months), local recurrence occurred in 10.8% of patients while regional metastases developed in 4.5% of patients. Mortality from SCC of the lip was only 2.2%. Conclusion: SCC of the lip generally has a favorable outcome. Recently, there have been no major advances in lip reconstruction but rather continued improvement on accepted techniques. Early detection is essential for the successful treatment of SCC of the lip, which requires a multidisciplinary approach.

Use of the modified Fries technique in the reconstruction of the lower lip after the removal of a malignant tumor

AB  - Background/aim: Lip carcinoma makes about 1/4 of all oral carcinomas. Primary treatment of the lower lip carcinoma means radical excision of the full thickness of the lower lip tissue together with the tumor. The reconstruction of the defect accomplished after the removal of the lower lip tumor is a challenge for the surgeon and requires a detailed preoperative planning, the right choice of the method for reconstruction and the knowledge of the adequate surgical techniques needed. The aim of this study was to present a ten-year clinical results and experience concerning the reconstruction of lower lip defects longer than 4 cm by means of the modified Fries method in patients treated for the lower lip carcinoma. Methods. The reconstruction of the lower lip by means of the modified Fries method was performed in 37 patients. The following parameters were analyzed: sex, age, the time elapsed from the occurrence of the first symptoms until the first consultation with the doctor, the size of tumors, the size of the postexcision defect, TNM classification, the application of side diagnostic procedures, pathohistological diagnosis of postoperative complications. The sensibility of the lower lip region was monitored, as well as the obtained esthetic effects. Results. The results classified according to the mentioned parameters were processed, analyzed and displayed in detail. Clinical experience concerning the application of this reconstruction technique was presented and analyzed. Conclusion. The modified Fries technique can successfully be applied in the reconstruction of the lower lip defects longer than 4 cm because its application leads to satisfactory functional and acceptable esthetic results.
AB  - Uvod/cilj: Karcinomi usana čine približno oko 1/4 svih karcinoma usne duplje. Primarno lečenje karcinoma donje usne podrazumeva radikalnu eksciziju pune debljine tkiva donje usne zajedno sa tumorom. Nadoknada defekta nastalog nakon uklanjanja tumora donje usne predstavlja izazov za hirurga i zahteva detaljno preoperativno planiranje, pravilan izbor načina rekonstrukcije i poznavanje odgovarajućih hirurških tehnika. Cilj rada je da se kroz desetogodišnju retrospektivnu studiju prikažu naši klinički rezultati i iskustva u nadoknadi velikih defekata donje usne modifikovanom metodom po Friesu kod bolesnika lečenih od karcinoma donje usne. Metode. Od 256 bolesnika lečenih zbog tumora donje usne u periodu od 1992. do 2001. godine, kod 37 je primenjena rekonstrukcija modifikovanom metodom po Friesu. Analizirani su sledeći parametri: pol i godine starosti bolesnika, vreme proteklo od pojave prvih simptoma do javljanja lekaru, veličina tumora, veličina postekscizionog defekta, TNM klasifikacija, primena pomoćnih dijagnostičkih procedura, patohistološka dijagnoza, postoperativne komplikacije. Beležena su opažanja bolesnika u vezi senzibiliteta predela donje usne. Procenjivani su i ostvareni estetski efekti. Rezultati. Detaljno su obrađeni, analizirani i prikazani rezultati po navedenim parametrima. Izneta su naša klinička iskustva u primeni ove tehnike rekonstrukcije. Analizirane su njene prednosti i mane. Zaključak. Modifikovana tehnika po Friesu može se uspešno primeniti u rekonstrukciji defekata donje usne većih od 4 cm do potpunog nedostatka donje usne, jer se njenom primenom ostvaruju zadovoljavajući funkcionalni i prihvatljivi estetski rezultati.

AB  - Every occlusal therapy comprises an analysis, detection and removal of occlusal disorders as well as dental restoration. In this paper are presented different occlusal indicators (markers) which are now available on the market and their advantages and disadvantages are described.
AB  - Analiza okluzije, otkrivanje i otklanjanje okluzalnih smetnji, kao i izrada zubnih nadoknada su sastavni deo svake okluzalne terapije. Ovi postupci se ne mogu izvesti uspešno bez odgovarajućih okluzalnih indikatora. U radu su nabrojani različiti indikatori (markeri) trenutno dostupni na tržištu i opisane njihove prednosti i nedostatci.
