AB  - Introduction/Objective. We aimed to investigate the effects of diabetes mellitus (DM) on rat dental pulp repair by measuring time-dependent changes in expressions of vascular endothelial growth factor (VEGF) and bone morphogenetic protein 2 (BMP 2) following direct pulp capping. Methods. Two groups, each of 20 Wistar rats, received either streptozotocin (for DM induction) or the same volume of sterile saline. A week later, the pulp of maxillary and mandibular right incisors in diabetic and non-diabetic groups were exposed and capped with calcium hydroxide in order to provoke reparative response. The levels of VEGF and BMP 2 were determined in the pulp tissue lysates one and seven days after the pulp capping, using enzyme-linked immunosorbent assays. Results. Diabetic state per se increased VEGF level, with a peak at first day after the pulp capping (19.3 ± 0.9 pg/mg, p < 0.001), but did not affect BMP 2 levels. Significant increase of BMP 2 expression was noticed on the seventh day in capped pulp, but only in diabetic rats (16.7 ± 1 pg/mg, p = 0.001). Positive correlation between VEGF and BMP 2 was found on the seventh day following capping, only in diabetic pulp (r = 0.905, p = 0.003). Conclusion. Diabetes-induced increase in VEGF expression reflects changes in the inflammatory phase of pulp repair in DM. Increase in BMP 2 expression suggest that stimulating effect of calcium hydroxide appears seven days after diabetic pulp capping.
AB  - Uvod/Cilj Cilj ove studije bio je da se ispita efekat dijabetesa melitusa na reparaciju zubne pulpe pacova utvrđivanjem vremenski zavisnih promena u ekspresiji vaskularnog endotelnog faktora rasta (VEGF) i kostnog morfogenetskog proteina 2 (BMP 2) posle direktnog prekrivanja pulpe. Metode Istraživanje je sprovedeno na pacovima soja vistar, podeljenim u dve grupe od po 20 životinja, pri čemu je jedna grupa dobila streptozotocin (za indukciju dijabetesa melitusa), a druga sterilni fiziološki rastvor u istoj zapremini. Posle nedelju dana pulpe maksilarnih i mandibularnih donjih inciziva kod dijabetičnih i nedijabetičnih životinja su eksponirane i odmah zatim prekrivene kalcijum-hidroksidom da bi se izazvao reparatorni odgovor. Nivoi VEGF i BMP 2 su utvrđivani u lizatima pulpnog tkiva, prvog i sedmog dana posle direktnog prekrivanja imunoenzimskim testom ELISA. Rezultati Dijabetično stanje je dovelo do rasta nivoa VEGF, sa maksimumom utvrđenim prvog dana posle prekrivanja pulpe (19,3 ± 0,9 pg/mg, p < 0,001), ali nije uticalo na nivoe BMP 2. Značajan porast BMP 2 je utvrđen sedmog dana posle prekrivanja pulpe, ali samo kod dijabetičnih pacova (16,7 ± 1 pg/mg, p = 0,001). Kod ovih životinja u istom periodu posle prekrivanja nađena je pozitivna korelacija između nivoa VEGF i BMP 2 (r = 0,905, p = 0,003). Zaključak Dijabetesom indukovan porast ekspresije VEGF ukazuje na promene u inflamatornoj fazi pulpne reparacije. Porast ekspresije BMP 2 ukazuje da se stimulativan reparatorni efekat kalcijum-hidroksida javlja sedmog dana posle prekrivanja dijabetične pulpe.

Retention force of overdenture retained with telescopic crowns – a comparison of polyether ether ketone and zirconia ceramic telescopic crowns

AB  - Introduction/Objective. Recently, new materials for double crowns have been introduced, such as zirconia and polyether ether ketone (PEEK). However, some characteristics of these materials, such as retentive force and duration of “settling in phase,” have not been investigated sufficiently. During the “settling in phase,” telescopic overdenture has not yet achieved its definitive retention force, and it can be harmful for periodontal tissue if the value is above optimal for a long period of time. The objective was to measure the in vitro overall pull-off force of telescopic crowns where primary crowns were made from zirconia ceramics and a survey of the “settling in phase” duration. Methods. Forty zirconia primary telescopic crowns were produced on prepared canine teeth. Twenty secondary crowns were of PEEK and other 20 of zirconia with electroplated gold copings. The pull-off force measurements were conducted utilizing a dynamometer until a constant value was obtained. Results. The specimens of the PEEK group showed higher initial retentive force values. Settling in phase was finished between 800 and 900 cycles of separation for both groups. Comparing the value of the pull-off force between individual different cycles, a statistically significant reduction was recorded up to the 800th cycle, while between the 800th and the 900th cycle there was no difference. Conclusions. The settling in phase was finished between 800 and 900 cycles of separation in both groups. Final retentive force values for both tested telescopic groups were in the optimal range which is 5–9 N per one telescopic crown.
AB  - Uvod/Cilj Poslednjih godina uvedeni su u praksu novi materijali za dvostruke krune, kao što su cirkonija i polietereterketon (PEEK). Međutim, neke karakteristike ovih materijala nisu dovoljno ispitane, kao što su retenciona sila i trajanje "faze uhodavanja". "Faza uhodavanja" je inicijalni period upotrebe teleskopske proteze kada finalna retenciona sila još uvek nije postignuta, i može imati štetni uticaj na parodontalno tkivo ako su u tom periodu sile previsoke i predugo traju. Cilj je bio da se izmeri in vitro ukupna sila razdvajanja teleskopskih kruna, gde su primarne krune izrađene od cirkonijumske keramike, i ispitati trajanje faze uhodavanja. Metode Četrdeset primarnih teleskopskih kruna od cirkonijumske keramike je izrađeno na preparisanim očnjacima. Dvadeset sekundarnih kruna je izrađeno od PEEK-a, i još 20 kruna od cirkonije sa galvanizovanim zlatom. Za merenje sile razdvajanja korišćen je dinamometar. Spajanje i razdvajanje teleskopskih kruna i merenje sile razdvajanja je vršeno dok nije dobijena konstantna vrednost. Rezultati Uzorci iz grupe PEEK pokazali su višu inicijalnu vrednost retencione sile. Faza uhodavanja je završena između 800 i 900 ciklusa razdvajanja kod obe grupe. Kada se uporede individualne vrednosti sile razdvajanja između različitih ciklusa, statistički značajno smanjenje je zabeleženo do 800. ciklusa, dok između 800. i 900. ciklusa nije bilo razlike. Zaključak Faza uhodavanja je završena između 800 i 900 ciklusa razdvajanja u obe grupe. Finalna retenciona sila kod obe testirane grupe pokazala je optimalne vrednosti, koje iznose 5-9 N po teleskopskoj kruni.

Efficacy and safety of 4% articaine with epinephrine for the anterior middle superior alveolar nerve block comparing to the computer controlled and conventional anesthetic delivery: Prospective, randomized, cross-over clinical study

AB  - Bacground/Aim. The efficient dental anesthesia, which is related to the clinically adequate depth, duration and the width of anesthetic field, is an important prerequisite for successful dental treatment. The aim of this study was to investigate and compare the pulpal anesthesia and cardiovascular parameters after the anterior middle superior alveolar (AMSA) nerve block with 4% articaine with epinephrine administered by conventional cartridge-syringe and computer-controlled local anesthetic delivery system (CCLADS). Methods. This controlled double-blind cross-over randomized clinical study included 38 healthy volunteers. Efficacy of pulpal anesthesia after the AMSA nerve block was evaluated by measuring a success rate, onset and duration of pulpal anesthesia, using an electrical pulp tester. The parameters of cardiovascular function (systolic and diastolic blood pressure, heart rate) were monitored noninvasively. Results. Successful pulpal anesthesia of all tested teeth was obtained in 57.9% participants with CCLADS and in 44.7% participants with conventional syringe. The onset time was not significantly different between two investigated groups. The pulpal anesthesia duration was not significantly different neither within nor between investigated groups. The systolic and diastolic blood pressures were significantly decreased in both investigated groups, in comparison with the baseline values. Heart rate significantly decreased within CCLADS from 10th to 30th minute when compared to baseline. Conclusion. The efficacy of pulpal anesthesia and safety of cardiovascular profile of 0.6 mL of articaine with epinephrine (1:100.000) delivered with CCLADS were improved in comparison to the conventional syringe delivery. Significant changes of cardiovascular function were not observed.
AB  - Uvod/Cilj. Efikasna zubna anestezija u pogledu klinički adekvatne dubine, trajanja i širine anestetičkog polja je važan preduslov za uspešno zubno lečenje. Cilj rada bio je ispitivanje i upoređivanje parametra anestezije zubne pulpe i kardiovaskularnih parametra posle gornje prednje i srednje alveolarne (AMSA) sprovodne anestezije postignute 4% artikainom sa epinefrinom, primenom standardne karpulbrizgalice i kompjuterski kontrolisanog sistema za primenu anestetičkog rastvora (CCLADS). Metode. U ovom randomizovanom, prospektivnom, kontrolisanom, dvostruko slepom ukrštenom kliničkom istraživanju učestvovalo je 38 ispitanika. Kvalitet anestezije zubne pulpe posle AMSA anestezije praćen je na osnovu uspešnosti, latentnog perioda i trajanja anestezije zubne pulpe, primenom električnog pulp-testera. Parametri kardiovaskularne funkcije (sistolni i dijastolni krvni pritisak, srčana frekvencija) praćeni su neinvazivno, primenom aparata za monitoring. Rezultati. Uspešna anestezija zubne pulpe svih ispitivanih zuba bila je prisutna kod 57,9% ispitanika posle kod CCLADS i kod 44,7% ispitanika posle primene standardne karpul-brizgalice. Latentni period i trajanje anestezije zubne pulpe nisu se značajno razlikovali između ispitivanih grupa. Sistolni i dijastolni pritisak bili su značajno sniženi u praćenim vremenskim intervalima u odnosu na početne vrednosti. Srčana frekvencija je bila značajno snižena kod CCLADS grupe od 10 do 30 minuta u odnosu na početne vrednosti. Zaključak. Kontrolisanom kompjuterizovanom primenom (CCLADS) 0,6 mL 4% artikaina sa epinefrinom (1:100,000) za AMSA sprovodnu anesteziju, postignut je bolji kvalitet anestezije zubne pulpe u odnosu na primenu artikaina sa epinefrinom standardnom karpul brizgalicom. Nisu uočene bitne promene funkcija kardiovaskularnog sistema.

The aim of this study was to investigate and compare the pulpal anesthesia and cardiovascular parameters after the anterior middle superior alveolar (AMSA) nerve block with 4% articaine with epinephrine administered by conventional cartridge-syringe and computer-controlled local anesthetic delivery system (CCLADS). Methods. This controlled double-blind cross-over randomized clinical study included 38 healthy volunteers. Efficacy of pulpal anesthesia after the AMSA nerve block was evaluated by measuring a success rate, onset and duration of pulpal anesthesia, using an electrical pulp tester. The parameters of cardiovascular function (systolic and diastolic blood pressure, heart rate) were monitored noninvasively. Results. Successful pulpal anesthesia of all tested teeth was obtained in 57.9% participants with CCLADS and in 44.7% participants with conventional syringe. The onset time was not significantly different between two investigated groups. The pulpal anesthesia duration was not significantly different neither within nor between investigated groups. The systolic and diastolic blood pressures were significantly decreased in both investigated groups, in comparison with the baseline values. Heart rate significantly decreased within CCLADS from 10th to 30th minute when compared to baseline. Conclusion. The efficacy of pulpal anesthesia and safety of cardiovascular profile of 0.6 mL of articaine with epinephrine (1:100.000) delivered with CCLADS were improved in comparison to the conventional syringe delivery. Significant changes of cardiovascular function were not observed., Uvod/Cilj. Efikasna zubna anestezija u pogledu klinički adekvatne dubine, trajanja i širine anestetičkog polja je važan preduslov za uspešno zubno lečenje. Cilj rada bio je ispitivanje i upoređivanje parametra anestezije zubne pulpe i kardiovaskularnih parametra posle gornje prednje i srednje alveolarne (AMSA) sprovodne anestezije postignute 4% artikainom sa epinefrinom, primenom standardne karpulbrizgalice i kompjuterski kontrolisanog sistema za primenu anestetičkog rastvora (CCLADS). Metode. U ovom randomizovanom, prospektivnom, kontrolisanom, dvostruko slepom ukrštenom kliničkom istraživanju učestvovalo je 38 ispitanika. Kvalitet anestezije zubne pulpe posle AMSA anestezije praćen je na osnovu uspešnosti, latentnog perioda i trajanja anestezije zubne pulpe, primenom električnog pulp-testera. Parametri kardiovaskularne funkcije (sistolni i dijastolni krvni pritisak, srčana frekvencija) praćeni su neinvazivno, primenom aparata za monitoring. Rezultati. Uspešna anestezija zubne pulpe svih ispitivanih zuba bila je prisutna kod 57,9% ispitanika posle kod CCLADS i kod 44,7% ispitanika posle primene standardne karpul-brizgalice. Latentni period i trajanje anestezije zubne pulpe nisu se značajno razlikovali između ispitivanih grupa. Sistolni i dijastolni pritisak bili su značajno sniženi u praćenim vremenskim intervalima u odnosu na početne vrednosti. Srčana frekvencija je bila značajno snižena kod CCLADS grupe od 10 do 30 minuta u odnosu na početne vrednosti. Zaključak. Kontrolisanom kompjuterizovanom primenom (CCLADS) 0,6 mL 4% artikaina sa epinefrinom (1:100,000) za AMSA sprovodnu anesteziju, postignut je bolji kvalitet anestezije zubne pulpe u odnosu na primenu artikaina sa epinefrinom standardnom karpul brizgalicom. Nisu uočene bitne promene funkcija kardiovaskularnog sistema.",

Unilateral complex partial denture performance evaluation: 5 Years follow up clinical study

AB  - Introduction Removable partial denture (RPD) is common treatment option for unilateral partially edentulous patients not indicated for implant therapy. Unilateral complex partial denture (UCPD) could be an alternative approach to RPD treatment, but there is lack of evidence about UCPD treatment outcomes during the long-term clinical performance. The aim of this study was to use periodontal, prosthodontic and participant satisfaction measures to evaluate the long-term clinical performance of UCPD. Material and methods This 5 year follow-up clinical study evaluated pocket probing depth (PPD) and vertical clinical attachment loss (CAL-V) of direct abutment (DA), indirect abutment (IA) and control teeth (CT). Also complications and failures of UCPD were analysed using questionnaire of participant satisfaction with UCPD (stability, comfort and manipulation). Results Evaluation of data showed that CAL-V and PPD significantly increased over time for DA, IA and CT (p lt 0.0001), but the tooth function (DA, IA and CT) did not significantly influence changes in PPD and CAL-V. The fracture of one abutment tooth and increase of the number of artificial teeth deformations (p=0.039) were observed after 5 years. Participant satisfaction with denture after 1 year and 5 years vs. 7 days was significantly improved. Conclusion Despite limitations of this clinical study and assuming regular oral maintenance with proper indication, UCPD might be considered as good treatment option for Kennedy II rehabilitation in patients not indicated for implant therapy or who cannot tolerate extensive RPD design.
AB  - Uvod Parcijalna skeletirana proteza (PSP) najčešća je terapijska opcija kod jednostrane krezubosti kada nije indikovana implantološka terapija. Jednostrana kompleksna parcijalna skeletirana proteza (JKPSP) predstavlja alternativu konvencionalnoj PSP, jedino što ne postoje evidentni podaci o uspešnosti terapije nakon duže kliničke upotrebe. Cilj ovog rada bio je da se na osnovu analize objektivnih pokazatelja, odnosno analize periodontalnih i protetskih parametara, kao i na osnovu subjektivnih ocena pacijenta, izvrši provera uspešnosti JKPSP retiniranim atečmenom tipa reze sa oprugom nakon petogodišnjeg praćenja. Materijal i metod Kod deset ispitanika oba pola izvršeno je praćenje gubitka vertikalnog pripoja gingive i dubine parodontalnih džepova, kod primarnog retencionog zuba (PRZ), sekundarnog retencionog zuba (SRZ), koji su bili u sastavu JKPSP i kontrolnog zuba (KZ) suprotne strane vilice. Takođe, analizirani su protetske komplikacije (fraktura zuba, gubitak veštačkih zuba) i zadovoljstvo pacijenata na osnovu popunjenih upitnika, gde su oni ocenjivali stabilnost, komfor i rukovanje JKPSP. Rezultati Nakon analize dobijnih podataka uočeno je da se vrednost dubine parodontalnih džepova i gubitak vertikalnog pripoja gingive kod PRZ statistički značajno razlikuju u odnosu na SRZ i KZ (p  lt  0,0001). Zabeležene protetske komplikacije u vidu frakture retencionog zuba i deformacije veštačkih akrilatnih zuba nisu bile statistički značajne posle pet godina praćenja. Zadovoljstvo pacijenata JKPSP posle prve godine i posle pet godina nošenja se značajno popravilo u odnosu na prvih sedam dana posle predaje. Zaključak Nezavisno od ograničenja ove kliničke studije, uzimajući u obzir adekvatnu indikaciju i pravilno održavanje higijene, JKPSP se može smatrati dobrom opcijom za rehabilitaciju krezubosti Kenedi II klase kod pacijenata kod kojih nije indikovana terapija implantatima, odnosno koji ne prihvataju prisustvo velike spojnice. Pacijenti su bili zadovoljni protezama, njihovim komforom i lakoćom rukovanja protezama. .

Stress and strain analyses of removable partial denture abutment tooth in relation to the position of the minor connector

AB  - Introduction/objective: For optimum loading distribution, the angle formed by the occlusal rest and the vertical minor connector from which it originates should be less than 90°. The objective of the article was to visualize the optimum angle between the occlusal rest and the minor connector in terms of intensity and distribution of occlusal loads using finite element analysis. It was the intention, concerning biomechanical behavior, to document that the optimum angle between the occlusal rest and the minor connector should be less than 90°. Methods: Three different virtual models of partial edentulous Kennedy III class were created using the CATIA design computer program with different angles between the occlusal rest and the minor connector. Stress distribution after simulated occlusal loading was analyzed using the finite element method. Results: Comparing the results obtained for three models, the highest stress values were seen in model 3 (the angle between the occlusal rest and the small connector is greater than 90°) whether the load is applied in the middle or at the end of the saddle. Conclusion: Within limitations and on the basis of the study results, the minimum compressive stress was seen in model 1, where the angle between the occlusal rest and the minor connector was less than 90° whether the load is applied in the middle or at the end of the saddle. It is recommended that obtuse angle between the rest and the minor connector should be avoided due to potential hazardous stress concentration on abutment teeth.
AB  - Uvod/Cilj: Okluzalni naslon i mala spojnica treba da zaklapaju međusobni ugao manji od 90 stepeni, kako bi se obezbedilo najpovoljnije prenošenje opterećenja. Cilj rada je bio da se metodom konačnih elemenata prikaže ugao između okluzalnog naslona i male spojnice koji je najpovoljniji za prenošenje okluzalnog opterećenja. Namera je bila da se, posmatrano sa biomehaničkog aspekta, dokumentuje da je ugao manji od 90 stepeni između okluzalnog naslona i male spojnice najpovoljniji. Metod: Izrađena su tri različita virtuelna modela krezube vilice klase krezubosti Kenedi III u programu CATIA sa modelovanim različitim uglovima između okluzalnog naslona i male spojnice. Analiza distribucije napona i deformacija nakon simuliranog okluzalnog opterećenja izvršene su metodom konačnih elemenata. Rezultati: Posle simuliranog okluzalnog opterećenja sva tri modela najveći napon je uočen kod modela 3 (ugao između okluzalnog naslona i male spojnice veći od 90 ste- peni), bez obzira na to da li je opterećenje aplikovano na sredini ili na kraju sedla. Zaključak: U okviru ograničenja u istraživanju, najmanji kompresioni napon uočen je u modelu 1 (ugao između oklu- zalnog naslona i male spojnice manji od 90 stepeni) bez obzira na to da li je opterećenje aplikovano na sredini ili na kraju sedla. Preporučuje se da se tup ugao između okluzalnog naslona i male spojnice izbegava zbog mogućih štetnih koncentracija napona na retencionom zubu.

AB  - Contemporary dentistry is, first of all, characterized by diverse accelerated development, owing to improvements of information and other technologies, as well as the development of dental materials (shape-memory biomaterials, nanomaterials, biomaterials for application in tissue engineering, etc.). Expert doctrinaire attitudes move from the direction of operative interventions, whereby disease and acute symptoms are primarily treated, towards the strengthening of oral health by minimally invasive procedures. A particular place in patients’ total rehabilitation belongs to numerous esthetic procedures which, to a large extent, make up a wants-based service, led by the patients’ needs and affinities. This paper deals with differences between cosmetic and esthetic dentistry. The complexity of esthetic dentistry, which favors therapy with the change of function parameters in care for the patient, is emphasized. On the other hand, more attention is paid to the need to know and respect ethical and marketing principles that follow any activity of dentists, starting from the first contact with the patient, the selection of certified materials, to the implementation of the appropriate treatment plan. Well-directed communication and comprehensive awareness of the patient, the use of the visual analog scale, consideration of realistic resources in therapy, and the acceptance of de Bono model of adopted parallel thinking are determinants which help dentists define a problem adequately, find quality solutions, open alternative solutions, and reduce the potential risks in patients’ therapy.
AB  - Današnju stomatologiju karakteriše ubrzani razvoj zahvaljujući unapređenju informacionih i drugih tehnologija, kao i razvoju dentalnih materijala (biomaterijali sa memorisanim oblikom, nanomaterijali, biomaterijali za primenu u tkivnom inženjeringu i dr.). Stručni doktrinarni stavovi kreću se iz pravca operativnih intervencija, kojima se primarno saniraju bolest i akutni simptomi, ka jačanju oralnog zdravlja malo invazivnim postupcima. Posebno mesto u sveukupnoj rehabilitaciji bolesnika pripada brojnim estetskim procedurama, koje u velikoj meri čine praksu vođenu potrebama i afi- nitetima bolesnika. U radu se diskutuje o razlikama u poimanju kozmetske i estetske stomatologije. Naglašena je složenost estetske stomatologije, koja daje prednost terapiji sa promenom funkcijskih parametara u zbrinjavanju bolesnika. S druge strane, akcenat se stavlja na potrebu poznavanja i poštovanja etičkih i marketinških načela koja prate svaku aktivnost lekara, počev od prvog kontakta sa bolesnikom, odabira sertifikovanog materijala do realizacije odgovarajućeg terapijskog plana. Dobro usmerena komunikacija i sveobuhvatna informisanost bolesnika, upotreba VAS skale, kao i sagledavanje realnih resursa u terapiji odrednice su koje pomažu stomatologu da definiše problem na pravi način, iznađe kvalitetna rešenja, otvori alternativne solucije i smanji moguće rizike u terapiji bolesnika.

Investigation of antioxidant capacity of several luting cements processes by HPMC method

AB  - Background: Free radicals (FR) occur in oral cavity where lot of food was transferred to through entire life under specific saliva conditions. Many enzymes, microorganism, alcohol beverages, nicotine and other harmful or indifferent substances when in contact to oral tissues might provoke oxidation process under specific condition creating FR's. The similar role might have various dental materials. Aim of the study was to record the level of antioxidant (AO) activity of several permanent (P) luting cements alone or combined with quercetin AO substance. Materials/Methods: P cements were Zn-phosphate, Zn-polycarboxilate, GIC and composite resin cement. They were prepared as original prescription and their variant by 1% weight addition of quercetin. AO activity of cements was measured by HPMC test evaluated by Student t test. Results: There were statistically significant differences among Zn-phosphate, Zn-polycarboxilate and resin dental cements (p > 0,05). GIC displayed significantly higher AO values (p  lt  0,01) versus other three cements. There were no difference in AO capacity between sample of original P cements and their corresponding quercetin variants (p > 0,05). Conclusions: Conventional GIC displayed the most powerful AO activity among P luting cements. Addition of 1% antioxidant quercetin did not improve AO capacity of investigated cements.

Prosthetic treatment after teeth extractions in patients with type 2 diabetes mellitus

AB  - Introduction Good and well balanced diet provided by adequate mastication is part of therapy in patients with type 2 diabetes mellitus (DM). The critical period presents the time immediately after teeth extractions; hence, immediate denture is a rational therapeutical choice for diabetic patients. The presence of immediate denture and its compression might compromise wound healing process, affect chewing ability, food intake and consequently blood glucose level in type 2 DM patients. Objective The objective of this study was to compare socket opening diameters (SOD), chewing ability, changes in blood glucose level and food intake in type 2 DM patients with and without maxillary immediate complete denture (MICD) during a three-week wound healing period. Methods The study comprised 78 type 2 DM partially removable denture wearers (42 candidates for teeth extractions and 36 candidates for teeth extractions and insertion of MICDs). During the three-week period participants were followed for SOD, chewing ability and changes in blood glucose level and food intake. Results Patients with MICD showed significantly lower reduction of SOD (seventh, 14th, 21st day) and higher chewing ability (seventh, 14th, 21st day) in comparison to patients without an MICD. Significantly lower number of patients with an MICD had changes in blood glucose level and food intake. Conclusion Maxillary immediate complete denture presents a good therapeutic choice for type 2 DM patients, as it provides possibility of adequate mastication after teeth extractions and maintenance of nutritional status and blood glucose level.
AB  - Uvod Redovna i balansirana ishrana obezbeđena adekvatnom mastikacijom je značajan deo terapije pacijenata sa dijabetesom melitusom tipa 2 (DM tip 2). Kritičan period za ove bolesnike jeste vreme neposredno nakon ekstrakcije zuba, pa za njih imedijatna proteza predstavlja terapijski izbor. Prisustvo imedijatne proteze i kompresija koju izaziva mogu kompromitovati proces zarastanja, uticati na sposobnost žvakanja, količinu unete hrane, a time i nivo glikemije kod pacijenata obolelih od ovog tipa dijabetesa. Cilj rada Cilj istraživanja je poređenje dijametra ekstrakcionih alveola (DEA), sposobnosti žvakanja, promene količine unete hrane i glikemije kod DM tipa 2 pacijenata sa gornjom totalnom imedijatnom protezom (GTIP) i bez nje. Metode rada Istraživanje je obuhvatilo 78 DM tipa 2 pacijenata, nosilaca parcijalnih pločastih proteza (42 pacijenta indikovana za ekstrakcije zuba i 36 pacijenata indikovanih za ekstrakcije zuba i dobijanje GTIP). Tokom postekstrakcionog perioda od tri nedelje kod pacijenata su praćeni DEA, sposobnost žvakanja, promena količine unete hrane i glikemije. Rezultati Pacijenti sa GTIP pokazali su značajno manju redukciju DEA (7, 14, 21. dana), veću sposobnost žvakanja (7, 14, 21. dana) u odnosu na pacijente bez GTIP. Značajno manji broj pacijenata sa GTIP imao je promene u glikemiji i unošenju hrane u odnosu na grupu bez GTIP. Zaključak Imedijatna proteza predstavlja terapijski izbor kod pacijenata sa DM tipa 2, budući da omogućava dobru mastikaciju nakon ekstrakcija, uz održanje nutricionog statusa i glikemije.

The effect of recasting on biological properties of Ni-Cr dental alloy

AB  - Introduction Increases in market prices of gold over the last 20 years have led to expansion of basic dental alloys, which, primarily due to their good mechanical properties and acceptable prices, have found their place in everyday dental practice. However, within the procedure of making dental prosthetic restorations, the alloys are melted and cast, which leads to changes in their physical, mechanical and biological properties. Objective The objective of the study was to test biocompatibility of a Ni-Cr dental alloy (WIRON 99) depending on the number of melting and casting processes. Methods The working method included the testing of cytotoxicity of the alloy obtained by casting after one, after four, and after eight successive processes of melting. Cytotoxicity of samples was tested by means of a 24-hour and a three-day cytotoxicity test, done on L929 fibroblasts. Results A repeatedly melted and cast alloy shows a reduced biocompatibility and causes specific responses of the tissues in the surrounding area. Since the cytotoxic effect is more significant in the extended contact with the culture cells, a three-day cytotoxicity test showed discrete changes which were the indicator of cell growth inhibition in the cell culture. Conclusion The obtained results confirm the working hypothesis that repeated alloy melting and casting will decrease biocompatibility of dental alloys and will lead to specific responses of the tissue in the surrounding area.
AB  - Uvod Porast cene zlata na svetskom tržištu u poslednjih 20 godina doveo je do ekspanzije baznih dentalnih legura, koje su pre svega zbog dobrih mehaničkih svojstava i pristupačnih cena našle primenu u svakodnevnoj stomatološkoj praksi. U proceduri izrade protetskih radova legure se tope i izlivaju, što dovodi do promena njihovih fizičko-mehaničkih i bioloških svojstava. Cilj rada Cilj rada bio je ispitivanje biokompatibilnosti jedne Ni-Cr legure (WIRON 99) u zavisnosti od broja topljenja i livenja. Metode rada U radu je ispitivana citotoksičnost legure dobijene izlivanjem posle jednog topljenja, posle četiri i posle osam uzastopnih topljenja. Citotoksičnost uzoraka je ispitivana pomoću 24-časovnog i trodnevnog testa citotoksičnosti na L929 fibroblastima. Rezultati Više puta topljena i livena legura pokazala je smanjenu biokompatibilnost, dovodeći do specifičnih odgovora tkiva u neposrednom okruženju. Budući da je citotoksični efekat izraženiji u produženom kontaktu sa ćelijama iz kulture, trodnevni test citotoksičnosti je pokazao diskretne promene koje su pokazatelj inhibicije rasta ćelija u kulturi. Zaključak Dobijeni rezultati potvrđuju radnu hipotezu da više puta ponavljana topljenja i li venja legura smanjuju biokompatibilnost dentalnih legura i dovode do specifičnih odgovora tkiva u neposrednom okruženju.

However, within the procedure of making dental prosthetic restorations, the alloys are melted and cast, which leads to changes in their physical, mechanical and biological properties. Objective The objective of the study was to test biocompatibility of a Ni-Cr dental alloy (WIRON 99) depending on the number of melting and casting processes. Methods The working method included the testing of cytotoxicity of the alloy obtained by casting after one, after four, and after eight successive processes of melting. Cytotoxicity of samples was tested by means of a 24-hour and a three-day cytotoxicity test, done on L929 fibroblasts. Results A repeatedly melted and cast alloy shows a reduced biocompatibility and causes specific responses of the tissues in the surrounding area. Since the cytotoxic effect is more significant in the extended contact with the culture cells, a three-day cytotoxicity test showed discrete changes which were the indicator of cell growth inhibition in the cell culture. Conclusion The obtained results confirm the working hypothesis that repeated alloy melting and casting will decrease biocompatibility of dental alloys and will lead to specific responses of the tissue in the surrounding area., Uvod Porast cene zlata na svetskom tržištu u poslednjih 20 godina doveo je do ekspanzije baznih dentalnih legura, koje su pre svega zbog dobrih mehaničkih svojstava i pristupačnih cena našle primenu u svakodnevnoj stomatološkoj praksi. U proceduri izrade protetskih radova legure se tope i izlivaju, što dovodi do promena njihovih fizičko-mehaničkih i bioloških svojstava. Cilj rada Cilj rada bio je ispitivanje biokompatibilnosti jedne Ni-Cr legure (WIRON 99) u zavisnosti od broja topljenja i livenja. Metode rada U radu je ispitivana citotoksičnost legure dobijene izlivanjem posle jednog topljenja, posle četiri i posle osam uzastopnih topljenja. Citotoksičnost uzoraka je ispitivana pomoću 24-časovnog i trodnevnog testa citotoksičnosti na L929 fibroblastima. Rezultati Više puta topljena i livena legura pokazala je smanjenu biokompatibilnost, dovodeći do specifičnih odgovora tkiva u neposrednom okruženju. Budući da je citotoksični efekat izraženiji u produženom kontaktu sa ćelijama iz kulture, trodnevni test citotoksičnosti je pokazao diskretne promene koje su pokazatelj inhibicije rasta ćelija u kulturi. Zaključak Dobijeni rezultati potvrđuju radnu hipotezu da više puta ponavljana topljenja i li venja legura smanjuju biokompatibilnost dentalnih legura i dovode do specifičnih odgovora tkiva u neposrednom okruženju.",

Denture stomatitis and salivary vascular endothelial growth factor in immediate complete denture wearers with type 2 diabetes

AB  - Statement of problem. Diabetes mellitus type 2 is associated with a variety of oral mucosal changes, including an altered level of salivary vascular endothelial growth factor. However, the authors identified no studies concerning denture stomatitis and salivary vascular endothelial growth factor in immediate denture wearers with diabetes mellitus type 2. Purpose. The purpose of this study was to investigate the incidence of and risk factors for denture stomatitis in immediate complete denture wearers with and without diabetes mellitus type 2 and to investigate the relationship between levels of salivary vascular endothelial growth factor and clinical forms of denture stomatitis in both groups. Material and methods. Individuals without diabetes (n=42) or with diabetes mellitus type 2 (n=36) who were candidates for complete immediate dentures were included in the study. After 1 year of wearing relined immediate dentures, participants were evaluated for denture stomatitis and potential contributing factors. Salivary vascular endothelial growth factor levels were measured with enzyme-linked immunosorbent assays. Data were analyzed with the chi(2) test or the Student t test where appropriate, as well as with binary logistic regression analysis. Results. The incidence of denture stomatitis was 61% in participants with diabetes mellitus type 2 and 38% in those without diabetes. Low denture stability and diabetes mellitus type 2 were risk factors for denture stomatitis. Salivary vascular endothelial growth factor concentrations at the beginning of the study were 557.6 +/- 94.7 pg/mL in participants with diabetes mellitus type 2 and 103.5 +/- 21.6 pg/mL in those without diabetes. In Newton Type I and Newton Type II denture stomatitis, vascular endothelial growth factor levels were 460.9 +/- 55.4 pg/mL and 1445.2 +/- 422.1 pg/mL in individuals with diabetes and 73.2 +/- 10.0 pg/mL and 306.5 +/- 22.6 pg/mL in those without diabetes. Conclusions. Perceived denture stability and diabetes mellitus type 2 are independent risk factors for the occurrence of denture stomatitis. Altered salivary vascular endothelial growth factor levels and denture stomatitis are more prevalent in denture wearers with diabetes mellitus type 2.

AB  - Vascular endothelial growth factor (VEGF) is critical component in processes of angiogenesis, vasculogenesis and neurogeneis. Angiogenesis is important for wound healing and for restoring blood flow to tissues after injury or insult. When regulation of angiogenic growth factors, such as VEGF, fails, blood vessels are formed excessively or insufficiently. Important environmental effects on VEGF expression include: hypoxia, mechanical stress, nitric oxide, acid conditions, glycemia and hormones. VEGF is constitutively expressed in oral tissues such as gingiva, gingival cervical fluid, and saliva and is affected by various pathological processes. Diabetes mellitus type 2 (DM type 2) is associated with a range of basic complications such as, neuropathy, microvascular and macrovascular disease and altered wound healing. Also DM type 2 is linked with increased incidence of several oral conditions: periodontal disease, salivary gland dysfunction, burning mouth, delayed healing of wounds and mucosa ulcerations. There is a suggestion that these abnormalities, most likely are related to increased oxidative stress, compromised host defense, chronic systemic inflammation, vasculopathy and impaired salivary secretion. AB  - Faktor rasta vaskularnog endotela (Vascular endothelial growth factor, VEGF) ima značajnu ulogu u procesima angiogeneze, vaskulogeneze i neurogeneze. U fiziološkim uslovima, angiogeneza je značajna za zarastanje rana i obnovu krvotoka kroz tkiva posle povrede, dok u patološkim uslovima, kada se poremeti fiziološka regulacija angiogenih faktora rasta, uključujuci VEGF i njihovih inhibitora dolazi do prekomernog ili nedovoljnog stvaranja krvnih sudova. Na ekspresiju VEGF značajan efekat imaju lokalni faktori na mestu stvaranja ovog faktora rasta u koje se ubrajaju: hipoksija, mehanički stres, azot monoksid, ph, glikemija i hormoni. Stalno prisustvo VEGF je utvrdjeno u tkivima i tečnostima usne šupljine (gingiva, tečnost gingivalnog sulkusa, pljuvačka), kao i da se ono menja u toku patoloških procesa. Diabetes mellitus tip 2 (DM tip 2) je oboljenje u kome su najčešće sistemske komplikacije nefropatija, retinopatija i otežano zarastanje rana, direktno posledice mikro i makrovaskulopatija i neuropatija. Free-end saddle length influence on stress level in unilateral complex partial denture abutment teeth and retention elements

AB  - Background/Aim. Different types of dental restorations are used for the therapy of unilateral free-end saddle edentulism. Unilateral complex partial denture is one of the indications for the Kennedy class II partial edentulism. The abscence of major connector and denture plate is an advantage compared to the conventional restorations, because of better comfort and shorter period of adaptation. The aim of the study was to analyse the influence of free-end saddle length change on the behaviour of unilateral complex partial denture supporting structures. Methods. Stress levels of the canine and the first premolar as retentional teeth and the attachments were tested under the influence of physiological forces with the loading point shifting distally in relation to the saddle length change. A virtual real size 3D model of the fixed part of the restoration (the canine and the first premolar with milled crowns) was created using the CATIA computer program. It was connected to the mobile part of partial denture with the SD snap in latch attachment. Mobile part of the restoration was designed in the region of 2, 3 and 4 lateral teeth (second premolar, first, second and third molar). By using the finite element method (FEM) stress levels analysis was performed under the load of physiological forces of 150 N in the free-end saddle teeth zone. Results. The results of analysis show that physiological forces cause a different stress distribution on the abutment teeth and the attachment, depending on the saddle length. Conclusion. The stress level values obtained for the abutment teeth as well as the attachment are far lower than the marginal ones. The behaviour of the system changes under this defined stress, but no plastic deformation occurs.
AB  - Uvod/Cilj. U terapiji jednostrano slobodnog sedla koriste se različiti oblici zubnih nadoknada. Jednostrana kompleksna parcijalna proteza (JKPP) jedna je od indikacija za bezubost klase Kenedi II. Odsustvo velike spojnice i protezne ploče predstavlja prednost u odnosu na konvencionalne nadoknade zbog boljeg komfora i kraćeg perioda adaptacije. Cilj rada bio je analiza uticaja promene dužine slobodnog sedla na ponašanje potpornih struktura JKPP. Metode. Ispitivani su naponi očnjaka i prvog premolara kao retencionih zuba i veze (spoja) pod dejstvom fizioloških sila sa pomeranjem tačke opterećenja distalno, u zavisnosti od dužine sedla. Primenom kompjuterskog programa CATIA u realnoj veličini urađen je virtulni 3D model fiksnog dela nadoknade (očnjak i prvi premolar sa namenskim krunama) koji je veza SD snap in latch priključena na mobilni deo parcijalne proteze. Mobilni deo nadoknade postavljen je u predelu dva, tri, odnosno četiri bočna zuba (drugi premolar, prvi, drugi i treći molar). Primenom metode konačnih elemenata obavljena je analiza naponskih stanja pri opterećenju fiziološkim silama od 150 N u predelu zuba slobodnog sedla. Rezultati. Analiza proračuna pokazala je da pod dejstvom fizioloških sila dolazi do različite raspodele napona na retencione zube i spoj u zavisnosti od dužine sedla. Zaključak. Dobijene vrednosti za napone kako na retencionim zubima, tako i na spoju, daleko su manje od graničnih. Pri zadatim naponima dolazi do promene ponašanja, ali ne i do plastične deformacije sistema.

Primenom kompjuterskog programa CATIA u realnoj veličini urađen je virtulni 3D model fiksnog dela nadoknade (očnjak i prvi premolar sa namenskim krunama) koji je veza SD snap in latch priključena na mobilni deo parcijalne proteze. Mobilni deo nadoknade postavljen je u predelu dva, tri, odnosno četiri bočna zuba (drugi premolar, prvi, drugi i treći molar). Primenom metode konačnih elemenata obavljena je analiza naponskih stanja pri opterećenju fiziološkim silama od 150 N u predelu zuba slobodnog sedla. Rezultati. Analiza proračuna pokazala je da pod dejstvom fizioloških sila dolazi do različite raspodele napona na retencione zube i spoj u zavisnosti od dužine sedla. Zaključak. Dobijene vrednosti za napone kako na retencionim zubima, tako i na spoju, daleko su manje od graničnih. Pri zadatim naponima dolazi do promene ponašanja, ali ne i do plastične deformacije sistema.",

Comparative analysis of unilateral removable partial denture and classical removable partial denture by using finite element method

AB  - Introduction. Various mobile devices are used in the therapy of unilateral free-end saddle. Unilateral dentures with precise connectivity elements are not used frequently. In this paper the problem of applying and functionality of unilateral freeend saddle denture without major connector was taken into consideration. Objective. The aim was to analyze and compare a unilateral RPD (removable partial denture) and a classical RPD by calculating and analyzing stresses under different loads. Methods. 3D models of unilateral removable partial denture and classical removable partial denture with casted clasps were made by using computer program CATIA V5 (abutment teeth, canine and first premolar, with crowns and abutment tissues were also made). The models were built in full-scale. Stress analyses for both models were performed by applying a force of 300 N on the second premolar, a force of 500 N on the first molar and a force of 700 N on the second molar. Results. The Fault Model Extractor (FME) analysis and calculation showed the complete behavior of unilateral removable partial denture and abutments (canine and first premolar), as well as the behavior of RPD under identical loading conditions. Applied forces with extreme values caused high stress levels on both models and their abutments within physiological limits. Conclusion. Having analyzed stresses under same conditions, we concluded that the unilateral RPD and classical RPD have similar physiological values.
AB  - Uvod. U lečenju jednostrane krezubosti koriste se različiti oblici mobilnih nadoknada. Jednostrane kompleksne proteze sa preciznim veznim elementima nisu često u upotrebi. Radi rasvetljenja problema jednostrano slobodnog sedla i primene proteze koja ne poseduje veliku spojnicu, u ovom radu je prikazano istraživanje funkcionalnosti ove manje poznate nadoknade. Cilj rada. Cilj rada je bio da se ispitaju i uporede dobijeni naponi jednostrane kompleksne i konvencionalne parcijalne skeletirane proteze pod opterećenjem. Metode rada. U kompjuterskom programu CATIA V5 napravljeni su trodimenzionalni modeli jednostrane kompleksne proteze s odgovarajućim potpornim strukturama (retencioni zubi - očnjak i prvi premolar s namenskim krunicama, alveole i parodontalni prostor) i klasične skeletirane proteze s livenim kukicama. Radi validnosti dobijenih rezultata, modeliranje je obavljeno u prirodnoj veličini. Primenom metode konačnih elemenata izvršena je komparativna analiza vrednosti dobijenih napona pri opterećenju silama od 300 N u predelu drugog premolara, 500 N u predelu prvog molara i 700 N u predelu drugog molara. Rezultati. Proračun je dao sliku ponašanja celog modela jednostrane kompleksne parcijalne proteze s retencionim zubima i sliku celog modela klasične skeletirane parcijalne proteze koje su predstavljene u obliku napona pri različitim uslovima opterećenja. Uneta opterećenja izazvala su visok nivo napona na modelu i zubima nosačima, ali u granicama fiziološke podnošljivosti. Zaključak. Naponi na potpornim strukturama jednostrane kompleksne proteze koji su nastali usled dejstva primenjenih sila bili su u podnošljivim fiziološkim granicama. U uslovima istog opterećenja primenom metode konačnih elemenata dobijaju se veoma slične vrednosti napona jednostrane kompleksne proteze i konvencionalne skeletirane proteze.

Fixed restoration unit model, in real size, with the appropriate Supporting structures (canine and first premolar with present crowns, alveola. periodontal space) that are connected by the SD snap-in-latch attachment to the mobile portion of a partial denture. The mobile portion consists of in acrylate-coated metal base with three teeth (second premolar, first and second molars). The stress analysis, using the finite-element method, was performed Under the application of physiological loads of 25 N, 50 N. 75 N and 100 N, and excessive loads of 300 N, 500 N and 700 N in the second premolar region, as well as in the first and second molar region. The results of the analysis showed that the largest amount Of load under the application of physiological occlusal forces is positioned on the abutment teeth. Excessive forces are borne by the attachment. The stress analysis, performed on the unilateral complex partial denture model. Suggested that the obtained stress values are lower than the limit values at which the plastic deformation in the model occurs.

The effect of repeated casting on the biocompatibility of a dental gold alloy

AB  - Although highly noble Au alloys have been considered as biocompatible dental materials, little is known about whether recasting affects their biocompatibility. The aim of this work was to study the effect of repeated casting procedures on the microstructure and biocompatibilty of a noble Au dental alloy. The prepared samples of one- four- and eight times melted/cast Dentor S were used to study primary cutaneous irritation and sensibilization on experimental animals. The extract of the alloy prepared by conditioning the Dentor S samples in a cell culture medium was used to study the cytotoxicity on L929 cells. The elemental composition and surface changes of the alloy were examined by Energy Dispersive X-ray (EDX) analysis and optical microscopy, respectively. Our results showed that repeated casting decreased the biocompatibility of Dentor S, manifested as decreased metabolic and proliferative activities of L929 cells and that the effect correlated with the number of melting/casting procedures. However, neither sample of Dentor S caused irritation and sensibilization on experimental animals. EDX showed that recasting slightly increased the content of Au and Cu and slightly decreased the content of Pt, Pd and Zn. These changes correlated with the observations by optical microscopy. In conclusion, our results show that repeated casting of noble Au alloy changes its microstructure and decreases the biocompatibilty, suggesting that this procedure should be avoided in dental practice.
AB  - Visoko plemenite legure su biokompatibilni materijali, ali je malo poznato da li ponovljena livenja legura utiču na njihovu biokompatibilnost. Cilj ovog rada je bio da se utvrdi efekat ponovljenog topljenja i livenja na mikrostrukturu i biokompatibilnost visoko plemenitih dentalnih legura zlata. Pripremjena je serija uzoraka od jednom, četiri i osam puta topljene i livene Dentor S legure za ispitivanje primarne iritacije kože i senzibilizacije na eksperimentalnim životinjama, kao i in vitro ispitivanje citotoksičnosti na L929 ćelijama. Hemijski sastav i promena površine legure ispitivana je EDX analizom i optičkim mikroskopom. Naši rezultati su ukazali da ponavljana livenja smanjuju biokompatibilnost Dentor S legure, koja se manifestuje smanjenjem metabolitičke i proliferativne aktivnosti L929 ćelija i da je efekat u korelaciji s brojem topljenja i livenja. Ni jedan uzorak Dentor S legure ne uzrokuje iritaciju i senzibilizaciju eksperimentalnih životinja. EDX analiza dokazuje da ponovna livenja blago povećavaju procenat Au i Cu i blago smanjuju procenat Pt, Pd i Zn. Ove promene su u korelaciji sa zapažanjima na optičkom mikroskopu. abstract = "Although highly noble Au alloys have been considered as biocompatible dental materials, little is known about whether recasting affects their biocompatibility. The aim of this work was to study the effect of repeated casting procedures on the microstructure and biocompatibilty of a noble Au dental alloy. The prepared samples of one- four- and eight times melted/cast Dentor S were used to study primary cutaneous irritation and sensibilization on experimental animals. The extract of the alloy prepared by conditioning the Dentor S samples in a cell culture medium was used to study the cytotoxicity on L929 cells. The elemental composition and surface changes of the alloy were examined by Energy Dispersive X-ray (EDX) analysis and optical microscopy, respectively. Our results showed that repeated casting decreased the biocompatibility of Dentor S, manifested as decreased metabolic and proliferative activities of L929 cells and that the effect correlated with the number of melting/casting procedures. However, neither sample of Dentor S caused irritation and sensibilization on experimental animals. EDX showed that recasting slightly increased the content of Au and Cu and slightly decreased the content of Pt, Pd and Zn. These changes correlated with the observations by optical microscopy. In conclusion, our results show that repeated casting of noble Au alloy changes its microstructure and decreases the biocompatibilty, suggesting that this procedure should be avoided in dental practice., Visoko plemenite legure su biokompatibilni materijali, ali je malo poznato da li ponovljena livenja legura utiču na njihovu biokompatibilnost. Cilj ovog rada je bio da se utvrdi efekat ponovljenog topljenja i livenja na mikrostrukturu i biokompatibilnost visoko plemenitih dentalnih legura zlata. Pripremjena je serija uzoraka od jednom, četiri i osam puta topljene i livene Dentor S legure za ispitivanje primarne iritacije kože i senzibilizacije na eksperimentalnim životinjama, kao i in vitro ispitivanje citotoksičnosti na L929 ćelijama. Hemijski sastav i promena površine legure ispitivana je EDX analizom i optičkim mikroskopom. Naši rezultati su ukazali da ponavljana livenja smanjuju biokompatibilnost Dentor S legure, koja se manifestuje smanjenjem metabolitičke i proliferativne aktivnosti L929 ćelija i da je efekat u korelaciji s brojem topljenja i livenja. Ni jedan uzorak Dentor S legure ne uzrokuje iritaciju i senzibilizaciju eksperimentalnih životinja. EDX analiza dokazuje da ponovna livenja blago povećavaju procenat Au i Cu i blago smanjuju procenat Pt, Pd i Zn. Ove promene su u korelaciji sa zapažanjima na optičkom mikroskopu.
