Association of polymorphisms in TNF-alpha, IL-1 beta, GSTM and GSTT genes with apical periodontitis: is there a link with herpesviral infection?

AB  - Aim To investigate the possible association between TNF alpha (-308 G/A) and IL-1 beta (-511 C/T) single nucleotide polymorphisms (SNPs) and GSTT and GSTM deletion polymorphisms and risk of apical periodontitis (AP) development, and determine the association of different genotypes with the presence of herpesviral infection in AP. Methodology The study included 120 periapical lesions and 200 control samples. Gene polymorphism analysis was performed using either polymerase chain reaction (PCR) or PCR/ restriction fragment length polymorphism (RFLP). Relative gene expression of TNF-alpha and IL-1 beta was analysed using reverse transcriptase - real-time PCR. The presence of Epstein-Barr virus (EBV) and human cytomegalovirus (HCMV) was assessed by nested PCR. Chi-square and Fisher's exact tests and logistic regression analyses were done for polymorphisms, whilst Mann-Whitney U-test was performed for the expression analysis. The expected frequency of variants was analysed by the Hardy-Weinberg equilibrium test. Results TNF-alpha (-308 G/A) SNP increased AP susceptibility for heterozygous (odds ratio (OR) = 1.72, 95% confidence interval (CI) = 1.06-2.80, P = 0.027) and homozygous (OR = 8.55, 95% CI = 1.77-41.36, P  lt  0.001) carriers of the variant A allele. On the other hand, IL-1 beta (-511 C/T) polymorphism exerted a protective effect both in heterozygotes (OR = 0.540, 95% CI = 0.332-0.880, P = 0.013) and homozygotes (OR = 0.114, 95% CI = 0.026-0.501, P  lt  0.001). In addition, GSTM1 and GSTT1 null genotypes separately, as well as concomitantly, were associated with an increased risk for AP development (P  lt  0.001). The null GSTT1 genotype increased approximately twice the risk of Epstein-Barr infection (EBV) in AP (OR = 2.17, 95% CI = 1-4.71, P = 0.048), whilst TNF-alpha SNP decreased it, both in heterozygotes (OR = 0.20, 95% CI = 0.08-0.48, P  lt  0.001) and AA homozygotes (OR = 0.07, 95% CI = 0.01-0.37, P = 0.001). Conclusions GSTM and GSTT deletion polymorphisms, as well as TNF alpha (-308 G/A) SNP, are associated with increased risk, whereas IL-1 beta (-511 C/T) polymorphism decreases the risk of AP development. GSTT and TNF alpha polymorphisms also appear to modulate the risk of EBV infection in Serbian patients with AP.

How Much Country Economy Influences ECC Profile in Serbian Children-A Macro-Level Factor Analysis

AB  - Introduction: Serbia has universal health coverage (UHC) for pediatric dental care and similar country distribution for dentists and physicians per 1,000 inhabitants. However, a high prevalence of early childhood caries (ECC) with wide variation across the country was observed in previous studies. This paper aimed to analyze the association between economic and healthcare country macro-level factors with ECC prevalence and treatment. Method: The outcome variables were ECC prevalence and frequency of untreated ECC in 36- to 71-month-olds. Cross-sectional pathfinder survey on a nationally representative sample of children was conducted in order to obtain data. Independent variables included the following: gross domestic product (GDP), social and health care budget beneficiaries' expenditures, local self-government budget, unemployment rate, population density and density of physicians and dentists. Guided by the WHO's Basic Methods for Oral Health Surveys stratified cluster sample, 17 sites were randomly chosen to obtain adequate distribution of data regarding urban, peri-urban and rural areas in each analyzed statistical territorial unit. The variables were analyzed using the independent t-test or Mann-Whitney U test. A probability value of  lt 0.05 was considered significant. Results: The final sample included 864 children aged 36 to 71 months. Observed prevalence of ECC was 41.1%. Although no statistically significant difference was found, children with ECC compared to healthy children were living in parts of the country with averages of approximate to 122(sic) lower GDP per capita, approximate to 4(sic) lower social and health care expenditures per capita, 9 inhabitants per km(2) lower population density, almost 7(sic) per capita lower local self-government budget and a 0.6% higher unemployment rate. Furthermore, although without a statistically significant difference, untreated ECC was associated with approximate to 302(sic) lower GDP per capita, approximate to 12(sic) lower social and health care expenditures per capita, 34 inhabitants per km2 lower population density, almost 20(sic) per capita lower local self-government budget and a 1.7% higher unemployment rate. Conclusions: This study, performed in a nationally representative sample of preschool children, revealed the association of economic macro-level factors with ECC prevalence and its (non-) treatment. Further research on a larger sample is necessary to confirm the results. These findings suggest that most of the public-health efforts regarding prevention and early treatment of ECC should be directed at regions with lower economic performance.

AB  - Introduction: The exact mechanisms of periapical bone resorption have not been fully elucidated. This study aimed to analyze the expression of Notch signaling molecules (Notch2, Jagged1, and Hey1) and proinflammatory cytokines (tumor necrosis factor alpha [TNF-alpha], interleukin [IL]-1 beta, and IL-6) in human apical periodontitis lesions with different receptor activator of nuclear factor kappa B ligand (RANKL)/osteo-protegerin (OPG) ratios and determine their potential correlation. Methods: The study group consisted of 50 periapical lesions collected in conjunction with apicoectomy. The relative gene expression of the investigated molecules (Notch2, Jagged1, Hey1, RANKL, OPG, TNF-alpha, IL-1 beta, and IL-6) in all tissue samples was analyzed using reverse transcriptase real-time polymerase chain reaction. The Student t test, Mann-Whitney U test and Spearman correlation were used for statistical analysis. Results: Based on the RANKUOPG ratio, periapical lesions were either RANKL predominant (RANKL > OPG, n = 33) or OPG predominant (RANKL  lt  OPG, n = 17). Symptomatic lesions occurred more frequently in RANKL-predominant compared with OPG-predominant lesions (24 vs 7, P=.029). Notch2,Jagged1, Hey1, and TNF-alpha were significantly overexpressed in lesions with predominant RANKL compared with lesions with predominant OPG (P =.001, P =.001, P =.027, and P =.016, respectively). Significant correlations were observed between the investigated genes in periapical lesions. Conclusions: Notch signaling appeared to be activated in periapical inflammation. An increase in Notch2, Jagged1, Hey1, and TNF-alpha expression in RANKL-predominant periapical lesions corroborates their joined involvement in extensive periapical bone resorption.

Glutathione-S-transferase M1 polymorphism and pro-inflammatory cytokines tumour necrosis factor-alpha and interleukin-1 beta are associated with preeclampsia in Serbian women

AB  - Problem Preeclampsia has a multifactorial origin with genetic, immunological, and environmental factors described as main contributors to its onset. This study aimed to investigate glutathione-S-transferase M1 (GSTM1) and glutathione-S-transferase T1 (GSTT1) gene polymorphisms, the expression of pro-inflammatory cytokines (TNF-alpha, IL-1 beta, IL-6), and the potential relationship between GST polymorphisms and cytokine expression levels in preeclampsia and uncomplicated pregnancy. Method of Study This prospective case-control study included 50 women with preeclampsia and 50 healthy pregnant women. DNA and RNA were extracted from women leukocytes. Deletion polymorphisms were analyzed by PCR, while cytokine mRNA expression was analyzed by real-time PCR. Results GSTM1 null genotype with present GSTT1 increased the risk for preeclampsia development. Deletion of GSTT1 without deletion of GSTM1 increased the risk for early preeclampsia. Relative mRNA expression of TNF-alpha was significantly higher in preeclampsia compared to healthy pregnant women (P = 0.006). Expression of IL-1 beta was significantly higher in severe and late preeclampsia compared to the control group (P = 0.005, P = 0.007, respectively). A significant positive correlation between TNF-alpha and IL-1 beta was observed (Spearman's rho = 0.312, P = 0.028) and between IL-1 beta and IL-6, in preeclampsia group (Spearman's rho = 0.296, P = 0.037). IL-1 beta was significantly increased in patients with GSTT1 null genotype (P = 0.015) while IL-6 was increased in patients with GSTM1 null genotype (P = 0.015). Conclusions GSTM1 null genotype represents a risk factor for preeclampsia development, while GSTT1 null genotype favors early preeclampsia. Preeclampsia is also associated with increased expression of pro-inflammatory cytokines, predominantly TNF-alpha and IL-1 beta.

Influence of nanostructured calcium aluminate and calcium silicate on the liver: histological and unbiased stereological analysis

AB  - Aim To examine the potential systemic toxicity of nanostructured materials based on calcium silicate and calcium aluminate, for potential application in Dentistry. Methodology Twenty-four Albino Wistar rats aged 2 months were used as an in vivo animal model for subcutaneous implantation of the investigated materials, placed in polyethylene tubes. Thirty days after implantation, the livers of the rats were analysed and following histological and stereological parameters were evaluated for volume density of hepatocytes and blood sinusoids, number and numerical density of hepatocytes, surface of hepatocytes and their nucleuses, nucleocytoplasmic ratio and mitotic index of hepatocytes. Stereological measurements were achieved using Cavalieri's principle, with grid P2 and unbiased analysis. Additionally, immunohistochemistry studies were performed to further analyse changes in liver tissue. Several haematological and biochemical parameters of blood of experimental animals were also analysed, as well as local tissue reactions around the implants. Statistical analysis was performed using parametric (anova and t-test) and nonparametric tests (Kruskal-Wallis and Mann-Whitney U-test) depending on data distribution. Results Implanted dental cements led to an increase in stereological and histological parameters in liver tissue compared to control rats. Although the investigated parameters mostly showed significant differences between control and experimental animals, the liver tissue of the experimental animals did not have visible signs of pathological changes. This was supported by the analysis of blood parameters which were not significantly different between control and experimental animals. Also, the subcutaneous tissues had minimal inflammatory reactions. Immunohistochemistry studies revealed that nanostructured materials induced proliferation of hepatocytes, but that the immunological response to the materials was not strong enough to induce proliferation of immunoreactive cells in liver in the observed time period. Conclusions This study was performed as a contribution to the attestation of the biocompatibility of dental cements based on calcium silicate and calcium aluminate. Although these materials induced several changes in the liver structure, they were not clinically relevant and represent a normal and reversible response of the liver to the presence of biocompatible materials in the body. Blood and immunohistochemistry analyses and local tissue reactions further confirmed that these materials possess good biocompatible potential.

AB  - ObjectivesThis study aimed to analyse Epstein-Barr virus (EBV) and human cytomegalovirus (HCMV) viral loads in symptomatic and asymptomatic apical periodontitis lesions, to determine levels of TNF-, IL-1 and IL-6 in these lesions and to investigate a possible correlation between herpesviral copy numbers and levels of proinflammatory cytokines. Materials and MethodsA total of 100 samples of apical periodontitis were subjected to HCMV and EBV copy numbers analysis by nested polymerase chain reaction (PCR) and TaqMan real-time PCR. The concentrations of TNF-, IL-1 and IL-6 were determined by ELISA method. SPSS software was used for statistical analysis. ResultsThere were no significant differences in the occurrence of EBV and HCMV between symptomatic and asymptomatic periapical lesions (p=.686, p=.879, respectively). Only 12 of 74 EBV (16.2%) and four of 54 HCMV (13.5%) nested PCR-positive samples showed increased viral copy numbers above the limit of 125 copies/ml. There was no significant correlation between the levels of analysed proinflammatory cytokines and herpesviral copy numbers in our sample. ConclusionThe observed low viral loads point to a relatively rare occurrence of active EBV and HCMV infection in our sample. Latent herpesviral infection does not enhance the production of investigated proinflammatory cytokines.

MethodologyThe experimental group consisted of 30 EBV-positive and 30 EBV-negative periapical lesions collected in conjunction with apicoectomy. The pulp tissues of 20 impacted third molars were used as healthy controls. The qualitative and quantitative analysis of EBV was performed by nested and real-time polymerase chain reaction (PCR), respectively. The levels of RANKL and OPG were analysed by reverse transcriptase real-time PCR. The levels of 8-OHdG and GSSG were determined by enzyme-linked immunosorbent assay (ELISA). Mann-Whitney U-test and Spearman's correlation were used for statistical analysis. ResultsThe levels of RANKL, OPG, 8-OHdG and GSSG were significantly higher in apical periodontitis lesions compared to healthy pulp controls (P=0.001, P lt 0.001, P lt 0.001 and P lt 0.05, respectively). RANKL and OPG mRNA expression was significantly higher in EBV-positive compared to EBV-negative periapical lesions (P lt 0.05). On the other hand, the calculation of continuous MS scores using standardized residuals in linear regression (Z scores) or factor scores of principal component analysis (PCA) is highly impractical for clinical use. Recently, a novel, easily calculated continuous MS score called siMS score was developed based on the IDF MS criteria for the adult population. Objective To develop a Pediatric siMS score (PsiMS), a modified continuous MS score for use in the obese youth, based on the original siMS score, while keeping the score as simple as possible and retaining high correlation with more complex scores. Subjects and methods The database consisted of clinical data on 153 obese (BMI >= 95th percentile) children and adolescents. Continuous MS scores were calculated using Z scores and PCA, as well as the original siMS score. Four variants of PsiMS score were developed in accordance with IDF criteria for MS in youth and correlation of these scores with PCA and Z score derived MS continuous scores was assessed. Results PsiMS score calculated using formula: (2xWaist/Height) + (Glucose(mmol/l)/5.6) + (triglycerides( mmol/l)/1.7) + (Systolic BP/130)-(HDL(mmol/l)/1.02) showed the highest correlation with most of the complex continuous scores (0.792-0.901). The original siMS score also showed high correlation with continuous MS scores. Conclusion PsiMS score represents a practical and accurate score for the evaluation of MS in the obese youth. The original siMS score should be used when evaluating large cohorts consisting of both adults and children.

AB  - This study examined the potential of a new porous calcium hydroxyapatite scaffold covered with poly (lactide-co-glycolide) (PLGA) as a bone substitute, identifying its advantages over Geistlich Bio-Oss (R), considered the gold standard, in in vivo biofunctionality investigations. Structural and morphological properties of the new scaffold were analyzed by scanning electron and atomic force microscopy. The biofunctionality assays were performed on New Zealand white rabbits using new scaffold for filling full-thickness defects of critical size. The evaluated parameters were: the presence of macrophages, giant cells, monoocytes, plasma cells, granulocytes, neo-angiogenesis, fibroplasia, and the percentage of mineralization. Parallel biofunctionality assays were performed using Geistlich Bio-Oss (R). The appearance of bone defects 12 weeks after the new scaffold implantation showed the presence of a small number of typical immune response cells. Furthermore, significantly reduced number of capillary buds, low intensity of fibroplasia and high degree of mineralization in a lamellar pattern indicated that the inflammation process has been almost completely overcome and that the new bone formed was in the final phase of remodeling. All biofunctionality assays proved the new scaffold's suitability as a bone substitute for applications in maxillofacial surgery. It showed numerous biological advantages over Geistlich Bio-Oss (R) which was reflected mainly as a lower number of giant cells surrounding implanted material and higher degree of mineralization in new formed bone.

AB  - Purpose: Survivin is thought to play an important role in carcinogenesis and is found to be associated with poor clinical outcome in various malignancies. Gene-31 G/C polymorphism has been identified as a risk factor for the development of several types of tumors. The purpose of this study was to investigate the association between survivin gene promoter-31C/G polymorphism and urothelial carcinoma (UC) risk in Serbian population and to compare the different expressions of survivin in UC of different disease stages, histological grades and tumor location in the upper or lower urinary tract. Methods: DNA from 94 patients with primary UC and from 82 healthy subjects was subjected to PCR restriction fragment length polymorphism analysis (PCR-RFLP) to identify individual genotypes. UC samples were subjected to immunohistochemical analysis to assess survivin expression in these lesions. Results: It was observed that the frequency of G/G genotype was greater in patients with UC (58.7%) than in controls (32%). Compared with study subjects carrying the C/G or C/C genotypes, significantly increased UC risk was found for individuals carrying the G/G genotype. Those carrying the G/G genotype had a significantly increased UC risk compared with those with C/G or C/C genotypes. Patients with UC carrying the G/G genotype had a greater prevalence of muscle -invading (stage T2-T4), high-grade (G2) tumor and immunohistochemicaly overexpressed survivin compared with those carrying the C/G or C/C genotypes. Conclusions: G/G genotype of the -31C/G polymorphism might be a risk factor for UC development.

After-Hours Versus Office-Hours Dental Injuries in Children: Does Timing Influence Outcome?

AB  - Aim. The aim of this study was to analyze the outcomes and factors associated with after-hours dental trauma. Methods. Study sample consisted of 1762 permanent teeth injuries in children, gender and age matched with office-hours injuries. Epidemiological and clinical data were collected from 4 university dental trauma centers. Results. During median follow-up time of 4.3 years, complications have occurred in 14.5% of injured teeth. Age, type, and degree of tissue injury and after-hours time of injury were significantly associated with complications. Unfavorable outcomes were 34% more likely in the after-hours group compared with office-hours. Urgent treatment was significantly delayed in after-hours group with a delay of more than 3 hours in 90.5% versus 38.9% in the office-hours group. Multivariate regression model showed that after-hours time of injury was significant predictor of complications. Conclusion. Delayed urgent treatment was one of the main factors associated with unfavorable outcome of after-hours injuries.

AB  - ObjectivesThe aim of this study was to investigate survivin, cyclin D1, and p21hras expression in keratocystic odontogenic tumors before and after decompression, as well as in pericoronal follicles. A potential correlation between the expression levels of these proteins was also investigated. Materials and methodsWe analyzed eighteen keratocystic tumors treated by decompression and subsequent enucleation along with seven pericoronal follicles using immunohistochemistry. ResultsKeratocystic tumor samples, both before and after decompression, were positive for each of the investigated proteins. In pericoronal follicles, survivin exhibited cytoplasmic staining in contrast to nuclear staining in keratocystic tumors. Cyclin D1 expression was negative in pericoronal follicles, and p21hras expression was similar in both groups. Survivin showed significantly higher expression after decompression, while cyclin D1 and p21hras remained unchanged (P=0.039, P=0.255, P=0.913, respectively). There was no correlation between these proteins neither before nor after decompression. ConclusionsWithin the limits of the study, we can conclude that following decompression, keratocystic odontogenic tumors preserve distinct immunohistochemical profiles of cyclin D1 and p21hras expression, despite substantial reduction in size of the lesions. Significant increase of survivin expression after decompression might be attributed to higher level of epithelial proliferation caused by this procedure.

A potential correlation between the expression levels of these proteins was also investigated. Materials and methodsWe analyzed eighteen keratocystic tumors treated by decompression and subsequent enucleation along with seven pericoronal follicles using immunohistochemistry. ResultsKeratocystic tumor samples, both before and after decompression, were positive for each of the investigated proteins. In pericoronal follicles, survivin exhibited cytoplasmic staining in contrast to nuclear staining in keratocystic tumors. Cyclin D1 expression was negative in pericoronal follicles, and p21hras expression was similar in both groups. Survivin showed significantly higher expression after decompression, while cyclin D1 and p21hras remained unchanged (P=0.039, P=0.255, P=0.913, respectively). There was no correlation between these proteins neither before nor after decompression. ConclusionsWithin the limits of the study, we can conclude that following decompression, keratocystic odontogenic tumors preserve distinct immunohistochemical profiles of cyclin D1 and p21hras expression, despite substantial reduction in size of the lesions. Significant increase of survivin expression after decompression might be attributed to higher level of epithelial proliferation caused by this procedure.",

Efficiency of photodynamic therapy in the treatment of peri-implantitis: A three-month randomized controlled clinical trial

AB  - Introduction Peri-implantitis is an inflammatory lesion of peri-implant tissues. Eradication of the causative bacteria and decontamination of the implant surface is essential in achieving predictable and stabile clinical results. Photodynamic therapy (PDT) is non-invasive adjuvant therapeutic method to surgery in the treatment of bacterial infection. Objective The aim of this study was to evaluate early clinical and microbiological outcomes of periimplantitis after surgical therapy with adjuvant PDT. Methods Fifty-two diagnosed peri-implantitis sites were divided into two groups. PDT was used for decontamination of implant surface in the study group; in the control group, chlorhexidine gel (CHX) followed by saline irrigation was applied. Several clinical parameters were recorded before the treatment (baseline values) and three months after surgical treatment. Samples for microbiological identification were collected before therapy, during the surgical therapy (before and after decontamination of implant surface), and three months thereafter, and analyzed with identification systems using biochemical analysis. Results The use of PDT resulted in significant decrease of bleeding on probing in comparison to CHX (p  lt  0.001). It showed significant decontamination of implant surfaces with complete elimination of anaerobic bacteria immediately after surgical procedure and three months later. Conclusion The results indicate that PDT can be used as an adjuvant therapy to surgery for decontamination of implant surface and surrounding peri-implant tissues within the treatment of peri-implantitis.
AB  - Uvod Periimplantitis je inflamatorni proces koji zahvata meka tkiva i potpornu kost oko oseointegrisanog implantata. Eliminacija patogenih mikroorganizama i dekontaminacija implantne površine predstavlju najbitniji korak u postizanju stabilnih kliničkih rezultata. Fotodinamska terapija (FDT) predstavlja dodatni neinvazivni metod u terapiji bakterijskih infekcija. Cilj rada Cilj rada bila je procena kliničkih i mikrobioloških parametara nakon hirurške terapije periimplantitisa uz dodatnu primenu FDT. Metode rada Sva dijagnostikovana mesta periimplantitisa (n = 52) bila su podeljena u dve grupe: u studijskoj grupi, za dekontaminaciju implantne površine tokom hirurške procedure korišćena je FDT; u kontrolnoj grupi, za dekontaminaciju implantne površine korišćen je hlorheksidin u gelu (CHX). Klinički parametri praćeni su pre terapijske procedure i tri meseca posle terapije. Uzorci za mikrobiološku analizu uzimani su pre i tri meseca posle terapije, kao i tokom hirurške procedure, pre i posle dekontaminacije implantne površine. Za identifikaciju izolovanih anaeroba korišćen je sistem koji radi po principu biohemijske analize izolovanih mikrobioloških sojeva. Rezultati studije su pokazali da primenom FDT dolazi do znatne redukcije krvarenja na provokaciju u poređenju sa primenom CHX (p  lt  0,001). Primena FDT, kao pomoćnog terapijskog sredstva, omogućava potpunu eliminaciju anaerobnih bakterija sa implantne površine. Zaključak Rezultati pokazuju da FDT može da se koristi kao pomoćno terapijsko sredstvo za dekontaminaciju implantne površine i periimplantnog tkiva u okviru terapije periimplantitisa.

Association Between Dentition Status and Malnutrition Risk in Serbian Elders

AB  - Purpose: The aim of this study was to find the correlation between the number, type, and placement of symmetric functional tooth units and the nutritional status using body mass index (BMI) and Mini Nutritional Assessment (MNA) in patients with both maxillary and mandibular removable partial dentures. Materials and Methods: A randomized study was conducted at the Clinic for Prosthodontics, School of Dentistry, University of Belgrade, Serbia, followed by determination of number of functional tooth units. After insertion of the prostheses, the nutritional status was assessed, using BMI and MNA form. Nutritional status was reviewed 12 months after prosthetic treatment. Number of teeth and age were analyzed using analysis of variance and t test. For other variables such as BMI baseline, BMI follow-up, MNA baseline, MNA follow-up, change in BMI, and change in MNA, Kruskal-Wallis Test or Mann-Whitney Test were used. Results: The results on the MNA changes showed that patients with symmetric natural functional tooth units had significantly higher values (2.9 +/- 0.8) compared with patients without natural functional tooth units (1.9 +/- 0.6) or with asymmetric natural functional tooth units (1.6 +/- 0.4). Patients with asymmetric artificial functional tooth units had significantly lower change in MNA (1.9 +/- 0.4) than those with symmetric artificial functional tooth units (2.6 +/- 0.9). Conclusion: The findings of this study suggest that adequate rehabilitation with symmetric positioning of the functional tooth units can improve the nutritional status of partially edentulous patients.

AB  - Objective: To assess the prevalence of metabolic syndrome (MS) in obese children and adolescents in Serbia. Subjects and methods: The study group consisted of 254 subjects (148 female and 106 male), aged 4.6-18.9 years with diet-induced obesity (body mass index >= 95th percentile). Presence of MS using the International Diabetes Federation definition was assessed in all subjects, as well as oral glucose tolerance test and insulin resistance indices. Results: Overall prevalence of MS in all subjects aged >= 10 years was 31.2%, namely, 28.7% in children aged 10 to  lt 16 years and 40.5% in adolescents >= 16 years. When adjusted for age, gender and pubertal development, higher degree of obesity was a strong predictor of MS. Multivariate analysis showed that taller subjects and those with higher degree of insulin resistance were at significantly higher risk of MS, independent of the degree of obesity. Conclusions: High prevalence of MS emphasizes the need for prevention and treatment of childhood obesity.

Radiographic assessment of lower third molar eruption in different anteroposterior skeletal patterns and age-related groups

AB  - Objective: To analyze radiographic predictors for lower third molar eruption among subjects with different anteroposterior skeletal relations and of different age groups. Materials and Methods: In total, 300 lower third molars were recorded on diagnostic digital orthopantomograms (DPTs) and lateral cephalograms (LCs). The radiographs were grouped according to sagittal intermaxillary angle (ANB), subject age, and level of lower third molar eruption. The DPT was used to analyze retromolar space, mesiodistal crown width, space/width ratio, third and second molar angulation (alpha, gamma), third molar inclination (beta), and gonion angle. The LC was used to determine ANB, angles of maxillar and mandibular prognathism (SNA, SNB), mandibular plane angle (SN/MP), and mandibular lengths. A logistic regression model was created using the statistically significant predictors. Results: The logistic regression analysis revealed a statistically significant impact of 6 angle and distance between gonion and gnathion (Go-On) on the level of lower third molar eruption (P  lt  .001 and P  lt  .015, respectively). The retromolar space was significantly increased in the adult subgroup for all skeletal classes. The lower third molar impaction rate was significantly higher in the adult subgroup with the Class II (62.3%) compared with Class III subjects (31.7%; P  lt  .013). Conclusion: The most favorable values of linear and angular predictors of mandibular third molar eruption were measured in Class III subjects. For valid estimation of mandibular third molar eruption, certain linear and angular measures (beta angle, Go-On), as well as the size of the retromolar space, need to be considered.

The radiographs were grouped according to sagittal intermaxillary angle (ANB), subject age, and level of lower third molar eruption. The DPT was used to analyze retromolar space, mesiodistal crown width, space/width ratio, third and second molar angulation (alpha, gamma), third molar inclination (beta), and gonion angle. The LC was used to determine ANB, angles of maxillar and mandibular prognathism (SNA, SNB), mandibular plane angle (SN/MP), and mandibular lengths. A logistic regression model was created using the statistically significant predictors. Results: The logistic regression analysis revealed a statistically significant impact of 6 angle and distance between gonion and gnathion (Go-On) on the level of lower third molar eruption (P  lt  .001 and P  lt  .015, respectively). The retromolar space was significantly increased in the adult subgroup for all skeletal classes. The lower third molar impaction rate was significantly higher in the adult subgroup with the Class II (62.3%) compared with Class III subjects (31.7%; P  lt  .013). Conclusion: The most favorable values of linear and angular predictors of mandibular third molar eruption were measured in Class III subjects. For valid estimation of mandibular third molar eruption, certain linear and angular measures (beta angle, Go-On), as well as the size of the retromolar space, need to be considered.",

In both periapical lesions and controls, EBV-1 occurred more often compared with EBV-2 (P = .001). Dual EBV and HCMV coinfection was more frequently detected in large-size periapical lesions (P = .038). Conclusions: Both HCMV and EBV are associated with inflammatory processes of periapical bone destruction. HCMV gB type II and EBV-1 are the most prevalent genotypes in apical periodontitis lesions.

Based on clinical data and history of disease, 100 human apical periodontitis lesions were categorised as either asymptomatic or symptomatic lesions. According to histological examination, lesions were divided into periapical granulomas and radicular cysts. Pulp tissues of 25 impacted wisdom teeth were used as controls. Homogenised tissue samples were centrifuged and supernatants were used for the determination of cytokine levels by enzyme-linked immunosorbent assay. Significantly higher levels of IL-1 and IL-6 were found in symptomatic lesions compared with asymptomatic lesions and control tissues (P lt 0.001, P lt 0.001, respectively). The concentration of IL-1 was significantly higher in radicular cysts compared with periapical granulomas (P=0.003). Symptomatic lesions, as judged by high local production of IL-1 and IL-6, represent an immunologically active stage of the disease.

Factors associated with positive outcome of avulsion injuries in children

AB  - Introduction/Aim. Avulsions are severe dental injuries with high impact on patients' quality of life due to prolonged treatment and possible severe complications. The aim of the study was to analyze the epidemiological factors related to the occurrence, treatment and outcome of avulsions in Serbian children. Methods. This research included 2,194 patients aged 1-18 years with traumatic dental injuries. The history, demographic, clinical and radiographic data of the patients were observed from dental medical records. Results. A total of 266 avulsions were observed in 207 patients. The frequency of avulsions was 12% in primary dentition and 5% in permanent dentition. A statistically significant relationship between place, cause of avulsion and outcome was observed. Replantation of permanent teeth was performed in 46.7% with a mean time 6.9 h. Almost a half of permanently avulsed teeth (48.7%) were not retrieved from the accident site and 11.3% of replanted teeth were transported in adequate media. The observed incidence of complications was 31.9%. Conclusions. Re- plantation was not performed in more than a half of all eligible teeth due to either loss or inadequate/delayed transport, which emphasizes the need for preventive strategies and health education in population.
AB  - Uvod/Cilj. Avulzije spadaju među najteže povrede zuba kod dece i utiču na njihov psihosocijalni razvoj zbog dugotrajne terapije i mogućih teških komplikacija. Cilj istraživanja bio je analiza epidemioloških faktora povezanih sa nastankom, terapijom i ishodom avulzija kod dece u Srbiji. Metode. Ovo istraživanje obuhvatilo je 2 194 ispitanika uzrasta 1-18 godina sa povredama zuba. Podaci su dobijeni iz kartona povreda i stomatoloških kartona, a obuhvatali su: anamnestičke podatke, demografske podatke, podatke iz kliničkog i radiološkog pregleda. Rezultati. Zabeleženo je ukupno 266 avulzija kod 207 ispitanika. Učestalost avulzija u mlečnoj denticiji iznosila je 12%, a u stalnoj 5%. Uočena je statistički značajna povezanost između mesta, uzroka povrede i ishoda avulzije. Replantacija stalnih zuba je izvršena kod 46,7% ispitanika sa prosečnim vremenom 6,9 h. Skoro polovina svih avulziranih stalnih zuba (48,7%) nije donešena sa mesta povrede, dok je svega 11,3% replantiranih zuba donešeno u adekvatnom medijumu. Učestalost komplikacija iznosila je 31,9%. Zaključak. AB  - In numerous recent papers plasma chemistry of non equilibrium plasma sources operating at atmospheric pressure has been linked to plasma medical effects including sterilization. In this paper we present a study of the effectiveness of an atmospheric pressure plasma source, known as plasma needle, in inhibition of the growth of biofilm produced by methicillin resistant Staphylococcus aureus (MRSA). Even at the lowest powers the biofilms formed by inoculi of MRSA of 10(4) and 10(5) CFU have been strongly affected by plasma and growth in biofilms was inhibited. The eradication of the already formed biofilm was not achieved and it is required to go to more effective sources.

Previous studies of HRQOL in elderly heart failure subjects have included mostly outpatients, and little is known about determinants of HRQOL in hospitalized elderly population, especially in Serbia. In this study, we tried to identify factors that influence HRQOL in elderly patients hospitalized with chronic heart failure in Serbia. Methods: The study population consisted of 136 patients aged 65 years or older hospitalized for chronic heart failure. HRQOL was assessed using the Minnesota Living with Heart Failure questionnaire. Predictors of HRQOL were identified by multiple linear regression analysis. Results: Univariate analysis showed that patients with lower income, a longer history of chronic heart failure, and longer length of hospital stay, as well as those receiving aldosterone antagonists and digoxin, taking multiple medications, in a higher NYHA class, and showing signs of depression and cognitive impairment had significantly worse HRQOL. Presence of depressive symptoms (P lt 0.001), higher NYHA class (P=0.021), lower income (P=0.029), and longer duration of heart failure (P=0.049) were independent predictors of poor HRQOL. Conclusion: Depressive symptoms, higher NYHA class, lower income, and longer duration of chronic heart failure are independent predictors of poor HRQOL in elderly patients hospitalized with chronic heart failure in Serbia. Further, there is an association between multiple medication usage and poor HRQOL, as well as a negative impact of cognitive impairment on HRQOL. Hence, measures should be implemented to identify such patients, especially those with depressive symptoms, and appropriate interventions undertaken in order to improve their HRQOL.
