AB  - Introduction/Objective The pain that originates from musculoskeletal structures of the mastication system is one of the symptoms belonging to the category of temporomandibular disorders or temporomandibular dysfunction (TMD). The objective of the research was to evaluate the effect of therapy with stabilizing occlusal splint in the control of painful symptoms of TMD in comparison with the effect of drug therapy. Methods Using standard Research Diagnostic Criteria for Temporomandibular Disorders diagnostic protocol proposed by Dworkin and LeResche, a group of 44 patients with painful TMD was included. The patients were divided into three treatment groups by random selection. The first group was treated with stabilization occlusal splint for a period of one month. In the two control groups, therapy with non-steroidal anti-inflammatory drug ibuprofen (Brufen, Mylan, Canonsburg, PA, USA) or a combination therapy of ibuprofen and diazepam, a medication from the benzodiazepine family (Diazepam, Hemofarm, Vršac, Serbia) was carried out over a period of three weeks. In order to assess the effects of the therapy with stabilizing occlusal splint and the drug therapy, before and after the therapy, pain intensity measurements were performed with visual analogue scale and digital pressure algometer. Results A significant reduction in the intensity of painful symptoms was achieved in all three therapeutic groups. No significant differences in the effectiveness of pain reduction between the proposed therapeutic modalities were noted. Conclusion The obtained results confirm that the therapy with stabilization occlusal splint is a valid procedure in the reduction of pain in patients with TMD.
AB  - Uvod/Cilj Bol porekla mišićno-skeletnih struktura mastikatornog sistema predstavlja jedan od simptoma koji pripadaju kategoriji temporomandibularnih poremećaja ili temporomandibularnih disfunkcija (TMD). Cilj istraživanja je bio da se proceni efekat terapije stabilizacionim okluzalnim splintom u kontroli bolnih simptoma TMD u poređenju sa efektom terapije lekovima. Metode Korišćenjem standardnog dijagnostičkog protokola (RDC/TMD) predloženog od strane Dvorkina i Lerešea, izdvojena je grupa od 44 bolesnika sa bolnim temporomandibularnim disfunkcijama. Bolesnici su podeljeni u tri terapijske grupe slučajnim izborom. Prva grupa je podvrgnuta terapiji stabilizacionim okluzalnim splintom u periodu od mesec dana. U dve kontrolne grupe je sprovedena terapija nesteroidnim antiinflamatornim lekom ibuprofenom (brufen, Mylan) ili kombinacijom ibuprofena i leka iz grupe benzodiazepina - diazepama (diazepam, Hemofarm) u periodu od tri nedelje. U cilju procene efekata terapije stabilizacionim okluzalnim splintom i terapije lekovima, pre i posle sprovedene terapije izvedena su merenja intenziteta bola vizuelnom analognom skalom i digitalnim pritisnim algometrom. Zaključak Dobijeni rezultati potvrđuju da je terapija stabilizacionim okluzalnim splintom validna procedura u smanjenju bola kod bolesnika sa TMD.

Biological complications of the cement-retained implantsupported restorations

AB  - Dental cements have been established in practice since 1878, and technological advances in the field of implant pros-thetics have directly influenced the development of materials in this area. Although the retention type of implant-supported restorations can be dual, it is nowadays known that the overall rate of technical complications is higher for cement-retained restorations by 2% over screw-retained restorations. Biological complications, including peri-implant mucositis, periimplantitis, bone loss around implants, soft-tissue recession and fistula suppuration, are also more frequent with cement-retained restorations. The most common of these, periimplantitis is often related to factors depending on a clinician.
AB  - Materijali za fiksiranje zubnih protetskih nadoknada utemeljeni su u stomatološkoj praksi od 1878. godine, a tehnološki napredak na polju implant-protetike direktno je uticao i na razvoj materijala u ovoj oblasti. Iako mehanizam vezivanja fiksnih nadoknada može biti dvojak, danas se zna da je ukupna stopa tehničkih komplikacija veća kod cementno retiniranih nadoknada za 2% u odnosnu na retenciju šrafom. Biološke komplikacije u koje se ubrajaju periimplantatni mukozitis, periimplantitis, gubitak kosti oko implantata, recesija mekih tkiva i pojava fistule, takođe su učestalije kod cementno retiniranih zubnih nadoknada. Najzastupljenija među njima, periimplantitis, često se dovodi u vezu sa "lekar-zavisnim" parametrima.

Stress assessment in patients with clinically diagnosed sleep bruxism

AB  - Background/Aim. Many studies investigated association between stress, anxiety or personality traits and sleep bruxism (SB), but results are still contradictory. We aimed to investigate whether there is a relation between clinically diagnosed sleep bruxism and salivary cortisol levels as one of the major stress biomarkers and to examine psychological factors and personality traits specific to sleep bruxism. Methods. A total of 23 sleep bruxism patients and 42 healthy non-sleep bruxism adults participated in this study. Diagnose of sleep-bruxism was assessed by self-report and clinical examination and also confirmed by bed-partner. Morning saliva was collected from all participants for analyses of the cortisol level. Sleep bruxism patients underwent a psychodiagnostic personality interpretation using the Minnesota Multiphasic Personality Inventory - MMPI-202 test. Results. Statistically significant difference between levels of morning salivary cortisol in the group of SB patients and the control group was recorded (t = 2.943, p  lt  0.01). Analysis of the personality profiles indicated that the sleep bruxism patients avoid contact with unpleasant feelings, especially depression, suppress the aggression and censor the expression of anger and rage. Conclusion. This study showed that patients with sleep bruxism have higher levels of salivary cortisol. Personality traits such as depression, hypomania and suppressed aggression were found to be common characteristics in patients with sleep bruxism. Present findings might support the hypothesis that sleep bruxism and psychological states such as stress may be related, but the cross-sectional nature of this study does not allow us to draw conclusions about the causal relationship between stress, personality traits and sleep bruxism.
AB  - Uvod/Cilj. Mnoge studije ispitivale su povezanost između stresa, anksioznosti, crta ličnosti i bruksizma, ali su rezultati i dalje kontradiktorni. Cilj ove studije bio je da se ispita da li postoji povezanost između klinički dijagnostikovanog bruksizma koji se javlja tokom spavanja i koncentracije kortizola u salivi koja se smatra jednim od glavnih biomarkera stresa, kao i da se ispitaju psihološki faktori i crte ličnosti osoba sa bruksizmom. Metode. Studija je obuhvatila 23 bolesnika sa bruksizmom i 42 zdrava ispitanika bez znakova bruksizma. Dijagnoza bruksizma uspostavljena je na osnovu spostvene izjave i kliničkog pregleda kao i potvrde partnera. Uzorci jutarnje pljuvačke prikupljani su radi analize kortizola. Bolesnici sa bruksizmom podvrgnuti su psihodijagnostičkom ispitivanju pomoću Minesota multifaznog testa ličnosti - MMPI-202. Rezultati. U grupi bolesnika sa bruksizmom registrovana je značajno veća koncentracija kortizola u pljuvačci u poređenju sa pacijentima bez znakova bruksizma (t = 2.943, p  lt  0.01). Psihodijagnostička interpretacija profila ličnosti pokazala je da ispitanike sa bruksizmom karakteriše izbegavanje kontakta sa neprijatnim osećanjima, posebno depresijom i potiskivanje agresivnosti, odnosno izražena autocenzura na otvoreno ispoljavanje ljutnje i besa. Zaključak. Kod bolesnika sa bruksizmom registrovane su povećane koncentracije kortizola u salivi. Psihodijagnostička analiza ukazuje da su depresija, hipomanija i supresija agresije izražene kod bolesnika sa bruksizmom. Nalazi bi mogli da podrže hipotezu povezanosti bruksizma u toku spavanja i stresa, ali priroda ove studije ne dozvoljava izvođenje zaključaka o uzročnoj povezanosti stresa, crta ličnosti i bruksizma u toku spavanja.

AB  - Wear resistance is one of the most important physical properties of the artificial teeth used in acrylic dentures. The goal of this research was to synthesize a new composite material made of matrix Poly-(methyl methacrylate)-PMMA with different percentages (2 % and 3 % of volume fractions) of zinc-oxide nanoparticles (ZnO NPs) as reinforcing elements, to improve its mechanical properties. The dynamic mechanical behaviour of this composite was studied through the DMA method in comparison to the pure PMMA supported by the characterization of their microstructures. Then the wear resistance was analysed on the samples, which were prepared in the form of teeth. In this context their vertical height loss was measured after 100,000 chewing cycles on a chewing simulator, before and after the artificial thermal ageing. Investigations showed that the PMMA/ZnO NP composites dampened the vibrations better than the pure PMMA, which could be assigned to the homogenous distribution of ZnO NPs in the PMMA matrix. It was found that the mean vertical height loss for the pure PMMA teeth was significantly higher (more than 4 times) compared to composite teeth made with ZnO NPs. Introducing the thermal artificial ageing led to the finding that there was no effect on the height loss by the composite material with 3 % of volume fractions of ZnO NPs. Based on this it was concluded that PMMA/ZnO NPs composites showed improved in-vitro wear resistance compared to acrylic-resin denture teeth, so this new composite material should be preferred when occlusal stability is considered to be of high priority.

AB  - Nasal defects can occur as a result of head and neck trauma, or due to partial or complete ablation or resection of the tumor in the nasal area and surroundings. Smaller defects can be reconstructed surgically while large defects are mainly reconstructed combined surgically and prosthetically. The aim of this paper was to present prosthetic reconstruction of the nasal defect by colored vinyl polysiloxane prosthesis retained with craniofacial basal disc implants. An adequate aesthetics and stability of the prosthesis was achieved during mandible and mimic muscles movements.
AB  - Defekti nosa mogu nastati kao posledica traume glave i vrata ili usled delimične ili potpune ablacije, odnosno resekcije tumora predela nosa i okoline. Manji defekti nosa se rekonstruišu hirurški, dok se veliki defekti uglavnom rekonstruišu kombinovano, kako hirurški tako i protetski. Cilj ovoga rada bio je da se prikaže protetska rekonstrukcija defekta nosa protezom izrađenom od vinil-polisiloksana u boji sa retencijom preko kraniofacijalnih bazalnih disk-implantata. Ovakvom protetskom nadoknadom postignute su zadovoljavajuća estetika i stabilnost proteze pri pokretima mandibule i mimičnih mišića.

Characterisation of NiTi orthodontic archwires characteristic functional properties

AB  - The aim of this paper was to analyse the characteristic properties of different commercially available NiTi archwires (diameter 0.014 ''). Characteristic properties were determined by using DSC analysis for identification of phase temperatures and a tensile test for obtaining the mechanical properties, while semi-quantitative EDS analysis was used for their chemical composition. Analysis of phase temperatures showed that nickel- titanium orthodontic archwires, in the austenitic phase, were exhibiting superelastic effect in the oral environment (T=37 degrees C). The uniaxial stress-strain curves showed different values for the beginning and the end of loading due to the stress and transformation range. The investigation of the chemical composition showed an equiatomic NiTi alloy. The aim of the first phase of biocompatibility assessment of Ni-Ti archwires was to investigate the potential cytotoxicity on a model of rat thymocytes, where a direct contact exists between the archwire and the cells. The test was chosen because of the high sensitivity of thymocytes to proapoptotic stimuli and because the test includes the culture of non-adherent, non-proliferating cells, such as thymocytes, which is much more convenient for the evaluation of a material's cytotoxicity in direct contact, compared to the adherent cells such as fibroblasts. The experiment was designed to cultivate the Ni-Ti archwire samples with rat thymocytes, using different surface-over-volume ratios of the archwires in complete medium. The cytotoxic effect was evaluated by measuring the apoptosis. The aim of the second phase of the study was to investigate further the cytotoxic effects of Ni-Ti archwires in the short-term (24h) and long-term (48h) cultures of rat's thymocytes, which are highly sensitive to pro-apoptotic stimuli, where a direct contact exists between archwires and the cells.

Occlusal stabilization splint for patients with temporomandibular disorders: Meta-analysis of short and long term effects

AB  - Background Psychological discomfort, physical disability and functional limitations of the orofacial system have a major impact on everyday life of patients with temporomandibular disorders (TMDs). In this study we sought to determine short and long term effects of stabilization splint (SS) in treatment of TMDs, and to identify factors influencing its efficacy. Methods MEDLINE, Web of Science and EMBASE were searched for randomized controlled trials (RCTs) comparing SS to: non-occluding splint, occlusal oral appliances, physiotherapy, behavioral therapy, counseling and no treatment. Random effects method was used to summarize outcomes. The effect estimates were expressed as odds ratio (OR) or standardized mean difference (SMD) with 95% confidence interval. Subgroup analyses were carried out according to the use of Research Diagnostic Criteria (RDC/TMD) and TMDs origin. Strength of evidence was assessed by GRADE. Meta-regression was applied. Results Thirty three eligible RCTs were included in meta-analysis. In short term, SS presented positive overall effect on pain reduction (OR 2.08; p = 0.01) and pain intensity (SMD -0.33; p = 0.02). Subgroup analyses confirmed SS effect in studies used RDC/TMD and revealed its effect in patients with TMDs of muscular origin. Important decrease of muscle tenderness (OR 1.97; p = 0.03) and improvement of mouth opening (SMD -0.30; p = 0.04) were found. SS in comparison to oral appliances showed no difference (OR 0.74; p = 0.24). Meta-regression identified continuous use of SS during the day as a factor influencing efficacy (p = 0.01). Long term results showed no difference in observed outcomes between groups. Low quality of evidence was found for primary outcomes. Conclusion SS presented short term benefit for patients with TMDs. In long term follow up, the effect is equalized with other therapeutic modalities. Further studies based on appropriate use of standardized criteria for patient recruitment and outcomes under assessment are needed to better define SS effect persistence in long term.

AB  - Implant stability is a principal precondition for the success of implant therapy. Extraoral implants (EO) are mainly used for anchoring of maxillofacial epithesis. However, assessment of implant stability is mostly based on principles derived from oral implants. The aim of this study was to investigate clinical stability of EO craniofacial disk implants (single, double, and triple) by resonance frequency analysis at different stages of the bone's healing. Twenty patients with orbital (11), nasal (5), and auricular (4) defects with 50 EO implants placed for epithesis anchorage were included. Implant stability was measured 3 times; after implant placement, at 3 months and at least after 6 months. A significant increase in implant stability values was noted between all of the measurements, except for triple-disk implants between third and sixth months, and screw implants between 0 and third months. Disk implants showed lower implant stability quotient (ISQ) values compared with screw implants. Triple-disk implants showed better stability compared with single and double-disk implants. Based on resonance frequency analysis values, disk implants could be safely loaded when their ISQ values are 38 (single disks), 47 (double disks), and 48 (triple disks). According to resonance frequency analysis, disk implant stability increased over time, which showed good osseointegration and increasing mineralization. Although EO screw implants showed higher ISQ values than disk implants, disk-type implants can be safely loaded even if lower values of stability are measured.

Fiber reinforced composite bridge as a replacement for missing upper permanent lateral incisor: A case report

AB  - Hypodontia of upper lateral incisors is significant aesthetic problem in young people. Ideal solution to this problem would be an implant placement, but there is a problem of unfinished growth and development. It is therefore necessary to have a temporary solution in order to maintain normal stomatognathic system functions and solve aesthetic problems until the final restoration is placed. The aim of this study was to show the possibility of treating hypodontia of permanent upper lateral incisors by creating adhesive bridge in one visit. A 15-year-old patient was referred to the Department for Prosthodontics, School of Dental Medicine in Belgrade with hypodontia of permanent upper lateral incisors. The patient had braces until 14 years of age after which she was rehabilitated with partial denture that was neither aesthetically nor functionally acceptable solution for her and significantly affected her emotional security. The adhesive bridge was made using GC composite fibers and a set of GC composite materials for the purpose of temporary replacement of missing teeth. This treatment provided good functional and aesthetic results. The space for the placement of two endosseous implants is preserved and a period of adaptation positively influenced the patient's social development in sensitive adolescent age.
AB  - Hipodoncija gornjih lateralnih sekutića predstavlja značajan estetski problem mladih osoba. Idealno rešenje ovog problema bi predstavljala ugradnja implantata i izrada krunice, ali je prisutan problem nezavršenog rasta i razvoja. Zbog toga je neophodno izraditi privremeno rešenje, kako bi se normalno odvijale funkcije stomatognatnog sistema i rešili estetski problemi, sve do izrade trajne nadoknade. Cilj ovog rada bio je da se prikaže mogućnost zbrinjavanja hipodoncije stalnih gornjih lateralnih sekutića izradom adhezivnog mosta u jednoj poseti. Pacijentkinja starosti 15 godina javila se na Kliniku za stomatološku protetiku Stomatološkog fakulteta u Beogradu sa hipodoncijom stalnih gornjih lateralnih sekutića. U anamnezi je navela da je nosila fiksni ortodontski aparat do svoje 14. godine, a potom bila rehabilitovana parcijalnom pločastom protezom, kojom nije bila zadovoljna ni estetski ni funkcionalno, što je značajno uticalo na njenu emocionalnu nesigurnost. Izrađen je adhezivni most primenom GC kompozitnih vlakana i setom GC kompozitnih materijala sa ciljem privremene nadoknade neizniklih zuba. Primenjenom terapijom je postignut zadovoljavajući funkcionalni i estetski rezultat do momenta realizacije trajnog protetskog rešenja. Sačuvan je prostor za ugradnju dva endosealna implantata, a period adaptacije je pozitvno uticao na socijalni razvoj osobe u osetljivom pubertetskom dobu. .

Contouring the emergence profile of peri-implant soft tissue by provisionals on implants: Case report

AB  - Aesthetic outcome of implant therapy involves the appropriate architecture of peri-implant soft tissue and interdental papilla. The dynamic compression technique of soft tissue is mentioned in contemporary literature as one of the methods for achieving optimal emergence profile. The aim of this case report was to present soft tissue prosthetic modeling with temporary crowns on implants for obtaining an emergence profile of final restoration. A 25-year-old female patient with missing left maxillary central incisor was referred to the Department of Oral Surgery School of Dental Medicine University in Belgrade for dental implant placement. After detailed planning, implant placement of Straumann Bone level implant NC O 3, 3×12 mm (StraumannR Bone Level NC), in the position of 21 was performed. Upon completion of osseointegration period, screw retained laboratory temporary crown was fabricated. During the next 3 months emergence profile was scalloped by creating additional pressure on the soft tissue with periodic adding the composite resin material to a temporary crown. The created emergence profile was transferred to the master cast by using customized impression coping, making possible fabrication of the final implant restoration according to the design made with provisional restoration. Soft tissue conditioning using temporary dental restorations on implants presents a non-invasive method with predictable aesthetic result.
AB  - Estetski uspeh implantološke terapije podrazumeva odgovarajuću arhitekturu periimplantatnih mekih tkiva i interdentalne papile. Tehnika dinamičke kompresije mekih tkiva pominje se u savremenoj literaturi kao jedna od metoda za postizanje optimalnog izlaznog profila. Cilj ovog rada je bio da se predstavi mogućnost protetičkog modelovanja mekih tkiva zubnom nadoknadom na implantatu za dobijanje izlaznog profila definitivne nadoknade. Žena stara 25 godina sa nedostatkom centralnog sekutića u gornjoj vilici s leve strane upućena je na Kliniku za oralnu hirurgiju Stomatološkog fakulteta Univerziteta u Beogradu radi ugradnje implantata. Posle detaljnog planiranja, u predelu zuba 21 ugrađen je Straumann Bone level implant NC O 3,3×12,0 mm (Straumann® Bone Level NC). Po završenom periodu oseointegracije izrađena je laboratorijska privremena krunica retinirana zavrtnjem. Tokom sledeća tri meseca izlazni profil je oblikovan periodičnim stvaranjem dodatnog pritiska na meka tkiva dodavanjem kompozitnog materijala na privremenu krunicu. Izlazni mekotkivni profil je pomoću individualizovanog transfera pozicije implantata prenet na radni model, što je omogućilo izradu konačne keramičke hibridne krunice prema dizajnu koji je kreiran privremenom nadoknadom. Priprema mekih tkiva privremenim zubnim nadoknadama na implantatima je neinvazivna klinička metoda s predvidljivim estetskim rezultatom i zadovoljavajućom visinom i širinom interdentalne papile.

AB  - Orbital defects after tumor resection (exenteration of orbital content) have been traditionally reconstructed with adhesive-retained craniofacial prostheses, also known as epistheses. The breakthrough in rehabilitation of facial defects with implant-retained prostheses has come with development of modern silicones (vynilpolysiloxane) and bone anchorage called osseointegration. Craniofacial implant technology offers improved reconstructive options to patients. This paper describes therapeutical procedure on a patient who received craniofacial implant-retained orbital prosthesis after orbital exenteration. The patient reported excellent prosthesis handling and stability.
AB  - Orbitalna oštećenja nastala resekcijom tumora (egzenteracija sadržaja orbite) se uglavnom rekonstruišu kraniofacijalnim protezama pričvršćenim adhezivima koji se nazivaju i epitezama. Izuzetne mogućnosti u rehabilitaciji defekata lica sa implantatno retiniranim protezama nastale su razvojem savremenih silikona (vinilpolisiloksana) i oseointegracijom implantata. Kraniofacijalna implantatna tehnologija danas pruža široke i poboljšane rekonstruktivne mogućnosti. U ovom radu je opisana terapijska procedura na pacijentu sa defektom orbite gde je izrađena implantatno retinirana orbitalna proteza. Pacijent je posle završene terapije bio zadovoljan zbog jednostavnog rukovanja i izuzetne stabilnosti proteze.

AB  - Aims: To investigate the potential role of polymorphisms in matrix metalloproteinase-9 (MMP-9), glutathione S-transferase M1 (GSTM1) and T1 (GSTT1), and methylenetetrahydrofolate reductase (MTHFR) genes as risk factors for development of temporomandibular disorders (TMD) in a Serbian population. Methods: This case-control study included 282 subjects: 100 with TMD and 182 healthy controls. Genotyping was done by means of polymerase chain reaction (PCR)/restriction fragment length polymorphism (RFLP) for single nucleotide polymorphisms (SNPs) analysis (C-1562T MMP-9 and C677T MTHFR) or multiplex PCR and real-time PCR methods for deletion analysis (GSTM1, GSTT1) of DNA obtained from buccal swabs. The association of gene variants with TMD risk was determined by calculating odds ratios (OR) and their 95% confidence intervals (CI). Results: A statistically significant difference in genotype and allele frequencies was found between the TMD group and controls for the MMP-9 SNP. Heterozygotes (CT) were significantly more frequent in the TMD group than in the control group and carriers of the T allele had an approximately twofold increase of TMD risk (OR = 2.13, 95% CI = 1.24-3.67, P = .005). The null GSTT1 genotype as well as the combined non-null GSTM1/null GSTT1 were associated with lower risk of TMD (OR = 0.28, CI = 0.10-0.74, P = .004 and OR = 0.16, CI = 0.03-0.58, P  lt  .001, respectively). GSTM1 alone and MTHFR polymorphisms did not show an association with TMD. Conclusion: The C-1562T SNP in the promoter region of the MMP-9 gene, the GSTT1 null, as well as the combined GSTM1 non-null and GSTT1 null genotypes are modulators of TMD risk in a Serbian population.

AB  - The aim of this study is to investigate the mechanical properties of polymethyl methacrylate (PMMA) and thermoplastic polycarbonate (TPC) materials in order to produce night bruxoguards. For this purpose we used a static tensile test. In the next step the microstructures of PMMA and TPC were observed. Within this framework special attention was paid to the examination of the tensile-test tube fracture surfaces for both materials. This approach revealed that PMMA is a brittle and TPC is a plastic material. Certain mechanical properties and a review of the crucial areas confirmed that the TPC material is extremely favourable for making occlusal splints.

Transcutaneous electrical nerve stimulation therapy in reduction of orofacial pain

AB  - Introduction Patients with craniomandibular disorders suffer from hypertonic, fatigued and painful masticatory muscles. This condition can lead to limitation of mandibular jaw movements. All of these symptoms and signs are included in myofascial pain dysfunction syndrome. Transcutaneous electrical nerve stimulation (TENS) has been used for treatment of these patients. Objective The aim of this study was to assess the effect of TENS therapy on chronic pain reduction in patients with the muscular dysfunction symptom. Methods In order to evaluate the effect of TENS therapy before and after the treatment, Craniomandibular Index (Helkimo) was used. Pain intensity was measured by VAS. Patients had TENS treatment over two-week period. BURST TENS modality was used. Current intensity was individually adjusted. Results Two patients did not respond to TENS therapy. Complete pain reduction was recorded in 8 patients, while pain reduction was not significantly different after TENS therapy in 10 patients. Conclusion TENS therapy was confirmed as therapeutic procedure in orofacial muscle relaxation and pain reduction.
AB  - Uvod Hiperaktivnost mastikatorne muskulature, pojačan tonus mišića, zamor i bol, uz smanjenje opsega funkcijskih kretnji donje vilice, jesu znaci i simptomi miofascijalnog bolnog disfunkcijskog sindroma. Različiti terapijski modaliteti koriste se u lečenju, među kojima je i terapija transkutanom električnom nervnom stimulacijom (TENS). Cilj rada Cilj rada bila je klinička potvrda efekta terapije TENS na intenzitet hroničnog bola kod osoba sa simptomima disfunkcije mišića. Metode rada Radi procene terapije TENS korišćen je Helkimov kraniomandibularni indeks. Za kvantifikaciju intenziteta bola pre i posle lečenja primenjena je vizuelno-analogna skala (VAS). Pacijenti su podvrgnuti terapiji tri puta nedeljno u trajanju od 40 minuta tokom dve nedelje. Primenjen je modalitet burst (kratki impulsi dužine 10 ms i frekvencije 2-3 Hz) uz individualno podešavanje intenziteta strujnog impulsa. Rezultati Nisu primećeni negativni sporedni efekti terapije TENS. Potpuno smanjenje intenziteta bola zabeleženo je kod osam ispitanika. Kod 10 ispitanika smanjenje bola nije bilo značajno. Dva pacijenta nisu reagovala na primenjenu terapiju. Zaključak Terapija TENS se preporučuje osobama s manjim stepenom kraniomandibularnih disfunkcija bez obzira na intenzitet bola.

AB  - In this research, the structure changes along the depth gradient layers of titanium substrate, after etching with NaOH and subsequent thermal treatment at various temperatures between 300 and 800 degrees C, were investigated by XRD, FTIR and AES. Particularly, the changes of Ti substrate after etching with NaOH, subsequent ionic exchange of Na+ with Ca2+ ions and thermal treatment at 700 degrees C were analysed. Due to this approach, it was possible to get insight into the chemical changes and changes of Ti oxidation states and consequent phase analysis, along the depth of the titanium oxide coatings. In addition, Secondary Electron Imaging (SEI) showed very interesting nanotopology of all samples. Particularly interesting topology, consisting of very thin nano-designed walls between mutually interconnected pores, was observed for the sample in which Na+ were replaced with Ca2+ ions. This structure might be suitable for deposition of hydroxyapatite by biomimetic or plasma methods and as an appropriate scaffold for cell adhesion and proliferation.

AB  - Implementation of the methods of 3D digitization in dental practice is a modern and sophisticated alternative to the method of conventional impressions. Among many different methods of 3D digitization applied in dentistry, optical methods can be considered as today mostly applied, especially in the field of prosthodontics. The developments of this kind of systems have converged in two main directions - extraoral and intraoral systems. The main quality parameters for evaluating measuring performances of 3D digitization methods and systems include accuracy and precision. By introducing LED blue light in intra-and extraoral cameras, the Cerec system has significantly improved the quality of 3D digitization results. As the technical features of cameras in both types of systems are identical and the difference of the digitization methods is connected only to operating environments, the main goal of the research was to analyze whether different digitization conditions significantly influence the systems' measuring performances. Analyzes were focused on investigating accuracy and precision of two mentioned specialized dental optical systems on the basis of CAD inspection, with special attention on evaluating statistically significant differences within the obtained results. Statistically significant difference was not found in systems' precision, while the extra oral system was found as of significantly higher accuracy.

AB  - Background/Aim. Dimensional stability and accuracy of an impression after chemical disinfection by immersion in disinfectants are crucial for the accuracy of final prosthetic restorations. The aim of this study was to assess the deformation of addition and condensation silicone impressions after disinfection in antimicrobial solutions. Methods. A total of 120 impressions were made on the model of the upper arch representing three full metal-ceramic crown preparations. Four impression materials were used: two condensation silicones (Oranwash L - Zhermack and Xantopren L Blue - Heraeus Kulzer) and two addition silicones (Elite H-D + regular body - Zhermack and Flexitime correct flow - Heraeus Kulzer). After removal from the model the impressions were immediately immersed in appropriate disinfectant (glutaraldehyde, benzalkonium chloride - Sterigum and 5.25% NaOCl) for a period of 10 min. The control group consisted of samples that were not treated with disinfectant solution. Consecutive measurements of identical impressions were realized with a Canon G9 (12 megapixels, 2 fps, 6x/24x), and automated with a computer Asus Lamborghini VX-2R Intel C2D 2.4 GHz, by using Remote Capture software package, so that time-depending series of images of the same impression were obtained. Results. The dimensional changes of all the samples were significant both as a function of time and the applied disinfectant. The results show significant differences of the obtained dimensional changes between the group of condensation silicones and the group of addition silicones for the same time, and the same applied disinfectant (p = 0.026, F = 3.95). Conclusion. The greatest dimensional changes of addition and condensation silicone impressions appear in the first hour after their separation from the model.
AB  - Uvod/Cilj. Dimenzionalna stabilnost i preciznost otiska posle hemijske dezinfekcije potapanjem u dezinficijens predstavljaju osnovu za preciznost definitivnih zubnih nadoknada. Cilj ovog istraživanja bio je da se proceni dimenzionalna stabilnost otisaka izrađenih od adicionih i kondenzacionih silikona posle dezinfekcije u antimikrobnim rastvorima. Metode. Napravljen je uzorak od 120 otisaka, dobijenih otiskivanjem modela gornje vilice sa ispreparisanim zubima za metalokeramičke krune. Od otisnih materijala korišćena su dva kondenzaciona silikona (Oranwash L - Zhermack i Xantopren L Blue - Heraeus Kulzer) i dva adiciona silikona (Elite H-D + regular body - Zhermack i Flexitime correct flow - Heraeus Kulzer). Odmah po odvajanju od modela, otisci su potopljeni u odgovarajući dezinficijens (glutaraldehid, benzalkonijum-hlorid - Sterigum i 5,25% NaOCl) u trajanju od 10 min. Kontrolnu grupu činili su uzorci koji nisu tretirani dezinficijensom. Uzastopna merenja istovetnih otisaka vršena su pomoću fotoaparata Canon G9 (12 megapiksela, 2 fps, 6x/24x), a automatizovana sa računara Asus VX 2R Lamborghini- Intel C2D 2,4 GHz, korišćenjem opcija softverskog paketa Remote Capture tako da su dobijene vremenske serije fotografija istog otiska. Rezultati. Utvrđene su izražene dimenzionalne promene svih uzoraka kako u funkciji vremena tako i u funkciji primenjenog dezinficijensa. Rezultati pokazuju postojanje značajnih razlika dimenzionalnih promena između grupe kondenzacionih silikona i grupe adicionih silikona za isto vreme i isti primenjeni dezinficijens (p = 0,026, F = 3,95). Zaključak. Najveće dimenzionalne promene otisaka uzetih adicionim i kondenzacionim silikonima beleže se u prvom satu po odvajanju sa modela.

Reliability of conventional shade guides in teeth color determination

AB  - Background/Aim. Color matching in prosthodontic therapy is a very important task because it influences the esthetic value of dental restorations. Visual shade matching represents the most frequently applied method in clinical practice. Instrumental measurements provide objective and quantified data in color assessment of natural teeth and restorations. In instrumental shade analysis, the goal is to achieve the smallest ΔE value possible, indicating the most accurate shade match. The aim of this study was to evaluate the reliability of commercially available ceramic shade guides. Methods. VITA Easyshade spectrophotometer (VITA, Germany) was used for instrumental color determination. Utilizing this device, color samples of ten VITA Classical and ten VITA 3D - Master shade guides were analyzed. Each color sample from all shade guides was measured three times and the basic parameters of color quality were examined: ΔL, ΔC, ΔH, ΔE, ΔElc. Based on these parameters spectrophotometer marks the shade matching as good, fair or adjust. Results. After performing 1,248 measurements of ceramic color samples, frequency of evaluations adjust, fair and good were statistically significantly different between VITA Classical and VITA 3D Master shade guides (p = 0.002). There were 27.1% cases scored as adjust, 66.3% as fair and 6.7% as good. In VITA 3D - Master shade guides 30.9% cases were evaluated as adjust, 66.4% as fair and 2.7% cases as good. Conclusion. Color samples from different shade guides, produced by the same manufacturer, show variability in basic color parameters, which once again proves the lack of precision and nonuniformity of the conventional method.
AB  - Uvod/Cilj. Određivanje boje zuba u protetskoj terapiji predstavlja veoma važan zadatak jer utiče na prirodan izgled i estetsku vrednost zubnih nadoknada. Vizuelni metod određivanja boje zuba najčešće se koristi u kliničkoj praksi. Instrumentalna merenja pružaju objektivne i kvantifikovane podatke u proceni boje prirodnih zuba i restauracija. U instrumentalnoj analizi boje cilj je da se postigne najmanja moguća vrednost ΔE, što predstavlja najtačniji izbor nijanse. Cilj ovog istraživanja bio je da se utvrdi pouzdanost najčešće korišćenih ključeva za određivanje boje zuba. Metode. Possibilities of reconstruction and implant-prosthetic rehabilitation following mandible resection

AB  - Introduction. Mandible reconstruction is still very challenging for surgeons. Mandible defects could be the consequence of ablative surgery for malignancies, huge jaw cysts, infection and trauma. Segmental resection of the mandible may compromise orofacial function and often lead to patients psychological disorders. Despite very frequent use of microvascular flaps, autogenous bone grafts are still very reliable technique for mandible reconstruction. Comprehensive therapy means not only mandible reconstruction, but prosthodontic rehabilitation supported by dental implants, which can significantly improve patients quality of life. The aim of this paper was to evaluate possible techniques of mandible reconstruction and to present a patient who had been submitted to mandible resection and reconstruction with autogenous iliac bone graft and prosthodontic rehabilitation with fixed denture anchoraged by disc-shaped implants in early loading protocol. Case report. Mandible reconstruction was performed simultaneously with resection. Autogenous iliac bone graft was taken, reshaped and placed in two parts, to the required optimal contour of the mandible. After graft consolidation, decision was made for prosthodontics rehabilitation with fixed dentures supported by implants. In addition to the standard preoperative procedures, planning was done based on a biomodel gained by rapid prototyping after CT scan. It offered a real 3D planning to obtain a proper shape, dimension and the position of implants. Conclusion. If bone dimensions of a reconstructed mandible are insufficient, like in the presented case, the use of basal osseointegrated implants may be a method of choice. Avoiding bone augmentation procedures, as well as early loading protocol for this type of implants, shorten the total rehabilitation time, which is very convenient for patients. Fixed denture supported by dental implants is the best solution for comprehensive rehabilitation after mandible resection.
AB  - Uvod. Rekonstrukcija mandibule još uvek predstavlja izazov za hirurga. Defekti mandibule mogu biti posledica radikalnog hirurškog tretmana malignih tumora, velikih vili- čnih cista, infekcija i povreda. Nedostatak dela vilične kosti kompromituje sve orofacijalne funkcije, a česti su i psihološki poremećaji. Pored sve učestalije primene mikrovaskularnih režnjeva, slobodni koštani graftovi su još uvek veoma pouzdan metod rekonstrukcije mandibule. Sveobuhvatna terapija pored rekonstrukcije mandibule podrazumeva i implantološko-protetičku rehabilitaciju, kojom se znatno poboljšava kvalitet života bolesnika. Cilj ovog rada bio je da se kroz pregled literature ocene metode rekonstrukcije mandibule i da se prikaže bolesnica kod koje je nakon segmentalne resekcije mandibule izvršena rehabilitacija fiksnom zubnom nadokanadom nošenom implantatima oblika diska. Prikaz bolesnika. U istom aktu sa resekcijom izvršena je i rekonstrukcija slobodnim koštanim ilijačnim graftom, koji je preoblikovan i postavljen iz dva dela, kako bi se uspostavio optimalan kontinuitet i forma mandibule. Po konsolidaciji grafta, postavljena je indikacija za izradu fiksne zubne nadoknade nošene implantatima. Pored standardnih preoperativnih procedura, izvršeno je realno trodimenzionalno planiranje na biomodelu dobijenim softverskom analizom CT podataka. Na taj način određen je oblik, veličina i najpovoljniji položaj implantata. Zaključak. Ukoliko su koštane dimenzije rekonstruisane mandibule nedovoljne, kao kod prikazane bolesnice, primena bazalnih oseointegrišućih implantata oblika diska može biti metoda izbora. Njihovom upotrebom izbegava se dodatna nadoknada kosti što uz rano opterećenje predstavlja pogodnost za bolesnike, jer se znatno skraćuje vreme rehabilitacije. Izrada fiksne zubne nadoknade nošene implantatima je najbolji način definitivne rehabilitacije bolesnika sa rekonstruisanom mandibulom.

Autogenous iliac bone graft was taken, reshaped and placed in two parts, to the required optimal contour of the mandible. After graft consolidation, decision was made for prosthodontics rehabilitation with fixed dentures supported by implants. In addition to the standard preoperative procedures, planning was done based on a biomodel gained by rapid prototyping after CT scan. It offered a real 3D planning to obtain a proper shape, dimension and the position of implants. Conclusion. If bone dimensions of a reconstructed mandible are insufficient, like in the presented case, the use of basal osseointegrated implants may be a method of choice. Avoiding bone augmentation procedures, as well as early loading protocol for this type of implants, shorten the total rehabilitation time, which is very convenient for patients. Fixed denture supported by dental implants is the best solution for comprehensive rehabilitation after mandible resection., Uvod. Rekonstrukcija mandibule još uvek predstavlja izazov za hirurga. Defekti mandibule mogu biti posledica radikalnog hirurškog tretmana malignih tumora, velikih vili- čnih cista, infekcija i povreda. Nedostatak dela vilične kosti kompromituje sve orofacijalne funkcije, a česti su i psihološki poremećaji. Pored sve učestalije primene mikrovaskularnih režnjeva, slobodni koštani graftovi su još uvek veoma pouzdan metod rekonstrukcije mandibule. Sveobuhvatna terapija pored rekonstrukcije mandibule podrazumeva i implantološko-protetičku rehabilitaciju, kojom se znatno poboljšava kvalitet života bolesnika. Cilj ovog rada bio je da se kroz pregled literature ocene metode rekonstrukcije mandibule i da se prikaže bolesnica kod koje je nakon segmentalne resekcije mandibule izvršena rehabilitacija fiksnom zubnom nadokanadom nošenom implantatima oblika diska. Prikaz bolesnika. U istom aktu sa resekcijom izvršena je i rekonstrukcija slobodnim koštanim ilijačnim graftom, koji je preoblikovan i postavljen iz dva dela, kako bi se uspostavio optimalan kontinuitet i forma mandibule. Po konsolidaciji grafta, postavljena je indikacija za izradu fiksne zubne nadoknade nošene implantatima. Pored standardnih preoperativnih procedura, izvršeno je realno trodimenzionalno planiranje na biomodelu dobijenim softverskom analizom CT podataka. Na taj način određen je oblik, veličina i najpovoljniji položaj implantata. Zaključak. Ukoliko su koštane dimenzije rekonstruisane mandibule nedovoljne, kao kod prikazane bolesnice, primena bazalnih oseointegrišućih implantata oblika diska može biti metoda izbora. Njihovom upotrebom izbegava se dodatna nadoknada kosti što uz rano opterećenje predstavlja pogodnost za bolesnike, jer se znatno skraćuje vreme rehabilitacije. Izrada fiksne zubne nadoknade nošene implantatima je najbolji način definitivne rehabilitacije bolesnika sa rekonstruisanom mandibulom.",

Prosthetic therapy of the lateral facial defect

AB  - Introduction. Facial defects may arise as a result of head and neck trauma or facial tumor ablation. Minor defects can be reconstructed surgically while large defects usually need combined surgical and prosthetic reconstruction. The aim of this study was to present the prosthetic reconstruction of the lateral facial defect using facial colored acrylic prosthesis. Case Report. A male patient with a maxillary defect on the left side and a large lateral facial defect on the same side received an obturator prosthesis as well as a facial colored acrylic prosthesis (facial-orbit) retained by the glasses frame. Satisfied aesthetics was accomplished. However, the stability of the prostheses during mandibular movements could not be achieved which resulted in saliva leakage over the lips. Conclusion. Better functional and aesthetic outcome could be achieved by combined surgical and prosthetic treatment of such large facial defect.
AB  - Uvod. Defekti lica mogu nastati kao posledica povrede glave i vrata ili usled ablacije tumora predela lica ili okoline. Manji defekti se rekonstruišu hirurški, dok se velika oštećenja uglavnom rekonstruišu kombinovano, primenom hirurških i protetičkih metoda. Cilj rada je bio da se prikaže protetička rekonstrukcija lateralnog defekta lica facijalnom protezom od akrilata u boji. Prikaz bolesnika. Muškarcu s defektom levog dela maksile i velikim lateralnim defektom lica s iste strane izrađene su opturator- proteza i proteza lica (orbito-facijalna) od akrilata u boji retiniranom preko okvira naočara. Time je postignut zadovoljavajući estetski izgled, ali nije postignuta stabilnost pri kretnjama mandibule, s posledičnim curenjem pljuvačke preko komisure usana. Zaključak. Bolji funkcionalni i estetski ishod bi se mogli postići hirurško- protetičkom terapijom ovako velikog defekta lica.

Immediate implant loading with fixed dental restorations: An animal model study

AB  - Background/Aim. Immediate loading is considered to be the most innovative technique in contemporary implant dentistry. Recent clinical and experimental findings have demonstrated that only implants with high primary stability can be subjected to immediate loading protocol with predictable results. It is generally accepted that the most important prerequsite for successful osseointegration is achievement and maintenance of implant stability. The aim of this in vivo study was to investigate the possibility for successful application of immediate loading protocol in implant systems with different surface properties. Methods. In the experimental study 2 mongrel dogs were edentulated bilaterally in the mandibular and maxillary premolar areas. After 3 months implants were placed in a pattern 4 different commercially available implants per quadrant (n = 32): Mk III TiUnite (Nobel Biocare, Sweden), ITI TPS (Straumann, Switzerland), 3IOsseotite (Implant Innovation, USA) and XiVE Cell-Plus (Friadent, Germany). Implants were subjected to immediate loading with 4 unit gold cast bridges, 2 days post implantation. The assessment of implant stability and immediate loading possibilities were done by performing Resonance frequency analysis (RFA). Results. After a 6- month loading period all bridges were in function and all implants occurred well osseointegrated. When summarizing the Implant Stability Quotient (ISQ) values, it was noted that resonance frequency was significantly higher for mandibular implants. The results of this experimental setting showed that all evaluated surfaces achieved good implant stability. Increase of ISQ values was found for all implants in the mandible and partially decrease of ISQ values for maxillary implants after 6 months of functional loading with 4 unit bridges. Conclusions. Investigated endooseal implants did not show different degree of osseointegration, because there was not statistically significant difference among observed parameters (ISQh i ISQp) between implant systems.
AB  - Uvod/Cilj. Imedijatno opterećenje implantata smatra se jednim od najznačajnijih dostignuća savremene dentalne implantologije. Rezultati novijih kliničkih i eksperimentalnih studija pokazali su da samo implantati visoke primarne stabilnosti mogu biti izloženi protokolu imedijatnog opterećenja zubnom nadoknadom sa predvidivim ishodom. Cilj studije bio je da se ispita mogućnost uspešne primene protokola imedijatnog opterećenja implantata različitog mikrodizajna fiksnim zubnim nadoknadama. Metode. U eksperimentalno istraživanje uključena su dva psa, obezubljena obostrano u premolarnoj regiji gornje i donje vilice. Nakon tri meseca od ekstrakcije zuba, ugrađena su, po predviđenoj šemi, četiri različita implantata po kvadrantu (n = 32): Mk III TiUnite (Nobel Biocare, Sweden), ITI TPS (Straumann, Switzerland), 3I-Osseotite (Implant Innovation, USA) i XiVE Cell-Plus (Friadent, Germany). Aspects of titanium-implant surface modification at the micro and nano levels

AB  - The shape and chemical composition, as well as the macro- and microtopography, of an implant surface have been studied widely as the major factors that positively influence implant osseointegration. Titanium and titanium alloys have been used extensively over the past 20 years as biomedical materials in orthopedic and dental surgery because of their good mechanical properties, corrosion resistance, no cell toxicity, and very poor inflammatory response in pen-implant tissue, which confirms their high biocompatibility. Their favorable biological performance is attributed to a thin native oxide film that forms spontaneously on the titanium surface. It is well established that surface roughness plays an important role in implant fixation. Accordingly, some authors have indicated the existence of an optimal range of surface roughness. The titanium surface can be either chemically or physically modified, or both, in order to improve biomaterial tissue integration. Different treatments are used to modify the titanium surface. Hydroxyapatite coatings, preceded or not by acid etching, are used to create a rough, potentially bioactive surface. Oxide blasting treatments, either with or without chemical etching, are used to develop rough surfaces. Thick oxide films obtained by anodic or thermal oxidation have been used to accelerate the osseointegration process. The ideal microtopography of the surface is still unknown, however, because it is very difficult to associate surface properties with clinical results. As more accurate knowledge is required, several Ti surfaces have been analyzed and the endosseous implant surface modified on the micro level has been thoroughly studied. Additionally, the production of gold (Au) nanoparticles to be added to the micron-scale modified surface has been performed. In this respect, an appropriate overview of our results is given.

Occlusal splints in reversible occlusal therapy of craniomandibular dysfunction

AB  - Craniomandibular dysfunction (CMD) is a set of structural and functional disorders of different etiology that affects temporomandibular joint (TMJ) and orofacial muscles. The most common etiologic factors are psychogenic, occlusal, trauma and congenital anomalies of craniofacial structures. About 75% of the examined population have mild symptoms of CMD while 3-4% have more severe symptoms which require medical attention. The main symptoms why people seek for medical attention are: facial pain which increases with chewing and irradiates in surrounding areas and pain in TMJ which irradiates in the ear canal, temporal area or neck. Painful restriction of mandible during mouth opening and eccentric movements is frequent as well as mandible deviation or deflection. Sound effects in TMJ such as popping or clicking during mouth opening are common. Initial and least invasive therapeutic procedure is reversible occlusal therapy using splints. There are two main types of occlusal splints: stabilization and relaxation. First type of splints works on condyle stabilization in orthopedically stable position; it is superoanterior condylar position in articular fossa with position of intercondylar discs between condyle and articular fossa when working cusps of the antagonists are in maximal contact with the splint. Another type of splint causes disocclussion of posterior teeth and eliminates negative effects of occlusal interference in the intercuspal position or during eccentric mandibular movements. During therapy, occlusal splint temporarily changes occlusal relationships as well as relations within TMJ, causing reduction of CMD symptoms. The best therapeutic effect for reduction of CMD symptoms is achieved by combination of physical therapy and medication.
AB  - Kraniomandibularne disfunkcije (KMD) jesu skup strukturnih i funkcijskih poremećaja različite etiologije koji zahvataju temporomandibularne zglobove (TMZ) i orofacijalnu muskulaturu. Najčešći etiološki faktori su psihogeni; slede okluzalni faktori, povrede i kongenitalne anomalije u predelu lica i vilica. Rasprostranjenost blagih simptoma KMD je oko 75%, dok 3-4% ispitanika ima teže simptome i potrebna im je lekarska pomoć. Glavne tegobe zbog kojih se osobe sa KMD javljaju lekaru jesu bol u predelu lica koji se pojačava pri žvakanju i širi se u okolne delove i bol u predelu TMZ koji se širi u ušni kanal, temporalno ili u vrat. Česta su i bolna ograničenja pokreta mandibule pri otvaranju usta i ekscentričnim kretnjama sa devijacijom ili defleksijom sredine donje vilice. Neretko se čuju i zvučni efekti preskakanja ili kliktanja u TMZ pri pokretanju donje vilice. Inicijalna i najmanje invazivna terapijska mera je reverzibilna okluzalna terapija splintovima. Okluzalni splintovi mogu biti stabilizacioni i relaksacioni. Prvi imaju zadatak da stabilizuju kondile u ortopedski stabilnom položaju - superoanteriorni položaj kondila u fosi artikularis sa pravilno interponiranim diskusima, kada se potporne kvržice antagonista nalaze u maksimalnom kontaktu sa pločom splinta. Drugi splintovi dovode do disokluzije bočnih zuba i eliminacije štetnog efekta okluzalnih interferenci u interkuspalnom položaju ili pri ekscentričnim kretnjama mandibule. Tokom terapije okluzalnim splintom privremeno se menjaju okluzalni i odnosi unutar TMZ do smanjenja simptoma KMD. Najbolji terapijski efekat suzbijanja simptoma KMD postiže se u kombinaciji s određenim vidovima fizikalne i medikamentne terapije.

Stress-strain analysis of an abutment tooth with rest seat prepared in a composite restoration

AB  - Placing a composite restoration on abutments for the removable of partial dentures gives favorable aesthetic results with minimal intervention. The objective of this paper is to analyze the stress distribution of a tooth with occlusal rest-seat preparation in the composite and compare it to the biomechanical behavior of an intact tooth, assuming that the stress and strain distribution throughout the intact tooth provides the control scenario. For this finite-element study two different models were designed. The first model was the three-dimensional (3D) model of an intact tooth, and the other one was a 3D model of a tooth with a composite restoration and an appropriate occlusal rest-seat preparation. Load stimulations were performed when the rest was fully seated on its corresponding rest seat and abutment tooth in order to obtain data about the biomechanical behavior of the abutment tooth compared to the intact tooth's stress-distribution pattern. The results of our analyses are presented and analyzed qualitatively. The occlusal loading effect along the sound tooth exhibits a wider high-stress area, localized in correspondence with the occlusal enamel, than the restored teeth. This is due to the rigidity of the enamel. The reduction in the stress values occurs in the composite restoration, which is less rigid. Its lower rigidity allows larger cusp movements. The stress-distribution pattern of the restored tooth with the rest seat showed that introducing an occlusal restoration does not differ from the intact tooth globally, but locally. Our findings indicate that the composite rest-seat restoration absorbs the loading, so reducing the stresses inside the tooth's structure.
