AB  - Background: The analysis of nasal anatomy, and especially the nasal bones including the osseocartilaginous vault, is significant for functional and aesthetic reasons. Objectives: The objective was to understand the anatomy of the nasal bones by establishing new descriptions, terms, and definitions because the existing parameters were insufficient. Adequate terminology was employed to harmonize the anthropometric and clinical measurements. Methods: A two-part harvest technique consisting of resecting the specimen and then creating a replica of the skull was performed on 44 cadavers to obtain specific measurements. Results: The nasal bones have an irregular, variable shape, and three distinct angles can be found along the dorsal profile line beginning with the nasion angle (NA), the dorsal profile angulation (DPA) and the kyphion angulation (KA). In 12% of cases, the caudal portion of the nasal bones was straight and without angulation resulting in a "V-shape" configuration. In 88% of cases, the caudal portion of the bone was angulated, which resulted in an "S-shape" nasal bone configuration. The intervening cephalic bone, nasion to sellion (N-S), represents the radix while the caudal bone, sellion to r (S-R), represents the bony dorsum. Conclusions: By standardizing and measuring existing nasal landmarks and understanding the different anatomic configurations of the nasal bones, rhinoplasty surgeons can better plan their operations within the radix and bony and osseocartilaginous vaults.

Intratympanic Corticosteroid Perfusion in the Therapy of Meniere's Disease

AB  - Introduction Over the last two decades the intratympanic perfusion of corticosteroids has been used as a minimally invasive surgical therapy of Meniere's disease. According to experimental studies the antiinflammatory, immunoprotective, antioxidant and neuroprotective role of the locally perfused corticosteroids was noticed in the inner ear structures. The recovery of action potentials in the cells of the Corti organ was confirmed as well as a decreased expression of aquaporine-1, a glycoprotein responsible for labyrinth hydrops and N and K ions derangement. Objective The study showed results of intratympanic perfusion therapy with dexamethasone in patients with retractable Meniere's disease who are resistant to conservative treatment. Methods Single doses of 4 mg/ml dexamethasone were given intratympanically in 19 patients with retractable Meniere's disease. Six single successive doses of dexamethasone were administered in the posteroinferior quadrant of the tympanic membrane. Follow-up of the patients was conducted by using a clinical questionnaire a month after completed perfusion series as well as on every third month up to one year. Results One month after completed first course of perfusions, in 78% of patients, vertigo problems completely ceased or were markedly reduced. The recovery of hearing function was recorded in 68% and marked tinnitus reduction in 84% of patients. After a year of follow-up, in 63% of patients the reduction of vertigo persisted, while hearing function was satisfactory in 52%. Tinitus reduction was present in 73% of patients. Conclusion Intratympanic perfusion of dexamethasone in patients with Meniere's disease is a minimally invasive therapeutic method that contributes to the reduction of the intensity of vertigo recurrent attacks, decrease of the intensity of tinnitus and improvement of the average hearing threshold. Patients with chronic diseases and Meniere's disease who are contraindicted for systemic administration of cortocosteroids (hypertension, diabetes, glaucoma, peptic ulcer, etc.) have an additional therapeutic option by dexamethasone intratympanic perfusion.

Moleculobiological characteristics of oral squamous cell carcinomas

AB  - Oral squamous cell carcinomas (OSCCs) are associated with poor prognosis, and despite advances in therapy approaches, no major improvement in survival has been achieved in the recent years. Efforts are now directed toward finding new biological markers that could predict tumor behavior more accurately. OSCCs, as the majority of malignant tumors, arise from progressive accumulation of genetic and epigenetic lesions, transforming normal cells into malignant. In this paper, an analysis of current studies directed to understanding the underlying mechanisms of OSCC pathogenesis was presented. The emphasis was put on mutational analysis of cancer genes, as well as on the role of viral infections and methylation processes in OSCC. Finally, an overview of studies that tried to determine the possibility for developing OSCC was given.
AB  - Oralne skvamocelularne karcinome (OSCK) odlikuje uglavnom loša prognoza i, uprkos pomacima u terapijskim postupcima, poslednjih godina nije ostvaren napredak u preživljavanju osoba s ovim tumorom. Velike nade se polažu u molekularnu medicinu i pronalaženje novih bioloških markera pomoću kojih bi preciznije nego što to dopuštaju klinički i histopatološki parametri moglo da se predvidi ponašanje tumora. OSCK, kao i većina drugih malignih oboljenja, rezultat su postupne akumulacije raznovrsnih genetičkih i epigenetičkih promena u ćelijama, koje od normalnih postaju neoplastične. U ovom radu dat je presek nekih od pravaca istraživanja na polju molekularne biologije oralnih karcinoma, s osvrtom na studije koje se bave ispitivanjem somatskih mutacija u kancerskim genima, učešćem onkogenih virusa u patogenezi i značaju procesa metilacije za OSCK. Takođe su pomenute studije posvećene utvrđivanju eventualnog postojanja predispozicije za razvoj OSCK.

AB  - Symptoms of voice disorders can adversely affect the teachers ability to function in the classroom and prevent them from developing effective working relationships with students and other staff. Sick leave, speech pathology management, and surgical intervention can be expensive, in terms of time and costs. Severe voice problems can also lead to the permanent loss of the teaching position. The teacher would therefore be considered a professional voice user and as such needs to be aware of indicators of vocal distress and the factors that contribute to voice disorders.
AB  - U ovom radu smo naveli niz saveta i uputstava kako predavači, kao jedna od najopterećenijih vokalnih profesija, da što lakše prepoznaju početne simptome poremećaja glasa i kako da i sami doprinesu njegovom poboljšanju i očuvanju primenom preventivnih mera vokalne higijene.

AB  - Although myc genes are an important family of oncogenes involved in human carcinogenesis, data concerning its role in head and neck tumors are quite limited. Moreover, information regarding precancerous lesions is almost inexisting. In order to better understand mechanisms of progression of premalignant lesions into malignant and the possible role of c-myc amplification, the status of this oncogene was analyzed in two types of pathological changes known to possess malignant transformation potential oral leukoplakia and chronic laryngitis. In the first type of lesions a high percentage of c-myc activation (33%) was observed pointing to a probable role of myc in leukoplakia malignant transformation, while in the other type of lesion there were no signs of myc amplification.
AB  - C-myc spada u red onkogena sa važnom ulogom u humanoj kancerogenezi, međutim malo je podataka u literaturi o promenama ovog gena u malignim i premalignim lezijama glave i vrata. Sa ciljem da se bolje upoznaju mehanizmi evolucije prekanceroznih lezija u prave kancere sa naglaskom na c-myc onkogen, analiziran je njegov status u dva tipa promena za koje se zna da imaju maligni potencijal: oralnim leukoplakijama i hroničnim laringitisima. U prvom tipu lezija ustanovljen je visok stepen (33%) aktivacije myc-onkogena, što bi ukazivalo na značaj ove promene u malignoj transformaciji leukoplakija, dok u drugom nije nađen nijedan uzorak sa ovom promenom.
