Changes in pulp sensitivity across the menstrual cycle in healthy women and women with temporomandibular disorders

Menstrual cycle may contribute to experimental pain measures in healthy women and women with chronic pain.

Objectives
The present study aimed to investigate variation in pulp sensitivity across the menstrual cycle in healthy women and women with temporomandibular disorders (TMD) and to explore the effect of TMD pain and psychosocial variables on the pulp response.

Methods
This longitudinal study involved 47 regularly menstruating women aged 20‐45, 24 healthy and 23 with diagnosed painful TMD. The electric and cold stimuli were performed by electric pulp tester and refrigerant spray, respectively, on mandibular lateral incisors, in five menstrual phases (menstrual, follicular, periovulatory, luteal and premenstrual). Research Diagnostic Criteria for TMD were used to assess TMD, chronic pain, depression and somatisation. Regression analysis was performed to investigate the effect of the predictor variables on the pulp sensitivity.

Results
Significant phase‐related differences were observed for pain intensity to cold stimuli. Higher pain sensitivity was reported in menstrual in comparison with luteal phase (P = .019) among healthy women, and in menstrual in comparison with follicular (P = .033), periovulatory (P = .003) and luteal (P = .007) phases in TMD women. No significant differences were recorded for electric stimuli. Regression analysis identified depression as the determinant of cold and electric response in menstrual phase, regardless of age and presence of TMD.

Conclusion
Menstrual phase in healthy and TMD women with regular menstrual cycle is characterised with higher pulp sensitivity to cold stimuli. Depressive symptoms independently influence pulp response in this phase.

Treatment after inadequate immediate replantation of accidentally extracted immature mandibular premolar during primary molar extraction

AB  - During extraction of the primary mandibular right second molar in an 11-year-old girl, the unerupted second premolar was accidentally extracted. Clinical and radiographic examination showed that the immediately replanted immature premolar was not oriented and positioned correctly. Four hours later, treatment consisted of manual extrusion of the permanent tooth bud, rotation, and gentle repositioning into its original position. Adequate replantation was confirmed by a post-operative radiograph. After 2 years and 4 months, clinical examination revealed normal, healthy appearance of the replanted tooth, no sensitivity to percussion, no tenderness to palpation, and a slight response to a cold pulp sensibility test. A radiograph showed completely developed root with closed apical foramen, slightly irregular root morphology and shorter root length, complete obliteration of the pulp, and no signs of periapical pathosis.

Oral health difficulties in children and adolescents with autism spectrum disorder: Parental perception

AB  - Introduction/Objective Autism spectrum disorder (ASD) is characterized by features that have the potential to make oral hygiene and dental appointments challenging. The aim of this study was to investigate difficulties related to oral hygiene and dental appointments that may be encountered in children and adolescents with ASD, in comparison to their typically developing peers. Methods A 48-item questionnaire was prepared for the purpose of the study and distributed to parents of children and adolescents with ASD in three specialized daycare centers, as well as to parents of typically developing children and adolescents at the Clinic for Pediatric and Preventive Dentistry in Belgrade, Serbia. Ninety-two questionnaires were considered and statistically analyzed in the SPSS program, using χ2 and Mann-Whitney U-test. Results The following statistically significant differences were found between children and adolescents with ASD and their typically developing peers: general difficulties in everyday oral hygiene, need of help for basic oral hygiene tasks, tooth brushing frequency, sensory difficulties related to toothbrush and toothpaste, level of anxiety prior to dental appointment, cooperation during appointment, sensory difficulties related to touch, operatory light and sound of dental unit, number of treatments under general anesthesia, and the number of refused dental treatments. Conclusion Children and adolescents with ASD face significantly more difficulties concerning everyday oral hygiene and dental appointments in comparison to their typically developing peers. Dentists' awareness of issues that are specific to this population of patients is important in order to enable quality dental care.
AB  - Uvod/Cilj Poremećaji autističnog spektra (PAS) mogu biti uzrok poteškoća vezanih za održavanje oralne higijene, kao i za posete stomatologu. Cilj rada je bio da se utvrde razlike između dece i adolescenata sa PAS i kontrolne grupe neurotipičnih vršnjaka sa aspekta održavanja oralne higijene i poseta stomatologu. Metode Upitnik od 48 pitanja distribuiran je roditeljima dece i adolescenata sa PAS u tri specijalizovana dnevna boravka, kao i roditeljima kontrolne grupe neurotipičnih vršnjaka. Devedeset dva upitnika je uzeto u obzir i statistički analizirano u programu SPSS. Rezultati Statistički značajne razlike između dece i adolescenata sa PAS i njihovih tipično razvijenih vršnjaka utvrđene su u sledećim kategorijama: teškoće u svakodnevnoj oralnoj higijeni, potreba za pomoći u održavanju oralne higijene, učestalost pranja zuba, senzorne teškoće vezane za četkicu i zubnu pastu, nivo anksioznosti pre posete stomatologu, saradnja tokom stomatološkog tretmana, senzorne teškoće vezane za dodir stomatologa, svetlo i zvuk stomatološkog uređaja, broj tretmana pod opštom anestezijom i broj odbijenih stomatoloških usluga. Zaključak Deca i adolescenti sa PAS suočavaju se sa znatno većim poteškoćama u pogledu svakodnevne oralne higijene i poseta stomatologu u poređenju sa tipično razvijenim vršnjacima. Svest stomatologa o pitanjima koja su specifična za ovu populaciju pacijenata je važna kako bi se deci i adolescentima sa PAS omogućila kvalitetna stomatološka nega.

Prosthetics in paediatric dentistry

AB  - Premature loss of teeth in children may lead to both functional and esthetic problems. Missing teeth in both anterior and posterior regions may cause malfunctions in mastication and proper pronunciation. If the missing teeth are not replaced, further complications may occur, including adjacent tooth migration, loss of alveolar bone, and irregular occlusion. Considering the sensitive nature of children, loss of teeth may cause the development of insecurities and low self esteem problems. Due to dynamic nature of growth in children and adolescents, prosthetic appliances must not hinder development of orofacial system, and must meet adequate esthetic and functional standards. Dental prosthetic appliances in paediatrics must be planned with respect to the special conditions that led to tooth loss or damage. Multi-disciplinary approach is needed, under constant supervision of paediatric dentist and orthodontist, as well as regular checkups with clinical and radiographical examinations.

AB  - One of the main objectives of contemporary dentistry is to preserve healthy tooth structure by applying techniques of noninvasive treatment. Air abrasion is a minimally invasive non mechanical technique of tooth preparation that uses kinetic energy to remove carious tooth structure. A powerful narrow stream of moving aluminum-oxide particles hit the tooth surface and they abrade it without heat, vibration or noise. Variables that affect speed of cutting include air pressure, particle size, powder flow, tip's size, angle and distance from the tooth. It has been proposed that air abrasion can be used to diagnose early occlusal-surface lesions and treat them with minimal tooth preparation using magnifier. Reported advantages of air abrasion include reduced noise, vibration and sensitivity. Air abrasion cavity preparations have more rounded internal contours than those prepared with straight burs. This may increase the longevity of placed restorations because it reduces the incidence of fractures and a consequence of decreased internal stresses. However, air abrasion cannot be used for all patients, i.e. in cases involving severe dust allergy, asthma, chronic obstructive lung disease, recent extraction or other oral surgery, open wounds, advanced periodontal disease, recent placement of orthodontic appliances and oral abrasions, or subgingival caries removal. Many of these conditions increase the risk of air embolism in the oral soft tissues. Dust control is a challenge, and it necessitates the use of rubber dam, high-volume evacuation, protective masks and safety eyewear for both the patient and the therapist.
AB  - Jedan od osnovnih ciljeva savremene stomatologije jeste očuvanje zdravog zubnog tkiva primenom preventivnih metoda i tehnika neinvazivnog lečenja. Vazdušna abrazija je minimalno invazivna tehnika preparacije kaviteta koja koristi kinetičku energiju za uklanjanje karijesa. Usmeravanje pravolinijskog mlaza čestica aluminijum- oksida na površinu zuba koja se obrađuje dovodi do abrazije supstrata bez toplote, vibracija i zvuka. Faktori koji utiču na brzinu preparacije uključuju intenzitet vazdušnog pritiska, veličinu čestica abraziva, brzinu protoka čestica, prečnik vrha kanile, veličinu ugla kanile i udaljenost vrha kanile od površine zuba koja se obrađuje. Njena upotreba se savetuje u dijagnostici rane karijesne lezije gleđi na okluzalnim površinama zuba i minimalnoj preparaciji kaviteta. Pojedini autori preporučuju i upotrebu optičkih pomagala. Prednosti tehnike vazdušne abrazije uključuju smanjenje buke, vibracija i operativne osetljivosti, koja je veoma subjektivna i promenljiva kod pacijenata. Kaviteti preparisani vazdušnom abrazijom imaju zaobljene ivice, za razliku od kaviteta preparisanih standardnim tehnikama. To može povećati dugotrajnost postavljenih ispuna, jer se smanjuje mogućnost nastanka njihovog odlamanja, neutralisanjem sila unutrašnjeg napona u poređenju sa stresom koji se javlja kod angularnih preparacija. Primena tehnike vazdušne abrazije je kontraindikovana kod osoba alergičnih na prašinu, obolelih od astme ili hronične opstruktivne bolesti pluća, pacijenata sa postekstrakcionim ranama, parodontopatijom, ortodontskim aparatima ili dubokim karijesom, jer se ovom tehnikom preparacije ne odstranjuje uspešno mekana dentinska struktura. Mnoga ova stanja povećavaju rizik od nastanka vazdušne embolije u mekim oralnim tkivima. Kontrola rasejanja čestica abraziva predstavlja izazov kliničkom stomatologu, a zahteva i primenu koferdama, jakih aspiratora, zaštitnih maski i zaštitnih naočara kako kod pacijenta, tako i kod terapeuta.

Hard dental tissue minimal-invasive preparation using contemporary polymer rotating instruments and laser

AB  - Goal of contemporary dentistry is to decrease the patient's discomfort during treatment. Dentists aim to achieve maximum with the newly developed dental materials as well as with new cavity preparation techniques in the shortest time span. Since the development of the first constructed borer (drilling machine) for caries removal, the preparation techniques have considerably changed. The progress of dental materials as well as the cavity preparation techniques has led us to contemporary carbide tungsten and diamond borers that are used with obligatory water cooling. The innovation within this field represents newly developed polymer borers that can detect the difference between carious lesions and healthy tooth structure. In this way the cavity preparation may be performed without damaging dental healthy tissue. This is possible owing to their hardness which is lower than the hardness of intact dentin. Polymer borer preparation is painless with less vibration, while the increase in temperature is negligible. Lasers have been used in clinical dentistry since 1980s so it can be said that they represent a new technology. The function of lasers is based on ablation which requires water. Erbium lasers have shown the highest potential with their ability to produce effective ablation of hard dental tissues. Laser application in dentistry requires special training as well as some protective measures. Laser advantages, compared to traditional preparation techniques, involve the absence of vibration, painless preparation, possibility of preparation without anesthetic and easier patient's adjustment to dental intervention which is of importance, especially in pediatric dentistry.
AB  - Savremena stomatologija teži ka što manjoj traumatizaciji pacijenta prilikom rada uz maksimalni učinak. Savremeni materijali uz nove vidove preparacije kaviteta pružaju mogućnost sanacije karijesnih lezija u najkraćem vremenskom roku. Vidovi preparacije kaviteta su se znatno promenili otkako je uvedeno prvo mašinski napravljeno svrdlo za uklanjanje karijesa. Tehnološki napredak materijala za ispune i načina preparacije doveli su do savremenih karbidnih i dijamantskih svrdla koja se koriste uz obavezno vodeno hlađenje. Inovaciju na polju rotirajućih instrumenata predstavljaju polimerna 'pametna' svrdla, koja uklanjaju karijesnu leziju bez mogućnosti povređivanja i uklanjanja zdrave zubne strukture. Ovo je moguće zahvaljujući čvrstoći koju poseduju, koja je manja od čvrstoće zdravog dentina. Zahvaljujući prirodi materijala od kojih su izrađena, ova svrdla omogućuju bezbolniji rad uz manje vibracija i slabije zagrevanje zubne supstance. Laseri su relativno novo tehnološko sredstvo u stomatologiji, a njihova klinička upotreba počela je osamdesetih godina dvadesetog veka. Laseri funkcionišu po principu ablacije, za šta je neophodno prisustvo vode. Najveći potencijal među ispitivanim laserima pokazali su erbijumski laseri, kojima je moguće uraditi efikasnu ablaciju čvrstih zubnih tkiva. Za korišćenje lasera u stomatologiji neophodni su posebna obuka i odgovarajuće mere zaštite. Prednosti lasera u poređenju sa drugim metodama preparacije čvrstih zubnih tkiva su nepostojanje vibracija, bezbolan rad, mogućnost rada bez anestezije i lakše prilagođavanje pacijenta stomatološkim intervencijama, što je veoma značajno u dečjoj stomatologiji.

Push-out bond strength of an experimental self-adhesive resin cement

AB  - The adhesion of fiber posts luted with simplified adhesive systems has been a matter of great interest over the past years. The aim of this study was to assess the post retentive potential of experimental self-adhesive resin cement (EXP) when used alone and in combination with a self-etch adhesive. Fiber posts were placed in endodontically treated teeth and divided into four groups (n = 6) according the luting material, as follows: group 1 (EXP alone); group 2 (EXP used with self-etch adhesive); group 3 (marketed dual-cured cement used with self-etch adhesive); and group 4 (marketed self-adhesive cement). The push-out test was used to assess the retentive strength of fiber posts (expressed in MPa), and specimens were analyzed under a stereomicroscope to determine failure mode. The adhesive interface between the cement and root canal dentin for each group was evaluated using scanning electron microscopy. The post retentive potential of group 1 (EXP) (7.48 +/- 4.35 MPa) was comparable with that of marketed cements from group 4 (6.79 +/- 3.68 MPa) and group 3 (8.77 +/- 4.58 MPa). When EXP was used in combination with self-etch adhesive (group 2), significantly higher push-out bond-strength values were measured (15.87 +/- 4.68 MPa) compared with the other groups.

Post-retentive ability of new flowable resin composites

AB  - Purpose: To investigate the applicability of flowable composites as post luting agents by assessing the push-out strength of posts. Methods: 36 intact single rooted human premolars were selected. The endodontic treatment was performed and half of the specimens were restored with light transmitting posts (DT Light Post Illusion) and the other half with opaque posts (Tech 21 X-OP). In both groups the following combinations of adhesive/cement were tested: OptiBond Solo Plus/Nexus Third Generation (NX3), XP Bond/SureFil SDR Flow (SDR), and Vertise Flow (VF). Push-out test was used to assess the retentive strength of fiber posts, which was expressed in megapascals (MPa). Specimens were analyzed under a stereomicroscope to determine failure mode (adhesive between luting agent and post, adhesive between luting agent and dentin or mixed failure). Push-out data and failure mode distribution were analyzed by two-way ANOVA and Chi-square test, respectively (P lt  0.05). Results: The statistical analysis revealed that only the type of luting material significantly influenced push-out bond strength of the post (P lt  0.001). SDR (9.00 +/- 2.17 MLPa) performed similarly to the control group NX3 (7.15 +/- 1.74 MPa), while VF (4.81 +/- 1.51 MPa) should significantly lower bond strength. Failure modes differed significantly among groups.

abstract = "Purpose: To investigate the applicability of flowable composites as post luting agents by assessing the push-out strength of posts. Methods: 36 intact single rooted human premolars were selected. The endodontic treatment was performed and half of the specimens were restored with light transmitting posts (DT Light Post Illusion) and the other half with opaque posts (Tech 21 X-OP). In both groups the following combinations of adhesive/cement were tested: OptiBond Solo Plus/Nexus Third Generation (NX3), XP Bond/SureFil SDR Flow (SDR), and Vertise Flow (VF). Push-out test was used to assess the retentive strength of fiber posts, which was expressed in megapascals (MPa). Specimens were analyzed under a stereomicroscope to determine failure mode (adhesive between luting agent and post, adhesive between luting agent and dentin or mixed failure). Push-out data and failure mode distribution were analyzed by two-way ANOVA and Chi-square test, respectively (P lt  0.05). Results: The statistical analysis revealed that only the type of luting material significantly influenced push-out bond strength of the post (P lt  0.001). SDR (9.00 +/- 2.17 MLPa) performed similarly to the control group NX3 (7.15 +/- 1.74 MPa), while VF (4.81 +/- 1.51 MPa) should significantly lower bond strength. Failure modes differed significantly among groups.",

AB  - Introduction: Preserving intact coronal and radicular tooth structure, especially maintaining cervical tissue to create a ferrule effect, is considered to be crucial for the optimal biomechanical behavior of restored teeth. The ferrule effect has been extensively studied and still remains controversial from many perspectives. The purpose of this study was to summarize the results of research conducted on different issues related to the ferrule effect and published in peer-reviewed journals listed in PubMed. Methods: The search was conducted using the following key words: "ferrule" and "ferrule effect" alone or in combination with "literature review," "fracture resistance," "fatigue," "finite element analysis," and "clinical trials." Results: The findings from reviewed articles were categorized into three main categories: laboratory studies, computer simulation, and clinical trials. Laboratory studies were further classified into subchapters based on the main aspect investigated in relation to the ferrule effect. Conclusions: The presence of a 1.5- to 2-mm ferrule has a positive effect on fracture resistance of endodontically treated teeth. If the clinical situation does not permit a circumferential ferrule, an incomplete ferrule is considered a better option than a complete lack of ferrule. Including a ferrule in preparation design could lead to more favorable fracture patters. Providing an adequate ferrule lowers the impact of the post and core system, luting agents, and the final restoration on tooth performance. In teeth with no coronal structure, in order to provide a ferrule, orthodontic extrusion should be considered rather than surgical crown lengthening. If neither of the alternative methods for providing a ferrule can be performed, available evidence suggests that a poor clinical outcome is very likely. (J Endod 2012;38:11-19)

AB  - Objectives. The aim of this study was to investigate the effect of fiber post light transmitting ability to the continuity of resin cement-root dentin (C-RD) and resin cement-fiber post (C-FP) interface, elastic modulus and hardness of a dual-cure resin cement. Methods. Spectrophotometric measurements were applied for the determination of light transmission at coronal, middle and apical level as well as at the apical tip through Tech 21 X-OP (TECH) and DT Light Post (DT). Posts were cemented using dual-cured resin cement (Calibra). Roots were sectioned longitudinally through the post. Epoxy resin replicas were made and used to evaluate C-RD and C-FP interface under SEM. Modulus of elasticity (E) and Vicker's hardness (VH) of the cement layer were assessed. Results. No light transmission was detected through TECH. Light transmission through DT decreased from coronal to apical and rose at the apical tip. TECH presented a significantly lower percentage of continuous C-RD and C-FP interface in comparison to DT. Coronal third of C-RD interface in TECH specimens had a significantly higher percentage of continuity than apical third. No regional differences in continuity of C-RD interface were found in DT specimens. E and VH were significantly lower when TECH was used, and decreased from coronal to apical for both posts. Significance. Cementation of fiber post with no light transmitting ability using a dual-cured resin cement resulted in lower E and VH of the cement layer, and lower percentage of continuous C-RD and C-FP interface in comparison to cementation of light transmitting fiber post.

I and 11: experimental patented industrial treatment consisting of zirconium oxide coating and silanization (RTD); III: industrial treatment followed by adhesive application (XPBond, Dentsply Caulk); IV: adhesive (XPBond); V: adhesive (Prime&Bond NT, Dentsply Caulk); VI: silane (Calibra Silane, Dentsply Caulk); VII: no treatment. Adhesives were used in the self-curing mode. Two cements (Sealbond, RTD - group 1, and Calibra, Dentsply Caulk - groups 11 to VII) were applied on the posts to produce cylindrical specimens. Post/cement interfaces were evaluated under SEM. The surface of the industrially coated posts was examined using energy dispersive analysis by x-ray. Cylinders were cut to obtain microtensile sticks that were loaded in tension at a crosshead speed of 0.5 mm/min until failure. Statistical analysis was performed using Kruskal-Wallis analysis of variance followed by Dunn's multiple range test for post-hoc comparisons (p  lt  0.05). Weibull analysis was also performed. Results: The post/cement bond strength was significantly higher on fiber posts treated industrially (I: 23.14 +/- 8.05 MPa; II: 21.56 +/- 7.07 MPa; III: 22.37 +/- 7.00 MPa) or treated with XPBond adhesive (IV: 21.03 +/- 5.34 MPa) when compared to Prime&Bond NT application (V: 14.05 +/- 5.06 MPa), silanization (VI: 6.31 +/- 4.60 MPa) or no treatment (VII: 4.62 +/- 4.31) of conventional fiber posts (p  lt  0.001). Conclusion: The experimental industrial surface treatment and the adhesive application enhanced fiber post to resin cement interfacial strength. Industrial pretreatment may simplify the clinical luting procedure.

Surface treatments for improving bond strength to prefabricated fiber posts: A literature review

AB  - This literature review summarizes the research on fiber post surface treatments and provides information related to their benefit in enhancing bond strength to composites, based on the results of original scientific full papers from peer-reviewed journals listed in Pub Med. The search was conducted using the terms "fiber post," "surface treatment" "surface conditioning," "etching" and "sandblasting." A consistent number of in vitro studies that investigated the surface treatment of fiber posts in an attempt to improve bond strength have been published to date. Their results have been summarized in the following categories: chemical treatments and micromechanical treatments of fiber post surfaces (or a combination of both principles). The majority of available literature data is based on studies that investigated different "chairside" post superficial treatments. According to the in vitro results, surface conditioning improves fiber post bonding properties, and the bond strength of pre-treated fiber posts to restorative materials is satisfactory. Long-term clinical studies are needed prior to making a general recommendation for their use.

AB  - This study compared the effect of two drills and five cleaning regimens on post space debridement. One hundred extracted premolars were instrumented and obturated with warm vertical compaction of gutta percha. The teeth were divided into two groups according to the drill used to remove gutta percha/sealer and for post space preparation: a Largo drill (Largo; Dentsply, St Quentin en Yvelines, France) or a MTwo-PF drill (Sweden&Martina, Due Carrare, Padova, Italy). The following cleaning regimens were used: EDTA, ultrasonics, ultrasonics + EDTA, phosphoric acid, and distilled water. Scanning electron microscopic images of the post spaces were taken, and the presence of debris and of open dentin tubules were evaluated. The ultrasonics + EDTA, phosphoric acid, and EDTA groups were comparable in open tubules scores for both drills and in debris scores after the use of MTwo-PF (p > 0.05). The ultrasonics and control groups performed significantly worse (p  lt  0.05). The MTwo-PF drill resulted as effective as the Largo drill in obtaining a good post space cleaning, especially when followed by ultrasonics + EDTA irrigant regimen.

AB  - The aim of this study was to determine micro tensile bond strength of two and one-step self-etching adhesive systems to dentin. 25 intact human third molars were used. Flat surfaces of mid-coronal dentin were exposed and the teeth were divided into 5 groups (n=5). Composite build-ups were made using the following self-etching systems / composite materials: 2-step self-etching system Contax / Ecusphere Carat (DMG), 2-step self-etching system AdheSE / Tetric Ceram (Ivoclar Vivadent), 1-step 2-components self etching system Futurabond NR / Grandio (Voco) and 1-step 1-component self-etching adhesive G Bond / Gradia Direct (GC Corp.). Two-step etch-and-rinse system PQ Clear with Amelogen Universal composite resin (Ultra dent) was used in the control group. Bond strength was assessed with the non-trimming micro tensile technique. Statistical analysis was performed using the 1-way ANOVA, followed by the Tukey test for post-hoc comparisons (p lt 0.05). Investigated self-etching systems showed satisfactory values of micro tensile bond strength to dentin after 24 hours. Two-step self-etching system AdheSE resulted in significantly higher bond strength than all the other groups (p lt 0.001). Micro tensile bond strength of self-etching systems Contax, Futurabond NR and G Bond was comparable to the etch-and-rinse system PQ Clear. .
AB  - Cilj rada bio je da se odredi jačina veze dvofaznih i jednofaznih samonagrizajućih adhezivnih sistema sa dentinom. Površine dentina su eksponirane na 25 intaktnih trećih molara i zubi su podeljeni u 5 grupa (n=5). Izrađene su kompozitne nadogradnje korišćenjem sledećih samonagrizajućih adheziva / kompozitnih materijala: dvofazni samonagrizajući sistem Contax / Ecusphere Carat (DMG), dvofazni samonagrizajući sistem AdheSE / Tetric Ceram (Ivoclar Vivadent), jednofazni dvokomponentni samonagrizajući sistem Futurabond NR / Grandio (Voco) i jednofazni jednokomponentni samonagrizajući sistem G Bond / Gradia Direct (GC Corp.). Dvofazni adhezivni sistem sa potpunim nagrizanjem PQ Clear sa Amelogen Universal kompozitnim materijalom (Ultradent) korišćen je u kontrolnoj grupi. Jačina veze određena je testom otpornosti mikrouzoraka na kidanje. Rezultati su statistički analizirani jednofaktorskom analizom varijanse (ANOVA) i Tukey testom (p lt 0.05). Ispitivani samonagrizajući sistemi pokazali su zadovoljavajuće vrednosti jačine veze sa dentinom nakon 24 časa. Jačina veze dvofaznog samonagrizajućeg sistema AdheSE bila je značajno viša nego jačina veze u ostalim grupama (p lt 0.001). Jačine veze samonagrizajućih sistema Contax, Futurabond NR i G Bond nisu se statistički značajno razlikovale od jačine veze sistema sa potpunim nagrizanjem PQ Clear. .

AB  - Purpose: To evaluate the effect of different intermediate resin agents on composite-to-composite microtensile bond strength (mu TBS). Methods: Composite disks (8 x 4 mm) of Gradia Direct Anterior (N= 15) and Filtek Supreme XT (N=15), stored in a saline solution at 37 degrees C for I month, were sandblasted (Microetcher, 50 mu m-Al2O3), cleaned (35% H3PO4), and randomly divided into five groups (N=3) according to the intermediate agent applied. Two flowable composites (Gradia LoFlo; Filtek Supreme XT Flow) and two adhesives (Adper Scotchbond 1XT, Scotchbond Multi-Purpose) were used. As a control, no intermediate agent was applied., Each disk was incrementally repaired (W mm) with the same resin as the respective substrate. By serially cutting each repaired specimen, multiple sticks of about 0.8 mm(2) in cross-section were obtained and loaded in tension until failure at a cross-head speed of 0.5 mm/minute. Data were statistically analyzed. Failure mode was examined by stereo- and scanning electron-microscopy (SEM). Additional specimens were prepared to morphologically evaluate the composite-to-composite interfacial quality by SEM analysis. Results: Composite substrate and intermediate agent were significant factors (Two-way ANOVA, P lt  0.001). Bond strengths achieved with Gradia Direct Anterior were higher than Hick Supreme XT. The application of flowable composites resulted in statistically superior mu TBS than adhesives and control (Tukey test, P lt  0.001). Failure mode was predominantly cohesive (flowable composites), cohesive/adhesive (adhesives) and adhesive (no treatment). Flowable composites and adhesives showed a good interfacial quality.

AB  - Purpose: To summarize research conducted on self-adhesive cements and provide information on their properties, based on the results of original scientific full-length papers from peer-reviewed journals listed in PubMed. Materials and Methods: The search was conducted using the term "self-adhesive cement OR (trade names of currently available products)". Results: Only in vitro studies that investigated two commercially available self-adhesive cements have been published so far. The results were summarized into the following categories: adhesion to tooth substrates (enamel, dentin, root dentin), adhesion to restorative materials (endodontic posts, ceramics, titanium abutments), marginal adaptation, microleakage, mechanical properties, biocompatibility, chemical adhesion and fluoride release, and ratings in clinical use. Conclusion: The majority of available literature data is based on studies that investigated one of the self-adhesive cements that are currently available to clinicians. According to the in vitro results, self-adhesive cement adhesion to dentin and various restorative materials is satisfactory and comparable to other multistep resin cements, while adhesion to enamel appears to be a weak link in their bonding properties. Long-term clinical performance of these materials needs to be assessed prior to making a general recommendation for their use.

The results were summarized into the following categories: adhesion to tooth substrates (enamel, dentin, root dentin), adhesion to restorative materials (endodontic posts, ceramics, titanium abutments), marginal adaptation, microleakage, mechanical properties, biocompatibility, chemical adhesion and fluoride release, and ratings in clinical use. Conclusion: The majority of available literature data is based on studies that investigated one of the self-adhesive cements that are currently available to clinicians. According to the in vitro results, self-adhesive cement adhesion to dentin and various restorative materials is satisfactory and comparable to other multistep resin cements, while adhesion to enamel appears to be a weak link in their bonding properties. Long-term clinical performance of these materials needs to be assessed prior to making a general recommendation for their use.",

Methods: Single posts in three different sizes (l, 2 and 3; DT Light Posts, RTD; DT), as control, and seven different combinations of experimental small posts (Abrasive Technology), reproducing the sizes of several endodontic files were cemented in endodontic resin blocks. The combinations of small posts were made combining 2-4 small posts so as to reach the sizes of standardized DT posts. The posts were loaded at an angle of 45 to the long axis of the block using a crosshead speed of 1 mm/minute until specimen failure. Ten posts/combinations were tested. Statistical analysis was performed using one-way ANOVA followed by Tamhane test for the post hoc comparisons (P lt  0.05). Results: For the single posts, fracture resistance increased when increasing the diameter of the post. The use of multiple posts resulted in fracture resistances comparable to that of the single post for two of the three diameters studied (DT1 and DT2). For the largest diameter (DT3), the use of multiple posts resulted in lower fracture resistances than did the single post. (Am J Dent 2008;21:373-376).

AB  - Purpose: To evaluate the effect of different curing modes of dual-curing luting systems and root regions on the pushout strength of fiber posts to intraradicular dentin. Materials and Methods: Forty-two extracted premolars with a single root canal were sectioned at the cementoenamel junction and the roots were enclodontically treated. The roots were divided into two groups according to two dual-curing luting systems: (1) XP BOND-Dual Cure/Calibra resin cement; (2) XP BOND-Dual Cure/FluoroCore 2. For each luting system, three different curing modes were applied to the dentin adhesive and resin cement: "Self-cure and Self-cure (SC&SC)", "Self-cure and Dual-cure (SC&DC)", and "Dual-cure and Dual-cure (DC&DC)". Translucent Easy fiber posts (Dentsply Maillefer) were luted in the roots. A thin-slice push-out test was performed, and the data of push-out strength were analyzed using three-way ANOVA with luting system, curing mode, and root region (apical, middle and coronal) as factors. Tukey's test was used for post-hoc comparisons. Results: The push-out strength of XP BOND-Dual Cure/Calibra was significantly lower when Calibra resin cement was self-cured than when it was dual-cured (SC&SC: 6.04 +/- 2.65 MPa; SC&DC: 10.69 +/- 3.01 MPa; DC&DC: 10.72 +/- 3.63 MPa; p  lt  0.05). The curing modes did not affect the push-out strength of XP BOND-Dual Cure/FluoroCore 2 (SC&SC: 7.90 +/- 3.94 MPa; SC&DC: 8.32 +/- 2.73 MPa; DC&DC: 9.27 +/- 4.12 MPa; p > 0.05). The Coronal push-out strength was significantly higher than the apical push-out strength (p  lt  0.05). Conclusion: Retention of fiber posts in root canals was affected by the curing modes of dual-curing luting system and root regions.

The effect of baseline factors (tooth type, number of residual coronal walls, and type of definitive restoration) on restoration failure was assessed. Materials and Methods: The consecutive sample design included 150 patients. A total of 162 teeth (57 anterior and 105 posterior) were restored. Sixty-nine teeth had 3 or 4 residual coronal walls, while 93 teeth had 2 or fewer walls. Crowns and direct resin composite restorations were placed in 121 and 41 teeth, respectively. After 23 to 25 months, all patients were evaluated. Logistic regression was used to identify the joint effect of variables recorded at baseline (P  lt  .05). Results: The only failure modes observed were post debonding (4.3%, 2 in anterior teeth and 5 in posterior teeth) and endodontic failure (3.0%, 2 in anterior teeth and 3 in posterior teeth). All post debondings occurred in teeth with 2 or fewer coronal walls that were crown covered, All endodontic failures occurred in crown-covered teeth (1 failure in a tooth with 3 walls and the remaining 4 failures in teeth with 2 or fewer walls). Logistic regression found no statistical significance for any of the variables recorded at baseline. Conclusion: Restorations placed with the use of a fiber post and core resulted in 4.3% post debondings and 3.0% endodontic failures after 2 years of clinical service.

Mechanical failure of restored teeth with fiber posts can be related to the amount of residual coronal structure.

AB  - Purpose: To evaluate the effect of different silane agents and air-drying temperatures on the repair strength of a microfilled hybrid composite. Materials and Methods: Composite cylinders (8 x 4 mm) of Gradia Direct Anterior (GC, N = 36), stored in a saline solution at 37 degrees C for 1 month, were sandblasted (50-mu m aluminium oxide), cleaned (35% phosphoric acid) and randomly divided into six groups (n = 6). Two prehydrolyzed silane primers (Monobond-S, lvoclar-Vivadent, Porcelain Primer, Bisco), a non prehydrolyzed silane primer (Porcelain Liner M, Sun Medical) and three silane/adhesive coupling agents (Porcelain Bond Activator-PBA/Clearfil New Bond, PBA/Clearfil SE Bond, PBA/Clearfil Tri-S Bond, Kuraray) were investigated. Siiane-coated surfaces were air dried at two different temperatures (23 degrees C and 38 degrees C) and repairs (8 x 8 mm) were fabricated (Gradia Direct Anterior). Unrepaired composite cylinders (8 x 8 mm, n = 6) were used as control to evaluate the cohesive strength of the material. Microtensile bond strength measurements (mu TBS) were performed. Results: The silane agent applied (p  lt  0.001), the airdrying temperature (p  lt  0.001) and their interaction (p  lt  0.001) were significant factors (two-way ANOVA, Tukey test; p  lt  0.05). Silane primers achieved inferior mu TBS when air dried at 23 degrees C as compared to silane/adhesive blends. Warm air-drying was significantly beneficial to composite repairs mediated by silane primers. Comparable results were achieved by silane/adhesive couplings at 23 degrees C and 38 degrees C. At 38 degrees C all the intermediate agents resulted in repair mu TBS that were comparable to the 24-h cohesive strength of the composite (one-way ANOVA, Dunnett t-tests; p  lt  0.05). Conclusion: The chemical interactions between silane primers and compozite substrate may be optimized through warm airdrying. Silane/adhesive couplings were not influenced by the air drying temperature.

Adhesion between fiber posts and resin luting agents: A microtensile bond strength test and an SEM investigation following different treatments of the post surface

AB  - Purpose: (1) To evaluate the interfacial strength between FRC Postec posts and three luting agents (Multilink, Variolink 11, and MultiCore Flow) following different surface treatments, and (2) to observe the effect of sandblasting (Rocatec Pre) on the surface morphology of methacrylate-based fiber posts. Materials and Methods: The posts received one of the following surface treatments: (1) sandblasting, (2) sandblasting + silanization, (3) silanization or (4) no treatment. The three luting agents were bonded to the post and the post-cement bond strength was evaluated with the microtensile test. SEM observation of sandblasted and nontreated posts was performed. Post-cement interfaces were also evaluated. Data were statistically analyzed with two-Way ANOVA with post treatment and luting agent as factors. Tukey's test was applied for post-hoc comparisons. Results: Post treatment and the interaction between type of luting agent and type of post treatment were significant factors for bond strength (p  lt  0.001). The type of luting agent did not significantly influence bond strength (p = 0.07). Sandblasting + silanization performed better than sandblasting or no treatment (p  lt  0.001). Silanization resulted in significantly higher bond strengths than no treatment (p = 0.045). No differences were detected between sandblasting + silanization and silanization. SEM observation revealed an increased surface roughness and exposure of fibers in sandblasted posts. Conclusion: Silanization was confirmed to be a reliable method for improving the bond strength of resin luting agents to fiber posts. Bond strength of resin luting agents to fiber posts was not influenced by the type of luting agent. The sandblasting procedure modified the methacrylate-based post surface texture.
