Asthma and periodontal health in children

AB  - Background/Aim. Oral health is an important part of overall health. Good oral health is important for oral diseases prevention and health maintenance of respiratory system. The aim of the study was to evaluate oral hygiene and periodontal health parameters of asthmatic children and to compare them with children without asthma as well as to evaluate those parameters according to type of used medications and time of taking medications in children with asthma. Methods. This epidemiological study included 68 children with asthma and 68 children without asthma or any other chronic disease aged from 6 to 16 years. Parameters used in this study were Greene-Vermillion index, Löe-Silness gingival index and Community Periodontal Index (CPI). Results. Good oral hygiene (31.1%) was more present in children without asthma whereas poor hygiene (20.0%) was more frequent in children with asthma (p  lt  0.001). Healthy gingiva was more frequent in children without asthma (25%) while mild (58.8%) and moderate gingival inflammation (5.9%) were more frequent in the group of children with asthma (p  lt  0.01). Mean CPI values were higher in children with asthma (p  lt  0.001). Mean values of Plaque Index, Gingival Index and CPI did not show statistically significant difference in relation to type of administered medication. However, taking medications in the afternoon was related to higher mean values of Plaque Index and Gingival Index (p  lt  0.05) within the group of children with asthma. Conclusion. Children with asthma had poorer oral hygiene and were diagnosed with greater values of oral hygiene and periodontal indices compared with the group of children without asthma. For this reason, it is necessary to promote oral health and establish good oral hygiene habits in asthmatic children.
AB  - Uvod/Cilj. Oralno zdravlje je važan deo opšteg zdravlja. Dobro oralno zdravlje je važno kako za prevenciju oralnih oboljenja tako i za održavanje zdravlja respiratornog sistema. Cilj istraživanja bio je da se proceni oralna higijena, stanje zdravlja parodontalnih tkiva dece obolele od astme, te da se te vrednosti uporede sa pronađenim vrednostima kod dece bez astme, kao i da se dobijene vrednosti procene u odnosu na vrstu i vreme upotrebljenog leka u grupi dece sa astmom. Metode. U epidemiološku studiju je uključeno 68 dece sa astmom i 68 dece bez astme ili bilo koje druge hronične bolesti, uzrasta od 6 do 16 godina. Parametri korišćeni u studiji su bili plak indeks po Greene-Vermillionu, Löe- Silnessov gingivalni indeks i Indeks stanja parodoncijuma u zajednici (Community Periodontal Index - CPI). Rezultati. Dobra oralna higijena bila je prisutnija kod dece bez astme (31,1%), dok je loša češće bila zastupljena kod dece sa astmom (20,0%) (p  lt  0,001). Zdrava gingiva je bila zastupljena češće kod dece bez astme (25%), dok je blaga (58,8%) i umerena (5,9%) upala desni bila prisutnija u grupi dece sa astmom (p  lt  0.01). Prosečne vrednosti CPI indeksa bile su veće kod dece sa astmom (p  lt  0.001). Prosečne vrednosti plak indeksa, gingivalnog indeksa i CPI nisu ukazale na značajnu razliku u odnosu na vrstu primenjenog leka. Međutim, uzimanje lekova u popodnevnim satima je bilo povezano sa višim vrednostima plak i gingivalnog indeksa (p  lt  0,05) u grupi dece sa astmom. Zaključak. Deca sa astmom su imala lošiju oralnu higijenu i kod njih su utvrđene veće vrednost plak i parodontalnih indeksa u poređenju sa grupom dece bez astme. Iz tog razloga neophodno je promovisanje oralnog zdravlja kao i uspostavljanje dobrih oralno higijenskih navika kod dece sa astmom.

Stress and strain analyses of removable partial denture abutment tooth in relation to the position of the minor connector

AB  - Introduction/objective: For optimum loading distribution, the angle formed by the occlusal rest and the vertical minor connector from which it originates should be less than 90°. The objective of the article was to visualize the optimum angle between the occlusal rest and the minor connector in terms of intensity and distribution of occlusal loads using finite element analysis. It was the intention, concerning biomechanical behavior, to document that the optimum angle between the occlusal rest and the minor connector should be less than 90°. Methods: Three different virtual models of partial edentulous Kennedy III class were created using the CATIA design computer program with different angles between the occlusal rest and the minor connector. Stress distribution after simulated occlusal loading was analyzed using the finite element method. Results: Comparing the results obtained for three models, the highest stress values were seen in model 3 (the angle between the occlusal rest and the small connector is greater than 90°) whether the load is applied in the middle or at the end of the saddle. Conclusion: Within limitations and on the basis of the study results, the minimum compressive stress was seen in model 1, where the angle between the occlusal rest and the minor connector was less than 90° whether the load is applied in the middle or at the end of the saddle. It is recommended that obtuse angle between the rest and the minor connector should be avoided due to potential hazardous stress concentration on abutment teeth.
AB  - Uvod/Cilj: Okluzalni naslon i mala spojnica treba da zaklapaju međusobni ugao manji od 90 stepeni, kako bi se obezbedilo najpovoljnije prenošenje opterećenja. Cilj rada je bio da se metodom konačnih elemenata prikaže ugao između okluzalnog naslona i male spojnice koji je najpovoljniji za prenošenje okluzalnog opterećenja. Namera je bila da se, posmatrano sa biomehaničkog aspekta, dokumentuje da je ugao manji od 90 stepeni između okluzalnog naslona i male spojnice najpovoljniji. Metod: Izrađena su tri različita virtuelna modela krezube vilice klase krezubosti Kenedi III u programu CATIA sa modelovanim različitim uglovima između okluzalnog naslona i male spojnice. Analiza distribucije napona i deformacija nakon simuliranog okluzalnog opterećenja izvršene su metodom konačnih elemenata. Rezultati: Posle simuliranog okluzalnog opterećenja sva tri modela najveći napon je uočen kod modela 3 (ugao između okluzalnog naslona i male spojnice veći od 90 ste- peni), bez obzira na to da li je opterećenje aplikovano na sredini ili na kraju sedla. Zaključak: U okviru ograničenja u istraživanju, najmanji kompresioni napon uočen je u modelu 1 (ugao između oklu- zalnog naslona i male spojnice manji od 90 stepeni) bez obzira na to da li je opterećenje aplikovano na sredini ili na kraju sedla. Preporučuje se da se tup ugao između okluzalnog naslona i male spojnice izbegava zbog mogućih štetnih koncentracija napona na retencionom zubu.

Changes in Oral Health Related Quality of Life (OHRQoL) and Satisfaction with Conventional Complete Dentures Among Elderly People

AB  - Purpose: To assess the changes in oral health-related quality of life (OHRQoL) and satisfaction among older adults after receiving new complete dentures, and the association of age, gender, education, medical history, past prosthetic history (number of previous complete dentures and time since current complete dentures) and quality of existing complete dentures with patients' ratings of satisfaction with their complete dentures. Materials and Methods: The study comprised 114 community-dwelling adults >= 65 year of age, all wearing complete dentures in both jaws. The sample was selected from four senior day centres. Data were collected using clinical oral examinations and a self-administered questionnaire. Dentures were clinically evaluated for retention, stability, occlusion, articulation and vertical dimension. The questionnaire recorded data on sociodemographic information, previous denture history, oral satisfaction scale (OSS), and OHRQoL through the Oral Impacts on Daily Performance (OIDP) scale. Results: There was a significant improvement in OHRQoL after treatment (mean OIDP score: 2.7) compared to before treatment (mean OIDP score: 6.6). The largest changes were in relation to impacts on eating (33.3% to 15.9%) and smiling (17.5% to 4.5%). Six months after placement of the dentures, patient satisfaction improved compared to before treatment (p  lt  0.001). The results of this study demonstrated that satisfaction was correlated with age, gender, and past prosthetic history in the patients rehabilitated with complete dentures. Conclusions: The provision of new dentures among older adults resulted in significantly better satisfaction and OHRQoL than wearing existing dentures. There was no significant correlation between the clinical assessment of the dentures and patients' satisfaction with them.

Numerical modeling of the edentulous mandible with a complete denture using multiblock method

AB  - In this paper, finite element model of edentulous lower jaw (mandible) is generated, as well as the model of support layer and complete denture using various software. The aim of this study was to produce 3D digital mandibular complete denture, mucosa, and supporting bone models using reverse engineering and evaluate effect of mucosa thickness and resiliency on stress distribution of complete denture. In this study we used a model of mandibular complete denture and 5 mm thickness of mucosa (which represent the resiliences soft). The influence of resilient mucous membranes layer thickness is analysed by assigning loads to simulate the force due to chewing on the lower right and left central incisor and lower right and left first molar. Based on the numerical results, we conclude that the maximum stress values were concentrated in cortical bone.

abstract = "In this paper, finite element model of edentulous lower jaw (mandible) is generated, as well as the model of support layer and complete denture using various software. The aim of this study was to produce 3D digital mandibular complete denture, mucosa, and supporting bone models using reverse engineering and evaluate effect of mucosa thickness and resiliency on stress distribution of complete denture. In this study we used a model of mandibular complete denture and 5 mm thickness of mucosa (which represent the resiliences soft). The influence of resilient mucous membranes layer thickness is analysed by assigning loads to simulate the force due to chewing on the lower right and left central incisor and lower right and left first molar. Based on the numerical results, we conclude that the maximum stress values were concentrated in cortical bone.",

Validity and reliability of the Oral Impacts on Daily Performance (OIDP) scale in the elderly population of Bosnia and Herzegovina

AB  - Validity and reliability of the Oral Impacts on Daily Performance (OIDP) scale in the elderly population of Bosnia and Herzegovina Objectives: To adapt the Oral Impacts on Daily Performance (OIDP) index for elderly people in Bosnia and Herzegovina and test its validity, reliability and responsiveness to change. Background: Clinical measures alone may not be adequate for assessing the oral health of individuals. Subjective oral health indicators tested within a particular cultural context may not be relevant across cultures. Materials and methods: The study population comprised 231 free-living adults aged 65 years or older. The OIDP was cross-culturally adapted from English into the Serbian language and its psychometric properties were tested. Data were collected using a clinical examination and a questionnaire containing the OIDP. Results: In terms of reliability, Cronbach's alpha coefficient was 0.82 and the intraclass correlation coefficient 0.88. The very high correlation of OIDP with self-rated oral health (r = 0.78) verified criterion validity, while construct validity was demonstrated through its significant and graded associations with other subjective health measures. OIDP change scores on a treated subsample showed moderate effect size (0.59) and were associated with perceptions of oral health change, providing evidence for its responsiveness to change. Conclusion: The Bosnian version of the OIDP showed satisfactory validity, reliability and responsiveness to change confirming its appropriateness for use among older populations in Bosnia and Herzegovina.

Prevalence, severity, and clinical determinants of oral impacts in older people in Bosnia and Herzegovina

AB  - This study aimed to assess the prevalence and the severity of oral impacts and the relationships between oral health-related quality of life (OHRQoL) and clinical measures of oral function in a sample of older adults in Bosnia. The sample comprised 261 community-dwelling adults of >= 65 yr of age. Participants were randomly drawn from three senior day-centres for elderly people. Data were collected using clinical examination and a questionnaire. The OHRQoL was assessed through the Oral Impacts on Daily Performances (OIDP) measure, in terms of the prevalence, intensity, and extent of oral impacts. Overall, 55.2% reported at least one oral impact in the last 6 months. Difficulty eating (43.3%) and difficulty speaking (34.1%) were the most common impacts. These were also the most severely affected performances, while going out and enjoying the contact of other people were the least severe. A higher number of natural teeth, natural occluding pairs, and anterior and posterior occluding pairs were all significantly associated with a lower prevalence of oral impacts and correlated with the OIDP score, even after adjusting for sociodemographic factors. The prevalence of oral impacts was high and there was a strong and consistent association between OHRQoL and clinical dental indicators of function.
