The presence of periopathogenic bacteria in subgingival and atherosclerotic plaques- An age related comparative analysis

AB  - Introduction: There is a known connection between periodontitis and atherosclerosis and the presence of periopathogens in blood vessels. However, changes of the oral microflora related to the aging process and its possible effects on atherosclerosis, have yet to be analyzed. The aim of this study was to assess temporal changes in the frequency of periodontal bacteria in the subgingival plaque and in atherosclerotic blood vessels of patients with atherosclerosis. Methodology: The study included 100 patients with atherosclerosis and periodontitis, divided into two groups, below and over 60 years of age. Clinical examinations were performedand subgingival plaque specimens were collected as well as biopsy specimens from the following arteries: coronary (34), carotid (29), abdominal (10), femoral (10), mammary (13) and iliac (4). Subgingival and artery specimens were subjected to PCR detection of 5 major periodontal pathogens: Porphyromonas gingivalis (Pg), Prevotella intermedia (Pi), Aggregatibacter actinomycetemcomitans (Aa), Tannerella forsythensis (Tf) and Treponema denticola (Td). Results: Tf was the most and Td the least frequent bacteria in both age groups and in both types of samples. The frequencies of bacteria in subgingival versus atherosclerotic samples were: Tf (76%: 53%), Pi (71%: 31%), Pg (60%: 38%), Aa (39%: 14%) and Td (21%: 6%). Only Aa and Pi showed a significant difference of prevalence between younger and older patients. The most colonized artery was a. coronaria, followed by a. carotis, a. abdominalis, a. mammaria, and a. femoralis. Conclusions: Patient's age and the distance of a given blood vessel from the oral cavity influenced microbiological findings in the atherotic plaque.

Correlation between sagittal jaw position and jaw relationship in children with skeletal class III malocclusion

AB  - Introduction/Objective The evaluation of sagittal jaw relationship is crucial in orthodontic diagnostics, orthodontic treatment planning, and monitoring of its results. A large number of parameters have been established for their assessment. The aim of this study was to determine the significance of the correlation between the indicators of sagittal jaw relationship, ANB, AOBO, AFBF, NAPg, and OJ, one with another, and with indicators of the sagittal position of the jaws, SNA and SNB, in children with skeletal class III malocclusion. Methods A total of 100 children with mixed dentition, of both genders, based on the profile cephalometric analysis, were divided into two equal groups: group 1 (test group) - children with skeletal class III (n = 50), group 2 (control group) - children with skeletal class I (n = 50). Results In children with skeletal class III malocclusion, significant correlations were found among the indicators of sagittal jaw relationships ANB, AOBO, AFBF, NAPg, OJ, mutually, except between AOBO and AFBF. Significant correlations of these parameters were also found with the SNB angle, but not with the SNA angle. Conclusion Confirmed significant correlation between tested indicators in the sagittal jaw relation indicates that, if we find skeletal jaw relationship class III, or just a simple reversed incisors overbite in children in routine application of any of the above mentioned parameters in everyday work, it should be indubitably checked and monitored using a larger number of parameters, especially those that define the sagittal position of the mandible.
AB  - Uvod/Cilj Procena sagitalnih međuviličnih odnosa od ključnog je značaja u ortodontskoj dijagnostici, planiranju ortodontskog lečenja i praćenju njegovih rezultata. Za njihovu procenu ustanovljen je veliki broj parametara. Cilj ovog rada bio je da utvrdi značajnost korelacija između pokazatelja sagitalnih međuviličnih odnosa, ANB, AOBO, AFBF, NAPg i OJ, međusobno i sa pokazateljima sagitalnog položaja vilica, SNA i SNB, kod dece sa malokluzijom III skeletne klase. Metode Ukupno 100 dece sa mešovitom denticijom, oba pola, selekcionisano je na osnovu kefalometrijskih analiza profilnih telerendgenskih snimaka na dve jednake grupe: grupa 1 (ispitna grupa) - deca sa malokluzijom III skeletne klase (n = 50), grupa 2 (kontrolna grupa) - deca sa I skeletnom klasom (n = 50). Rezultati Kod dece sa malokluzijom III skeletne klase utvrđene su značajne korelacije između svih ispitivanih pokazatelja sagitalnih međuviličnih odnosa (ANB, AOBO, AFBF, NAPg, OJ), osim između AOBO i AFBF. Značajne korelacije ovih parametara ostvarene su, takođe, i sa uglom SNB, dok sa uglom SNA nisu. Zaključak Utvrđena značajna korelacija između ispitivanih pokazatelja sagitalnih međuviličnih odnosa ukazuje da ukoliko se u svakodnevnom radu rutinskom primenom bilo kog od pomenutih parametara kod dece utvrdi skeletni međuvilični odnos III klase ili samo jednostavan obrnut preklop sekutića, treba ga obavezno proveriti i pratiti primenom većeg broja parametara, posebno onih koji definišu sagitalni položaj mandibule.

Analysis of intracellular enzymes in saliva of patients with aggressive periodontitis

AB  - Introduction. Aggressive periodontitis is characterized by rapid destruction of periodontal tissue. It is more commonly found in young adults. Analysis of biochemical markers in saliva is very important to monitor the level of periodontal tissue destruction. The aim of this study was to analyze the intracellular enzymes aspartate aminotransferase (AST), alanine aminotransferase (ALT) and alkaline phosphatase (ALP) in the saliva of patients with aggressive periodontitis. Material and Methods. The study included 22 patients with aggressive periodontitis and 28 healthy subjects (control group). Enzyme activity was measured in mixed non-stimulated saliva using kinetic methods in a spectrophotometer and expressed in international units per litre (U/L). Results. The activity of enzymes ALT and ALP was significantly higher in saliva of patients with aggressive periodontitis compared to healthy subjects, while AST activity was not significantly different. Also, there was no correlation between the activity of intracellular enzymes in saliva and clinical parameters such as gingival index, bleeding index, probing depth in patients with aggressive periodontitis. Conclusion. Obtained results indicate that salivary enzymes AST, ALT and ALP can be used as biochemical markers to aid in diagnosis of aggressive periodontitis.
AB  - Uvod. Agresivni periodontitis se odlikuje veoma brzim propadanjem parodontalnog tkiva i češće se javlja kod mladih ljudi. Analiza biohemijskih markera u pljuvački je vrlo značajna za utvrđivanje stepena oštećenja parodontalnog tkiva. Cilj studije je bio analiza unutarćelijskih enzima - aspartat- aminotransferaze (AST), alanin-aminotransferaze (ALT) i alkalne fosfataze (ALP) - u pljuvački osoba obolelih od agresivnog periodontitisa. Materijal i metode rada. U studiju su uključena 22 pacijenta s agresivnim peridontitisom, dok je kontrolnu grupu činilo 28 zdravih ispitanika. Aktivnost enzima je analizirana u mešovitoj nestimulisanoj pljuvački kinetičkim metodama na spektrofotometru, a izražena je u internacionalnim jedinicama po litru (U/l). Rezultati. Aktivnost enzima ALT i ALP bila je statistički značajno povećana u pljuvački ispitanika obolelih od agresivnog periodontitisa u odnosu na grupu zdravih ispitanika, dok se aktivnost AST nije statistički značajno razlikovala. Takođe, nije postojala korelacija između aktivnosti unutarćelijskih enzima u pljuvački i kliničkih parametara (gingivalnog indeksa, indeksa krvarenja, dubine parodontalnog džepa). Zaključak. Dobijeni rezultati pokazuju da su enzimi AST, ALT i ALP u pljuvački potencijalni biohemijski markeri za pomoć u dijagnostikovanju agresivnog periodontitisa.
