Marked epithelial to mesenchymal transition in surgical margins of oral cancer-an in vitro study

AB  - Epithelial to mesenchymal transition (EMT) is a feature of several types of human cancer, including oral squamous cell carcinoma (OSCC). In the present study, tumor and margin cell cultures obtained from patients with OSCC were used to determine the expression patterns of certain EMT-associated markers, including vimentin, alpha -smooth muscle actin, SLUG and SNAIL. In addition, other EMT-associated features, including clonal, proliferative and migratory potential were compared between the two cell types. Cell cultures were generated from tumor and margin tissue samples from 6 patients and cultured up to the fifth passage. EMT marker expression was assessed by reverse transcription-quantitative PCR. Cell proliferation, colony formation and scratch wound healing assays were conducted to characterize the two cell types in terms of proliferation rates, clonality and motility. All of the studied markers were expressed in tumor and margin cells. Although no significant differences were noted with regard to the aforementioned markers, their expression tended to be higher in margin cultures than in tumor cultures. The expressions of the EMT markers were also higher in the fifth passage compared with those noted at the first with a few exceptions. The rates of proliferation and cell migration were decreased during passages, while the number of colonies was increased in both types of cell culture. Tumor and margin cells indicated certain similarities with regard to EMT transition characteristics.

Predictive Value of Opto-magnetic Imaging Spectroscopy in Discriminating Oral Squamous Cell Carcinoma from Non-tumor Tissue in Surgical Margins

AB  - PurposeNegative surgical margins status is of extreme importance for better prognosis and lower recurrence rate in patients undergoing surgical treatment for oral squamous cell carcinoma (OSCC). Standard histopathology is time consuming and current optical procedures come with various limitations. We examined the potential of a new imaging technique-opto-magnetic imaging spectroscopy (OMIS) in discriminating tumor (OSCC) from adjacent non-tumor oral cavity tissue.MethodsA total of 46 samples from 21 patients were included in this study. Samples were taken from a fresh surgical specimen of primary OSCC. Digital imaging of samples was performed within 60 min of resection using OMIS device. Images were processed through a specifically designed convolution algorithm based on light-matter interaction. As a result a convolution spectrum was generated. Predictive value of spectral data was assessed using correlation tests and Naive Bayes classification model.ResultsThere was no significant correlation in distribution of characteristic peaks between tumor and non-tumor tissue (P>0.05). Tumor tissue showed more magnetic activity compared to non-tumor tissue. Naive Bayes classifier with kernel density estimation discriminated tumor from non-tumor tissue with the accuracy of 82.61%, 86.96% sensitivity, 78.26% specificity and the AUC value of 0.917.ConclusionOMIS seems to be a promising optical method for ex vivo characterization of OSCC and non-tumor tissue. Further investigation is necessary to determine how tissue type and level of pathological transformation impact OMIS results. Ultimately, this could aid surgeons in using this method as an in vivo indicator for surgical resection with safe margins.

AB  - The aim of this study was to investigate the biocompatibility of nanostructured materials based on highly active calcium silicates mixed with different radiocontrast agents in comparison to MTA(+) using in vitro and in vivo model. Morphology of materials' samples was analyzed using SEM while the phase compositions were identified by XRD. pH values of materials' suspensions were conducted by pH-meter. The cytotoxicity of materials' solutions was tested by MTT test (100, 50, 25 and 12.5mg/ml). LDH and H-3-thymidine assay were utilized for biocompatibility investigations of materials' eluates (24h, 7 day and 21 day). Eighteen Guinea pigs were used for intramuscular implantation, as teflon tubes with freshly prepared materials were placed into intramuscular pockets. All samples were composed of round and needle-like particles equally distributed with Ca/Si ratio 2.7 at%, with the presence of hydrated calcium silicate phases. The pH values of ALBO-MPCA(1) and ALBO-MPCA(2) were high alkaline, while in case of MTA(+) they were lower and continuously declined (p lt 0.05). Investigated materials didn't exhibit dose-dependent effect on metabolic activity of L929 cells (p>0.05). Significant differences in the percentage of cytotoxicity between diluted and undiluted extracts between all tested materials after 24h and 7 day were noticed (p lt 0.05). Increase in L929 cells proliferation was noticed in case of undiluted eluates of ALBO-MPCA(1) and ALBO-MPCA(2) after 7 day (p lt 0.05). There were no statistically significant differences in the intensity of inflammatory response between investigated materials and control group after 60 day (p>0.05). Evaluation of biocompatibility of both ALBO-MPCA(1) and ALBO-MPCA(2) indicate their potential clinical use. [GRAPHICS] .

The amplification of c-erb-B2 in cancer-free surgical margins is a predictor of poor outcome in oral squamous cell carcinoma

AB  - The tumour subtype, TNM classification, and histopathological data are sometimes not sufficient for understanding and assessing the behaviour of oral cancers. In an attempt to find additional markers of tumour biology and behaviour, this study sought to determine the incidence and consequently the relevance of c-erb-B2, c-Myc, and H-ras gene alterations in tumour-free margins of oral squamous cell carcinoma (OSCC). Fifty samples of OSCC were analyzed for c-erb-B2 and c-Myc amplification by real-time polymerase chain reaction and for H-ras point mutations by sequencing. A relatively high incidence of genetic lesions was detected: 22% of cases had c-erb-B2 and 30% had c-Myc amplification, whilst only 12% harboured H-ras mutations. Kaplan-Meier analysis and the log-rank test showed statistically significant differences in 5-year survival rates and relapse between patients with tumour margins positive for c-erb-B2 amplification and those with margins that were negative (P = 0.002). H-ras and c-Myc alterations could not be associated with tumour behaviour. Molecular analysis of margins, targeting cancer genes, could identify additional, independent predictors of risk and outcome in OSCC.

AB  - ObjectivesThe aim of this study was to investigate survivin, cyclin D1, and p21hras expression in keratocystic odontogenic tumors before and after decompression, as well as in pericoronal follicles. A potential correlation between the expression levels of these proteins was also investigated. Materials and methodsWe analyzed eighteen keratocystic tumors treated by decompression and subsequent enucleation along with seven pericoronal follicles using immunohistochemistry. ResultsKeratocystic tumor samples, both before and after decompression, were positive for each of the investigated proteins. In pericoronal follicles, survivin exhibited cytoplasmic staining in contrast to nuclear staining in keratocystic tumors. Cyclin D1 expression was negative in pericoronal follicles, and p21hras expression was similar in both groups. Survivin showed significantly higher expression after decompression, while cyclin D1 and p21hras remained unchanged (P=0.039, P=0.255, P=0.913, respectively). There was no correlation between these proteins neither before nor after decompression. ConclusionsWithin the limits of the study, we can conclude that following decompression, keratocystic odontogenic tumors preserve distinct immunohistochemical profiles of cyclin D1 and p21hras expression, despite substantial reduction in size of the lesions. Significant increase of survivin expression after decompression might be attributed to higher level of epithelial proliferation caused by this procedure.

AB  - Objectives: to investigate p16(INK4a) and p14(ARF) tumor suppressor gene methylation status, determine telomere length and assess the importance of these epigenetic and genetic parameters in the development of pleomorphic adenoma and carcinoma ex pleomorphic adenoma of the parotid salivary glands. Materials and Methods: Genomic DNA from paraffin-embedded samples of 50 pleomorphic adenomas and 10 carcinomas ex pleomorphic adenoma was subjected to methylation specific polymerase chain reaction for hypermethylation analyses and real time polymerase chain reaction for the relative telomere length calculations. Results: Promoter hypermethylation of the two genes was a very frequent event in both neoplasms between 60% and 90% of samples were hypermethylated - but without significant difference between the groups. The mean relative telomere length in the pleomorphic adenoma group was significantly increased in comparison to the control group (P = 0.00), and significantly decreased in comparison to the carcinoma group (P = 0.05). Telomeres were also longer in myxoid and cellular histological subtypes of adenomas than in the classic type (P = 0.044 and P = 0.018, respectively). Longer telomeres were more frequent in tumors with hypermethylated p14(ARF) alleles (P = 0.013). Conclusion: Promoter hypermethylations seems to be an important mechanism of p16(INK4a) and Pl4(ARF) inactivation in parotid gland tumors. Telomeric lengthening appears to be involved in the pathogenesis of both benign and malignant tumors of the parotid glands.

AB  - Mucoepidermoid carcinoma (MEC) harbors a recurring t(11;19) translocation with an associated novel fusion oncogene-MECT1-MAML2. The CRTC1-MAML2oncogene disrupts normal cell-cycle and differentiation, contributing to tumor development. The objectives of this study were to establish the incidence of CRTC1-MAML2 fusion in Serbian patients and estimate its relevance as a genetic marker of MEC behavior. In this retrospective study, 20 cases of MEC of salivary glands were tested for the presence of CRTC1-MAML2 fusion using reverse transcriptase-polymerase chain reaction. Clinicopathological parameters and survival data were examined in relation to fusion status. The CRTC1-MAML2 fusion was detected in 40% of MECs and its presence was associated exclusively with low-intermediate grade tumor histology (P = 0.02) and favorable clinical outcome, with 100 % overall survival rate (P=0.046). The study has shown that the presence of the CRTC1-MAML2 fusion can serve as an additional diagnostic and prognostic marker for mucoepidermoid carcinomas.

AB  - Background: Genetic studies of salivary gland neoplasms were mainly focused on chromosomal changes, and some specific patterns of chromosome translocations have been described. However, molecular alterations, in particular the role of HER-2/H-ras/c-myc signalling cascade in pleomorphic adenoma pathogenesis (PA), are less well characterized. In addition, data on single nucleotide polymorphisms (SNPs) as potential susceptibility factors for PA development are also quite scarce. Methods: Mutational analyses were performed by means of real-time PCR (HER-2 and c-myc amplification analysis), PCR-SSCP and sequencing (H-ras point mutation detection). Polymorphisms analysis was performed by PCR-RFLP (survivin and MMP-9 genes). Results: Amplification of HER-2 and c-myc has been found in 13% and 9% of PA cases respectively. Point mutations in H-ras codons 12/13 have been detected in 17% of PAs. No correlation could be established between these alterations and clinical characteristics of PAs, whereas they might play a role in a subset of malignant salivary gland tumours. As for survivin -31 G/C polymorphism, C allele carriers had a 4-fold decrease of the risk of developing PA (p=0.05). Carriers of the variant allele T of the -1562C/T SNP in MMP-9 gene had a 4-fold increase of the risk of developing PA (p lt 0.001). Conclusions: A longer follow-up of PA patients harbouring mutations could uncover a prognostic role of HER-2 and c-myc amplification as predictors of adenoma transformation into carcinoma. Both survivin and MMP-9 promoter polymorphisms represent susceptibility factors for the development of PAs in the Serbian population.
AB  - Uvod: U ispitivanjima mehanizama nastanka tumora pljuvačnih žlezda uglavnom dominiraju citogenetičke studije, pa su tako detektovane i opisane različite hromozomske translokacije sa specifičnim obrascem javljanja. Međutim, molekularne promene u ovim tumorima i dalje su relativno slabo poznate, a pogotovo je malo podataka o potencijalnom značaju signalnog puta HER-2/H-ras/c-myc u razvoju i progresiji pleomorfnih adenoma (PA). Takođe, retki su i podaci 0 polimorfizmima pojedinačnog nukleotida (SNP) kao faktora predispozicije za nastanak PA. Metode: Analize somatskih mutacija urađene su metodama real-time PCR (analiza amplifikacije HER-2 i c-myc), PCR-SSCP i sekvenciranja (detekcija tačkastih mutacija u H-ras). Ana I iza polimorfizama pojedinačnih nukleotida (SNP) vršena je prime- nom metode PCR-RFLP (u genima za survivin i MMP-9). Zaključak: Dužim praćenjem pacijenata sa PA mogla bi da se ustanovi prognostička uloga HER-2 i c-myc amplifikacija kao indikatora za transformaciju adenoma u karcinom. Polimorfizmi u promotorima gena za survivin i MMP-9 predstavljaju modulatore rizika za razvoj pleomorfnih adenoma u srpskoj populaciji.

Cemento-ossifying fibroma of jaws-correlation of clinical and pathological findings

AB  - Cemento-ossifying fibroma (COF) belongs to the group of bone-related lesions of the oral cavity. The aim of this study was refine its histologic features and to correlate histopathological picture and clinical behavior of the tumor. The quantity of bone spherules, their cellularity, the existence of hemorrhage, inflammation, and endochondral ossification in the stroma of the tumor were analyzed and correlated with patients main symptoms of ten patients with COF. All patients had swelling that lasted between 3 and 20 months (mean 10.4 months). Other clinical symptoms were facial asymmetry, eye bulb protrusion, and teeth displacement. The evolution of the tumor was longer in the mandible (12.3 months) than in maxilla (9.5 months). The main histomorphological diagnostic criterion for COF was psammoma-like structures. In lesions with longer case history, the number of "psammomatoid" bodies was greater. In lesions with shorter course of the disease, their number was lower and stromal hemorrhage and inflammation were presented. One patient had secondary superposition of aneurysmal bone cyst in the prime COF of mandible. Histological appearance of COF may be influenced by the tumor evolution. A siginificant correlation between the clinical course of the disease and histological parameters has been certified.

AB  - Analyses of the viscerocranial parameters in dogs has been avoided due to inconsistencies in defining the reference points for linear and angular measurements. In our study we classified the elements which refer to the balance of skeletal structures and teeth in relation to other osseous elements. By X-ray-photoimaging spatial relations within the osseous system of the head of German Shepherd dogs angular parameters (gonial angle Go, the angles Iv, SNA, SNB, ANB and NSBa) were constructed. Sagittal positions or inclinations of the upper and lower incisors were determined by angles C and D and their relationship was determined by the interincisal angle E. The position of the upper and lower eyeteeth in relation to the basic plane of the maxilla and mandible was determined by the numerical values of angles F and G. The opportunity to define the type of growth, and the changes between the face and the skull base, was provided by constructing the following angles of the polygon, as well as by the exact determination of their individual and total values:  lt SarGo,  lt NSAr,  lt ArGoIv,  lt GoIvMe.
AB  - Telerendgenske analize viscerokranijalnih parametara su izbegavane usled toga što referentne tačke za linearna i angularna merenja nisu bile definisane. U našem radu, klasifikovani su elementi koji se odnose na ravnotežu skeletnih struktura i zuba u odnosu na ostale kostne anatomske entitete glave. Prostorni odnosi na kostnom sistemu glave pasa rase nemački ovčar, utvrđeni su konstrukcijom i numeričkim određivanjem najznačajnijih uglovnih parametara: gonijalni ugao Go, uglovi Iv, SNA, SNB, ANB, NSBa. Sagitalni položaji ili nagibi gornjih i donjih sekutić a određeni su preko uglova C i D, a odnos između ova dva parametra konstrukcijom interincizalnog ugla E. Bcl-2 expression in oral squamous cell carcinoma

AB  - Apoptosis is a genetically regulated process involved in tissue size regulation, morphogenesis, and elimination of genetically damaged cells. A pallet of genes is involved in the control of apoptosis, such as bcl-2 family whose oncogenic potential has been demonstrated in oral tumorigenesis. Different members of bcl-2 family may promote or inhibit apoptosis by synthesizing anti- and proapoptotic proteins. One of antiapoptotic proteins, bcl-2, with a crucial role in apoptosis regulation was the object of our study. By means of immunohistochemistry we estimated the level of overexpression of bcl-2 proteins in a series of the 26 formalin fixed, paraffin-embedded samples of oral squamous cell carcinoma (OSCC). Analyzed tumors originated from different sites of oral cavity; 7/26 belonged to stage II, 14/26 to stage III, and 5/26 to stage IV Immunoreactivity was scored according to the percentage and intensity of positive cytoplasmic bcl-2 staining. All tumors had low percentage of positively stained bcl-2 cells, with mean values for lower/higher intensity of 8.3 +/- 2.5/34.4 +/- 7, 7.5 +/- 1.1/31.9 +/- 4.3, and 8.4 +/- 5.8/31.5 +/- 5.8 within stages II,III, and IV, respectively. Low level of bcl-2 expression in our sample seems to be associated with higher survival rate: 77% for the 5-year follow-up period. Comparing clinicopathologic and risk factors data within each and between three groups of analyzed tumors (lip-tongue P = 0.58, tongue-floor of the mouth, P = 0.21, lip-floor of the mouth, P = 0.50) there was no significant difference. AB  - After rentgencraniornetric and roentgencephalometric analysis of the most significant reference points for dogs of the German Shepherd breed aged 12 months, we considered it would be of importance, both from the scientific and cynologic point of view, to determine all linear parameters which might be used in the exact type classification of the neuroviscerocranium of these animals. Correct proportions of certain regions of the face and head were exactly determined by defining and numerical specifying the following lines line N (Nasion)- S (Sella), bispinal or palatine plane alpha (SnA- SnP) occlusive plane- beta, plane gamma (Go- Me) and line N-A. The line N-Iv denotes the middle height of the face which is specific for the mesocephalic type of dogs head. In addition to these, there are also the lines Co Condylion) - Go (Gonion), Go (Gonion) - Me (Menton) and line S (Sella) Go (Gonion). A full inspection of the harmony and anatomic balance in the proportions of the neuroviscerocranium skeleton was achieved by specifying the following linear parameters for the frontal photograph: the lines Zy- Zy (Zygon), Eu- Eu (between the Eurion points), the lines Em- Em, ecm- ecm Eim- Eim, Go- Go, the distances En- En, Ein- Bin, Ecm- Ecm, Ecp- Ecp and Ecl- Ecl.

Diagnosis and therapy of oral cavity diseases in small domestic animals

AB  - In parallel with the stepped up urbanisation of modern man, there is an increasing number of house pets of different breeds and species who suffer certain biological and physiological changes because of the specific manner of breeding and upkeep. The altered conditions of their natural environment can lead to disorders in the animal genetic fund, which is why numerous diseases include cases of constitutional flaws (caries, periodontosis and related complications, cysts, abscesses, malformations of hereditary origin - hypodontia, andontia, impacted teeth, and others). The paper presents cases of the most frequent diseases of teeth and supporting tissues, as well as the optimal manner of therapy. It also points out certain limitations in practicing veterinary orthodontia aimed at avoiding situations when certain congenital or acquired anomalies are corrected but result in the animal no longer meeting the required standards for its species.
AB  - Uporedo sa ubrzanom urbanizacijom modernog čoveka, zapaža se i povećan broj kućnih ljubimaca različitih vrsta i rasa, koji zbog specifičnog načina odgajanja trpe određene biološke i fiziološke promene. Izmenjeni uslovi prirodnog okruženja mogu da prouzrokuju poremećaje genetskog fonda životinja, što je razlog da se među mnogim bolestima javljaju slučajevi konstitucionalnih mana (karijes, paradontopatija i njihove komplikacije ciste, apscesi, malformacije naslednog porekla – hipodoncija, anodoncija impakcija zuba i slično). U radu su prikazani slučajevi najčešćih oboljenja zuba i njihovog potpornog aparata, kao i optimalni načini njihove terapije. Ukazano je i na određena ograničenja kojih moramo da se pridržavamo u veterinarskoj ortodonciji, kako bi se izbegle situacije korekcije pojedinih urođenih ili stečenih anomalija zbog kojih životinja odstupa od propisanih standarda za svoju vrstu.
