Influence of magnetic field to microbes in oral environment

AB  - From the very beginning of life, living organisms were exposed to the influence of geoelectric and geomagnetic fields, with established balance with this respect. Development of civilization has been distorted balance and harmony, imposing the extreme need for magnetism. Respecting positive effects of magnetic field to tissues, especially 'bactericide' effect, this investigation was conducted on the aim to assess the influence of magnetic field to oral microbes. Materials and method. This investigation was carried to in vitro. To obtain adequate samples of deposits or specimens of dental plaque, were from certain microbes which were isolated from human mouth. Microbes isolated from samples were Streptococcus parasanguis, Staphylococcus epidermidis, Rodococcus equi and Candida albicans. Turbidimetry array was applied for investigation of this kind. The method of spectrophotometry was applied, to obtain optical den­sity (OD) value and to establish indirectly a growth of intensity of sowed microbe's stains on the basis of modified OD of agar. Investigation was carried on two level of density of microbes of 1 CFU/ml as well as of 10 CFU/ml after 24h period and 48h period of incubation of microbes. Results. Positive effect of magnetic field upon dental plaque of bacteria reduction in vitro was recognized. At the baseline-initial 24 hours period of measurement of exposition to magnetic field, the count of all isolated mi­crobes has been significantly reduced, in the both of agar of increased density as well as of agar with reduced OD. Over the time, the decrement of influence of magnets versus microbes in vitro has been detected. Conclusion. Count of this influence of the magnet to microbes is of extreme importance in parodontology. This way, the positive influence of magnet to supporting tissues and quality and durability of overdenture was confirmed. .
AB  - Sva živa bića su izložena dejstvu geoelektričnog i geomagnetskog polja i u tom smislu je ostvarena ravnoteža. Sa razvojem civilizacije ta ravnoteža se narušava u smislu da slabi uticaj geomagnetnog polja i postoji t.z.v. 'glad' za magnetizmom. Imajući u vidu pozitivan uticaj magneta na sva biološka tkiva, posebno 'baktericidni' efekat israživanje je sprovedeno sa ciljem da se ispita uticaj magnetnog polja na oralnu mikrofloru. Materijal i metod: Istraživanja su sprovedena in vitro. Uzorci, tj. brisevi zubnih naslaga poticali su sa zuba pacijenata Stomatološkog fakulteta u Beogradu. Iz uzoraka su izolovani: Streptococcus parasanguis, Staphylococcus epidermidis, Rodococcus equi i Candida albicans. Korišćena je metoda turbidimetrija gde se spektrofotometrom na osnovu optičke gustine bujona indirektno utvrđuje intenzitet rasta zasejanih mikro-organizama. Ispitivanje je vršeno sa 2 gustine mikroorganizama 1 i 10 CFU/ml i to nakon 24 i 48 h inkubacije mikroorganizama. Rezultati pokazuju da je došlo do redukcije broja bakterija u in vitro uslovima. U prvih 24 h izloženosti uticaju magnetnog polja ta redukcija je bila značajna kako u bujonima veće tako i manje gustine. Tokom vremena uticaj magneta na brojnost mikroflore u in vitro uslovima opada. Zaključak Ovakav uticaj magneta na brojnost mikroorganizama je značajan u parodontologiji pa samim tim ima pozitivan uticaj na potporna tkiva ispod supraden-talnih proteza, na njihov kvalitet i trajnost. .

Radiographic changes in alveolar bone height on overdenture abutments: a longitudinal study

AB  - Objective: Stabilisation of partial dentures is an important part of prosthodontic treatment, particularly in patients with an excessive reduction in alveolar bone supporting their abutment teeth. In these cases, overdenture prostheses are one possible treatment choice. Background: The aim of this study was to evaluate the effects of overdentures on the condition of alveolar bone of the remaining teeth, and to evaluate the overdenture concept in the prosthetic treatment of partially edentulous patients. Materials and methods: The investigation was conducted on 22 patients during observation periods of 6 and 10 years. After preparation, all abutment teeth were provided with either amalgam restorations or gold copings. An overdenture in the lower jaw was fabricated for every patient. A special film holder was used to repeat the X-ray procedure in every patient in the same manner. The radiographs were evaluated using the grid scale. The alveolar bone level of all remaining teeth was evaluated on dental films by measuring the distance between the root apex and coronal border of the alveolar bone. Results: A statistically significant difference (p  lt  0.01) was calculated 6 years after delivery stage between distal alveolar bone levels of experimental abutments under overdentures and the remaining teeth of the control group. Alveolar bone of abutment teeth was considerably preserved, particularly in seven patients observed 10 years after provision of their dentures. Conclusion: Within the limitation of this study, the validity of the overdenture treatment in partially edentulous patients is concluded. Good oral hygiene was crucial for the success of treatment with overdentures.

AB  - Aim: The aim of this study was to evaluate the effects of overdentures on the condition of alveolar bone around the remaining teeth over 10 years. Materials and Methods: After preparation of all the remaining abutment teeth in the lower jaw, overdentures were fabricated for 22 patients. The radiographs were made at the delivery stage, then 6 and 10 years after delivery. The radiographs were evaluated using the grid scale and the film holder made it possible to repeate x-ray procedure in every patient in the same manner. Results: We concluded that we achieved the maximum therapeutic effect. Conclusion: Good oral hygiene is the key of success with overdentures.
AB  - Cilj: Cilj ove studije bio je da se izmere efekti supradentalnih proteza na stanje alveolarne kosti u predelu zadržanih zuba za vreme od 10 godina. Materijal i metod: Nakon pripreme svih zadržanih "patrljaka zuba" u donjoj vilici, načinjene su donje totalne supradentalne proteze za 22 pacijenta. Dentalne radiografije tih zuba su urađene na početku, pri predaji proteza, a zatim 6 i 10 godina nakon predaje. Dentalni rentgenski snimci upoređivani su uz pomoć žičane mrežice i držača rentgen filma koji su pripremljeni za svakog pacijenta i svaki zub idividualno čime je bila obezbeđena izrada identično pozicioniranih dentalnih radiografija u različitim vremenskim periodima. Rezultati: Dobijeni rezultati ove studije ukazuju da je postignut maksimalan terapeutski efekat u pacijenata sa malim brojem preostalih, parodontloški loših zuba. Zaključak: Dobra oralna higijena je ključ uspeha tretmana supradentalnim protezama.

Alveolar bone condition in retained teeth region of patient with complete lower overdenture - case report

AB  - In everyday dental practice, prosthetic treatment of patients having a small number of remaining natural teeth is more or less common and the remaining teeth are kept for improvement of retentive abilities of removable prosthodontics. The essence of the concept discussed herein is that the remaining teeth are to be left in place for purpose of preservatingof the alveolar bone and providing permanent and stable support for complete dentures. Patient M.J.,female had an edentulous upper jaw and 3 remaining lower incisors. After adequate preparation-endodontic treatment, as well as parodontal treatment, clinical crowns were reduced, and then cast cap crowns positioned on parts of dental tissues. Finally, the upper complete denture and lower overdenture were positioned. After the positioning of dentures, a special retroalveolar film holder was placed, dental retroalveolar radiographs were positioned and radiographs were made. The holder was designed to provide intraoral radiograph in selected repeatable positions of radiographs and in the same conditions- standardized procedure, so that it was possible to compare radiograms. Measurements were made on each film for each tooth from its mesial, as well as distal sides, between apexes and coronal borders of alveolar bones. Patient M.J. received dentures and the first radiograph was made at the baseline when cast cap crowns and dentures were positioned in her mouth. Followe-ups were in an identical mode of radiograph and assessing radiographs after 1, 7 and 12 years. We concluded that we achieved a maximum therapeutic effect in such a long period of time. The influence of patients' motivation and better ability to persist in keeping and maintaing oral higiene were main factors for success.
AB  - Danas se u svakodnevnoj stomatološkoj praksi u slučajevima maksimalne krezubosti veoma često koriste preostali zubi za poboljšanje retencije mobilnih nadoknada. Međutim, suština ovde primenjenog koncepta je da su preostali zubi zadržani sa ciljem da čuvaju alveolarnu kost, obezbeđujući time trajniju i stabilniju potporu totalnoj protezi. Pacijent M J, u gornjoj vilici je bila bezuba, a u donjoj je imala svega 3 preostala zuba (sekutića). Ovi zubi su nakon adekvatne pripreme (endodontski tretman redukcija kliničke krune, parodontalni tretman) zaštićeni livenim kapicama a nakon toga su izrađene mobilne nadoknade: u gornjoj vilici klasična totalna, a u donjoj totalna supradentalna proteza. Na predaji proteza je izrađen specijalni držač Rtg filma i načinjen retroalveolarni snimak ovih zuba. Kao držač poslužila je specijalna kašika izrađena od autoplimerizujućeg akrilata na radnom modelu. S obzirom na normiranu poziciju snimka postupak rtg snimanja bilo je moguće ponoviti na identičan način posle 1, 7 i 12 godina. Može se zaključiti da je postignut maksimalni terapijski efekat u ovom dugom vremenskom periodu. abstract = "In everyday dental practice, prosthetic treatment of patients having a small number of remaining natural teeth is more or less common and the remaining teeth are kept for improvement of retentive abilities of removable prosthodontics. The essence of the concept discussed herein is that the remaining teeth are to be left in place for purpose of preservatingof the alveolar bone and providing permanent and stable support for complete dentures. Patient M.J.,female had an edentulous upper jaw and 3 remaining lower incisors. After adequate preparation-endodontic treatment, as well as parodontal treatment, clinical crowns were reduced, and then cast cap crowns positioned on parts of dental tissues. Finally, the upper complete denture and lower overdenture were positioned. After the positioning of dentures, a special retroalveolar film holder was placed, dental retroalveolar radiographs were positioned and radiographs were made. The holder was designed to provide intraoral radiograph in selected repeatable positions of radiographs and in the same conditions- standardized procedure, so that it was possible to compare radiograms. Measurements were made on each film for each tooth from its mesial, as well as distal sides, between apexes and coronal borders of alveolar bones. Patient M.J. received dentures and the first radiograph was made at the baseline when cast cap crowns and dentures were positioned in her mouth. Followe-ups were in an identical mode of radiograph and assessing radiographs after 1, 7 and 12 years. We concluded that we achieved a maximum therapeutic effect in such a long period of time. The influence of patients' motivation and better ability to persist in keeping and maintaing oral higiene were main factors for success., Danas se u svakodnevnoj stomatološkoj praksi u slučajevima maksimalne krezubosti veoma često koriste preostali zubi za poboljšanje retencije mobilnih nadoknada. Međutim, suština ovde primenjenog koncepta je da su preostali zubi zadržani sa ciljem da čuvaju alveolarnu kost, obezbeđujući time trajniju i stabilniju potporu totalnoj protezi. Pacijent M J, u gornjoj vilici je bila bezuba, a u donjoj je imala svega 3 preostala zuba (sekutića). Ovi zubi su nakon adekvatne pripreme (endodontski tretman redukcija kliničke krune, parodontalni tretman) zaštićeni livenim kapicama a nakon toga su izrađene mobilne nadoknade: u gornjoj vilici klasična totalna, a u donjoj totalna supradentalna proteza. Na predaji proteza je izrađen specijalni držač Rtg filma i načinjen retroalveolarni snimak ovih zuba. Kao držač poslužila je specijalna kašika izrađena od autoplimerizujućeg akrilata na radnom modelu. S obzirom na normiranu poziciju snimka postupak rtg snimanja bilo je moguće ponoviti na identičan način posle 1, 7 i 12 godina. Može se zaključiti da je postignut maksimalni terapijski efekat u ovom dugom vremenskom periodu.
