Prevalence of non-carious cervical lesions among the general population of the Republic of Srpska, Bosnia and Herzegovina

AB  - Introduction As non-carious cervical lesions (NCCLs) may compromise aesthetics and function, knowledge of their aetiological covariables enhances management of clinical complaints and success of restorative treatments. Aims The primary aim of this study was to assess the presence of NCCLs among the general population of the Republic of Srpska, Bosnia and Herzegovina and the potential association with patient-related risk factors. Methods A prevalence study of NCCLs included 738 respondents from eight towns/municipalities. Two dental practitioners examined all respondents. NCCLs were diagnosed according to the Smith and Knight tooth wear index, measured using a Williams periodontal probe. Data regarding risk factors were obtained through a structured questionnaire. Multivariate logistic regression was used to analyse the association of risk factors and the occurrence of NCCLs. Results Non-carious cervical lesions were diagnosed in 384 (52%) respondents. Multivariate regression analysis showed that several variables were independently associated with the risk of developing NCCLs, including frequent consumption of acid food (P = 0.001), frequent consumption of acid drinks (P = 0.001), retaining drink in the mouth (P = 0.001), alcohol consumption (P = 0.030), bruxism (P = 0.018) and gastro-oesophageal reflux (P = 0.023). First mandibular premolars were the most affected teeth (left: 46.0%; right: 44.0%), followed by the second right maxillary premolars (37.3%), second left maxillary premolars (33.6%) and finally by the first right maxillary premolars (34.0%). Conclusion The results of the current study suggest that NCCLs occur frequently and have a multifactorial aetiology. The lowest prevalence was recorded among individuals younger than 20 years of age. As the majority of risk factors are modifiable, regular dental care could lead to the early detection of NCCLs.

AB  - This study aimed to compare the accuracy of three electronic apex locators (EALs), Raypex 5, Apex NRG and ProPex II, in locating the apical foramen (AF) in vivo. Thirty multi-rooted teeth were included in the study. Distances from the file tip to the radiographic apex from the buccal (R1) and mesial (R2) aspect were measured after extraction. The distance from the file tip to the AF (L) was measured microscopically. Raypex 5, Apex NRG and ProPex II were accurate to within +/- 0.5mm in 84.6%, 78.6% and 66.7% of cases, respectively. R1, R2 and L measurements between the EALs were not statistically different (P>0.05). R1 and L correlated positively (r(2)=0.817; P lt 0.001). There were no significant differences in all measurements between vital and non-vital root canals (P>0.05). There was no statistically significant difference between the EALs. There was a positive correlation between electronic and radiographic measurements when locating the AF.

Frequency and quality of root canal fillings in an adult Serbian population

AB  - Introduction Estimation of frequency and quality of root canal fillings is the way to evaluate endodontic treatment needs and success/failure rates of performed endodontic procedures. Objective The aim of this study was to determine the frequency and quality of root canal fillings and the frequency of apical periodontitis on endodontically treated teeth in a group of adult patients from Serbia. Methods In order to analyze the presence and quality of root canal fillings and the frequency of periapical radiolucencies on endodontically treated teeth, 3526 teeth were examined on orthopantomograms of 153 adult patients. Results Overall, 12.5% of examined teeth were root filled, and 51.8% of them had radiographic signs of apical periodontitis. The analysis of root fillings quality revealed the presence of more inadequate ones (55.9%). The frequency of apical periodontitis was significantly higher in teeth with inadequate than in those having adequate root canal obturation (72.2% and 25.9%, respectively). Conclusion The frequency of apical periodontitis on root-filled teeth in this group of patients was high, indicating a low success rate of performed endodotic procedures and high endodontic retreatment needs.
AB  - Uvod Utvrđivanje učestalosti i kvaliteta punjenja kanala zuba jeste način da se procene potrebe za endodontskim lečenjem i stepen uspeha primenjenih endodontskih procedura. Cilj rada Cilj ovog rada je bio da se odrede učestalost i kvalitet punjenja kanala korena zuba, kao i učestalost apeksnih parodontitisa endodontski lečenih zuba kod odraslih pacijenata u Srbiji. Metode rada Radi utvrđivanja postojanja i kvaliteta punjenja kanala korena i učestalosti rasvetljenja u periapeksu endodontski lečenih zuba, analizirano je 3.526 zuba na ortopantomogramima 153 odrasle osobe. Rezultati Ukupno je endodontski lečeno 12,5% pregledanih zuba, od kojih je 51,8% imalo radiografske znake apeksnih parodontitisa. Analiza kvaliteta punjenja kanala pokazala je da je bilo više neadekvatnih punjenja (55,9%). Učestalost apeksnih parodontitisa je bila značajno veća kod zuba s neodgovarajućim kanalnim opturacijama (72,2%) u odnosu na one s odgovarajućim punjenjem kanala korena (25,9%). Zaključak Kod ispitanih pacijenata utvrđena je visoka učestalost apeksnih parodontitisa zuba s punjenim kanalima korena, što ukazuje na slabu uspešnost primenjenih endodontskih zahvata, kao i na potrebu za ponovljenim endodontskim lečenjem.

AB  - Introduction. The main goal of obturation is prevention of reinfection of the root canal system and consequently appearance of periapical lesion. Epidemiological studies have confirmed high prevalence of periapical lesions in endodontically treated teeth with inadequate root canal fillings. The aim of this study was to assess the quality of root canal fillings in teeth treated at students' practical sessions. Iatrogenic errors during endodontic interventions were also assessed. Methods. Two hundred dental records of the patients treated by students in the study program Dentistry at the School of Medicine in Foca during the academic year 2010/2011 were randomly chosen. The length and homogeneity of the fillings or the presence of iatrogenic errors were recorded. A root canal filling was considered adequate if it had correct length and homogeneity in the absence of iatrogenic errors. Two students' curricula V1 and V2 were evaluated for the treatment outcome. Results. Out of 128 endodontically treated teeth, adequate length of the canal filling to the physiological apex was observed in 57% of teeth, homogeneity in 78.9%, while both of these criteria were satisfied in 66 teeth (51.6%). Better quality of root canal fillings was achieved by students of the study curricula V2 and the difference between these two programs was significant (p lt 0.05). Better quality of root canal fillings in both study programs was recorded for incisors and canines as compared to premolars. Conclusion. The quality of endodontic treatment performed by students was considered adequate in 51.6% of cases. The type of curriculum for endodontic course had a significant impact on the quality of endodontic treatment performed by students.
AB  - Uvod. Opturacija kanala korena zuba vrši se radi prevencije ponovne infekcije kanalnog sistema i obezbeđivanja uslova za efikasno ozdravljenje periapeksnog tkiva. Epidemiološke studije su ukazale na visoku stopu prevalencije periapeksnih promena kod endodontski lečenih zuba sa neodgovarajućim punjenjem kanala. Cilj rada bio je da se proveri kvalitet punjenja kanala korena endodontski lečenih zuba na studentskim vežbama i ustanove lekarske greške nastale tokom endodontske intervencije. Materijal i metode rada. Za potrebe istraživanja metodom slučajnog uzorka odabrano je 200 kartona pacijenata kojima je vršen endodontski tretman na studentskim vežbama koje su u okviru studijskog programa Stomatologija izvođene na Medicinskom fakultetu u Foči tokom 2010/2011. školske godine. Parametri posmatranja bili su dužina i homogenost punjenja kanala i postojanje lekarskih grešaka tokom zahvata. Adekvatnim se smatralo punjenje kanala koje ima odgovarajuću dužinu i homogenost i bez lekarskih grešaka. Analizirana su dva studentska programa iz oblasti endodoncije (V1 i V2) i utvrđen ishod lečenja obolelih zuba. Rezultati. Od 128 endodontski lečenih zuba, odgovarajuća dužina punjenja kanala do fiziološkog suženja utvrđena je u 57% uzoraka, homogenost u 78,9% uzoraka, dok su oba ova kriterijuma bila zadovoljena kod ukupno 66 zuba (51,6%). Kvalitetnije punjenje kanala korena uradili su studenti programa V2, a razlika između dva studentska programa bila je statistički značajna (p lt 0,05). Kvalitetna opturacija kanala u oba studijska programa zabeležena je kod sekutića i očnjaka u odnosu na premolare. Zaključak. Kvalitet endodontskog lečenja na studentskim vežbama bio je odgovarajući u nešto više od polovine slučajeva. Kurikulum iz oblasti endodoncije ima značajan uticaj na kvalitet endodontskog lečenja na studentskim vežbama.

abstract = "Introduction. The main goal of obturation is prevention of reinfection of the root canal system and consequently appearance of periapical lesion. Epidemiological studies have confirmed high prevalence of periapical lesions in endodontically treated teeth with inadequate root canal fillings. The aim of this study was to assess the quality of root canal fillings in teeth treated at students' practical sessions. Iatrogenic errors during endodontic interventions were also assessed. Methods. Two hundred dental records of the patients treated by students in the study program Dentistry at the School of Medicine in Foca during the academic year 2010/2011 were randomly chosen. The length and homogeneity of the fillings or the presence of iatrogenic errors were recorded. A root canal filling was considered adequate if it had correct length and homogeneity in the absence of iatrogenic errors. Two students' curricula V1 and V2 were evaluated for the treatment outcome. Results. Out of 128 endodontically treated teeth, adequate length of the canal filling to the physiological apex was observed in 57% of teeth, homogeneity in 78.9%, while both of these criteria were satisfied in 66 teeth (51.6%). Better quality of root canal fillings was achieved by students of the study curricula V2 and the difference between these two programs was significant (p lt 0.05). Better quality of root canal fillings in both study programs was recorded for incisors and canines as compared to premolars. Conclusion. The quality of endodontic treatment performed by students was considered adequate in 51.6% of cases. The type of curriculum for endodontic course had a significant impact on the quality of endodontic treatment performed by students., Uvod. Opturacija kanala korena zuba vrši se radi prevencije ponovne infekcije kanalnog sistema i obezbeđivanja uslova za efikasno ozdravljenje periapeksnog tkiva. Epidemiološke studije su ukazale na visoku stopu prevalencije periapeksnih promena kod endodontski lečenih zuba sa neodgovarajućim punjenjem kanala. Cilj rada bio je da se proveri kvalitet punjenja kanala korena endodontski lečenih zuba na studentskim vežbama i ustanove lekarske greške nastale tokom endodontske intervencije. Materijal i metode rada. Za potrebe istraživanja metodom slučajnog uzorka odabrano je 200 kartona pacijenata kojima je vršen endodontski tretman na studentskim vežbama koje su u okviru studijskog programa Stomatologija izvođene na Medicinskom fakultetu u Foči tokom 2010/2011. školske godine. Parametri posmatranja bili su dužina i homogenost punjenja kanala i postojanje lekarskih grešaka tokom zahvata. Adekvatnim se smatralo punjenje kanala koje ima odgovarajuću dužinu i homogenost i bez lekarskih grešaka. Analizirana su dva studentska programa iz oblasti endodoncije (V1 i V2) i utvrđen ishod lečenja obolelih zuba. Rezultati. Od 128 endodontski lečenih zuba, odgovarajuća dužina punjenja kanala do fiziološkog suženja utvrđena je u 57% uzoraka, homogenost u 78,9% uzoraka, dok su oba ova kriterijuma bila zadovoljena kod ukupno 66 zuba (51,6%). Kvalitetnije punjenje kanala korena uradili su studenti programa V2, a razlika između dva studentska programa bila je statistički značajna (p lt 0,05). Kvalitetna opturacija kanala u oba studijska programa zabeležena je kod sekutića i očnjaka u odnosu na premolare. Zaključak. Kvalitet endodontskog lečenja na studentskim vežbama bio je odgovarajući u nešto više od polovine slučajeva. Kurikulum iz oblasti endodoncije ima značajan uticaj na kvalitet endodontskog lečenja na studentskim vežbama.",

Histopathology of subcutaneous tissue reaction to endodontic root canal sealers

AB  - The aim of the study was to evaluate tissue response of two root canal sealers: Endomethasone (cink oxide eugenol-based sealers) and Ketac Endo Aplicap (glass ionomer based sealers). The sealers were freshly mixed and injected into subcutaneous connective tissue of the dorsal surface of experimental animals (Wistar Furth rats). The inflammatory reaction caused by the sealers was evaluated 7, 30 and 60 days after implantation using descriptive hisopathological analysis. The tissue sections were taken from injection sites. Each incision included skin,subcutaneous connective tissue and underlying muscular tissue. All blocks were proceeded using standard histological procedures. The tissue reactions were graded as mild,moderate or severe inflammation. After the seven days both sealers showed a moderate reaction with a decreasing trend. Both materials caused mild inflammation which diminished at the end of the experimental period. The results of this study demonstrate that Ketac Endo and Endomethasone is well tolerated by subcutaneus connective tissue of experimental animals.
AB  - Cilj ovih proučavanja je bio ispitivanje biokompatibilnosti dva materijala za definitivno kanalno punjenje: Endomethasone (materijal na bazi cink oksid eugenola) i Ketac Endo Aplocap (glas jonomer cementa za kanalno punjenje). Sveže pripremljeni materijali su injicirani u potkožno vezivno tkivo leđnog dela eksperimentalnih životinja (pacovi Wistar soja). Intenzitet inflamatorne reakcije proveravan je nakon 7, 30 i 60 dana na osnovu patohistološke analize. Isečak je uzet sa mesta aplikacije materijala i obuhvatao je deo kože, potkožno tkivo i deo mišićnog sloja. Tkiva su podvrgnuta standardnoj histološkoj obradi. Reakcija tkiva na ubačeni materijal označena je kao blaga, srednja i jako izražena inflamacija. Posle prvog perioda od 7 dana oba materijala su izazvala intenzivnu zapaljensku reakciju tkiva sa kasnijom tendencom smanjenja inflamacije. Oba materijala su izazvala umereno zapaljenje koje je nestalo do kraja eksperimentalnog perioda. Postignuti rezultati ukazuju da su Ketac Endo i Endomethasone biokompatibilni u potkožnom vezivnom tkivu eksperimentalnih životinja.

Endodontic treatment of lower anterior teeth with multiple canals

AB  - Introduction. Knowledge of root canal morphology is one of the main prerequisites for successful endodontic treatment. We present the case of a patient with multiple canals in lower anterior teeth which were treated endodontically. Case Report. To a 45-year-old patient endodontic treatment was applied on lower left lateral incisor, lower right central and lateral incisor and lower right canine. These teeth were previously prepared for prosthetic crowns. All involved teeth had two canals which were not visible on the primary radiographs. Gracility of remaining tooth structure and canals and the presence of two canals in each tooth required additional radiographs for verification of multiple canals. Careful biomechanical instrumentation and irrigation with use of lubrication due to narrow canals was performed. The canals were obturated with gutta-percha using lateral compaction technique and Acroseal (Septodont) paste. On the control checkup after eight months, the patient was asymptomatic, while clinical finding was normal. Conclusion. Complexity of presented case was even more significant because these teeth were prepared to support prosthetic superstructure and crowns, and every 'forgotten' canal is an iatrogenic error.
AB  - Uvod. Poznavanje morfoloških osobina kanala korena zuba jedan je od osnovnih preduslova uspešne endodontske terapije. Prikazujemo slučaj pacijenta sa višekanalnim sistemima donjih frontalnih zuba koje je bilo neophodno endodontski lečiti. Prikaz slučaja. Kod žene stare 45 godina urađena je endodontska terapija na donjem levom lateralnom sekutiću, donjem desnom centralnom i lateralnom sekutiću i donjem desnom očnjaku. Zubi su prethodno bili pripremljeni za protetičke krunice. Svi uključeni zubi su imali po dva kanala koja nisu bila vidljiva na primarnom radiogramu. Gracilnost preostale krunice zuba, gracilnost kanala i zastupljenost po dva kanala u svakom zubu zahtevali su dodatna snimanja radi potvrde udvojenosti. Bila je neophodna pažljiva biomehanička obrada uz primenu lubrikanta i irigansa zbog otežane prohodnosti kanala. Kanali su napunjeni lateralnom kompaktažom gutaperke uz primenu paste Acroseal (Septodont). Na kontrolnom pregledu osam meseci kasnije pacijentkinja nije imala subjektivnih tegoba, a klinički nalaz je bio normalan. Zaključak. Sagledavanje kompleksnosti ovakvog slučaja radi endodontskog lečenja je značajno, jer su ovi zubi bili dalje opterećeni protetičkim nadogradnjama i krunicama, pa bi svaki 'zaboravljeni' kanal bio lekarska greška u okviru preprotetičke pripreme.

AB  - The most important requirements for successful endodontic treatment are accurate diagnosis and good knowledge of canal morphology. A mandibular second premolars rarely have more than one root and one root canal. Data from the literature indicate low percentage of a mandibular premolars with three or more root canals. Unusual morphology of the root canal system is diagnostic and therapeutic challenge. It is very important to analyze preoperative radiograph and to explore thoroughly the entrance of the root canal. The aim of this study was to present two patients who had lower second premolars with three canals that required endodontic therapy.
AB  - Jedan od osnovnih uslova uspešnog endodontskog lečenja je precizna dijagnostika i dobro poznavanje morfologije kanalnog sistema zuba. Donji drugi premolar retko ima više od jednog korena i jednog kanala. Podaci iz literature ukazuju na izuzetno mali procenat donjih drugih premolara sa tri i više kanala korena. Neuobičajeni morfološki izgled kanalnog sistema predstavlja dijagnostički i terapijski izazov. Za postavljanje tačne dijagnoze od izuzetne važnosti su analiza radiograma načinjenog pre hirurškog zahvata i pažljiva eksploracija ulaza u kanale korena. U ovom radu prikazana su dva pacijenta sa trokanalnim donjim drugim premolarima koje je bilo potrebno endodontski lečiti.

Analysis of sealing ability of root canal sealers using scanning electronic microscopy technique

AB  - Introduction. An ideal endodontic sealer should adhere firmly both to dentin and to gutta-percha. Objective. The aim of this study was to evaluate the adhesion of the root canal filling to dentin and gutta-percha using scanning electronic microscopy (SEM). Methods. The sealing ability of endodontic sealers to dentinal walls of the root canal was assessed in recently extracted human single canal premolars. Twenty teeth were prepared using the crown-down technique and irrigated with 3% NaOCl. A total of 20 samples were divided into two groups. The root canals were obturated using Ketac-Endo Aplicap and GutaFlow. The sealing ability and adhesion properties at the sealer-dentin interface were studied using SEM and the results were rated from 1 to 3; extremely good adhesion (rated 1), good adhesion (rated 2) and a relatively good adhesion (rated 3). Results. The results showed extremely good adhesion on Ketac-Endo and GuttaFlow dentin interface. GuttaFlow has strong adhesion (rated 1) to gutta-percha in comparison with Ketac-Endo to gutta-percha interface (rated 2). Conclusion. New GuttaFlow filling material has a strong sealing ability and excellent adhesion to dentinal walls and gutta-percha cones. AB  - Uvod. Osnovne osobine materijala za trajno punjenje kanala korena zuba podrazumevaju idealno zaptivanje, kako tvrdog zubnog tkiva, tako i gutaperka- poena. Cilj rada. Cilj rada je bio da se ispita adhezivnost materijala za trajno punjenje kanala korena za dentin i gutaperka-kočiće korišćenjem skening-elektronske mikroskopije (SEM). Metode rada. Sposobnost zaptivanja dentinske površine obrađenog zida endodontskim silerima ispitana je na sveže izvađenim jednokorenim premolarima. Dvadeset zuba je preparisano krunično- apeksnom tehnikom uz ispiranje troprocentnim rastvorom natrijum-hipohlorita (NaOCl). Uzorci su svrstani u dve grupe: prva je opturisana sa materijalom Ketac-Endo, a druga sa GuttaFlow. Sposobnost zaptivanja i adezivnost na granici dodira silera i dentina, odnosno silera i gutaperke, analizirana je primenom SEM. Adhezivnost je označena kao: ekstremno dobra (ocena 1), dobra (ocena 2) i relativno dobra (ocena 3). Rezultati. Dobijeni rezultati su pokazali izuzetno dobru adhezivnost materijala Ketac-Endo i GuttaFlow za dentinsku površinu ispreparisanih zidova kanala korena zuba. GuttaFlow je pokazao veoma dobru vezu za gutaperka-kočić (ocena 1) u odnosu na Ketac-Endo (ocena 2). Zaključak. Novi materijal GuttaFlow ima izuzetno dobru adhezivnost, kako za dentin kanala korena, tako i za gutaperka-poene. Ketac-Endo ostvaruje veoma dobru sposobnost zaptivanja za dentin i dobru adhezivnost, ali nešto slabiju za gutaperku u odnosu na GuttaFlow.

Biocompatibility of root canal obturation materials implanted in rats muscular tissue

AB  - The purpose of this study was to investigate the biocompatibility of different dental materials for teeth root canal obturation, implanted in rat muscular tissue, by analyzing the type of muscular tissue inflammatory reaction level. In the preparation of Wistar rats M. biceps femoris, three different materials for root canal obturation were implanted: Sealapex (Kerr, Romulus, USA), AH-26 (DE Trey, Zurich, Switzerland) and Spongio cement material based on polymethil metacrylate - PMMA (Tomić, 1981, US patent 4296209). The used control material was sterile wax. The rats were sacrificed 30 and 60 days after implantation and the response of the tissue on implanted material was evaluated with standard histological methods on 6 _m sections stained with hematoxilin and eosin. Implantation of Sealapex in rat muscle tissue produced a strong acute inflammation, infiltration with massive polymorphonuclear and numerous giant cells. Acute inflammation still persisted 60 days after implantation. AH-26 produced a strong reaction around the alien body with predominantly limphocitic infiltration. Giant cells and macrophages contained phagocited pieces of material. After 60 days expressive chronic myositis was present with a mass of giant cells and proliferation of connective tissue. Thirty days later the results of histological investigations suggested that implanted PMMA based material still produced a connective fibrous capsula made of fibroblasts and fibrocytes around the implanted material. In the muscular tissue myositis developed forming granulomatous tissue. After 60 days a capsula was made of mature hyalinisated connective tissue without signs of inflammation in the surrounding muscular tissue. The reaction is similar to the reaction to inert control material, such as sterile wax. These findings suggest biocompatibility of PMMA based material for canal obturation.
AB  - Cilj ovog rada je bio da se, na osnovu analize tipa i stepena inflamatorne reakcije koje izazivaju različiti materijali za punjenje kanala korena zuba - sileri, ispita njihova biokompatibilnost nakon unošenja u mišićno tkivo pacova. U poprečno presečen m. biceps femoris laboratorijskih pacova Wistar soja inplantirane su paste za opturaciju kanala Sealapex (Kerr, Romulus, USA), AH - 26 (DE Trey, Zurich, Switzerland) i Spongio cement preparat na bazi PMMA (Tomić, 1981, US patent 4296209), a kao kontrolni materijal stomatološki roze vosak. Životinje su bile žrtvovane nakon 30 i 60 dana, a materijal je pripreman za patohistološ ke pretrage standardnom tehnikom i bojen hematoksilin eozinom. Spongio cement u mišićnom tkivu oko implantiranog materijala daje hronič no zapaljenje, uz stvaranje vezivne kapsule sagrađene od fibroblasta i fibrocita. Nakon 60 dana, kapsula je sagrađena od zrelih ćelija i hijeliniziranih kolagenih vlakana. Implantacija Sealapexa u mišićno tkivo pacova, produkuje jaku hroničnu inflamaciju sa masivnom infiltracijom polimorfonukleara i džinovskih ćelija, koja postoji i nakon 60 dana od implantacije. Pasta AH-26 izaziva burnu reakciju mišićnog tkiva na strano telo sa izraženom limfocitarnom infiltracijom. Nakon 60 dana, prisutna je hronična inflamacija uz prisustvo brojnih džinovskih ćelija i makrofaga koje sadrže fagocitovane deliće materijala, dok vezivno tkivo proliferiše uz stvaranje cističnih tvorevina.

Root canal treatment of mandibular second premolar tooth with taurodontism

AB  - INTRODUCTION Taurodontism is a morphoanatomical change in the shape of a tooth. An enlarged body of a tooth with smaller than usual roots is a characteristic feature. Internal tooth anatomy correlates with this appearance, which means that a taurodontal tooth has a large pulp chamber and apically positioned furcations. This dental anomaly may be associated with different syndromes and congenital discoders. CASE OUTLINE The case report presents the patient of a rare case of taurodontism in the mandibular second premolar with chronic periodontitis. Endodontic treatment was performed after dental history and clinical examination. Special care is required in all segments of endodontic treatment of a taurodontal tooth from the identification orifice, canal exploration, determining working length, cleaning and shaping and obturation of the root canal. Precurved K-file was used for canal exploration and location of the furcation. One mesial and one distal canal with the buccal position were identified in the apical third of the root canal. The working lengths of two canals were determined by radiographic interpretation with two K-files in each canal and verified with the apex locator. During canal instrumentation, the third canal was located in the disto-lingual position. The working length of the third canal was established using the apex locator. CONCLUSION Thorough knowledge of tooth anatomy and its variations can lead to lower percentage of endodontic failure. Each clinical case involving these teeth should be investigated carefully, clinically and radiographically to detect additional root canals. High quality radiographs from different angles and proper instrumentarium improve the quality of endodontic procedure.
AB  - Uvod Taurodontizam je morfološko- anatomska promena oblika zuba koja se odlikuje produženim telom zuba i korenovima kraćim od uobičajenih. Unutrašnja anatomija zuba je u korelaciji s ovom pojavom, što znači da kod taurodontičnog zuba postoje velika pulpna komora i račvanje kanala u apeksnom delu. Ova anomalija zuba može biti udružena sa drugim sindromima i urođenim anomalijama. Endodontsko lečenje ovakvih zuba veoma je teško i komplikovano. Prikaz bolesnika Prikazan je dvadesettrogodišnji muškarac kod kojeg je dijagnostikovan veoma redak slučaj taurodontizma mandibulnog drugog premolara s hroničnim parodontitisom. Na osnovu anamneze i pažljivog kliničkog pregleda odlučeno je da se primeni endodontski zahvat na ovom zubu. Na sve faze endodontske terapije obraćena je posebna pažnja: od pronalaženja ulaza u kanal i ispitivanja njegove prohodnosti, preko određivanja radne dužine, obrade i čišćenja, do konačnog punjenja kanala. Za proveru lokacije na mestu račvanja i prohodnosti kanala korišćen je prethodno zakrivljeni instrument. Utvrđeni su mezijalni i distalni kanali apeksne trećine, bukalno postavljeni. Ubacivanjem kanalnih instrumenata u svaki od njih određena je radna dužina radiografskim metodom, koja je potom proverena apeks-lokatorom. Tokom obrade kanala otkriven je i treći kanal distolingvalno, čija je radna dužina određena apeks-lokatorom. Zaključak Dobro poznavanje anatomije zuba s varijacijama omogućiće znatno manje neuspeha od endodontskog lečenja i očuvanje zuba. Svaki slučaj treba podvrgnuti pažljivom kliničkom i radiološkom ispitivanju kako bi se otkrili dodatni kanali. Primena visokoprecizne radiografije iz nekoliko uglova i korišćenje savremenih instrumenta poboljšavaju kvalitet endodontskog lečenja.

Antimicrobial activity of root canal sealers

AB  - All root canal sealers should have antimicrobial activity so that they could act against any remaining bacteria after root canal obturation. The aim of this study was to evaluate the antimicrobial activity of five root canal sealers: AH26, a resin based paste; Apexit, calcium hydroxide based paste; Endomethasone and Tubliseal, zinc oxide eugenol based materials and Ketac Endo Aplicap, glass ionomer based sealer. Matherials And Methods: Antimicrobial activity was tested against S. mutans 70C and L.casei ATCC 27773 using ADT (agar diffusion inhibitory test) on TYC SB, blood and MRS agars. Freshly prepared paste or cement was placed in grooves of 4 mm in diameter in agar plates. Prepared agar plates were incubated at 37ºC in GAS PAC system for 24hours. The antimicrobial effect was determined according to the diameter of growth inhibition zone around tested materials. Three measurements were done for each material and values were recorded in millimeters. Statistical analysis was done using Mann Whitney test. The results confirmed that epoxy resin and zinc oxide eugenol based sealers had the greatest antimicrobial effect. Calcium hydroxide and glass ionomer based sealers showed significantly lower antimicrobial activity compared to AH26, Endomethasone and Tubliseal. Conclusion: The greatest antimicrobial activity was found for epoxy resin based sealer (AH26) for both tested microorganisms.
AB  - Svi materijali za definitivno punjenje kanala korena bi trebalo da imaju antimikrobna svojstva kako bi posle opturacije mogli delovati na eventualno zaostale mikroorganizme. Cilj: Cilj ovog istraživanja je bio da se ispita antimikrobni efekat pet materijala za opturaciju kanala: AH 26 paste na bazi epoksi smole, Apexit-a, paste na bazi kalcijum hidroksida, materijala na bazi cink oksid eugenola Endomethasone-a i Tubliseal-a i glas jonomer cementa Ketac Endo Aplicap. AB  - The aim of this study was to examine sealing ability of 3 endodontic sealers to dentinal walls and gutta-percha using scanning electron microscopic analysis (SEM). The sealing ability of endodontic sealers to dentinal walls of the root canal was assessed in recently extracted human anterior teeth. 18 teeth were prepared with step back technique and irrigated with 2.5% NaOCl and 3% hydrogen peroxide. Teeth were divided into 3 groups in accordance to the sealer used for root canal obturation: AH 26 (Dentsply, De Trey, Germany), Sankin Type II (Sankin Industry, Japan) and Sealapex (Kerr, USA). The sealing ability and adhesion properties on the sealer-dentin interface were studied using SEM. Results were rated from 1 to 3: 1 - extremely good adhesion; 2 - good adhesion; 3 - relatively good adhesion. The results showed extremely good adhesion on the AH 26-dentin interface and good adhesion of AH 26 to the gutta-percha cones. Sankin Type II showed a good adhesion to the dentinal walls and gutta-percha cones, and Sealapex showed a relatively good adhesion to the canal walls and also a relatively good adhesion to the gutta-percha cones.

Tissue toxicity of root canal sealers

AB  - The aim of this study was to evaluate tissue response to root canal sealers Tubuliseal and Sealapex. The sealers were freshly mixed and injected in the dorsal subcutaneous connective tissue of 12 Wistar rats.The observation periods were 7 days, 21days and 60 days. Four operative areas were formed ( 2 for test sealers, Tubliseal or Sealapex and 2 for control material) on each animal. Tissue sections were taken from selected sites. Each section included skin, subcutaneous connective tissue and underlying muscle tissue.All blocks were processed with the use of standardized histological procedures.The tissue reactions were studied under light microscopy. Different grades of tissue reaction to the tested materials were recorded as mild, moderate or severe inflammation. After seven days both root canal sealers showed severe inflammatory reaction of connective tissue in experimental animals.Tubuliseal caused prolonged moderate and mild inflammation. Sealapex caused mild inflammation which diminished at the end of the observation period. The results of this study demonstrated that Sealapex was better tolerated by tissue than Tubliseal.
AB  - Cilj ovog rada je bio da se ispita efekat implantiranih materijala za trajnu opturaciju kanala korena, Tubliseal-a i Sealapex-a na meka tkiva. Sveže zamešan material je injiciran u leđno potkožno vezivno tkivo kod 12 pacova Wistar soja. Na svakom pacovu su formirana 4 operativna polja ( po 2 za testirani material Tubliseal ili Apexit ,a 2 za kontrolni materijal). Opservacioni period je iznosio 7, 21 i 60 dana. Analiza je urađena na isečku tkiva koji je obuhvatio deo kože, potkožnog tkiva i deo mišićnog sloja. Preparati su histološki pripremljeni,a analiza preparata je izvršena svetlosnim mikroskopom. Reakcija tkiva na testirani materijal označena je kao blaga, umerena i jaka inflamacija. Posle 7 dana oba materijala su izazvala jaku inflamatornu reakciju potkožnog vezivnog tkiva eksperimentalnih životinja.U kasnijem periodu je došlo do smanjenja inflamacije. Tubliseal je izazavao prolongiranu umerenu, a kasnije blagu inflamaciju. Sealapex je dao blagu inflamaciju koja se izgubila na kraju opservacionog perioda. Rezultati ispitivanja ukazuju da je Sealapex tolerantniji za tkivo u odnosu na Tubliseal.

Frequency and quality of endodontically treated teeth in adult population

AB  - The aim of this study was to investigate the frequency and quality of endodontically treated teeth in adult patients. The panoramic radiographs of 175 adult patients visiting Faculty of Stomatology were examined for endodontic treatment and periapical status. Root canal fillings were categorized as "adequate" or "inadequate" with regard to root canal filling length and lateral seal. Results were analyzed statistically using chi-squared test. From total of 4854 teeth examined, root canal treatment was found in 508 teeth (10.5%). Teeth in the maxilla were more frequently root-filled (24.4 and 24.5%), as compared to those in the mandible (13.4 and 13.9%). The difference was statistically significant (p > 0.01). The highest incidence of root canal treatment was scored for the maxillary central incisors (20.1%). The adequate root canal fillings were detected in 178 (35%), and inadequate root canal fillings were detected in 330 teeth (65%). The periapical radiolucencies were found in 338 (69%) of all examined teeth and in 66.5 of the endodontically treated teeth. The number of teeth with apical periodontitis without endodontic treatment was 94 (1.9%) of all examined teeth 149 (85.1%) patients had 1 or more teeth with apical periodontitis.
