Salivary interleukin-8 levels in children suffering from Type 1 diabetes mellitus

AB  - Objective: The aim of this study was to investigate the differences between the salivary levels of IL-8 in patients with Type I diabetes mellitus (DM) with (DM+P) or without (DM-P) concomitant periodontitis and healthy subjects. The correlations between the levels of these cytokines and clinical periodontal parameters were also established Methods: Twenty children and adolescents with Type I DM (10 diagnosed with periodontitis, 10 presenting no signs of periodontitis) and a control group consisting of 20 healthy children and adolescents aged 7-18 years were recruited for this study. Results: The Salivary IL-8 level was statistically significantly (p lt 0.005) elevated in subjects with Type 1 DM (474.47 +/- 716.76) compared to non-diabetic control group (101.99 +/- 68.32). There was no difference (p>0.05) in the salivary IL-8 level when subjects with Type I DM with concomitant periodontitis were compared to diabetics without periodontitis. When the salivary IL-8 level in subjects with Type I DM was correlated with the clinical parameters, no statistical significance was found Conclusion: An elevated salivary IL-8 level in subjects with Type 1 DM without concomitant periodontitis plays a major role in the development of diabetic micro and macroangiopathy and pathogenesis of atherosclerosis. Consequently, this may offer a basis for the assessment of risk, prophylaxis and treatment of diabetic complications.

Orthodontic-surgical treatment of the skeletal class III malocclusion: A case report

AB  - Background. Class III malocclusions are considered to be ones of the most difficult problems to treat. Their causes are multifactorial and include genetic and/or environmental factors. Class III malocclusions are generally classified into 2 categories: skeletal and dental. The diagnosis is important due to the different treatment approaches. Generally a dental class III can be treated with orthodontics alone, while a true skeletal class III requires a combination of orthodontics and surgery. Case report. We presented a female patient with skeletal Class III malocclusion. The treatment was complete with positive overbite and acceptable occlusion using a combination of fixed orthodontic appliance treatment as well as the surgical operation. The patient was happy with her new appearance and function. Conclusion. Class III discrepancy should be diagnosed and classified according to its etiology and treated with appropriate surgery, including, if necessary, not only mandibular, but also maxillary surgery, in order to achieve a normal facial appearance. In any case, as the field of orthodontics continues to develop technologically and philosophically, we can expect that advances in diagnosis and treatment planning are imminent and inevitable.
AB  - Uvod. Malokluzije III klase smatraju se među najtežim za lečenje. Faktori koji dovode do njihovog formiranja su različiti, počev od naslednih do onih koji se javljaju tek posle rođenja. Ove malokluzije se inače dele na dve velike grupe: dentoalveolarne i skeletne. Zbog različitih pristupa samom lečenju kako dentoalveolarnih, tako i skeletnih oblika ove malokluzije, najvažnije je postaviti tačnu dijagnozu. Dentoalveolarni oblici III klase mogu se lečiti samo ortodontski, dok teži slučajevi skeletnih oblika moraju da kombinuju ortodontsko-hirurško lečenje. Prikaz slučaja. U ovom radu prikazana je bolesnica sa malolkuzijom III skeletne klase. Lečenje je završeno sa pozitivnim zadovoljavajućim preklopom i okluzijom ortodontskim prehirurškim lečenjem, kao i hirurškim zahvatom. Bolesnica je bila zadovoljna novim promenama kako intraoralnim, tako i ekstraoralnim, uočljivim na samom licu kao i postignutom funkcijom. Zaključak. Mimoilaženje vilica III klase neophodno je dijagnostikovati i svrstati prema poreklu i uzroku i lečiti primenom odgovarajuće hirurgije uključujući, prema potrebi, ne samo hirurgiju mandibule, već i maksile. U svakom slučaju, možemo očekivati stalno usavršavanje u postavljanju dijagnoze i lečenju s obzirom na činjenicu da se ortodoncija razvija i tehnološki i filozofski.

Assessment of available bone in the region of the mental foramen for insertion of dental implants

AB  - The aim of this study was to investigate the quantity of the residual bone above the mental foramen/canal relevant to insertion of dental implants. Measurements were performed on 39 dried mandibles that were sectioned perpendicular through the mental foramen. The thickness of bone was measured from the alveolar crest to the mental foramen and the mental canal, as well as from the mental canal to the lingual cortex. Mean distance between the alveolar crest and the mental foramen was 14.79 mm, ranging from 5.3 mm to 21.2 mm. The bony height measured between the alveolar crest and the mental canal was 17.34 mm, ranging from 6.67 mm to 24.43 mm. The mean difference was 2.99 mm, ranging from 0.5 to 6.33 mm. Bone thickness measured between the mental canal and the lingual cortex was 5.43 mm, ranging from 2.43 mm to 8.97 mm. It is concluded that the distance between the alveolar crest and the mental foramen can be used as a reliable guide for the length of an implant to be inserted in the region of the mental foramen. Such an implant can be 2 mm longer than this distance.

Mandibular nerve block at the oval foramen using reliable landmarks: refinement of a previously described procedure

AB  - The aim of this study was to improve the injection technique when blocking the mandibular nerve at the oval foramen by using consistent landmarks. The lower lateral orbital angle (LLOA) and the junction of the external acoustic canal and the mastoid process (AM) proved to be reliable landmarks. Distances between landmarks for the localization of the puncture point (PP), cranial indices as well as the angle of the injection needle to the sagittal plane were calculated on 22 human dried skulls. The PP was found to lie exactly in the middle between the LLOA and AM. The angle of the injection needle to the sagittal plane was 71.86degrees. No correlation was found between the cranial index and the angulation of the needle or the distances for the localization of the PP. From this study a modified puncture technique was developed.
