Apolipoprotein epsilon 4 Allele and Dental Occlusion Deficiency as Risk Factors for Alzheimer's Disease

AB  - Compromised dentition has been suggested to pose a significant risk factor for dementia. It was mainly investigated through insufficient tooth number, disregarding contact between opposing teeth (dental occlusion). The epsilon 4 allele of apolipoprotein (APOE4) is the primary genetic marker for the late onset of Alzheimer's disease (AD). However, APOE4 and dental occlusion have not yet been investigated as possible associated risk factors for AD. The study was aimed to examine the impact of dental status and different APOE gene variants on AD occurrence. Secondly, sociodemographic variables were investigated as factors potentially associated with AD. The case-control study included two groups: 116 patients with AD (according to the NINDS-ADRDA criteria) and 63 controls (Mini-Mental State Examination scores >= 24). The analysis of APOE gene polymorphism was conducted through PCR reaction. Dental examination included recording of number of teeth, presence of fixed or removable dentures, and number of functional tooth units (FTU). Regression analysis was used to investigate the joint effect of the clinical and genetic variables on AD. Results showed that patients with AD were more often carriers of epsilon 3/epsilon 4 genotype and epsilon 4 allele, had lower number of teeth and FTU, and were less likely to be married, live in home, and had less chronic diseases, compared to the controls. Regression analysis showed that presence of APOE4 allele and the number of total FTU remained associated with AD, even when adjusted for age, sex, and level of education. In conclusion, deficient dental occlusion and presence of APOE4 may independently increase risk for AD.

Retention force of overdenture retained with telescopic crowns – a comparison of polyether ether ketone and zirconia ceramic telescopic crowns

AB  - Introduction/Objective. Recently, new materials for double crowns have been introduced, such as zirconia and polyether ether ketone (PEEK). However, some characteristics of these materials, such as retentive force and duration of “settling in phase,” have not been investigated sufficiently. During the “settling in phase,” telescopic overdenture has not yet achieved its definitive retention force, and it can be harmful for periodontal tissue if the value is above optimal for a long period of time. The objective was to measure the in vitro overall pull-off force of telescopic crowns where primary crowns were made from zirconia ceramics and a survey of the “settling in phase” duration. Methods. Forty zirconia primary telescopic crowns were produced on prepared canine teeth. Twenty secondary crowns were of PEEK and other 20 of zirconia with electroplated gold copings. The pull-off force measurements were conducted utilizing a dynamometer until a constant value was obtained. Results. The specimens of the PEEK group showed higher initial retentive force values. Settling in phase was finished between 800 and 900 cycles of separation for both groups. Comparing the value of the pull-off force between individual different cycles, a statistically significant reduction was recorded up to the 800th cycle, while between the 800th and the 900th cycle there was no difference. Conclusions. The settling in phase was finished between 800 and 900 cycles of separation in both groups. Final retentive force values for both tested telescopic groups were in the optimal range which is 5–9 N per one telescopic crown.
AB  - Uvod/Cilj Poslednjih godina uvedeni su u praksu novi materijali za dvostruke krune, kao što su cirkonija i polietereterketon (PEEK). Međutim, neke karakteristike ovih materijala nisu dovoljno ispitane, kao što su retenciona sila i trajanje "faze uhodavanja". "Faza uhodavanja" je inicijalni period upotrebe teleskopske proteze kada finalna retenciona sila još uvek nije postignuta, i može imati štetni uticaj na parodontalno tkivo ako su u tom periodu sile previsoke i predugo traju. Cilj je bio da se izmeri in vitro ukupna sila razdvajanja teleskopskih kruna, gde su primarne krune izrađene od cirkonijumske keramike, i ispitati trajanje faze uhodavanja. Metode Četrdeset primarnih teleskopskih kruna od cirkonijumske keramike je izrađeno na preparisanim očnjacima. Dvadeset sekundarnih kruna je izrađeno od PEEK-a, i još 20 kruna od cirkonije sa galvanizovanim zlatom. Za merenje sile razdvajanja korišćen je dinamometar. Spajanje i razdvajanje teleskopskih kruna i merenje sile razdvajanja je vršeno dok nije dobijena konstantna vrednost. Rezultati Uzorci iz grupe PEEK pokazali su višu inicijalnu vrednost retencione sile. Faza uhodavanja je završena između 800 i 900 ciklusa razdvajanja kod obe grupe. Kada se uporede individualne vrednosti sile razdvajanja između različitih ciklusa, statistički značajno smanjenje je zabeleženo do 800. ciklusa, dok između 800. i 900. ciklusa nije bilo razlike. Zaključak Faza uhodavanja je završena između 800 i 900 ciklusa razdvajanja u obe grupe. Finalna retenciona sila kod obe testirane grupe pokazala je optimalne vrednosti, koje iznose 5-9 N po teleskopskoj kruni.

Effects of Prosthetic Therapy in the Treatment of Edentulous Patients with Obstructive Sleep Apnea

AB  - Background: This study is based on the assumption that the reduced vertical dimension of occlusion (VDO) in patients wearing complete dentures could be one of the causes of OSA appearance. The aim of the study was to examine the effect of prosthetic treatment which corrects the reduced VDO in edentulous patients with OSA. Methods: The study involved 20 edentulous patients with apnea/hypopnea index per hour of sleep (AHI/h) > 5.7  lt  32.5. The methodology of research required a multidisciplinary chronological protocol from collection of anamnestic data, intraoral examination, polysomnographic (PSG) registration during sleep, and diagnosis by using the magnetic resonance imaging (MRI) to fabrication of the complete dentures in accordance with the established clinical procedures. Volume analysis (in cubic millimeters) of the pharynx was performed as sum of the segmented areas multiplied by their thickness. Results: Prosthetic therapy in edentulous patients with OSA reduced the value of AHI/h. The average value of AHI/h based on the sample data before the treatment was 17.6 +/- 8.9 while three months after the treatment with complete dentures AHI/h amounted to 9.7 +/- 6.5 (p  lt  0.001). After three months from prosthetic rehabilitation, MRI showed a mean volume increase of 27% of pharynx in edentulous patients with OSA. Conclusion: The prosthetic treatment with complete dentures which corrects VDO provides adequate support to orofacial structures, increases the volume of the pharynx and reduces the symptoms of OSA in edentulous patients.

Dental treatment of the elderly people with disabilities

AB  - Introduction. The growing population of the elderly people and a proportional increase in the number of the elderly with different types of disabilities, necessitates a multidisciplinary approach to the assessment of their oral health and dental treatment. The ultimate aim is to retain a pain-free functional dentition and decrease the risk of future disease. Material and Methods. A PubMed search was performed and the authors contributed their experience in implementing preventive and therapeutic measures. Oral health problems of the elderly with disabilities. Two main factors influence the oral health: multimorbidity and polypharmacy. Oral health problems expected in this population are teeth abrasion, teeth fractures, root caries, periodontitis and problems with wearing dentures due to stomatitis caused by Candida albicans. Oral health assessment and treatment guidelines. This article provides guidelines for assessment and treatment planning, taking into consideration multimorbidity, polypharmacy, dementia and capacity of caregivers. Preventive measures. Preventive measures are crucial for long-term oral health of this population, and this paper provides guidelines for preventive treatment depending on the degree of functional dependence. Prosthetic treatment. Although some elderly with disabilities are suitable for conventional prosthetic treatment, often there are contraindications and specific considerations that must be taken into account. Conclusion. Oral health needs of the elderly people with disabilities should not be neglected and the success of treatment depends on the education of dental professionals and cooperation with other health professionals of the medical team.
AB  - Uvod. Zbog rastuće populacije starih ljudi i proporcionalnog povećanja učestalosti broja starih ljudi sa različitim tipovima hendikepa, multi disciplinarni pristup proceni njihovog oralnog zdravlja je neophodan. Krajnji cilj je postići funkcionalnu denticiju bez bolova i smanjiti rizik od budućih oboljenja. Materijal i metode. Pretraživanje literature u bazi PubMed je obavljeno, ali je takođe iskustvo autora u primeni preventivnih i terapijskih mera uzeto u obzir. Problemi oralnog zdravlja starih ljudi sa hendikepom. Dva glavna razloga imaju uticaj na oralno zdravlje: multi morbiditet i polifarmacija. Problemi oralnog zdravlja koji se očekuju u ovoj populaciji su abrazija zuba, frakture zuba, karijes korena, parodontopatija i problemi sa nošenjem proteza usled stomatitis uzrokovanog kandidom (Candida albicans). Procena oralnog zdravlja i smernice za planiranje tipa tretmana. Ovaj članak obezbeđuje korisne smernice za procenu i planiranje tretmana, uzimajući u obzir multi morbiditet, polifarmaciju, demenciju i kapacitet negovatelja. Preventivne mere. Preventivne mere su ključne za dugotrajni uspeh oralnog zdravlja ove populacije i ovde su prikazane smernice za preventivne mere u zavisnosti od stepena funkcionalne zavisnosti. Protetski tretman. Iako su neki stari ljudi sa hendikepom pogodni za konvencionalni protetski tretman, većina ovih pacijenata ima neke kontra indikacije i specifičnosti koje treba uzeti u obzir. Zaključak. Potrebe za oralnim zdravljem starih ljudi sa hendikepom ne smeju biti zanemarene, a uspeh tretmana zavisi od edukacije stomatologa kao i saradnje sa ostalim specijalistima medicinskog tima.

A PubMed search was performed and the authors contributed their experience in implementing preventive and therapeutic measures. Oral health problems of the elderly with disabilities. Two main factors influence the oral health: multimorbidity and polypharmacy. Oral health problems expected in this population are teeth abrasion, teeth fractures, root caries, periodontitis and problems with wearing dentures due to stomatitis caused by Candida albicans. Oral health assessment and treatment guidelines. This article provides guidelines for assessment and treatment planning, taking into consideration multimorbidity, polypharmacy, dementia and capacity of caregivers. Preventive measures. Preventive measures are crucial for long-term oral health of this population, and this paper provides guidelines for preventive treatment depending on the degree of functional dependence. Prosthetic treatment. Although some elderly with disabilities are suitable for conventional prosthetic treatment, often there are contraindications and specific considerations that must be taken into account. Conclusion. Oral health needs of the elderly people with disabilities should not be neglected and the success of treatment depends on the education of dental professionals and cooperation with other health professionals of the medical team., Uvod. Zbog rastuće populacije starih ljudi i proporcionalnog povećanja učestalosti broja starih ljudi sa različitim tipovima hendikepa, multi disciplinarni pristup proceni njihovog oralnog zdravlja je neophodan. Krajnji cilj je postići funkcionalnu denticiju bez bolova i smanjiti rizik od budućih oboljenja. Materijal i metode. Pretraživanje literature u bazi PubMed je obavljeno, ali je takođe iskustvo autora u primeni preventivnih i terapijskih mera uzeto u obzir. Problemi oralnog zdravlja starih ljudi sa hendikepom. Dva glavna razloga imaju uticaj na oralno zdravlje: multi morbiditet i polifarmacija. Problemi oralnog zdravlja koji se očekuju u ovoj populaciji su abrazija zuba, frakture zuba, karijes korena, parodontopatija i problemi sa nošenjem proteza usled stomatitis uzrokovanog kandidom (Candida albicans). Procena oralnog zdravlja i smernice za planiranje tipa tretmana. Ovaj članak obezbeđuje korisne smernice za procenu i planiranje tretmana, uzimajući u obzir multi morbiditet, polifarmaciju, demenciju i kapacitet negovatelja. Preventivne mere. Preventivne mere su ključne za dugotrajni uspeh oralnog zdravlja ove populacije i ovde su prikazane smernice za preventivne mere u zavisnosti od stepena funkcionalne zavisnosti. Protetski tretman. Iako su neki stari ljudi sa hendikepom pogodni za konvencionalni protetski tretman, većina ovih pacijenata ima neke kontra indikacije i specifičnosti koje treba uzeti u obzir. Zaključak. Potrebe za oralnim zdravljem starih ljudi sa hendikepom ne smeju biti zanemarene, a uspeh tretmana zavisi od edukacije stomatologa kao i saradnje sa ostalim specijalistima medicinskog tima.",

The aim of this in vitro study was to test and analyze the sealing ability of three endodontic materials used for permanent obturation, in between the dentin walls and the gutta-percha points, using a scanning electron microscope. Material and Methods. Forty-five recently extracted single-root teeth, treated by a step-back technique, were divided into three groups (15 teeth in each); the canals were filled with three different permanent obturation materials: N2 - zinc oxide root canal cement, Gutta Flow (Coltene), and Endomethasone N (Septodont). Their sealing ability and adhesive properties were analyzed using field emission gun scanning electron microscope, at the time when they were applied between the dentin walls of the canal and the gutta-percha. Results. The results of the scanning electron microscope analysis have shown that all the three sealers have good adhesion properties when used over the root canal walls in the apical third. Good adaptation of the filling used for the root canal walls in the middle and the cervical third was found only in teeth obturated using Gutta Flow, while samples obturated by N2 - zinc oxide root canal cement and Endomethasone showed the weakest adhesion, and greatest number of cracks between the sealer and the canal wall. Conclusion. When using a single-cone obturation technique, compared to other obturation materials, Gutta Flow shows considerably better adaptation to the root canal wall and the gutta-percha points in the apical, middle, and the cervical third of the root.

Cilj ove in vitro studije je testiranje i analiziranje sposobnosti zaptivanja tri različita endodontska materijala koji se upotrebljavaju za definitivno punjenje između dentinskih zidova i gutaperke poena, primenom elektronskog mikroskopa. Materijal i metode. Četrdeset pet ekstrahovanih jednokorenih zuba, tretiranih step-back tehnikom bili su podeljeni u tri grupe (po 15 zuba u svakoj od njih), kanali su bili ispunjeni trima različitim materijalima za definitivnu opturaciju kanala: cink-oksid eugenolom, Gutta Flow (Coltene) i Endomethasone N (Septodont). Elektronskim mikroiskopom sa emisijom polja analizirani su adhezivna sposobnost i zaptivanje materijala za punjenje između dentinskih zidova korencskog kanala i gutaperke. Rezultati. Kada se primenjuje single-cone tehnika za opturaciju kanala, Gutta Flow pokazuje znatno bolju adaptaciju za zid korenskog kanala i gutaperke u apikalni deo kao i u srednjoj i cervikalnoj trećini korena u poređenju sa drugim materijalima za opturaciju kanala.

Oral health-related quality of life of institutionalized elderly in Serbia

AB  - Background/Aim. Elderly residents in nursing homes have a great risk of periodontal and tooth diseases. Improving oral health can also improve residents' general health and quality of life. The objective of our study was to investigate oral health related quality of life of institutionalized elderly in Serbia using Geriatric Oral Health Assessment Index (GOHAI). Methods. The Serbian version of the GOHAI questionnaire was developed in accordance with the recommended backward-forward method. A total of 301 participants completed the Serbian version of the GOHAI questionnaire. The questionnaire sought information about sociodemographic characteristics and self-reported perception of general and oral health. Clinical examination included assessment of periodontal and dental status. Reliability, internal consistency, and concurrent and convergent validity of GOHAI scores were examined. Results. There were 197 female and 104 male participants with the average age of 78.6 (SD ± 7.8) and average time spent in nursing home 4.9 (SD ± 4.7) years. The average score of the GOHAI was 48.4 (SD ± 8.4). Low GOHAI scores were associated with perceptions of poor oral and general health. Cronbach's alpha coefficient for the Serbian version of GOHAI was 0.79. This instrument showed a high level of internal consistency and homogeneity between questions. The respondents who perceived that they needed dental treatment at the time had significantly lower GOHAI scores. A total of 48.5% of the participants reported 'always' having difficulties when chewing. On the other hand, a small number of participants (0.3%) used medications 'always' to relieve dental pain. Conclusion. The Serbian version of the GOHAI showed acceptable reliability and validity. The GOHAI final score was considered low, indicating low oral health self-perception by the institutionalized elderly in Serbia.
AB  - Uvod/Cilj. Stariji korisnici domova za stara lica izloženi su velikom riziku od nastanka periodontalnih bolesti i bolesti zuba. Poboljšanje oralnog zdravlja može poboljšati opšte zdravlje i kvalitet života korisnika domova za stara lica. Cilj našeg istraživanja bio je da se ispita oralno zdravlje i njegova povezanost sa kvalitetom života korisnika domova za stara lica u Srbiji pomoću indeksa Geriatric Oral Health Assessment (GOHAI). The use of essential oils based antiseptic solution in the treatment of denture stomatitis

AB  - Introduction Local therapy of denture stomatitis (DS) associated with Candida species fungi infection usually involves the application of nystatin and miconazole. Due to the fact that these drugs may be less efficient against biofilm and possible resistance development, a new approach in the treatment includes the use of antiseptic agents. The aim of the study was to compare clinical and microbiological therapeutic outcomes of antiseptic solution Listerine® and Daktanol® antifungal oral gel in the treatment of DS associated with Candida species fungi. Material and Methods The study included 30 patients affected by DS, divided into the two treatment groups, control group (n=15) treated by Daktanol® gel and experimental group (n=15) treated by the antiseptic solution Listerine®. Successful treatment was evaluated based on palatal mucosa inflammation reduction classified according to the Newton classification and the difference in the number of fungal colony- forming units (CFU) isolated by smears before and after the treatment that lasted 14 days. Results Reduction in inflammation intensity and fungal CFU number on palatal mucosa (p lt 0,01) as well as on denture base (p lt 0,01) were observed in both groups of subjects after the treatment. Conclusion Antiseptic solution Listerine® and Daktanol® antifungal gel both reduced palatal mucosal inflammation and CFU number of fungi in mouth without significant differences among them. CFU number of fungi isolated from denture base was significantly lower after the treatment with Listerine® (p lt 0.05).
AB  - Uvod Lokalna terapija proteznog stomatitisa (PS) udruženog sa infekcijom gljivicama iz roda Candida najčešće se sprovodi upotrebom nistatina i mikonazola. Zbog otežanog dejstva ovih lekova na biofilm, kao i zbog razvoja moguće rezistencije, pažnja se usmerava na terapijske efekte koji se mogu postići primenom antiseptičkih sredstava. Cilj rada je da se uporede klinički i mikrobiološki ishodi primene antiseptičkog rastvora Listerine® i oralnog gela sa antimikotskim dejstvom Daktanol® u lečenju PS udruženog sa pojavom gljivica iz roda Candida. Metode rada Studija je uključivala 30 ispitanika obolelih od PS, podeljenih u dve terapijske grupe: kontrolnu (n = 15) lečenu gelom Daktanol® i eksperimentalnu (n = 15) – antiseptikom Listerine®. Uspešnost terapije procenjivana je na osnovu smanjenja intenziteta zapaljenja palatinalne sluzokože procenjenog prema klasifikaciji Newton pre početka lečenja i razlike u broju brisom izolovanih gljivičnih kolonija (CFU) pre i posle terapije, koja je trajala 14 dana. Rezultati Kod obe grupe ispitanika došlo je do smanjenja intenziteta zapaljenja i smanjenja CFU i na nepcu (p  lt  0,01) i na bazi proteze (p  lt  0,01) nakon terapije. Zaključak Antiseptički rastvor Listerine® i antimikotik Daktanol® dovode do smanjenja intenziteta zapaljenja palatinalne sluzokože i do značajnog smanjenja CFU u terapiji PS, bez međusobno značajne razlike, osim u smanjenju broja CFU izolovanih sa baze proteza nakon terapije, gde je rastvor Listerine® pokazao veću efikasnost (p  lt  0,05).

Material and Methods The study included 30 patients affected by DS, divided into the two treatment groups, control group (n=15) treated by Daktanol® gel and experimental group (n=15) treated by the antiseptic solution Listerine®. Successful treatment was evaluated based on palatal mucosa inflammation reduction classified according to the Newton classification and the difference in the number of fungal colony- forming units (CFU) isolated by smears before and after the treatment that lasted 14 days. Results Reduction in inflammation intensity and fungal CFU number on palatal mucosa (p lt 0,01) as well as on denture base (p lt 0,01) were observed in both groups of subjects after the treatment. Conclusion Antiseptic solution Listerine® and Daktanol® antifungal gel both reduced palatal mucosal inflammation and CFU number of fungi in mouth without significant differences among them. Zbog otežanog dejstva ovih lekova na biofilm, kao i zbog razvoja moguće rezistencije, pažnja se usmerava na terapijske efekte koji se mogu postići primenom antiseptičkih sredstava. Cilj rada je da se uporede klinički i mikrobiološki ishodi primene antiseptičkog rastvora Listerine® i oralnog gela sa antimikotskim dejstvom Daktanol® u lečenju PS udruženog sa pojavom gljivica iz roda Candida. Self-perception and satisfaction with dental appearance and aesthetics with respect to patients' age, gender, and level of education

Objective The aim of this study was to investigate the influence of age, education level, gender, and different dental status and the appearance of the upper anterior teeth (color, size, shape, position and alignment of the anterior teeth) on the satisfaction of the respondents with dental appearance and aesthetics of their upper anterior teeth and their desire for improvement. Methods The study encompassed 480 people aged 20 to 50 years with an average age of 30.84 years. There were 236 male and 244 female subjects. The respondents were interviewed using a questionnaire specially designed for the purpose of this research. For the study, the subjects were divided into the following three age groups: the younger age group (20-30 years of age), the middle age group (31-40 years of age), and the older age group (41-50 years of age). Results The conducted study did not reveal statistical significance with respect to gender in any of the examined parameters (p > 0.05). A little more than one half of the respondents in each age group were satisfied with their dental appearance and aesthetics (60.3% of the respondents in the age group of 20-30 years, 55.7% in the age group of 31-40, and 53.7% in the age group of 41-50 years of age). Satisfaction with dental appearance and aesthetics increases linearly with the increase in the level of education and was the highest among the respondents with university degree (33.3%). Conclusion Female respondents were more dissatisfied with their dental appearance and aesthetics as compared with male respondents, but the difference was found to be non-significant. Patients with higher education level were more satisfied with their dental appearance and aesthetics than those with lower education.
AB  - Uvod Pacijentovo subjektivno vrednovanje dentalnog izgleda i estetike postaje sve važniji faktor prilikom estetskih tretmana i protetskih terapija. Cilj rada Cilj istraživanja bio je da istraži uticaj godina starosti, nivoa obrazovanja, pola ispitanika i različiti zubni status i izgled gornjih prednjih zuba (boja, veličina, oblik, poredak prednjih zuba) na zadovoljstvo ispitanika dentalnim izgledom i estetikom gornjih prednjih zuba i postojanje želje za unapređenjem dentalne estetike. Metode rada Istraživanje je sprovedeno na 480 osoba od 20 do 50 godina, prosečne starosti 30,84 godina. Bilo je 236 ispitanika muškog pola i 244 ispitanika ženskog pola. Ispitanici su intervjuisani putem upitnika specijalno napravljenog za potrebe ovog istraživanja. Za potrebe istraživanja ispitanici su podeljeni prema godinama starosti u tri starosne grupe: mlađa starosna grupa (20-30 godina), srednja starosna grupa (31-40 godina), starija starosna grupa (41-50 godina). Rezultati Sprovedena studija nije utvrdila statističku signifikantnost u odnosu na pol ni u jednom od ispitivanih parametara (p > 0,05). Polovina ispitanika u svakoj starosnoj grupi bila je zadovoljna dentalnim izgledom i estetikom: 60,3% ispitanika u starosnoj grupi 20-30 godina, 55,7% u starosnoj grupi 31-40 godina i 53,7% u starosnoj grupi 41-50 godina. Zadovoljstvo dentalnim izgledom i estetikom ima linearno pravilo porasta zadovoljstva sa porastom stepena obrazovanja i najveće je kod ispitanika sa završenim fakultetom (33,3%). Zaključak Žene su bile nezadovoljnije svojim dentalnim izgledom i estetikom u odnosu sa muškarce, ali razlika nije statistički značajna. Pacijenti sa visokim stepenom obrazovanja bili su zadovoljniji dentalnim izgledom i estetikom nego ispitanici sa nižim stepenom obrazovanja.

Difference in apolipoprotein e genotype distribution between dentate and edentulous elderly patients with Alzheimer disease

AB  - Association between dementia and tooth loss has been shown although the nature of that association is not clear. It has also been shown that risk of dementia was increased in apolipoprotein E4 allele carriers. The objective of this study was to determine the frequency of APOE alleles and their association with the dental status in elderly demented patients. Dental status of 67 patients with dementia was recorded. DNA was isolated from buccal swabs and genotyping was done by PCR-RFLP. The majority of participants had E3/E4 genotype (55.2%) and these heterozygotes were significantly more frequent than any other genotype (p lt 0.001). There was no significant association between dental status and genotype. However, partial edentulousness with very few teeth in both jaws (1-9 teeth) was significantly more frequent among demented patients with E3/E4 genotype (p=0.021). Patients with Alzheimer disease most frequently had E3/E4 genotype and had very few or no teeth.

Caregivers' attitudes, knowledge and practices of oral care at nursing homes in Serbia

AB  - Background/Aim. Within the elderly population, residents in nursing homes, there is a greather risk of caries, periodontal disease and teeth loss. Assistance of caregivers in maintaininig good oral hygiene besides improving oral health can improve of residents general health and the quality of their lives. The aim of this study was to examine the attitudes of caregivers and knowledge about oral health, as well as the practice regarding oral care they apply at nursing homes in Serbia. Methods. The survey was conducted at the Gerontology Center Belgrade, consisting of four nursing homes located in the urban area. The study included 58 caregivers. They were contacted on working days, in all work shifts, during January, February and March of 2013. They were asked to fill in a self-administered questionnaire consisting of 26 closed-type questions. Results. The care-givers mostly considered that it was very important to take care of oral health of the residents, but 69% responded that the level of their oral health was low or very low. As the main barriers to oral hygiene maintenance, the caregivers indicated lack of time. The caregivers had more knowledge about periodontal disease than about the main cause of caries and its prevention. Formal medical education had the influence on the knowledge about oral diseases. Oral hygiene procedures carried out by the mayority of caregivers were denture cleaning and tooth brushing. Conclusion. The caregivers were aware of the limitations in everyday oral care of nursing homes residents in Serbia, although solving these problems requires the involvement of the entire public health service.
AB  - Uvod/Cilj. U grupi starijih osoba, stanovnika domova za stara lica, postoji povećani rizik od nastanka karijesa, periodontalne bolesti i gubitka zuba. Pomoć u održavanju dobre oralne higijene, pored toga, može uticati i na opšte zdravstveno stanje i kvalitet života stanovnika domova za stara lica. Cilj ovog istraživanja bio je da se ispitaju stavovi i znanje negovatelja o oralnom zdravlju, kao i praksa koju primjenjuju u domovima za stara lica u Srbiji. Metode. Istraživanje je sprovedeno u ustanovi Gerontološki Centar Beograd, koji se sastoji od četiri doma za stara lica, locirana u urbanom području. U istraživanju je učestvovalo 58 negovatelja, s kojima smo kontaktirali radnim danima, u svim radnim smenama, tokom januara, februara i marta 2013. godine. Oni su zamoljeni da samostalno ispune upitnik sastavljen od 26 pitanja zatvorenog tipa. Rezultati. Negovatelji uglavnom smatraju da je veoma važno brinuti se o oralnom zdravlju korisnika domova, ali 69% je odgovorilo da je nivo oralnog zdravlja korisnika nizak ili vrlo nizak. Za glavnu prepreku u održavanju oralne higijene korisnika domova, negovatelji su označili nedostatak vremena. Njegovatelji su imali više znanja o parodontalnim bolestima, nego o glavnom uzročniku karijesa i njegovoj prevenciji. Formalno medicinsko obrazovanje imalo je uticaj na njihovo znanje o oralnim bolestima. Čišćenje proteza i pranje zuba su najčešće primenjivani postupci negovatelja u održavanju oralne higijene korisnika domova. Zaključak. Negovatelji su svesni ograničenja u svakodnevnom održavanju oralnog zdravlja korisnika domova za stara lica u Srbiji, kao i da rešavanje tih problema zahteva uključivanje celokupnog javnog zdravstvenog sektora.

The survey was conducted at the Gerontology Center Belgrade, consisting of four nursing homes located in the urban area. The study included 58 caregivers. They were contacted on working days, in all work shifts, during January, February and March of 2013. They were asked to fill in a self-administered questionnaire consisting of 26 closed-type questions. Results. The care-givers mostly considered that it was very important to take care of oral health of the residents, but 69% responded that the level of their oral health was low or very low. As the main barriers to oral hygiene maintenance, the caregivers indicated lack of time. The caregivers had more knowledge about periodontal disease than about the main cause of caries and its prevention. Formal medical education had the influence on the knowledge about oral diseases. Oral hygiene procedures carried out by the mayority of caregivers were denture cleaning and tooth brushing. Conclusion. The caregivers were aware of the limitations in everyday oral care of nursing homes residents in Serbia, although solving these problems requires the involvement of the entire public health service., Uvod/Cilj. U grupi starijih osoba, stanovnika domova za stara lica, postoji povećani rizik od nastanka karijesa, periodontalne bolesti i gubitka zuba. Pomoć u održavanju dobre oralne higijene, pored toga, može uticati i na opšte zdravstveno stanje i kvalitet života stanovnika domova za stara lica. Cilj ovog istraživanja bio je da se ispitaju stavovi i znanje negovatelja o oralnom zdravlju, kao i praksa koju primjenjuju u domovima za stara lica u Srbiji. Metode. Istraživanje je sprovedeno u ustanovi Gerontološki Centar Beograd, koji se sastoji od četiri doma za stara lica, locirana u urbanom području. U istraživanju je učestvovalo 58 negovatelja, s kojima smo kontaktirali radnim danima, u svim radnim smenama, tokom januara, februara i marta 2013. godine. Oni su zamoljeni da samostalno ispune upitnik sastavljen od 26 pitanja zatvorenog tipa. Rezultati. Negovatelji uglavnom smatraju da je veoma važno brinuti se o oralnom zdravlju korisnika domova, ali 69% je odgovorilo da je nivo oralnog zdravlja korisnika nizak ili vrlo nizak. Za glavnu prepreku u održavanju oralne higijene korisnika domova, negovatelji su označili nedostatak vremena. Njegovatelji su imali više znanja o parodontalnim bolestima, nego o glavnom uzročniku karijesa i njegovoj prevenciji. Formalno medicinsko obrazovanje imalo je uticaj na njihovo znanje o oralnim bolestima. Čišćenje proteza i pranje zuba su najčešće primenjivani postupci negovatelja u održavanju oralne higijene korisnika domova. Zaključak. Negovatelji su svesni ograničenja u svakodnevnom održavanju oralnog zdravlja korisnika domova za stara lica u Srbiji, kao i da rešavanje tih problema zahteva uključivanje celokupnog javnog zdravstvenog sektora.",

AB  - Introduction. As the number of institutionalized elderly in the world increases, so does the need for good medical and dental care for this group of people. The aim of this research was to determine the oral hygiene level, dental, periodontal and prosthetic status of intitutionalized elderly persons of over 65 years, in three residental homes for elderly in Belgrade. Clinical examination determined dental and parodontal status by using DFMT and CPITN index. Condition of oral soft tissues was also estimated. Existing dentures were evaluated in terms of retention, stabilization, attrition, hygiene, and need for repairment. Results. Of all examined people, 16.6% did not maintain oral hygiene; 2.3% received help with maintaing oral hygiene, although 36.5% was dependant on other people's care. Average DFMT was 26.36. CPITN was 2.23 on average. No denture and a need for one was observed in 41.1% examined institutionalized persons. Fifty-five point two percent of dentures needed repairment, and denture hygiene was poor in 50.0% of cases. Among all examined, 11.9% were edentulous and had no denture. Conclusion. To conclude, institutionalized elderly do not receive help in maintaining oral hygiene, alhough the majority is dependent on other people's care. Also, there is a large number of missing teeth and a need for complete parodontal and prosthetic treatment of most examined persons. It is essential to improve dental health care in residential homes for elderly.
AB  - Uvod. Povećanje broja starih institucionalizovanih osoba u svetu iziskuje dobru opštu zdravstvenu i stomatološku zaštitu ove grupe ljudi. Cilj rada bio je ispitati nivo oralne higijene, zatim dentalni, parodontalni i protetski status institucionalizovanih osoba starosti preko 65 godina, u tri staračka doma u Beogradu. Materijal i metod. Anketirana je i pregledana 301 osoba. Anketa je obuhvatala sociodemografske podatke, navike u održavanju oralne higijene, subjektivnu procenu opšteg zdravstvenog stanja i stomatoprotetskog statusa. Kliničkim pregledom utvrđen je status zuba i parodontalnih tkiva korištenjem DFMT i CPITN indeksa. Evidentirano je i zdravlje mekih tkiva usta. Postojeće proteze ocenjene su u pogledu retencije, stabilizacije, abradiranosti, higijene i neophodnosti reparature. Rezultati. Od ukupnog broja pregledanih, 16,6% nije održavalo oralnu higijenu. Pomoć u održavanju oralne higijene dobijalo je 2,3% ispitanika, iako je 36,5% bilo zavisno od tuđe nege. Prosečan DMFT iznosio je 26,36. CPITN indeks prosečno je iznosio 2,23. Protetski nije bilo zbrinuto 45,1% pregledanih osoba koje su imale potrebu za protezom. Reparaturu je zahtevalo 55,2% proteza, a higijena proteze bila je loša u 50,0% slučajeva. Od ukupnog broja pregledanih osoba, 11,9% bilo je bezubo i nije imalo totalne proteze. Zaključak. Može se zaključiti da stare institucionalizovane osobe ne dobijaju pomoć u održavanju oralne higijene, iako su u velikom broju zavisne od tuđe nege. Takođe, prisutan je veliki broj nedostajućih zuba i potreba za kompletnom parodontološkom i protetskom terapijom većine osoba. Neophodno je poboljšati stomatološku zdravstvenu zaštitu pacijenata u staračkim domovima.

AB  - Background/purpose: Common to all frail elderly patients in Serbia is the fact that only little or no attention is given to their dental care. A normative need for prosthetic treatment exists for all partially edentulous and edentulous patients who lack adequate prosthetic devices. The realistic need for treatment means the opportunity to perform the treatment that will be beneficial for each patient individually. The goal of this study was to determine the normative and realistic needs for prosthetic treatment among frail elderly patients in Serbia. Materials and methods: The study involved 117 elderly and frail patients provided service at the Geriatrics Institute in Belgrade, and among families in suburban municipalities. We determined the patients' dental status, cognitive status, general health condition, and desire for treatment. Results: Results showed that 82.9% of patients had normative need for prosthetic treatment. Realistic need for prosthetic treatment was significantly lower-that is 19.7% to 28.2% of patients can be provided appropriate prosthetic treatment in order to improve their oral functionality and quality of life. Conclusion: Although realistic need for prosthetic treatment is significantly lower than normative need, treatment shouldn't be denied to patients who can receive it, especially when the treatment recommended is less invasive.

Assessment of telescopic denture's settling in phase duration

AB  - Introduction. Double telescopic crowns are connecting elements of partial dentures and are considered to be prosthetic devices of high aesthetic and biological value. Their basic drawback is the assessment of retention force value which has to be strong enough to balance the opposite forces but, nevertheless, must not result in harming the periodontal tissue of retention teeth while taking the denture off. The retention force is the highest immediately after the denture's construction and is progressively declining until the end of the 'settling in phase', i.e. until the retention force value becomes well-established. Aim of the study was the measurement of the overall retention force of completed telescopic dentures and a survey of 'settling in phase' duration. Methods. The measuring was conducted on 20 telescopic dentures consisting of 2, 3 or 4 telescopic crowns made of gold alloy type IV and executed by dynamometer. Results. An optimal retention force was detected in only 37% of telescopic dentures. An insufficient retention force was observed in 16%, while in the remaining 47% an excessive force was detected. In the largest number of dentures (35%) the settling in phase began after 33 cycles, which shows that this phase takes on average 10,6 days. Conclusion. The 'settling in phase', during which retention force becomes stabilized, is completed in the first ten days after the delivery of the completed telescopic denture.
AB  - Uvod. Dvostruke teleskop krune predstavljaju vezne elemente parcijalne proteze i smatraju se zubnim nadoknadama visokih estetskih i bioloških vrednosti. Njihov osnovni nedostatak predstavlja procena veličine retencione sile, koja mora biti dovoljno velika da uravnotežava suprotne sile a da sa druge strane, prilikom skidanja, ne dovodi do oštećenja parodontalnog tkiva zuba nosača. Retenciona sila je najveća odmah po izradi proteze i ona progresivno opada sve dok se ne završi tzv. 'faza uhodavanja', odnosno dok vrednost retencione sile ne postane ustaljena vrednost. Cilj istraživanja bio je merenje ukupne retencione sile gotovih teleskop proteza i ispitivanje vremenskog trajanja 'faze uhodavanja'. Materijal i metod. Merenja si vršena na 20 teleskop proteza, koje su u svom sastavu imale 2, 3 ili 4 teleskop krune izrađene od legure zlata tip IV. Merenja su vršena pomoću dinamometra. Rezultati. Kod samo 37% teleskop proteza registrovana je optimalna retenciona sila. Kod 16% registrovana je nedovoljna retenciona sila, dok je kod ostalih 47% registrovana prekomerna retenciona sila. Kod najvećeg broja proteza (35%), faza uhodavanja nastupila je nakon 33 ciklusa, što znači da je potrebno u proseku 10,6 dana za 'fazu uhodavanja'. Zaključak. 'Faza uhodavanja' tokom koje dolazi do ustaljenja retencione sile u proseku se završava u okviru prvih deset dana nakon predaje završene teleskop proteze.

Subjective oral health indicators tested within a particular cultural context may not be relevant across cultures. Materials and methods: The study population comprised 231 free-living adults aged 65 years or older. The OIDP was cross-culturally adapted from English into the Serbian language and its psychometric properties were tested. Data were collected using a clinical examination and a questionnaire containing the OIDP. Results: In terms of reliability, Cronbach's alpha coefficient was 0.82 and the intraclass correlation coefficient 0.88. The very high correlation of OIDP with self-rated oral health (r = 0.78) verified criterion validity, while construct validity was demonstrated through its significant and graded associations with other subjective health measures. OIDP change scores on a treated subsample showed moderate effect size (0.59) and were associated with perceptions of oral health change, providing evidence for its responsiveness to change. Conclusion: The Bosnian version of the OIDP showed satisfactory validity, reliability and responsiveness to change confirming its appropriateness for use among older populations in Bosnia and Herzegovina.

AB  - This study aimed to assess the prevalence and the severity of oral impacts and the relationships between oral health-related quality of life (OHRQoL) and clinical measures of oral function in a sample of older adults in Bosnia. The sample comprised 261 community-dwelling adults of >= 65 yr of age. Participants were randomly drawn from three senior day-centres for elderly people. Data were collected using clinical examination and a questionnaire. The OHRQoL was assessed through the Oral Impacts on Daily Performances (OIDP) measure, in terms of the prevalence, intensity, and extent of oral impacts. Overall, 55.2% reported at least one oral impact in the last 6 months. Difficulty eating (43.3%) and difficulty speaking (34.1%) were the most common impacts. These were also the most severely affected performances, while going out and enjoying the contact of other people were the least severe. A higher number of natural teeth, natural occluding pairs, and anterior and posterior occluding pairs were all significantly associated with a lower prevalence of oral impacts and correlated with the OIDP score, even after adjusting for sociodemographic factors. The prevalence of oral impacts was high and there was a strong and consistent association between OHRQoL and clinical dental indicators of function.

AB  - It was the purpose of this study to determine and analyse strains in the bone of posterior mandible below the removable and fixed partial dentures using Digital Image Correlation Method. Dried mandible with shortened dental arch (Kennedy Class 1) was used in the experiment. The mandible model was prepared for accepting two types of restorations for bilaterally missing molars conventional therapy, and removable and fixed partial dentures were manufactured following standard prosthetic protocol. The models with prosthetic restorations placed in situ were subjected to loading of 50-300 N, and photographed using two digital cameras as part of the digital image correlation method equipment. Afterwards, the obtained data for strains within restored mandible during loading ciclus were analysed in the software Aramis and graphically presented. Percentage size of the mandible strains within the line section are from 0.14 to 0.19% for the removable partial denture experimental model and 0-0.14% for the fixed partial denture experimental model. The study has demonstrated that Digital Image Correlation method can be used to measure strain on the mandible surface and replacements during loading and that from biomechanical perspective both therapeutic modalities can be equally useful for the restoration of the mandible with bilaterally distal edentulous spaces.

AB  - Background/Aim. The Oral Impacts on Daily Performance (OIDP) is a well-known psychometric test used internationally to assess the oral health-related quality of life. The interview and self-administrated questionnaire both assess the degree to which oral health problems have affected the life of the participants over the previous 6 months. The aim of this study was to translate the OIDP index into Serbian and to assess its reliability in practice as its initial verification in the Serbian speaking area. Methods. Following an internationally established methods, the OIDP scale was translated using standardized methodology that consisted of forward translation, pilot study and backward translation. Results. A pilot study was carried out with 44 respondents (24 males i 20 females) using a preliminar Serbian version of the OIDP index. All patients were aged over 65 years. A total of 68.2% of the participants replied that they had at least one OIDP impact on daily life in the past 6 months. These troubles were most prominent during eating (47.7%) and speaking (36.4%), but there is a little impact of troubles in the domain of psychosocial sphere. The corrected item-total correlation coefficients for all items were above the minimum recommended level of 0.20 for including an item in a scale. The standardized Cronbach's alpha coefficient was 0.75. Conclusion. Based on these results, we can conclude that this index is suitable for use in everyday practice in Serbian speaking area providing useful information required to assess oral health-related quality of life.
AB  - Uvod/Cilj. Upitnik Oral Impacts on Daily Performance (OIDP) dobro je poznat psihometrijski test za procenu kvaliteta života u zavisnosti od oralnog zdravlja koji se koristi širom sveta. Primenom ovog upitnika procenjuje se stepen u kojem oralnozdravstveni problemi utiču na svakodnevne aktivnosti ispitanika u poslednjih šest meseci. Cilj ove studije bio je da se OIDP indeks prevede na srpski jezik, proveri njegova pouzdanost u praksi i time verifikuje na našem području. Metode. Prateći internacionalno ustanovljeni metod, OIDP upitnik preveden je korišćenjem standardizovane metodologije koja se sastojala od prevoda upitnika sa engleskog na srpski jezik, povratnog prevoda i pilot studije. Rezultati. U studiju su bila uključena 44 ispitanika (24 muškog i 20 ženskog pola), starosti preko 65 godina, pri čemu je korišćena preliminarna srpska verzija OIDP upitnika. Tegobe u domenu oralnog funkcionisanja najviše su bile izražene tokom jela (47,7%) i govora (36,4%), dok je postojao mali uticaj teškoća u domenu psihosocijalne sfere. Prilikom utvrđivanja interne konzistencije testa uočili smo da je korigovani koeficijent korelacije za sva pitanja bio unutar zadovoljavajućih kriterijuma (> 0,20). Cronbach's alpha koeficijent, koji se izračunavao za pouzdanost testa bio je 0,75. Zaključak. Na osnovu rezultata ovog istraživanja zaključeno je da je ovaj upitnik pogodan za korišćenje u svakodnevnoj praksi na srpskom govornom području i da može pružiti korisne informacije pri proceni kvaliteta života.

AB  - Correctly indicated and accurately made, partial denture with double crowns is a dental restoration of the highest biological, prophylactic and aesthetic values. However, insufficient knowledge of double crowns systems, as well as errors and omissions in the clinical and laboratory work invalidate this type of restoration and can have negative effects on the supporting tissues. Double crowns may come in form of telescope crowns and conical crowns. Double crowns as connecting elements should provide: retention, stabilization, physiological transmission of occlusal loads through the abutment teeth as well as the prophylactic value, comfort for the patient, durability and aesthetics. In order to achieve these qualities, it is necessary, among other things: to make proper indication, to execute adequate tooth preparation (grinding), to make the choice of materials for the construction, to establish proper connection of the outer crown and the metal framework and to choose a suitable aesthetic material for veneering. It is necessary to understand the mechanism of double crowns retention so that the optimal value of the retention force which should be 5-9 N per each patrix can be established. Insufficient retention force will reduce the comfort for the patient because would make the separation too easy, whereas an excessive force may negatively affect the periodontium of the abutment teeth. Retention force and durability of restorations with telescope and conical crowns depend also on the characteristics of the material they are made of. The most frequently used are gold and palladium-silver alloys. In recent years, titanium and the combination of ceramic and galvanized gold are used. Prophylactic effect of double crowns is also provided by a proper planning that will enable the physiological transfer of the forces on the abutment teeth and the health of free gingiva. A special challenge is to plan the connection of outer crown with the metal framework. The best results showed optimized approximal extensions. Double crown production demands experience of a clinician and dental technician in the performance of clinical and laboratory phases. A removal of sufficient tooth tissue during grinding in order to avoid getting oversized artificial teeth and a formation of the appropriate demarcation line which will provide sufficient stability of the system in the gingival area represent a particular challenge. Errors in laboratory work can be avoided with a good knowledge of the milling proceedings, a selection of a suitable material for the external crowns modeling and acquisition of precision castings.

AB  - Introduction: Attachments are precise interlocking devices. They consist of one component which is fixed to the abutment, and the other is integrated into a removable part of the complex denture. The main roles of attachment are in retention and stabilization of denture and to guide the denture into its proper position. Adequate retention of attachments is one of the main demonstrations of successfully made prosthetic device. The aim of this paper is to explain the mechanisms which reduce the retention force of removable partial dentures with attachments. Material and methods: The search strategy applied was a combination of Medical Subject Headings (MeSH) terms and free text words, including the following keywords 'wear', 'retention' and 'attachment'. The selected studies were analyzed with regard to wear parameters, type of attachment and used material. A total of fourteen studies were included. The selected articles were published in the past ten years. Results: These articles have provided evidence that the majority of attachment systems demonstrate a reduction in their retentive force. Wear has been unquestionably implicated as the etiologic factor for the loss of retention. Findings from the literature have also implicated several factors that influence the retentive force of the attachment system and its wear features and accelerate the retention loss. Conclusion: Proper indication, suitable materials, accuracy in casting and other laboratory procedures and motivation of patients make attachment retention reduction as little as possible.
AB  - Atečmeni su precizni vezni elementi koji povezuju fiksni i mobilni deo parcijalne skeletirane proteze u jedinstvenu celinu i imaju ulogu u retenciji, stabilizaciji i vođenju proteze u osnovni položaj. Adekvatna retencija atečmena u izradi parcijalne skeletirane proteze jedna je od mera uspešnosti protetskog rada. Cilj rada je bio da objasni koji mehanizmi smanjuju retenciju parcijalne proteze s atečmenima. Dosadašnja istraživanja pokazuju da su gubitku retencije podložni svi najčešće korišćeni sistemi atečmena. Kako bi taj gubitak bio što manji, neophodna je pravilna indikacija, odabir najpogodnijeg materijala za njihovu izradu, preciznost u izradi laboratorijskih faza i motivacija pacijenta da pravilno rukuje protezom.

Comparative analysis of unilateral removable partial denture and classical removable partial denture by using finite element method

AB  - Introduction. Various mobile devices are used in the therapy of unilateral free-end saddle. Unilateral dentures with precise connectivity elements are not used frequently. In this paper the problem of applying and functionality of unilateral freeend saddle denture without major connector was taken into consideration. Objective. The aim was to analyze and compare a unilateral RPD (removable partial denture) and a classical RPD by calculating and analyzing stresses under different loads. Methods. 3D models of unilateral removable partial denture and classical removable partial denture with casted clasps were made by using computer program CATIA V5 (abutment teeth, canine and first premolar, with crowns and abutment tissues were also made). The models were built in full-scale. Stress analyses for both models were performed by applying a force of 300 N on the second premolar, a force of 500 N on the first molar and a force of 700 N on the second molar. Results. The Fault Model Extractor (FME) analysis and calculation showed the complete behavior of unilateral removable partial denture and abutments (canine and first premolar), as well as the behavior of RPD under identical loading conditions. Applied forces with extreme values caused high stress levels on both models and their abutments within physiological limits. Conclusion. Having analyzed stresses under same conditions, we concluded that the unilateral RPD and classical RPD have similar physiological values.
AB  - Uvod. U lečenju jednostrane krezubosti koriste se različiti oblici mobilnih nadoknada. Jednostrane kompleksne proteze sa preciznim veznim elementima nisu često u upotrebi. Radi rasvetljenja problema jednostrano slobodnog sedla i primene proteze koja ne poseduje veliku spojnicu, u ovom radu je prikazano istraživanje funkcionalnosti ove manje poznate nadoknade. Cilj rada. Cilj rada je bio da se ispitaju i uporede dobijeni naponi jednostrane kompleksne i konvencionalne parcijalne skeletirane proteze pod opterećenjem. Metode rada. U kompjuterskom programu CATIA V5 napravljeni su trodimenzionalni modeli jednostrane kompleksne proteze s odgovarajućim potpornim strukturama (retencioni zubi - očnjak i prvi premolar s namenskim krunicama, alveole i parodontalni prostor) i klasične skeletirane proteze s livenim kukicama. Radi validnosti dobijenih rezultata, modeliranje je obavljeno u prirodnoj veličini. Primenom metode konačnih elemenata izvršena je komparativna analiza vrednosti dobijenih napona pri opterećenju silama od 300 N u predelu drugog premolara, 500 N u predelu prvog molara i 700 N u predelu drugog molara. Rezultati. Proračun je dao sliku ponašanja celog modela jednostrane kompleksne parcijalne proteze s retencionim zubima i sliku celog modela klasične skeletirane parcijalne proteze koje su predstavljene u obliku napona pri različitim uslovima opterećenja. Uneta opterećenja izazvala su visok nivo napona na modelu i zubima nosačima, ali u granicama fiziološke podnošljivosti. Zaključak. Naponi na potpornim strukturama jednostrane kompleksne proteze koji su nastali usled dejstva primenjenih sila bili su u podnošljivim fiziološkim granicama. U uslovima istog opterećenja primenom metode konačnih elemenata dobijaju se veoma slične vrednosti napona jednostrane kompleksne proteze i konvencionalne skeletirane proteze.

Adaptation of Oral Health Impact Profile (OHIP-14) index for measuring impact of oral health on quality of life in elderly to Serbian language

AB  - Background/Aim. There is an evidence of increasing necessity to use indices for measuring impact of oral health on the elderly life quality for the purposes of the state-of-the art clinical dental practice. It is at least because data obtained in that way suggest whether a treatment is required, help in deciding upon a kind of dental treatment, as well as in evaluating the extent of success of the applied therapeutical procedures. The aim of the study was to translate into Serbian the Oral Health Impact Profile-14 (OHIP-14) index and to assess its validity in practice as its initial verification in the Serbian speaking areas. Methods. The OHIP-14 index was translated into Serbian and check-up of its easy-to-understand done in a group of 30 common elderly (above 65 years of age) with a total or partial prosthetic restoration. The study individuals did not fill in a questionnaire by themselves, instead the questions were put orally. If needed, additional explanations were given with no any suggesting a possible answer. Results. Considering the given answers and the shown need for additional explanations to some questions indicated that the question number 5 in the OHIP-14, namely 'Have you been selfconscious because of your mouth or dentures?' (Serbian, 'Da li ste bili samosvesni usled problema sa Vašim ustima i protezama?') cannot be adequately translated into Serbian because even 28 (93.3%) of the tested individuals were not able to understand it inspite the given explanations. Other questions were well understood, thus allowing to use the answers to them as score for a certain discomfort. Of all the tested, 13 (43%) denied any psychosocial disorders or experienced them only rarely, while 60% of the tested answered affirmately to the question 'Do you consider your life generally less satisfactory due to the problems induced by teeth and dentures?' pointing out a significant impact of oral health condition on the quality of life. Conclusion. The OHIP-14 index renders valuable and dependable data on the quality of life of elderly with dentures, thus it is favorable to use it in practice. Due to difficulties in understanding the question number 5, however, it was omitted off the final Serbian language version. Anyhow, its validity in a more numerous test group should be evaluated, as well as other indices introduced into Serbian speaking area to let a comparison made and the most suitable questionnaire used.
AB  - Uvod/Cilj. Korišćenje indeksa koji mere uticaj stanja oralnog zdravlja na kvalitet života predstavlja sve više neophodnost u stomatološkoj praksi jer se na osnovu informacija dobijenih iz njih utvrđuju potrebe za tretmanom, donosi odluku o vrsti stomatološkog tretmana i vrši procena uspešnosti primenjenih terapijskih procedura. Cilj rada bio je da se indeks Oral Health Impact Profile-14 (OHIP-14) prevede na srpski jezik uz proveru pouzdanosti u praksi kao početak njegove verifikacije na srpskom govornom području. Metode. Indeks OHIP-14 preveden je na srpski jezik, a razumljivost učinjenog prevoda za prosečnog ispitanika starijeg životnog doba testirana je na grupi od 30 nosioca totalnih ili parcijalnih proteza starijih od 65 godina. Ispitanici nisu samostalno popunjavali upitnik, već su im pitanja postavljana usmeno. Po potrebi, bila su data dodatna objašnjenja, pri čemu se izbegavalo nametanje odgovora ispitanika. Rezultati. Analizirajući dobijene odgovore i potrebu za dodatnim objašnjenjima pojedinih pitanja, ustanovljeno je da pitanje pod rednim broje 5 iz indeksa OHIP-14 'Have you been self-conscious because of your mouth or detures?!' (srp. prev. 'Da li ste bili samosvesni usled problema sa Vašim ustima ili protezama?') nije moguće adekvatno prevesti na srpski jezik jer je, čak, kod 28 (93,3%) ispitanika ovo pitanje bilo nerazumljivo, uprkos dodatnim objašnjenjima. Ostala pitanja ispitanici su dobro razumeli i na osnovu njihovih odgovora bilo je moguće odrediti broj poena za određenu tegobu. Tako npr. njih 13 (43%) negiralo je tegobe psihosocijalne prirode ili se izjasnilo da im se takve tegobe retko javljaju, dok je 60% potvrdno odgovorilo na pitanje 'Da li ste osetili da Vam je život generalno manje zadovoljavajući zbog problema sa zubima i protezama?', što ukazuje na značajan uticaj stanja oralnog zdravlja na kvalitet života. Zaključak. Indeks OHIP-14 pruža korisne i pouzdane informacije o kvalitetu života osoba koje nose proteze i poželjno ga je koristiti u praksi. Pitanje pod rednim brojem 5, zbog nerazumljivosti, izostavljeno je iz konačne verzije upitnika prevedenog na srpski jezik. Međutim, potrebno je izvršiti proveru njegove validnosti na većem broju ispitanika, ali i uvesti još neki indeks na srpsko govorno područje kako bi se moglo izvršiti poređenje i dati preporuke za najpogodniji takav indeks na srpskom jeziku za osobe starijeg životnog doba.

There is an evidence of increasing necessity to use indices for measuring impact of oral health on the elderly life quality for the purposes of the state-of-the art clinical dental practice. It is at least because data obtained in that way suggest whether a treatment is required, help in deciding upon a kind of dental treatment, as well as in evaluating the extent of success of the applied therapeutical procedures. The aim of the study was to translate into Serbian the Oral Health Impact Profile-14 (OHIP-14) index and to assess its validity in practice as its initial verification in the Serbian speaking areas. Methods. The OHIP-14 index was translated into Serbian and check-up of its easy-to-understand done in a group of 30 common elderly (above 65 years of age) with a total or partial prosthetic restoration. The study individuals did not fill in a questionnaire by themselves, instead the questions were put orally. If needed, additional explanations were given with no any suggesting a possible answer. Results. Considering the given answers and the shown need for additional explanations to some questions indicated that the question number 5 in the OHIP-14, namely 'Have you been selfconscious because of your mouth or dentures?' (Serbian, 'Da li ste bili samosvesni usled problema sa Vašim ustima i protezama?') cannot be adequately translated into Serbian because even 28 (93.3%) of the tested individuals were not able to understand it inspite the given explanations. Other questions were well understood, thus allowing to use the answers to them as score for a certain discomfort. Of all the tested, 13 (43%) denied any psychosocial disorders or experienced them only rarely, while 60% of the tested answered affirmately to the question 'Do you consider your life generally less satisfactory due to the problems induced by teeth and dentures?' pointing out a significant impact of oral health condition on the quality of life. Conclusion. The OHIP-14 index renders valuable and dependable data on the quality of life of elderly with dentures, thus it is favorable to use it in practice. Due to difficulties in understanding the question number 5, however, it was omitted off the final Serbian language version. Anyhow, its validity in a more numerous test group should be evaluated, as well as other indices introduced into Serbian speaking area to let a comparison made and the most suitable questionnaire used., Uvod/Cilj. Korišćenje indeksa koji mere uticaj stanja oralnog zdravlja na kvalitet života predstavlja sve više neophodnost u stomatološkoj praksi jer se na osnovu informacija dobijenih iz njih utvrđuju potrebe za tretmanom, donosi odluku o vrsti stomatološkog tretmana i vrši procena uspešnosti primenjenih terapijskih procedura. Cilj rada bio je da se indeks Oral Health Impact Profile-14 (OHIP-14) prevede na srpski jezik uz proveru pouzdanosti u praksi kao početak njegove verifikacije na srpskom govornom području. Metode. Indeks OHIP-14 preveden je na srpski jezik, a razumljivost učinjenog prevoda za prosečnog ispitanika starijeg životnog doba testirana je na grupi od 30 nosioca totalnih ili parcijalnih proteza starijih od 65 godina. Ispitanici nisu samostalno popunjavali upitnik, već su im pitanja postavljana usmeno. Po potrebi, bila su data dodatna objašnjenja, pri čemu se izbegavalo nametanje odgovora ispitanika. Rezultati. Analizirajući dobijene odgovore i potrebu za dodatnim objašnjenjima pojedinih pitanja, ustanovljeno je da pitanje pod rednim broje 5 iz indeksa OHIP-14 'Have you been self-conscious because of your mouth or detures?!' (srp. prev. 'Da li ste bili samosvesni usled problema sa Vašim ustima ili protezama?') nije moguće adekvatno prevesti na srpski jezik jer je, čak, kod 28 (93,3%) ispitanika ovo pitanje bilo nerazumljivo, uprkos dodatnim objašnjenjima. Ostala pitanja ispitanici su dobro razumeli i na osnovu njihovih odgovora bilo je moguće odrediti broj poena za određenu tegobu. Tako npr. njih 13 (43%) negiralo je tegobe psihosocijalne prirode ili se izjasnilo da im se takve tegobe retko javljaju, dok je 60% potvrdno odgovorilo na pitanje 'Da li ste osetili da Vam je život generalno manje zadovoljavajući zbog problema sa zubima i protezama?', što ukazuje na značajan uticaj stanja oralnog zdravlja na kvalitet života. Zaključak. Indeks OHIP-14 pruža korisne i pouzdane informacije o kvalitetu života osoba koje nose proteze i poželjno ga je koristiti u praksi. Soft tissue and dental status in female residents in Centre for mentally handicapped women

AB  - In mentally handicapped individuals, attention paid on their oral health is usually insufficient. This population is edentulous or partially edentulous, with high rate of dental caries and periodontal disease, and without adequate prosthetic constructions. Oral examination was done in 70 female residents of 'Centre for mentally handicapped women' in Višegrad (Bosnia and Herzegovina). Before the clinical examination, mini mental test (Mini Mental State Examination) was used for the quick orientation about degree of mental disorder. Finally, the plan of dental rehabilitation was made according to patient's health condition and mental diagnosis. All examined patients had severely damaged oral functions, including periodontal disease, decayed roots and a great number of missing teeth. Not a single patient had prosthetic constructions. Patients with lower degree of mental disorder showed satisfactory cooperation and it was estimated that dental treatment could be under- taken in these individuals. Mentally handicapped patients show high need for dental treatment. Prosthetic rehabilitation is possible in some of mentally handicapped patients.
AB  - Kod osoba koje su mentalno hendikepirane najčešće se ne posvećuje dovoljno pažnje oralnom zdravlju. Takva populacija stanovništva je u većini slučajeva bezuba, krezuba, bez adekvatnih zubnih nadoknada ili sa velikim brojem karijesnih zuba. Pregledano je sedamdeset pacijentkinja smeštenih u domu za mentalno hendikepirane osobe u Višegradu (Bosna i Hercegovina). Urađen je stomatološki pregled i mini mental test koji služi za brzu orijentaciju o stepenu mentalnog oštećenja. Takođe je urađen plan rehabilitacije pacijenata u skladu sa njihovim mentalnim zdravljem i osnovnom dijagnozom. Svi pregledani pacijenti su imali ozbiljno narušeno oralno funkcionisanje sa postojećom parodontopatijom, zaostalim gengrenoznim korenovima i velikim brojem nedostajućih zuba. Pacijenti nisu imali nikakve zubne nadoknade. Pacijenti sa manjim stepenom mentalnog oštećenja pokazali su zadovoljavajući stepen saradnje i procenjeno je da je kod njih moguće izvršiti stomatološku sanaciju. Mentalno hendikepirani pacijenti imaju velike potrebe za stomatološkom sanacijom a kod određenog broja ovih pacijenata moguća je i protetska rehabilitacija.

Oral health related quality of life of the frail and dependent elderly

AB  - Necessary approach to a patient in the 21st century understands not only analyzing physical condition, but also social and psychological aspect of an individual. In frail and dependent elderly who are residents of the long term institutions and hospitals, every aspect of health is jeopardized. Missing natural teeth and functional prosthetic constructions makes their medical condition worse, because of impossibility of adequate nutrition intake and communication. Aim of this work is to determine in what degree oral condition influence quality of life of frail and dependent elderly. Fifty patients (32 women and 18 men) were questioned in Institute for Geriatric Medicine, Hospital center 'Zvezdara'. Oral examination was done, including dental status, condition of the mucosa and periodontal tissues, and the presence of prosthetic constructions and their functionality. Quality of life was examined with questionnary OHIP (Oral Health Impact Profile) which gives information about possibility of oral functioning (speech, mastication, swallowing, presence of pain etc.) in the present condition (with or without natural teeth or prosthetic constructions). It was determined that 36 patients had score which shows high negative influence of oral condition on the quality of life. In that group of patients, dominant were women. Prosthetic care and rehabilitation is necessary in population of the frail and dependent elderly, in order to, at least maintain quality of life as it is, and to avoid more complication of the general health condition.
AB  - Neophodan pristup zdravlju u 21. veku podrazumeva obavezan osvrt ne samo u fizičko stanje nego i u socijalni i psihički aspekt svake individue. Kod pacijenata smeštenih u bolnice i staračke domove svi aspekti zdravlja su ugroženi. Nedostatak prirodnih zuba i adekvatnih zubnih nadoknada dodatno narušavaju fizičko i mentalno zdravlje ove populacije zbog nemogućnosti adekvatne ishrane ili komunikacije sa okolinom. Cilj rada je utvrđivanje stanja oralnog zdravlja kao i njegovog uticaja na kvalitet života pacijenata zavisnih od tuđe nege. U okviru Instituta za gerijatriju Kliničko Bolničkog Centra Zvezdara ispitano je 50 pacijenata (32 žene i 18 muškaraca). Stomatološkim pregledom utvrđeni su dentalni status, stanje potpornih tkiva prisutnih zuba, prisustvo zubnih nadoknada i njihova funkcionalnost. Uticaj koji stanje oralnog zdravlja ima na kvalitet života ispitan je pomoću upitnika OHIP (Oral Health Impact Profile) koji ispituje mogućnost funkcionisanja (govor, žvakanje, gutanje, prisustvo boli itd.) sa postojećim stanjem zuba i eventualno prisutnih zubnih nadokada. Utvrđeno je da 36 pacijenata ima skor koji pokazuje visok uticaj narušenog oralnog zdravlja na kvalitet života. Sa negativnim uticajem dominiraju osobe ženskog pola. Stomatološko zbrinjavanje i protetska rehabilitacija su neophodni kod pacijenata i oni se odvijaju u otežanim uslovima kako se već narušeno zdravstveno stanje i kvalitet života ne bi dodatno komplikovali.

AB  - The telescopic denture is one possible solution for prosthetic rehabilitation for a partially dentate arch with few remaining teeth, which is the reason why it is usually considered in elderly patients. The system of double crowns consists of the male component of attachment or patrix, which is cemented to the abutment tooth, and the female component of attachment or matrix, which is the removable part of the restoration. Retention force represents resistance to those forces which tend to separate the partial denture and supporting tissue in the occlusal direction. Its optimal value is 5-9 N per each patrix and it should strictly be between these limits as a stronger or weaker force will endanger the functionality of the denture or damage the supporting tissue of the abutment teeth. Objective: The goal of this study was to establish the exact value of the individual and total retention force of the completed telescopic system. Method: The Bredent dynamometer was used for measuring these values. The study involved measuring the individual force of separation of 50 patrix-matrix components, and total force of 20 partial dentures of various designs retained by double crowns made in the Dental Laboratory of the Clinic for Prosthetic Dentistry, Faculty of Stomatology, Belgrade University. The research was carried out in the period from January 2006 to October 2006. Results and conclusion: The results showed different values of separation force - from very low values to those exceeding optimal limits. A reduction in the force of separation is necessary in the case of excessive tightness. If there was looseness between patrix and matrix, the cylindrical components of double crowns need to be lined by a suitable composite material.

abstract = "The telescopic denture is one possible solution for prosthetic rehabilitation for a partially dentate arch with few remaining teeth, which is the reason why it is usually considered in elderly patients. The system of double crowns consists of the male component of attachment or patrix, which is cemented to the abutment tooth, and the female component of attachment or matrix, which is the removable part of the restoration. Retention force represents resistance to those forces which tend to separate the partial denture and supporting tissue in the occlusal direction. Its optimal value is 5-9 N per each patrix and it should strictly be between these limits as a stronger or weaker force will endanger the functionality of the denture or damage the supporting tissue of the abutment teeth. Objective: The goal of this study was to establish the exact value of the individual and total retention force of the completed telescopic system. Method: The Bredent dynamometer was used for measuring these values. The study involved measuring the individual force of separation of 50 patrix-matrix components, and total force of 20 partial dentures of various designs retained by double crowns made in the Dental Laboratory of the Clinic for Prosthetic Dentistry, Faculty of Stomatology, Belgrade University. The research was carried out in the period from January 2006 to October 2006. Results and conclusion: The results showed different values of separation force - from very low values to those exceeding optimal limits. A reduction in the force of separation is necessary in the case of excessive tightness. If there was looseness between patrix and matrix, the cylindrical components of double crowns need to be lined by a suitable composite material.",

The possibilities of prosthetic rehabilitation of shortened dental arches in elderly patients

AB  - Introduction. Functional consequences of posterior tooth loss are multiple; late and inadequate rehabilitation lead to further collapse of stomatognathic system, which is especially distinct in elderly population. Aim of this paper is to present advantages and disadvantages of different modalities of prosthetic treatment of shortened dental arch in elderly patients. Treatment with overdentures are convenient in this population because it is non-invasive and reversible. Periodontal problems and root caries are main complications of this treatment, but denture relaying after extraction and easy conversion into complete dentures facilitate resolving this complications. Removable partial dentures are one of the mostly indicated treatments for shortened dental arch. They improve masticatory efficiency, but disadvantages are in inferior aesthetic characteristics and comfort. Mechanical failures can be avoid by detailed analyze and correct planning. Treatment of shortened dental arch with fixed partial denture with distal extension can be very convenient solution for elderly patient. Osseointegration process and implant treatment promises every day more in prosthetic rehabilitation of elderly. Relayed prosthetic rehabilitation is recommended, not because of longer process of ossteointegration, but because of delayed recovery of soft tissues and general health condition of elderly. Conclusion. Each of before mentioned therapy modalities have its advantages and disadvantages in meaning of technical procedure, duration in function, possible complications and patient's comfort and satisfaction. In elderly patients, possible complications are more complex; therefore treatment must be done quickly, efficiently, and having in mind patient's general and oral health condition, level of cooperation and economic status.
AB  - Uvod. Funkcionalne posledice gubitka bočnih zuba su mnogobrojne i njihovo nepravovremeno i neadekvatno zbrinjavanje dovodi do daljeg urušavanja stomatognatnog sistema, koje je naročito izraženo kod pacijenata starije dobi. Cilj ovog rada je bio da se prikažu različite mogućnosti protetske terapije skraćenog zubnog niza i ukaže na njihove prednosti i nedostatke u protetskoj rehabilitaciji pacijenata starije dobi. Terapija supradentalnim protezama je podesan način lečenja kod ove populacije jer je neinvazivan i reverzibilan. Parodontalni problemi i karijes korena zuba su osnovne komplikacije ovakvog vida lečenja. Međutim podlaganje zubne nadoknade nakon ekstrakcije i laka konverzija u totalnu protezu olakšavaju rešavanje ovih komplikacija. Parcijalne proteze predstavljaju jedno od najčešće indikovanih rešenja za skraćen zubni niz. One poboljšavaju mastikatornu efikasnost, a nedostatak im je lošiji estetski učinak i smanjeni komfor. Mehaničke komplikacije u tretmanu parcijalnim protezama se mogu izbeći detaljnom analizom i pravilnim dizajnom proteza. Tretman skraćenog zubnog niza fiksnim nadoknadama sa visećim članom može biti veoma pogodno i komforno rešenje za pacijente starije dobi. Terapija implantatima i proces osteointegracije sve više obećavaju u lečenju krezubih pacijenata starije dobi. Preporučuje se odložena protetska rehabilitacija ne zbog lošije osteointegracije već zbog produženog oporavka mekih tkiva i opšteg zdravstvenog stanja pacijenata starije dobi. Zaključak. Brojni terapijski modalitet imaju svoje prednosti i nedostatke u smislu tehničkog izvođenja postupka, trajnosti nadoknade, mogućih komplikacija i pacijentovog komfora a samim tim i zadovoljstva. Kod pacijenata starije dobi, moguće komplikacije se usložnjavaju, a okluzalnu terapiju treba izvesti brzo, efikasno i u skladu sa opštim i oralnim stanjem zdravlja, stepenom saradnje sa pacijentom, ali i ekonomskim statusom.
