AB  - Objective: We aimed to investigate alteration in cellular signaling mediated by vascular endothelial growth factor (VEGF) and parameters of oxidative stress/nitric oxide generation, superoxide dismutase (SOD) and neuronal nitric oxide synthase (nNOS), underlying altered functional mechanical loading of TMJ (temporomandibular joint) during lateral mandibular deviation. Design: Thirty-eight 5-week-old male Wistar rats were divided into experimental group, which received acrylic resin appliance that shifted mandible to the left during closure, and control group. Computed tomography and histomorphometry were used for condyle analyses, while samples of condyle, synovial membrane and m. masseter were analyzed with enzyme-linked immunosorbent assay and spectrophotometry to determine VEGF and nNOS protein concentrations, and SOD activity. Results: Experimental group of rats developed smaller and asymmetrical mandibles. Less of new bone and cartilage formation and larger bone marrow cavities area were found in the experimental group. Higher VEGF expression in condyle and m. masseter as well as higher nNOS expression in m. masseter and synovial membrane were found in the experimental compared to the control group. Alteration of SOD activity was found in m. masseter and synovial membrane in the experimental group. Conclusions: Lateral mandibular deviation induces mandibular and condylar morphological changes as well as significant cellular signaling alterations in condyle, synovial membrane and masticatory muscle. Cellular VEGF protein overexpression and oxidative stress/nitric oxide disbalance could be the mechanisms underlying unbalanced functional TMJ loading due to mandibular deviation.

Effects of three types of functional appliances in class II malocclusion treatment: Sagittal and vertical changes

AB  - Introduction/Objective Class II malocclusions are sagittal malocclusions characterized by a distal relationship of posterior teeth. Depending on the underlying problem, Class II malocclusions can be skeletal or dentoalveolar. Class II malocclusion treatment modality will depend on the cause, severity, and age. Growth modification is the best treatment option in growing patients with skeletal Class II malocclusions. The aim of this study was to establish and compare sagittal and vertical skeletal and dental changes in patients treated with the 'M block' appliance, the Frankel functional regulator, and the Balters' bionator. Methods The sample consisted of 70 patients diagnosed with skeletal Class II malocclusions (ANB > 4°) and mandibular retrognathism (SNB  lt  80°). The patients were divided into three groups according to the type of appliance. All the patients went through the standard diagnostic procedure (anamnesis, clinical and functional analysis, study model, panoramic radiograph, and cephalometric analysis), and dental and skeletal age was determined. Treatment effects were analyzed on study models and cephalograms at the end of treatment. Results All the appliances led to significant mandibular anterior movement and sagittal growth, which reduced the ANB values. All three groups of patients presented with neutral growth pattern, upper incisor retrusion, and lower incisor protrusion at the end of treatment. Conclusion The results of this study indicate efficacy of all three appliances in skeletal Class II malocclusion treatment.
AB  - Uvod/Cilj rada Malokluzije II klase su sagitalne nepravilnosti zagrižaja koje karakteriše distalni odnos bočnih zuba. U zavisnosti od toga koje strukture su u nepravilnom odnosu, dele se na skeletne i dentoalveolarne. Terapija II klase zavisi od uzroka, izraženosti i uzrasta. Najbolji vid terapije ukoliko pacijenti i dalje rastu je modifikacija rasta. Cilj ove studije bio je da se utvrde i uporede sagitalne i vertikalne promene na skeletnim i dentalnim strukturama u toku lečenja M blok-aparatom, Frenklovim regulatorom funkcije tip I i bionatorom po Baltersu tip I. Metode Sedamdeset ispitanika sa dijagnozom skeletnog distalnog zagrižaja (ANB > 4°) i mandibularnog retrognatizma (SNB  lt  80°), prema vrsti aparata, podeljeni su u tri grupe. Svi su prošli kroz standardnu dijagnostiku (anamneza, klinička i funkcionalna analiza, analiza studijskih modela, ortopantomografskog i profilnog telerendgenskog snimka). Terapijski efekti i promene analizirani su na studijskim modelima i profilnom snimcima po završetku terapije. Rezultati Sva tri aparata dovela su do značajnog mezijalnog usmeravanja i sagitalnog rasta mandibule, što je smanjilo ANB ugao. U sve tri grupe je utvrđen neutralni rast, kao i retruzija gornjih i protruzija donjih sekutića. Zaključak Rezultati studije ukazuju na efikasnost sva tri ispitivana aparata u lečenju skeletnih malokluzija II klase.

Relationship between the vertical craniofacial disproportions and the cervicovertebral morphology in adult subjects

AB  - Introduction Orthodontic diagnosis includes the interpretation of the relations between the craniofacial and cervical system, given the potential impact of the irregularities from one system to another. Objective The aim of this study was to examine morphological characteristics of the cervical spine, depending on the parameters of the vertical craniofacial growth and gender in adult subjects. Methods The sample comprised lateral cephalograms of 120 subjects with different vertical facial growth, aged 17.5-35 years. Measured parameters were the following: anterior and posterior vertebral body height (ABHC2-C5, PBHC2-C5), anterior and posterior intervertebral space (AISC2-C5, PISC2-C5), distance between vertebrae and point sella (SC2, SC3, SC4), pterygomaxillare (PmC2), gonion (GoC2) and basion (BaC4); cervical spine angulation (OPT/CVT ) and inclination (OPT/HOR, CVT/HOR). Results Results showed that subjects with anterior facial growth rotation have greater values for BaC4, OPT/HOR, CVT/HOR, OPT/CVT, anterior and posterior vertebral body heights and intervertebral spaces, and lower values for GoC2 and PmC2. Higher values in males were found for anterior and posterior vertebral body heights, distances SC2, SC3, SC4, and BaC4. In females, the greater values were found for GoC2 and ORT/SVT. Conclusion Subjects with anterior facial growth rotation have greater cervical spine inclination and angulation, higher cervical vertebrae and intervertebral spaces, longer upper cervical spines and shorter distances GoC2 and PmC2. Males show smaller cervical column curvature, but higher cervical vertebrae and greater length of the upper cervical spine.
AB  - Uvod Ortodontska dijagnostika obuhvata tumačenje odnosa između kraniofacijalnog i cervikalnog sistema, pri čemu se ima u vidu potencijalni uticaj nepravilnosti jednog sistema na drugi. Cilj rada Cilj ove studije bio je da se ispitaju morfološke odlike cervikalnog dela kičmenog stuba u zavisnosti od vertikalnog kraniofacijalnog rasta i pola kod ispitanika sa završenim rastom. Metode rada Uzorak se sastojao od profilnih telerend- genskih snimaka 120 ispitanika s različitim vertikalnim kraniofacijalnim rastom, uzrasta od 17,5 do 35 godina. Na snimcima su mereni: prednja i zadnja visina tela pršljena (ABHC2-ABHC5, PBHC2-PBHC5), visina prednjeg i zadnjeg međupršljenskog prostora (AISC2-C5, PISC2-C5); rastojanje pršljenova od tačke sela (SC2, SC3, SC4), pterigomaksilare (PmC2), gonion (GoC2) i bazion (BaC4); angulacija (OPT/CVT) i inklinacija (OPT/HOR, CVT/HOR) cervikalnog dela kičme. Rezultati Rezultati su pokazali da su kod ispitanika s rastom lica prednjom rotacijom veće vrednosti za BaC4, OPT/HOR, CVT/HOR, OPT/CVT, prednje i zadnje visine tela pršljenova i međupršljenskih prostora, dok su vertikalna rastojanja GoC2 i PmC2 pokazala manje vrednosti. U grupi ispitanika muškog pola veće vrednosti pokazale su prednje i zadnje visine tela pršljenova, rastojanja SC2, SC3, SC4 i BaC4. Kod osoba ženskog pola veće vrednosti su bile za rastojanje GoC2 i ugao OPT/CVT. Zaključak Karakteristike osoba s rastom lica prednjom rotacijom su veća inklinacija i zakrivljenost cervikalne kičme, veće visine vratnih pršljenova i međupršljenskih prostora, veća dužina gornjeg dela cervikalne kičme i manja rastojanja GoC2 i PmC2. Kod osoba muškog pola uočava se manja zakrivljenost cervikalne kičme, ali veća visina vratnih pršljenova i veća dužina gornjeg dela cervikalne kičme.

AB  - Introduction During growth, proportions of craniofacial and cervical structures are changed. Craniofacial and cervicovertebral structures are morphologically and functionally connected, but their each other's influence is still unknown. Objective The aim of this study was to determine the changes in cervical lordosis and cervicovertebral morphology in different age periods and the possibility of estimating skeletal maturity, based on the percentage of anterior cervical vertebrae body height sum in the total anterior C2-C5 height. Methods The study included lateral radiographs of 120 patients of both sexes, divided into three different age groups: eight, 12-13 and 17-18 years of age. Five craniofacial and 15 cervical parameters were measured and analyzed. Results The results showed significant correlation between cervical lordosis angle and age, gender, anterior and posterior body height of C3, C4, C5, anterior C4-C5 and posterior C2-C3, C3-C4, C4-C5 intervertebral space, anterior body height of C2-C5. Overall values of all cervical body heights were more present in the total height of the spine in females, while all intervertebral spaces were more present in males. The percentage of anterior and posterior C2, C3, C4, C5 body height sum compared to total C2-C5 height increases with age. Conclusion The cervical lordosis becomes more curved and vertebral bodies occupy more space in females, while intervertebral spaces occupy more in males. Skeletal maturity could be estimated following vertebral percentage distribution in the total anterior C2-C5 part.
AB  - Uvod Tokom rasta proporcije kraniofacijalnih i cervikovertebralnih struktura se menjaju. Ove strukture su morfološki i funkcionalno povezane, ali je njihov međusobni uticaj i dalje nepoznat. Cilj rada Cilj ove studije je bio da se uoče promene krivine vratne kičme i morfologije vratnih pršljenova u različitim uzrasnim grupama, kao i mogućnost procene skeletne zrelosti zasnovane na procentualnoj zastupljenosti zbira prednjih visina vratnih pršljenova C2, C3, C4 i C5 u ukupnoj dužini prednje visine kičme od C2 do C5. Metode rada Studija je obuhvatila 120 ispitanika oba pola koji su svrstani u tri starosne grupe: 8, 12-13 i 17-18 godina. Pet kranijalnih i 15 cervikalnih parametara je mereno i analizirano. Rezultati Rezultati su pokazali statistički značajnu korelaciju između zakrivljenosti vratne kičme i godina, pola, prednje i zadnje visine tela pršljena C2, C3, C4, prednjeg C4-C5 i zadnjeg C2-C3, C3-C4, C4-C5 međupršljenskog prostora. Prosečne vrednosti visine tela vratnih pršljenova procentualno su bile češće kod ispitanica, a svi međupršljenski prostori kod osoba muškog pola. Procenat zbira prednje i zadnje visine pršljena C2, C3, C4 i C5 povećavao se sa godinama. Zaključak Krivina vratne kičme postaje zakrivljenija i tela pršljenova zauzimaju više prostora kod žena, a međupršljenski prostor više kod muškaraca. Procena skeletne zrelosti bi mogla da se prati na osnovu procentualne zastupljenosti visine tela pršljena u ukupnoj dužini prednjeg dela kičme (C2-C5).

Changes in soft tissue profile using functional appliances in the treatment of skeletal Class II malocclusion

AB  - Introduction The effects of orthodontic treatment are considered to be successful if the facial harmony is achieved, while the structures of soft tissue profile are in harmony with skeletal structures of neurocranium and viscerocranium. In patients with skeletal distal bite caused by mandibular retrognathism, facial esthetics is disturbed often, in terms of pronounced convexity of the profile and change in the position and relationship of the lips. Objective The aim of this study was to determine the extent of soft tissue profile changes in patients with skeletal Class II malocclusion treated with three different orthodontic appliances: Frankel functional regulator type I (FR-I), Balters' Bionator type I and Hotz appliance. Methods The study included 60 patients diagnosed with skeletal Class II malocclusion caused by mandibular retrognathism, in the period of early mixed dentition. Each subgroup of 20 patients was treated with a variety of orthodontic appliances. On the lateral cephalogram, before and after treatment, the following parameters were analyzed: T angle, H angle, the height of the upper lip, the position of the upper and lower lip in relation to the esthetic line. Within the statistical analysis the mean, maximum, minimum, standard deviation, coefficient of variation, two-factor analysis of variance with repeated measures and the factor analysis of variance were calculated using ANOVA, Bonferroni test and Student's t-test. Results A significant decrease of angles T and H was noticed in the application of FR-I, from 21.60° to 17.15°, and from 16.45° to 13.40° (p lt 0.001). FR-I decreased the height of the upper lip from 26.15 mm to 25.85 mm, while Hotz appliance and Balters' Bionator type I increased the height of the upper lip, thereby deteriorating esthetics of the patient. Conclusion All used orthodontic appliances lead to changes in soft tissue profile in terms of improving facial esthetics, with the most distinctive changes in the application of Frankel's functional regulator type I, which is the most successful appliance for achieving the overall facial harmony of the patient.
AB  - Uvod Da bi se učinak ortodontskog lečenja mogao smatrati uspešnim, neophodno je postići facijalnu harmoniju, pri čemu su strukture mekotkivnog profila u harmoniji sa skeletnim strukturama neurokranijuma i viscerokranijuma. Kod osoba sa skeletno-distalnim zagrižajem izazvanim mandibularnim retrognatizmom često je narušena facijalna estetika u vidu izraženog konveksiteta profila i promene u položaju i međusobnom odnosu usana. Metode rada Istraživanjem je obuhvaćeno 60 pacijenata sa dijagnozom skeletno-distalnog zagrižaja izazvanog mandibularnim retrognatizmom u periodu rane mešovite denticije. Svaka podgrupa od po 20 ispitanika lečena je različitim ortodontskim aparatima. Na profilnom telerendgenskom snimku pre i posle lečenja analizirani su sledeći parametri: ugao T, ugao H, visina gornje usne i položaj gornje i donje usne u odnosu na estetsku liniju. U okviru statističke analize izračunati su srednja, najveća i najmanja vrednost, standardna devijacija i koeficijent varijacije, urađene dvofaktorska analiza varijanse s ponovljenim merenjima i jednofaktorska analiza varijanse uz primenu testa ANOVA, Bonferonijevog (Bonferroni) testa i Studentovog t-testa. Rezultati Došlo je do značajnog smanjenja vrednosti uglova T i H pri primeni FR-I sa 21,60° na 17,15°, odnosno sa 16,45° na 13,40°, sa statističkom značajnošću na nivou p lt 0,001. Primenom FR-I smanjena je visina gornje usne sa 26,15 mm na 25,85 mm, dok se primenom Hocovog aparata i bionatora po Baltersu tip I povećala, čime se pogoršao estetski izgled pacijenta. Zaključak Svi korišćeni ortodontski aparati dovode do promena na mekotkivnom profilu u vidu poboljšanja facijalne estetike, pri čemu su promene najizrazitije pri korišćenju Frenkelovog regulatora Funkcije tip I, koji najuspešnije deluje na postizanje ukupne harmonije lica pacijenta.

Orthodontic treatment of nongrowing patient with class II division 2 malocclusion by Herbst appliance

AB  - Background. Inheritance is most casual etiological factor of Class II division 2 malocclusion. This kind of malocclusion is very difficult for treatment specially in older patients. Case report. In the female patient, 20 years old, at the beginning of the treatment at the School of Dentistry in Belgrade, lateral cephalogram showed skeletal and dentoalveolar Class II division 2 malocclusion. She was in the Herbst treatment for 8 months and 12 months more with a fixed multibracket appliance. The measurements were performed on lateral cephalograms before and after the treatment: ii, is, mi, ms, Pg and ss. The distance from these points to occlusal perpendicular line (Olp) were measured and compared from cephalogram before to cephalogram after the treatment. Temporomandibular joint (TMJ) tomograms were compared from before and after the treatment by superimposition. Correction was found in molar and incisor relation, overjet and overbite. There were found sagital skeletal changes and soft tissue profile improvement. Conclusion. Herbst appliance is effective in the treatment of Class II malocclusions, even in adult patients. Dental and skeletal changes as a result of Herbst treatment could be good choice instead of camouflage orthodontics or surgical decision.
AB  - Uvod. Malokluzije klase II odeljenja 2 uglavnom su nasledne etiologije. Pripadaju komplikovanijim slučajevima za ortodontsku terapiju, naročito kod starijih bolesnika. Prikaz bolesnika. Kod bolesnice stare 20 godina, na početku terapije koja je sprovedena na Stomatološkom fakultetu u Beogradu, na profilnom telerendgen snimku dijagnostikovana je skeletna i dentoalveolarna nepravilnost klase II odeljenja 2. Terapija Herbst-aparatom trajala je 8 meseci i još 12 meseci fiksnim aparatom. Na profilnom telerendgen snimku pre i posle terapije merena su rastojanja između okluzalne perpendikularne linije OLP i: najmezijalnije tačke aproksimalne površine gornjeg prvog molara (ms), najmezijalnije tačke aproksimalne površine donjeg prvog molara (mi), incizalne ivice gornjeg sekutića (is), incizalne ivice donjeg sekutića (ii), najudaljenije tačke prednje strane maksile (ss) i najisturenije tačke profila brade (Pg). Upoređene su vrednosti sa snimaka pre i posle terapije. Profilni snimci temporomandibularnog zgloba (TMZ) pre i posle terapije superponirani su i potom analizirani. Primenom Herbst aparata korigovan je odnos molara i odnos inciziva, kao i incizalni stepenik i incizalni preklop. Uočene su sagitalne skeletne promene i poboljšanje izgleda mekotkivnog profila bolesnika. Zaključak. Herbst aparat je efikasan u terapiji malokluzije klase II, čak i kod starijih bolesnika. abstract = "Background. Inheritance is most casual etiological factor of Class II division 2 malocclusion. This kind of malocclusion is very difficult for treatment specially in older patients. Case report. In the female patient, 20 years old, at the beginning of the treatment at the School of Dentistry in Belgrade, lateral cephalogram showed skeletal and dentoalveolar Class II division 2 malocclusion. She was in the Herbst treatment for 8 months and 12 months more with a fixed multibracket appliance. The measurements were performed on lateral cephalograms before and after the treatment: ii, is, mi, ms, Pg and ss. The distance from these points to occlusal perpendicular line (Olp) were measured and compared from cephalogram before to cephalogram after the treatment. Temporomandibular joint (TMJ) tomograms were compared from before and after the treatment by superimposition. Correction was found in molar and incisor relation, overjet and overbite. There were found sagital skeletal changes and soft tissue profile improvement. Conclusion. Herbst appliance is effective in the treatment of Class II malocclusions, even in adult patients. Dental and skeletal changes as a result of Herbst treatment could be good choice instead of camouflage orthodontics or surgical decision., Uvod. Malokluzije klase II odeljenja 2 uglavnom su nasledne etiologije. Pripadaju komplikovanijim slučajevima za ortodontsku terapiju, naročito kod starijih bolesnika. Prikaz bolesnika. Kod bolesnice stare 20 godina, na početku terapije koja je sprovedena na Stomatološkom fakultetu u Beogradu, na profilnom telerendgen snimku dijagnostikovana je skeletna i dentoalveolarna nepravilnost klase II odeljenja 2. Terapija Herbst-aparatom trajala je 8 meseci i još 12 meseci fiksnim aparatom. Na profilnom telerendgen snimku pre i posle terapije merena su rastojanja između okluzalne perpendikularne linije OLP i: najmezijalnije tačke aproksimalne površine gornjeg prvog molara (ms), najmezijalnije tačke aproksimalne površine donjeg prvog molara (mi), incizalne ivice gornjeg sekutića (is), incizalne ivice donjeg sekutića (ii), najudaljenije tačke prednje strane maksile (ss) i najisturenije tačke profila brade (Pg). Upoređene su vrednosti sa snimaka pre i posle terapije. Profilni snimci temporomandibularnog zgloba (TMZ) pre i posle terapije superponirani su i potom analizirani. Primenom Herbst aparata korigovan je odnos molara i odnos inciziva, kao i incizalni stepenik i incizalni preklop. Uočene su sagitalne skeletne promene i poboljšanje izgleda mekotkivnog profila bolesnika. Zaključak. Herbst aparat je efikasan u terapiji malokluzije klase II, čak i kod starijih bolesnika. Dentalne i skeletne promene kao rezultat terapije Herbst aparatom mogu ukazati na dobar izbor umesto terapije ortodontskom kamuflažom ili ortognatskom hirurgijom.",

Characteristics of soft tissue profile in patients with skeletal class III

Aim: The aim of this study was to determine characteristics of soft tissue profile in patients with skeletal class III and to find possible significant differences between these patients and patients in the control group (without malocclusion in sagittal direction). Methods: Twenty patients of the Department of Orthodontics, Belgrade School of Dentistry, were included in this study. Average age in this group was 8 years and 5 months. In experimental group there were 13 girls and 7 boys. Changes in soft tissue profile by clinical examination, photographic and cephalometric analysis were determinated during the whole diagnostic procedure. In the clinical examination, type of profile, characteristics of lips, facial harmony in vertical direction and the angle of mandibular plane were analyzed. On lateral cephalometrics, the following parameters were measured and analyzed: sagittal parameters SNA, SNB and ANB and parameters in Schwarz, Ricketts and Holdaway analysis of soft tissue profile. Results: Patients in the experimental group had significantly different changes in soft tissue profile. It was the consequence of existing skeletal malocclusion in sagittal direction. Statistically, differences were significant (p lt 0.01) Conclusion: Measurements and analysis showed that patients had typical severe concave profile. Most often, the position of soft tissue structures did not compensate skeletal anomalies.
AB  - Uvod: Kod pacijenata sa dijagnostikovanom III skeletnom klasom uočavaju se značajne promene u izgledu struktura mekotkivnog profila. Cilj: Osnovni cilj ovog proučavanja je bio da se utvrde karakteristike mekotkivnog profila pacijenata sa dijagnostikovanom III skeletnom klasom i da se praćeni postoje li statistički značajne varijacije u izgledu mekotkivnog profila u odnosu na pacijente bez skeletnih odstupanja u sagitalnom pravcu. Subjekti i metode: Ispitivanjem je obuhvaćeno 20 pacijenata Klinike za ortopediju vilica Stomatološkog fakulteta u Beogradu. U ispitivanom uzorku prosečna starost pacijenata bila je 8 godina i 5 meseci. Uzorak je obuhvatio 13 devojčica i 7 dečaka. Promene u strukturama mekotkivnog profila utvrđene su u okviru dijagnostičkog postupka kliničkim pregledom, analizom fotografije i profilnog telerendgen snimka. Kliničkim pregledom određen je tip profila lica, karakteristike usana, facijalna harmonija u vertikalnom pravcu i ugao nagiba mandibulame ravni. Na profilnom telerendgen snimku analizirani su parametri sagitalnog pravca SNA, SNB i ANB, kao i parametri mekotkivnog profila po Schwarzu, Rickettsu i Holdawayu. Rezultati: Postoje značajna odstupanja u vrednostima parametara mekotkivnog profila koja su direktna posledica postojeće sagitalne skeletne neusaglašenosti. Statistička značajnost uočenih razlika prisutna je na nivou p lt 0.01 i p lt 0.05 Zaključak: Izvršene analize ukazuju da pacijenti imaju tipičan konkavan profil, pri čemu, u većini slučajeva, meko-tkivne strukture svojim položajem ne kompenzuju postojeću skeletnu nepravilnost.

Possibilty of the lower third molar eruption: Radiographic analysis

AB  - Backgraund/Aim. To assess the possibility of the eruption of the lower third molar on the basis of the measured parameters: retromolar space, mesiodistal crown width of a molar and the third molar angulation. Methods. The investigation included 104 patients both sexes (43 boys, and 61 girls), 16 to 25 years old (meanage, 18 years). It was performed using the orthopanthomographic radiographs analysis of those patients. Each radiograph was covered by tracing paper, and the contoures of the followiny anatomic details were drawn: a) the crown and root contours of third molars, upper and lower central incisors, distal molars in occlusion, anterior edge of ramus mandible, b) lines: 1. the occlusal plane, 2. the line of retromolar space, 3. the mesiodistal crown width of third molar, 4. the axial shaft of the third molar and the distal angle between occlusal plane and the axial shaft of the third molar. The values were measured with an orthodontic caliper: the diameter of retromolar space, diameter of mesiodistal width, the value of distal angle between occlusal plane and axial shaft of molar. Results. A favourable angulation of the lower third molar (more than 60°) was found in, boys (left 27.90%, right 32.55%), girls (left 39.34%, right 37.77%). A favourable relationship between the diameters of mesiodistal width of the third molar and retromolar space was found in, boys, (left 13.59%, right 16.27%), girls, (left 8.19%, right 14.75%). A favorable relationship between the diameters of mesiodistal width of the third molar and the retromolar space and the angulation was found in boys, (left 9.30%, right 11.62%), girls, (left 6.56%, right 9.83%). Conclusion. There was not any statistically significant difference found between the relation of the retromolar value, third molar mesiodistal diameter, or of the third molar angulation to the left and the right side nor of their mutual relations in comparing boys and girls. A favorable prognosis was found in 9.33% of the patients.

Ovo istraživanje imalo je za cilj procenu mogućnosti nicanja donjeg trećeg molara na osnovu izmerenih parametara: retromolarnog prostora, meziodistalnog promera umnjaka i inklinacije umnjaka. Metode. Istraživanjem je obuhvaćeno 104 ispitanika (43 dečaka i 61 devojčica) oba pola, starosti 16−25 godina. Istraživanje se zasnivalo na ortopantomografskoj analizi. Na svaki ortopantomografski snimak zalepljen je paus papir i ucrtavane su konture anatomskih detalja: a) konture krunice i korena umnjaka, centralnih gornjih i donjih inciziva, poslednjih molara u okluziji, prednje ivice ramusa mandibule, b) linije: okluzalna ravan, linija retromolarnog prostora, meziodistalni promer krunice umnjaka, aksijalna osovina umnjaka i spoljašnji ugao između okluzalne ravni i aksijalne osovine umnjaka. Merene su vrednosti: veličina retromolarnog prostora, meziodistalni promer umnjaka, vrednost spoljašnjeg ugla između aksijalne osovine umnjaka i okluzalne ravni. Rezultati. Istraživanjem je ustanovljena povoljna inklinacija donjih umnjaka (više od 60°) kod dečaka − levo 27,90%, desno 32,55%; kod devojčica − levo 39,34%, desno 37,77%. Povoljan odnos meziodistalnog promera i retromolarnog prostora nađen je kod dečaka − levo 13,59%, desno 16,27%; kod devojčica − levo 8,19%, desno 14,75%. Povoljan odnos meziodistalnog promera, retromolarnog prostora i inklinacije nađen je kod dečaka − levo 9,30%, desno 11,62%; kod devojčica − levo 6,56%, desno 9,83%. Zaključak. Nije utvrđena statistički značajna razlika u odnosu veličine retromolarnog prostora, veličine meziodistalnog promera, stepena inklinacije umnjaka leve i desne strane, kao i njihovih odnosa upoređivanjem dečaka i devojčica. Povoljna prognoza nicanja donjih trećih molara nađena je kod 9,33% ispitanika.

Treatment of 2nd class division 1 malocclusion by using Herbs appliance

AB  - In this case was described the treatment in 14.5 years old female patient with II/1 class of malocclusion. On the base of clinical findings, study cast analysis, analysis of orthopan and profile cephalograms, there were assess II/1 class of malocclusion with deep bite, bimaxilar protrusion of frontal teeth and mild crowding in lower jaw. Patient was inefficacy treated by Andresen’s activator for two years. This occasion and patients age was reason to continue with the therapy by Herbst appliance for 6 months and fixed appliance 7 months more. By Herbst appliance was corrected sagital relation between jaws in very short treatment time and continuing treatment with fixed appliance was corrected intermaxilar teeth relation.
AB  - U ovom radu je prikazana terapija sprovedena kod pacijenta ženskog pola, uzrasta 14.5 godina, sa malokluzijom II klase 1. odelenja. Na osnovu kliničkog nalaza, morfološke analize sprovedene na studijskim modelima, analize ortopantomografskog i profilnog kefalometrijskog snimka, utvrđeno je da se radi o malokluziji II klase 1. odelenja , sa dubokim preklopom, protruzijom frontalnih zuba obe vilice i blagom teskobom u donjem zubnom nizu. Pacijent je bio u terapiji pomoću aktivatora po Andresen-u oko 2 godine ali bez značajnog uspeha. S obzirom na ovu okolnost i uzrast pacijenta, nastavljena je terapija pomoću Herbst aparata 6 meseci, i fiksnog aparata još 7 meseci. Herbst aparatom je u kratkom periodu korigovan sagitalni odnos vilica, nakon čega je fiksnim aparatom samo postignut pravilan međuvilični i unutar niza odnos zuba.

abstract = "In this case was described the treatment in 14.5 years old female patient with II/1 class of malocclusion. On the base of clinical findings, study cast analysis, analysis of orthopan and profile cephalograms, there were assess II/1 class of malocclusion with deep bite, bimaxilar protrusion of frontal teeth and mild crowding in lower jaw. Patient was inefficacy treated by Andresen’s activator for two years. This occasion and patients age was reason to continue with the therapy by Herbst appliance for 6 months and fixed appliance 7 months more. By Herbst appliance was corrected sagital relation between jaws in very short treatment time and continuing treatment with fixed appliance was corrected intermaxilar teeth relation., U ovom radu je prikazana terapija sprovedena kod pacijenta ženskog pola, uzrasta 14.5 godina, sa malokluzijom II klase 1. odelenja. Na osnovu kliničkog nalaza, morfološke analize sprovedene na studijskim modelima, analize ortopantomografskog i profilnog kefalometrijskog snimka, utvrđeno je da se radi o malokluziji II klase 1. odelenja , sa dubokim preklopom, protruzijom frontalnih zuba obe vilice i blagom teskobom u donjem zubnom nizu. Pacijent je bio u terapiji pomoću aktivatora po Andresen-u oko 2 godine ali bez značajnog uspeha. S obzirom na ovu okolnost i uzrast pacijenta, nastavljena je terapija pomoću Herbst aparata 6 meseci, i fiksnog aparata još 7 meseci. AB  - The aim of this study was to investigate whether there was an association between presence of a cleft and reduced size of deciduous tooth on the cleft side. In this investigation, 58 study models of patients with unilateral cleft lip and palate with deciduous dentition were analyzed. The largest mesiodistal dimension of deciduous teeth was measured by a sliding caliper. The size of the same teeth on the side of the cleft and on the non-cleft side was compared. The results of the investigation were statistically analyzed. A difference between the genders was found. In the sample of boys the lateral deciduous incisor and the second deciduous molars on the cleft side were smaller than those on the non-cleft side, while in girls the central and lateral deciduous incisors were smaller on the cleft side than on the non-cleft side. It was concluded that the presence of a unilateral cleft lip and palate was associated with smaller tooth size.
