Applying the model of excellence in dental healthcare

AB  - Introduction Models of excellence are considered a practical tool in the field of management that should help a variety of organizations, including dental, to carry out the measurement of the quality of provided services, and so define their position in relation to excellence. The quality of healthcare implies the degree within which the system of healthcare and health services increases the likelihood of positive treatment outcome. Objective The aim of the present study was to define a model of excellence in the field of dental healthcare (DHC) in the Republic of Serbia and suggest the model of DHC whose services will have the characteristics of outstanding service in the dental practice. Methods In this study a specially designed questionnaire was used for the assessment of the maturity level of applied management regarding quality in healthcare organizations of the Republic of Serbia. The questionnaire consists of 13 units and a total of 240 questions. Results The results of the study were discussed involving four areas: (1) defining the main criteria and sub-criteria, (2) the elements of excellence of DHC in the Republic of Serbia, (3) the quality of DHC in the Republic of Serbia, and (4) defining the framework of the model of excellence for the DHC in the Republic of Serbia. The main criteria which defined the framework and implementation model of excellence in the field of DHC in Serbia were: leadership, management, human resources, policy and strategy, other resources, processes, patients' satisfaction, employee's satisfaction, impact on society and business results. The model had two main parts: the possibilities for the first five criteria and options for the other four criteria. Conclusion Excellence in DHC business as well as the excellence of provided dental services are increasingly becoming the norm and good practice, and progressively less the exception.
AB  - Uvod Modeli izvrsnosti se smatraju praktičnim alatom u oblasti upravljanja koji treba da pomogne različitim organizacijama, pa i stomatološkim, da same izvrše merenje nivoa kvaliteta pruženih usluga i tako definišu gde se nalaze u odnosu na izvrsnost. Kvalitetna zdravstvena zaštita podrazumeva stepen u kojem sistem zdravstvene zaštite i zdravstvena usluga povećavaju verovatnoću pozitivnog ishoda lečenja. Cilj rada Cilj istraživanja bio je da se definiše model izvrsnosti za oblast stomatološke zdravstvene zaštite (SZZ) u Republici Srbiji i predloži model organizacije SZZ čija će usluga imati odlike izvrsne usluge u stomatološkoj praksi. Metode rada Za ovo istraživanje korišćen je posebno sačinjen upitnik za ocenu nivoa zrelosti primenjenog upravljanja kvalitetom u zdravstvenim organizacijama u Republici Srbiji. Upitnik se sastojao od 13 celina i 240 pitanja. Rezultati Rezultati istraživanja su razmatrani u četiri oblasti: 1) definisanje glavnih kriterijuma i podkriterijuma; 2) elementi izvrsnosti za SZZ u Republici Srbiji; 3) kvalitet u SZZ Republike Srbije; i 4) definisanje okvira modela izvrsnosti za SZZ u Republici Srbiji. Glavni kriterijumi koji su definisali okvir i primenu modela izvrsnosti u oblasti SZZ u Srbiji bili su: liderstvo, upravljanje ljudskim resursima, politika i strategija, ostali resursi, procesi, zadovoljstvo pacijenata, zadovoljstvo zaposlenih, uticaj na društvo i poslovni rezultati. Model je imao dve osnovne celine: mogućnosti za prvih pet kriterijuma i mogućnosti za ostala četiri kriterijuma. Zaključak Izvrsnost u poslovanju SZZ, kao i izvrsnost pruženih stomatoloških usluga, sve više postaje pravilo i dobra praksa, a sve manje izuzetak.

The application of air abrasion in dentistry

AB  - One of the main objectives of contemporary dentistry is to preserve healthy tooth structure by applying techniques of noninvasive treatment. Air abrasion is a minimally invasive non mechanical technique of tooth preparation that uses kinetic energy to remove carious tooth structure. A powerful narrow stream of moving aluminum-oxide particles hit the tooth surface and they abrade it without heat, vibration or noise. Variables that affect speed of cutting include air pressure, particle size, powder flow, tip's size, angle and distance from the tooth. It has been proposed that air abrasion can be used to diagnose early occlusal-surface lesions and treat them with minimal tooth preparation using magnifier. Reported advantages of air abrasion include reduced noise, vibration and sensitivity. Air abrasion cavity preparations have more rounded internal contours than those prepared with straight burs. This may increase the longevity of placed restorations because it reduces the incidence of fractures and a consequence of decreased internal stresses. However, air abrasion cannot be used for all patients, i.e. in cases involving severe dust allergy, asthma, chronic obstructive lung disease, recent extraction or other oral surgery, open wounds, advanced periodontal disease, recent placement of orthodontic appliances and oral abrasions, or subgingival caries removal. Many of these conditions increase the risk of air embolism in the oral soft tissues. Dust control is a challenge, and it necessitates the use of rubber dam, high-volume evacuation, protective masks and safety eyewear for both the patient and the therapist.
AB  - Jedan od osnovnih ciljeva savremene stomatologije jeste očuvanje zdravog zubnog tkiva primenom preventivnih metoda i tehnika neinvazivnog lečenja. Vazdušna abrazija je minimalno invazivna tehnika preparacije kaviteta koja koristi kinetičku energiju za uklanjanje karijesa. Usmeravanje pravolinijskog mlaza čestica aluminijum- oksida na površinu zuba koja se obrađuje dovodi do abrazije supstrata bez toplote, vibracija i zvuka. Faktori koji utiču na brzinu preparacije uključuju intenzitet vazdušnog pritiska, veličinu čestica abraziva, brzinu protoka čestica, prečnik vrha kanile, veličinu ugla kanile i udaljenost vrha kanile od površine zuba koja se obrađuje. Njena upotreba se savetuje u dijagnostici rane karijesne lezije gleđi na okluzalnim površinama zuba i minimalnoj preparaciji kaviteta. Pojedini autori preporučuju i upotrebu optičkih pomagala. Prednosti tehnike vazdušne abrazije uključuju smanjenje buke, vibracija i operativne osetljivosti, koja je veoma subjektivna i promenljiva kod pacijenata. Kaviteti preparisani vazdušnom abrazijom imaju zaobljene ivice, za razliku od kaviteta preparisanih standardnim tehnikama. To može povećati dugotrajnost postavljenih ispuna, jer se smanjuje mogućnost nastanka njihovog odlamanja, neutralisanjem sila unutrašnjeg napona u poređenju sa stresom koji se javlja kod angularnih preparacija. Primena tehnike vazdušne abrazije je kontraindikovana kod osoba alergičnih na prašinu, obolelih od astme ili hronične opstruktivne bolesti pluća, pacijenata sa postekstrakcionim ranama, parodontopatijom, ortodontskim aparatima ili dubokim karijesom, jer se ovom tehnikom preparacije ne odstranjuje uspešno mekana dentinska struktura. Mnoga ova stanja povećavaju rizik od nastanka vazdušne embolije u mekim oralnim tkivima. Kontrola rasejanja čestica abraziva predstavlja izazov kliničkom stomatologu, a zahteva i primenu koferdama, jakih aspiratora, zaštitnih maski i zaštitnih naočara kako kod pacijenta, tako i kod terapeuta.

Hard dental tissue minimal-invasive preparation using contemporary polymer rotating instruments and laser

AB  - Goal of contemporary dentistry is to decrease the patient's discomfort during treatment. Dentists aim to achieve maximum with the newly developed dental materials as well as with new cavity preparation techniques in the shortest time span. Since the development of the first constructed borer (drilling machine) for caries removal, the preparation techniques have considerably changed. The progress of dental materials as well as the cavity preparation techniques has led us to contemporary carbide tungsten and diamond borers that are used with obligatory water cooling. The innovation within this field represents newly developed polymer borers that can detect the difference between carious lesions and healthy tooth structure. In this way the cavity preparation may be performed without damaging dental healthy tissue. This is possible owing to their hardness which is lower than the hardness of intact dentin. Polymer borer preparation is painless with less vibration, while the increase in temperature is negligible. Lasers have been used in clinical dentistry since 1980s so it can be said that they represent a new technology. The function of lasers is based on ablation which requires water. Erbium lasers have shown the highest potential with their ability to produce effective ablation of hard dental tissues. Laser application in dentistry requires special training as well as some protective measures. Laser advantages, compared to traditional preparation techniques, involve the absence of vibration, painless preparation, possibility of preparation without anesthetic and easier patient's adjustment to dental intervention which is of importance, especially in pediatric dentistry.
AB  - Savremena stomatologija teži ka što manjoj traumatizaciji pacijenta prilikom rada uz maksimalni učinak. Savremeni materijali uz nove vidove preparacije kaviteta pružaju mogućnost sanacije karijesnih lezija u najkraćem vremenskom roku. Vidovi preparacije kaviteta su se znatno promenili otkako je uvedeno prvo mašinski napravljeno svrdlo za uklanjanje karijesa. Tehnološki napredak materijala za ispune i načina preparacije doveli su do savremenih karbidnih i dijamantskih svrdla koja se koriste uz obavezno vodeno hlađenje. Inovaciju na polju rotirajućih instrumenata predstavljaju polimerna 'pametna' svrdla, koja uklanjaju karijesnu leziju bez mogućnosti povređivanja i uklanjanja zdrave zubne strukture. Ovo je moguće zahvaljujući čvrstoći koju poseduju, koja je manja od čvrstoće zdravog dentina. Zahvaljujući prirodi materijala od kojih su izrađena, ova svrdla omogućuju bezbolniji rad uz manje vibracija i slabije zagrevanje zubne supstance. Laseri su relativno novo tehnološko sredstvo u stomatologiji, a njihova klinička upotreba počela je osamdesetih godina dvadesetog veka. Laseri funkcionišu po principu ablacije, za šta je neophodno prisustvo vode. Najveći potencijal među ispitivanim laserima pokazali su erbijumski laseri, kojima je moguće uraditi efikasnu ablaciju čvrstih zubnih tkiva. Za korišćenje lasera u stomatologiji neophodni su posebna obuka i odgovarajuće mere zaštite. Prednosti lasera u poređenju sa drugim metodama preparacije čvrstih zubnih tkiva su nepostojanje vibracija, bezbolan rad, mogućnost rada bez anestezije i lakše prilagođavanje pacijenta stomatološkim intervencijama, što je veoma značajno u dečjoj stomatologiji.

Inovaciju na polju rotirajućih instrumenata predstavljaju polimerna 'pametna' svrdla, koja uklanjaju karijesnu leziju bez mogućnosti povređivanja i uklanjanja zdrave zubne strukture. Ovo je moguće zahvaljujući čvrstoći koju poseduju, koja je manja od čvrstoće zdravog dentina. Zahvaljujući prirodi materijala od kojih su izrađena, ova svrdla omogućuju bezbolniji rad uz manje vibracija i slabije zagrevanje zubne supstance. Laseri su relativno novo tehnološko sredstvo u stomatologiji, a njihova klinička upotreba počela je osamdesetih godina dvadesetog veka. Laseri funkcionišu po principu ablacije, za šta je neophodno prisustvo vode. Najveći potencijal među ispitivanim laserima pokazali su erbijumski laseri, kojima je moguće uraditi efikasnu ablaciju čvrstih zubnih tkiva. Za korišćenje lasera u stomatologiji neophodni su posebna obuka i odgovarajuće mere zaštite. Prednosti lasera u poređenju sa drugim metodama preparacije čvrstih zubnih tkiva su nepostojanje vibracija, bezbolan rad, mogućnost rada bez anestezije i lakše prilagođavanje pacijenta stomatološkim intervencijama, što je veoma značajno u dečjoj stomatologiji.",

AB  - The research presented in this thesis is excellent service in a dental practice. How to define it , how to model , how to develop a model of the dental care system , whose services will have these features . For these reasons, this study is of particular importance . The main objective of this research is to develop a model for excellent dental organization that will by itself generate excellent dental care. Today, we have developed models of organizations that manage the quality of their business by applying the model of business excellence . Of course , we're talking about organizations that generate products or services of world class quality . Therefore, the secondary objective of this research is based on the experiences of developed countries , to carry out evaluations dostigntog upravljnja level of quality in the SHC in our country , and that is the basis of designing a model of excellent dental organization that will provide and excellent dental care.
AB  - Predmet istraživanja u ovoj tezi je izvrsna usluga u stomatološkoj praksi. Kako je definisati, kako je modelirati, kako razviti model organizacije stomatološke zdravstvene zaštite, čija usluga će imati ove karakteristike. Sve ove aspekte treba razmatrati kao model koga treba primeniti u Srbiji, gde se ovim pitanjima u poslednjih nekoliko godina pridaje posebna pažnja, ali ne za ovu oblast, već za neke druge grane medicine. Iz tih razloga ova istraživanja imaju poseban značaj. Osnovni cilj ovih istraživanja je da se razvije model za izvrsnu stomatološku organizaciju koja će sama od sebe da generiše izvrsnu stomatološku uslugu. Danas imamo razvijene modele organizacija koje upravljaju kvalitetom svog biznisa primenjujući modele poslovne izvrsnosti. Naravno, ovde se radi o organizacijama koje generišu proizvode ili usluge svetske klase kvaliteta. Zato je sekundarni cilj ovih istraživanja da se na bazi iskustva iz razvijenih zemalja, izvrši ocena dostigntog nivoa upravljnja kvalitetom u SZZ u našoj zemlji, pa da se na bazi toga projektuje model izvrsne stomatološke organizacije koja će davati i izvrsne stomatološke usluge.

AB  - Standardi serije ISO 9000, doneti 1987. godine, bili su pokretač razvoja i primene principa upravljanja kvalitetom u različitim oblastima, pa i u zdravstvenoj zaštiti. Danas u svetu postoji više od milion sertifikata za upravljanje kvalitetom, tako da se može reći da su oni bili osnovni pokretač unapređenja kvaliteta. Pored modela za upravljanje kvalitetom proizvoda i usluga, danas se u ovoj oblasti koriste različiti modeli. Cilj ovog rada je bio da se predstave metode i tehnike unapređenja kvaliteta zdravstvenih usluga, s posebnim osvrtom na usluge stomatološke zdravstvene zaštite i aspekt unapređenja kvaliteta u njoj.

Ovi modeli su prvo nastali u industriji a kasnije su se proširili i na usluge. Standardi ISO 9000 serije, doneti 1987. godine, bili su pokretač razvoja i primene menadžmenta kvalitetom ( QM-a) u različitim oblastima, pa i u zdravstvenoj zaštiti. Danas u svetu postoji preko milion sertifikata za QMS, tako da se može reći da su oni bili osnovni generator unapređenja kvaliteta. AB  - Business excellence models have a long history of development of sixty years. Today, the business excellence model can be essentially classified as world's most famous models/awards for excellence (Japanese, American and European), the most popular national models of excellence (the Australian, British, German, French), as well as models of excellence companies (Siemens, Philips, Toyota). In the world today there are about 120 models of excellence. The best way to improve quality of health organizations that have systems of quality management is by application of the concept of total quality (TQM). This approach ensures the improvement of overall performance of health organizations, primarily the internal organization including management, resources, processes and human resources, health services and performance of business results. The market requires high quality products and services to improve the quality of life, or TQM excellence models in all areas of social subjects operations. It is a process that never ends, and knowing the nature of man who was never satisfied with achieved, that is main driving force for social development. We are witnesses today that some countries are introducing awards to organizations which succeeded in implementation of TQM and models of excellence for quality achievement. The development of a model of business excellence (BE) for dental health care is the main subject of the current study. The aim of this study is to present and analyze the existing criteria for product excellence and based on that, to define criteria for models of excellence for dental health care in Serbia.
AB  - Istorija razvoja modela poslovne izvrsnosti duga je šezdeset godina. Oni se u osnovi mogu klasifikovati na: najpoznatije svetske modele - nagrade za izvrsnost (japanski, američki i evropski), najpoznatije nacionalne modele izvrsnosti (australijski, britanski, nemački, francuski) i kompanijske modele izvrsnosti (Siemens, Philips, Toyota). Danas u svetu postoji oko 120 modela izvrsnosti. Najbolji način za unapređenje kvaliteta zdravstvene organizacije jeste primena koncepta 'upravljanja ukupnim kvalitetom'. Ovaj pristup obezbeđuje unapređenje celokupnog poslovanja zdravstvene organizacije, prvenstveno interne organizacije, koja uključuje rukovodstvo, resurse, procese, kadrove, zdravstvene usluge i primenu poslovnih rezultata. Na tržištu se zahteva visok kvalitet proizvoda i usluga radi poboljšanja kvaliteta življenja, odnosno koncepta upravljanja ukupnim kvalitetom preko modela izvrsnosti poslovanja svih subjekata društva. To je proces bez kraja, poznavajući prirodu čoveka, koji nikada nije zadovoljan postignutim, što je osnovna pokretačka snaga razvoja društva. Svedoci smo da neke države uvode priznanja za organizacije koje su u određenom periodu napravile relevantan uspeh u primeni koncepta upravljanja ukupnim kvalitetom, odnosno u ostvarivanju modela izvrsnosti u postizanju kvaliteta. Ovde se istražuje razvoj modela poslovne izvrsnosti za oblast stomatološke zdravstvene zaštite. Cilj rada bio je da se predstave i analiziraju kriterijumi za model izvrsnosti, te na osnovu toga definišu kriterijumi modela izvrsnosti za stomatološku zdravstvenu zaštitu u Republici Srbiji.

Svedoci smo da neke države uvode priznanja za organizacije koje su u određenom periodu napravile relevantan uspeh u primeni koncepta upravljanja ukupnim kvalitetom, odnosno u ostvarivanju modela izvrsnosti u postizanju kvaliteta. Ovde se istražuje razvoj modela poslovne izvrsnosti za oblast stomatološke zdravstvene zaštite. Cilj rada bio je da se predstave i analiziraju kriterijumi za model izvrsnosti, te na osnovu toga definišu kriterijumi modela izvrsnosti za stomatološku zdravstvenu zaštitu u Republici Srbiji.",
