Susceptibility profile of Candida spp. isolated from humans and dogs with stomatitis to the essential oil of Thymus vulgaris

AB  - Candida spp. form a part of human and animal oral cavity flora. However Candida spp. is the main cause of dental related stomatitis in humans and stomatitis in dogs. Stomatitis treatment implies the use of azoles and polyenes to which yeasts build up resistance. The research is directed to the use of natural compounds such as essential oils. The aim of this paper is to define the antifungal activity of thyme oil on 15 clinical strains of Candida spp., isolated from humans and dogs and to determine if there is a difference in susceptibility between human and dog isolates. Sampling in patients with stomatitis was done by swabbing the denture or oral mucosa swab while sampling in dogs was done by swabbing the oral cavity mucosa after stomatitis has been diagnosed. In order to investigate the antifungal activity of thyme oil in vitro, microdilution method was used. Thyme oil expressed antifungal effects on all investigated strains. Also, our data show that the values of minimum fungicide concentration (MFC) and minimum inhibitory concentration (MIC) are lower in human strains. Explanation is that in most cases, stomatitis in humans is asymptomatic and thus not treated, so Candida strains have not developed resistance. On the other hand, stomatitis in dogs is followed by a marked clinical picture and treated is by antimicotics (mostly by azoles), therefore resistant Candida strains are more likely to occur.
AB  - Candida spp. je sastavni deo mikrobioma usne duplje čoveka i psa. Međutim Candida spp. predstavlja glavni uzročnik proteznog stomatitisa kod čoveka i stomatitisa kod psa. Terapija stomatitisa podrazumeva korišćenje azola i poliena na koje poslednjih godina gljivice razvijaju otpornost. Istrživanja se usmeravaju ka primeni prirodnih preparata kao što su etarska ulja. Cilj rada je utvrdi antifungalnu aktivnost ulja timijana na kliničke izolate Candida spp. čoveka i psa, i utvrdi da li postoji razlika u osetljivosti sojeva Candide spp. izolovanih kod čoveka i psa. Uzorkovanje kod pacijenata sa stomatitisom je vršeno brisom proteze ili brisom sluzokože. Kod pasa je uzrokovanje vršeno brisom sluzokože usne duplje kada je dijagnostikovan stomatitis. Zasejani sojevi su in vitro tretirani korišćenjem mikrodilucione metode da bi se utvrdilo da li ulje timijana deluje na kliničke izolate Candida spp. čoveka i psa. Rezultati pokazuju da su vrednosti minimalne fungicidne koncentracije (MFC) i minimalne inhibitorne koncentracije (MIC) manje kod čoveka. Stomatitis je kod ljudi najčešće je asimptomatski, pacijenti se ne podvrgavaju lečenju, pa sojevi Candida spp. nisu razvili rezistenciju na antifungicidne preparate. Kod pasa stomatitis je praćen burnijom kliničkom slikom, leči se antimikoticima (uglavnom azolima) pa je verovatnoća pojave rezistentnih sojeva Candida spp. veća.

Oral squamous cell carcinoma detection by salivary biomarkers in a Serbian population

AB  - Early detection of oral squamous cell cancer (OSCC) is the key to improve the low 5-year survival rate. Using proteomic and genomic technologies we have previously discovered and validated salivary OSCC markers in American patients. The question arises whether these biomarkers are discriminatory in cohorts of different ethnic background. Six transcriptome (DUSP1, IL8, IL1B, OAZ1, SAT1, and S100P) and three proteome (IL1B, IL8, and M2BP) biomarkers were tested on 18 early and 17 late stage OSCC patients and 51 healthy controls with quantitative PCR and ELISA. Four transcriptome (IL8, IL1B, SAT1, and S100P) and all proteome biomarkers were significantly elevated (p  lt  0.05) in OSCC patients. The combination of markers yielded an AUC of 0.86, 0.85 and 0.88 for OSCC total, T1-T2, and T3-T4, respectively. The sensitivity/specificity for OSCC total was 0.89/0.78, for T1-T2 0.67/0.96, and for T3-T4 0.82/0.84. In conclusion, seven of the nine salivary biomarkers (three proteins and four mRNAs) were validated and performed strongest in late stage cancer. Patient-based salivary diagnostics is a highly promising approach for OSCC detection. This study shows that previously discovered and validated salivary OSCC biomarkers are discriminatory and reproducible in a different ethnic cohort. These findings support the feasibility to implement multi-center, multi-ethnicity clinical trials towards the pivotal validation of salivary biomarkers for OSCC detection.

Computerized occlusal analysis in bruxism

AB  - Introduction. Sleep bruxism as nocturnal parafunction, also known as tooth grinding, is the most common parasomnia (sleep disorder). Most tooth grinding occurs during rapid eye movement - REM sleep. Sleep bruxism is an oral habit characterized by rhythmic activity of the masticatory muscles (m. masseter) that causes forced contact between dental surfaces during sleep. Sleep bruxism has been associated with craniomandibular disorders including temporomandibular joint discomfort, pulpalgia, premature loss of teeth due to excessive attrition and mobility, headache, muscle ache, sleep interruption of an individual and problems with removable and fixed denture. Basically, two groups of etiological factors can be distinguished, viz., peripheral (occlusal) factors and central (pathophysiological and psychological) factors. The role of occlusion (occlusal discrepancies) as the causative factor is not enough mentioned in relation to bruxism. Objective. The main objective of this paper was to evaluate the connection between occlusal factors and nocturnal parafunctional activities (occlusal disharmonies and bruxism). Method. Two groups were formed- experimental of 15 persons with signs and symptoms of nocturnal parafunctional activity of mandible (mean age 26.6 years) and control of 42 persons with no signs and symptoms of bruxism (mean age 26.3 yrs.). The computerized occlusal analyses were performed using the T-Scan II system (Tekscan, Boston, USA). 2D occlusograms were analyzed showing the occlusal force, the center of the occlusal force with the trajectory and the number of antagonistic tooth contacts. Results. Statistically significant difference of force distribution was found between the left and the right side of the arch (L%-R%) (t=2.773; p lt 0.02) in the group with bruxism. The difference of the centre of occlusal force - COF trajectory between the experimental and control group was not significant, but the trajectory of COF was longer in the group of bruxists (67.3±24.4mm). In addition, the significant difference of COF position in relation to the center of the elliptic fields was not found in bruxists (χ2=1.63; p> 0.05), but obtained results directly revealed uneven distribution of the occlusal forces which caused the excessive attrition and mobility of tooth. Conclusion. Our study failed to find direct correlation between occlusal factors and bruxism, so they are basically contributing factors.
AB  - Uvod. Bruksizam je noćna parafunkcionalna aktivnost mandibule i svrstava se u parasomniju ili poremećaj spavanja. Javlja se u REM fazi sna u vidu ritmične hiperaktivnosti mastikatornih mišića (m. masseter) koja destruktivno deluje skoro na sve komponente orofacijalnog sistema. Posledice ovog fenomena su trošenje zubnog tkiva, bol usled spazma mišića, miogene i artrogene disfunkcije sa mogućim degenerativnim intrakapsulnim promenama, pulpalgija, oboljenja parodoncijuma, glavobolje, naročito u jutarnjim časovima, i problemi različite prirode na pokretnim i fiksnim zubnim nadoknadama (trošenje akrilatnih zuba i akrilatnih faseta). U etiologiji bruksizma najčešće učestvuju periferni okluzalni i centralni psihogeni faktori. Uloga okluzije u etiologiji bruksizma nije dovoljno istražena niti definisana. Cilj rada. Cilj istraživanja je bio da se utvrdi veza između okluzalnih disharmonija i bruksizma, odnosno da se proceni značaj okluzije u nastanku bruksizma. Metod rada U istraživanje su uključene eksperimentalna i kontrolna grupa ispitanika. Eksperimentalnu grupu činilo je 15 ispitanika s evidentnim znacima i simptomima bruksizma, prosečne starosti od 26,6 godina, a kontrolnu grupu 42 osobe s izostankom bruksizma, prosečne starosti od 26,3 godine. Okluzija je analizirana kompjuterski, T-Scan II sistemom. Na dvodimenzionalnim kompjuterskim okluzogramima komentarisani su sledeći parametri: okluzalna opterećenja, centar okluzalnih sila i njegova putanja i broj antagonističkih parova zuba. Rezultati Kompjuterska analiza okluzije ukazuje na značajne razlike u opterećenju leve i desne strane (L%-D%) zubika (t=2,773; p lt 0,02) u korist ispitanika sa bruksizmom. Razlika u trajektorijumu centra okluzalnih sila (COF) nije značajna, ali je kod osoba sa bruksizmom trajektorijum COF duži (67,3±24,4 mm). Kod ispitanika sa bruksizmom takođe nije utvrđena statistički značajna razlika u položaju COF u odnosu na centar eliptičnih polja (χ2= 1,63; p>0,05), međutim, dobijeni rezultati ukazuju na evidentno neuravnoteženo opterećenje okluzije kod njih. Zaključak. Na osnovu rezultata kompjuterske analize okluzije može se zaključiti da okluzalni faktor doprinosi nastanku bruksizma.

Rezultati Kompjuterska analiza okluzije ukazuje na značajne razlike u opterećenju leve i desne strane (L%-D%) zubika (t=2,773; p lt 0,02) u korist ispitanika sa bruksizmom. Razlika u trajektorijumu centra okluzalnih sila (COF) nije značajna, ali je kod osoba sa bruksizmom trajektorijum COF duži (67,3±24,4 mm). Kod ispitanika sa bruksizmom takođe nije utvrđena statistički značajna razlika u položaju COF u odnosu na centar eliptičnih polja (χ2= 1,63; p>0,05), međutim, dobijeni rezultati ukazuju na evidentno neuravnoteženo opterećenje okluzije kod njih. Zaključak. Na osnovu rezultata kompjuterske analize okluzije može se zaključiti da okluzalni faktor doprinosi nastanku bruksizma.",

Significance of crown-down root canal preparation technique in endodontic therapy by using the hydroxylapatite sealer

AB  - Aim.To evaluate the crown-down preparation technique, and the use of hydroxylapatite based material for the definitive root canal obturation. Methods. The investigation included 20 single-canal roots with chronic periapical inflammatory lesion. Biomechanical medicamentous canal preparation was done using the double flared technique with balanced force, and the obturation was performed with hydroxylapatite sealer (unicone technique). Clinical and radiographic check-up performed 12 months after the treatment, used the following parameters: pain, swelling, percussion and palpation sensitivity, and the presence of fistula. Results. The obtained results showed a successful treatment in 18 cases, while in the 2 cases the treatment failed. Conclusion. These findings suggested that the crown-down preparation technique efficiently cleaned and shaped the root canal, and that the hydroxylapatite-based material created the homogenous and hermetic root canal obturation, so this methodology could be recommended for the endodontic therapy.
AB  - Cilj. Cilj ovog rada bio je da se klinički proceni značaj krunično-apeksne tehnike obrade kanala zuba uz primenu materijala na bazi hidroksilapatita za definitivno punjenje. Metode. Ispitivanje je obuhvatilo 20 jednokanalnih korenova jednokorenih zuba sa dijagnozom hronične periapeksne inflamacijske lezije. Biomehaničko-medikamentna tehnika dvostrukog širenja, uz primenu balansirane sile, bila je odabrana za preparaciju kanala, kao i materijal na bazi hidroksilapatita i monopoen tehnika za definitivno punjenje. Klinička i rendgen kontrola lečenih zuba je obavljena posle 12 meseci i praćeni su sledeći parametri: postojanje bola, otoka, osetljivosti na vertikalnu perkusiju i palpaciju, kao i postojanje fistule. Rezultati. Dobijeni rezultati su pokazali da je uspešan ishod terapije bio u 18 slučajeva, a terapija je u 2 slučaja bila neuspešna. Zaključak. Ovakav nalaz ukazuje na to da krunično-apeksna tehnika preparacije efikasno čisti i oblikuje prostor kanala, a primenom materijala na bazi hidroksilapatita ostvaruje se homogeno, hermetičko definitivno punjenje kanala, te se ovakva metodologija rada može preporučiti u endodontskoj terapiji.

AB  - This paper argues on importance of gingival and dental parameters in evaluation of esthetic characteristics of fixed restorations. Human beauty is a specific category which is not easy to define by means of any known criteria or with mathematic formula. But, it is also important for practitioners to take into consideration some objective rules determining what is really nice and appealing. In reconstructing the personal identity, it is essential to harmonize the esthetic features. In dental practice, this approach means to establish a close correlation among the facial, dental, gingival and physical components of orofacia! system. For the purpose of this study, the authors have divided the overview of esthetic principles into two parts. The first one deals with some objective parameters such as: gingival health, the zenith of gingival contour, the balance of gingival levels, relative tooth dimensions, the golden proportion of the teeth, the design of dental arch, interdental contact areas, interdental closure, the contour of incisal edge and the shape of interincisal angles. The esthetic integration of these parameters into a frame of smile and the whole face, as well as some subjective criteria are discussed in the second part of the paper.
AB  - Dentalni i gingivni parametri imaju veliki značaj u proceni estetskih obeležja fiksnih nadoknada. Činjenica da je ljudska lepota kategorija koja se ne može apsolutno definisati ni brižljivo odabranim kriterijuma, ni matematičkim formulama ipak ne zatvara vrata razmišljanju da postoji nešto što se zove objektivno važenje, saglasnost o tome šta jeste, a šta nije lepo. U želji da se sačuva ili rekonstruiše identitet jedinke, usklađivanje estetskih obeležja je od ogromne važnosti. U stomatološkom radu ovo podrazumeva uspostavljanje bliske komunikacije između facijalnih, dentalnih, gingivnih i fizičkih komponenti orofacijalnog sistema. s obzirom na složenost aktuelne problematike, autori su podelili naznačenu temu na dva dela. Ovi komplementarni delovi se bave sveobuhvatnim razmatranjima velikog broja objektivnih i subjektivnih faktora koji determinišu validnost dentalne kompozicije rekonstruisane fiksnim nadoknadama.

Concept of 'long centric'

AB  - The objective of this paper was to show the historical perspective of the „long centric" occlusal concept and its importance in the modern dentistry, especially from the gnathological aspect. The „long centric" concept represents therapeutic modality used in modern dentistry and occlusal adjustment in all patients showing differences in strong and weak closure of the lower jaw starting from the position of physiological rest/long centric" concept is applied only for anterior teeth and occlusal movements from rather than toward the center. Whenever the „long centric" parameters are not adequate, occlusal disturbance, resulting from the „wedge" effect during the initial closure of the lower jaw, is present. Different degrees of abrasion or hypermobility of the teeth are often the result of the above-mentioned occlusal disturbances and can potentially trigger bruxism and malfunction. Modus procedendi should be the regular approach of every dentist to any occlusion, because only the built-in „long centric" efficiently contributes to the occlusal stability of the anterior portion of the dentition. All occlusions should be routinely tested regarding their need for „long centric", especially when the extensive therapeutic interventions (conservative, prosthetics) of the occlusal complex are required.
AB  - Cilj rada je bio da prikaže istorijsku perspektivu okluzalnog koncepta„long centric"(LC) i njegov značaj u savremenoj stomatologiji, posebno sa gnatološkog aspekta. Koncept LC predstavlja terapijski modalitet koji se koristi u savremenoj stomatologiji i okluzalnom podešavanju kod pacijenata kod kojih se čvrsto i blago zatvaranje donje vilice iz položaja fiziološkog mira razlikuje. LC se odnosi samo na anteriorne zube i na okluzalnu slobodu od centra, a ne u centru. Ako potreba za ovim konceptom nije adekvatno zadovoljena, javlja se okluzalna smetnja zbog tzv. efekta klina u inicijalnom zatvaranju donje vilice. Različiti obrasci abrazije ili nekog stepena hipermobilnosti zuba su često rezultat takve smetnje i mogu imati potencijal „okidača" za bruksizam i parafunkciju. Prikazan modus procedenti određivanja potrebe za LC treba da bude rutinski prilaz svakog lekara bilo kojoj okluziji, jer ugrađen LC efikasno doprinosi okluzalnoj stabilnosti prednjeg segmenta denticije. Sve okluzije bi trebalo rutinski testirati u pogledu potrebe za LC, naročito kada postoji potreba za ekstenzivnim terapijskim intervencijama (konzervativna, protetska) na okluzalnom kompleksu.

AB  - Resin cements (composite luting cements) are used for cementation all ceramic crowns in contemporary dental practice. These materials which are characterized by type of fillers, initiation system, their viscosity, mechanical properties (wear resistance, elastic modulus, compressive strength, tensile and shear strength), and by their physical properties (CTE, radiopacity, polimerization shrinkage) became very popular in everyday practice. In that regard, this paper deals with some special characteristics of composite luting cements, as well as with the practical instructions during the cementation procedures connected to one of the commercial product of resin cements.
AB  - Savremena stomatološka praksa koristi za cementiranje keramičkih nadoknada kompozitne cementne materijale. Definisani različitim parametrima kao što su: odgovarajuća vrsta punioca, inicijatorni sistem, viskoznost, mehaničke karakteristike (modul elastičnosti, otpornost na habanje, čvrstoća na pritisak, istezanje i smicanje) i fizička svojstva (CTE, rendgenkontrasnost, polimerizaciona kontrakcija), strogo specifični za određene namene, ovi materijali zavređuju posebnu pažnju praktičara. Stoga, ovaj rad razmatra neke od njihovih specifičnosti sa osvrtom na pravila cementiranja i korektnu tehniku pri upotrebi jednog od poznatih preparata dvostrukopolimerizujućih kompozitnih cemenata.
