The Role of Varicella Zoster Virus in the Development of Periapical Pathoses and Root Resorption: A Systematic Review

AB  - Introduction: Varicella zoster virus (VZV) and subsequent herpes zoster (HZ) infection have been proposed as a causative agent of periapical pathoses and root resorption. This review aimed to identify, synthesize, and present a critical analysis of the available data on the association among VZV, subsequent HZ infection, and the development of periapical pathoses and root resorption and to analyze the level of evidence of available studies. Methods: The literature search covered MEDLINE, Science Citation Index Expanded, and Scopus. A qualitative critical appraisal of the included articles was performed. Results: The electronic database search. yielded 66 hits from PubMed, 73 hits from Web of Science, and 107 from Scopus: Seven case reports and 3 cross-sectional studies were included in the final review. When summarized, in 7 patients with a history of a previous HZ attack and with no other apparent cause, -23 teeth were diagnosed with apical periodontitis, 8 teeth with internal and 1 tooth with external root resorption. The cross-sectional studies investigated the presence of VZV DNA in samples of acute apical abscess. The VZV DNA was found only in 2 of 65 samples. Conclusions: All studies included in this systematic review.had a low level of evidence (4 and 5). Still, the potential role of VZV in the etiopathogenesis of periapical pathoses and root resorption cannot be ruled out. Future investigations should be directed toward the analysis of VZV pathologic effects on pulp blood vessels, which might cause local ischemia and tissue necrosis.

Efficacy analysis of application of reversible occlusal therapy in treating temporomandibular dysfunction - meta-analysis

AB  - Psychological discomfort, physical disability and functional limitations of the orofacial system have a major impact on everyday life of patients with temporomandibular disorders (TMDs). In this study we sought to determine short and long term effects of stabilization splint (SS) in treatment of TMDs, and to identify factors influencing its efficacy. Methods Search was conducted in the electronic databases MEDLINE, Web of Science and EMBASE. The search criteria were randomized controlled trials that compared SS to various control groups: non-occluding splint, occlusal oral appliances, physiotherapy, behavioral therapy, minimal treatment and no treatment. Synthesis of data was performed using the random effects model. The effect estimates were expressed as odds ratio (OR) or standardized mean difference (SMD) with 95% confidence interval. Subgroup analyses were carried out according to the use of Research Diagnostic Criteria (RDC/TMD) in primary studies, origin of TMDs, and individual therapeutic modalities. Strength of evidence was assessed by GRADE system. Sensitivity analysis showed the importance of introducing standardized diagnostic criteria in the evaluation of the observed treatments. Meta-regression was performed to determine the independent factors which influence the effect of SS. Results Thirty seven eligible RCTs were included in meta-analysis. In short term, SS presented positive overall effect on pain reduction (OR 2.08; p=0.01) and pain intensity (SMD - 0.33; p=0.02). Subgroup analyses confirmed SS effect in studies that used RDC/TMD...
AB  - Psihološka nelagodnost, fizičke i radne nesposobnosti i funkcionalna ograničenja orofacijalnog sistema imaju veliki uticaj na svakodnevni život pacijenata sa temporomandibularnim disfunkcijama (TMD). Cilj meta-analize je bio da se utvrde kratkoročni i dugoročni efekti stabilizacionog splinta (SS) u lečenju TMD-a, i da se identifikuju faktori koji utiču na njegovu efikasnost. Metod Pretraživanje se vršilo u elektronskim bazama MEDLINE, Web of Science i EMBASE. Kriterijum za pretraživanja bile su randomizovane kontrolisane studije koje su poredile SS prema različitim kontrolnim grupama: ne-okludirajući splint, oralni okluzalni aparati, fizikalna terapija, bihevioralna terapija, minimalni tretmani i bez tretmana. Sinteza podataka vršena je pomoću modela slučajnog efekta. Mere efekta za primarne i sekundarne ishode bili su unakrsni odnos šanse (UOŠ), ili standardizovana prosečna razlika sa 95% intervalom poverenja (IP). Porcelain veneers: Preparation design: A retrospective review

AB  - This paper discusses the preparation of tooth design for porcelain veneers. It follows the literature more than three past decades. From the very beginning, the porcelain veneers were placed to no/minimally prepared tooth substance, showing different problems in clinical use. Later, the technique of etching the porcelain and controlling the reduction of tooth structure presented the great steps forward in porcelain veneers accepting. The special accent concerning the preparative design was placed on variations of incisal edge preparation - the problem, which is still present in current practice. Additionally, the paper emphasizes the extremely demanding protocols in making the porcelain veneers, as well as their expanded clinical indications.
AB  - Rad diskutuje preparativni dizajn zuba u izradi porcelanskih faseta kroz literaturni revijalni osvrt duži od tri decenije. Upoznajući se sa porcelanskim fasetama, dentalna profesija nije prepoznavala na pravi način značaj preparacije zuba koji će nositi fasete, pa su fasete aplikovane na nepreparine površine zuba. Veliki korak napred u kliničkoj prihvatljivosti porcelanskih faseta donela je tehnika nagrizanja površine gleđi i kontrolisana redukcija ovog tkiva. Poseban akcenat u uspešnoj ekspolataciji faseta predstavlja iznalaženje odgovarajućeg dizajna incizalne ivice, kao i rešavanje problema fraktura tankih porcelanskih faseta. U vezi sa tim, rad diskutuje jedan od modaliteta u preparaciji incizalne ivice koji uključuje samo njeno skraćivanje do 2mm, but joint dizajn. Istraživanja pokazuju niz prednosti ovog dizajna u odnosu na palatinalni žleb: optimalan odnos keramike i cementa na palatinalnoj strani, smanjen rizik od postcemetirajućeg loma nastalog polimerizacionom kontrakcijom i prirodnim temperaturnim varijacijama u ustima i dr. Uz to, ovaj dizajn omogućava prezervaciju perifernog sloja gleđi, koji je kritičan u eliminaciji mikropukotine na palatinalnom spoju fasete i zuba, jasno se suprostavljajući silama smicanja. Skraćivanje incizalne ivice je jednostavnije, brže, a laboratorijski model je jasniji. Ravna površina ostavlja bolji oslonac keramičkoj faseti, pa su i rizici loma tankih palatinalnih ivica keramike kontrolisani ovim dizajnom. U novije vreme, prošireno indikativno polje u izradi faseta, koje je donelo brojne specifičnosti u preparativnoj tehnici i dizajnu zuba, biva kritikovano od 'back to future' promotera u struci, koji se zalažu za upotrebu tankih non-prep porcelanskih faseta. Uz to, revijalni pregled apostrofira značaj poštovanja veoma zahtevnih, različitih kliničkih faza u preparaciji zuba za prihvatanje soficticiranog konzervativnog modaliteta u struci, kakve su porcelanske fasete.
