Peri-implant soft and hard tissue condition after alveolar ridge preservation with beta-tricalcium phosphate/type I collagen in the maxillary esthetic zone: A 1-year follow-up study

AB  - Background/Aim. Alveolar ridge dimensional alterations following tooth extraction in the anterior maxilla often result in an inadequate bone volume for a correct implant placement. In order to obtain optimal bone volume various bone graft substitutes have become commercially available and widely used for socket grafting. The aim of this study was to examine and compare long-term clinical outcomes of dental implant therapy in the maxillary esthetic zone, after socket grafting with beta-tricalcium phosphate (TCP) combined with collagen type I, either with or without barrier membrane and flap surgery, after a 12-month follow-up. Methods. Twenty healthy patients were allocated to either C group (beta-TCP and type I collagen without mucoperiosteal flap coverage) or C+M group (beta-TCP and type I collagen barrier membrane with mucoperiosteal flap coverage). Following clinical parameters were assessed: implant stability (evaluated by a resonance frequency analysis - RFA), periimplant soft tissue stability (sulcus bleeding index - SBI, Mombelli sulcus bleeding index - MBI, periimplant sulcus depth, keratinized gingiva width, gingival level) and marginal bone level at the retroalveolar radiograms. Results. Within C+M group, RFA values significantly increased 12 weeks after implant installation compared to primary RFA values. Comparison between investigated groups showed a significantly reduced keratinized gingiva width in the C+M group compared to the C group after 3, 6, 9 and 12 months. Comparison between groups revealed significantly lower gingival level values in the C+M group at 9th and 12th month when compared to the C group. Conclusion. Implant treatment in the anterior maxilla could be effective when using a 9 months alveolar ridge preservation healing with combined treatment with beta-tricalcium phosphate and type I collagen, with regard to the peri-implant soft and hard tissue stability.
AB  - Uvod/Cilj. Posle ekstrakcije zuba, dimenzionalne promene alveolarnog grebena u estetskoj regiji gornje vilice za posledicu često imaju nedovoljnu količinu kosti za ugradnju zubnih implanata. U vezi sa tim, primenjuju se različiti koštani zamenici sa ciljem očuvanja dimenzija alveolarnog grebena posle ekstrakcije zuba. Cilj rada bio je da se, posle prezervacije alveolarnog grebena beta-trikalcijum fosfatom (TCP) sa kolagenom tip 1, sa barijernom membranom i mukoperiostalnim režnjem i bez nje, ispitaju i uporede klinički ishodi zarastanja posle ugradnje zubnih implanata u estetskoj regiji gornje vilice, tokom jednogodišnjeg perioda praćenja. Metode. Dvadeset zdravih bolesnika podeljeno je u dve grupe: C (beta TCP/kolagen tip 1 bez barijerne membrane i mukoperiostalnog režnja) i C+M (beta TCP/kolagen tip 1 sa barijernom membranom i mukoperiostalnim režnjem). Praćeni su uobičajeni klinički parametri ishoda terapije: implantna stabilnost (analiza rezonantne frekvence), stanje mekih tkiva (indeks krvarenja, plak indeks, širina pripojne mukoze, recesija gingive) i nivo periimplantnog koštanog tkiva na retroalveolarnom radiogramu. Rezultati. U C+M grupi, implantna stabilnost posle 12 nedelja bila je značajno veća u odnosu na primarnu stabilnost. U C+M grupi, širina keratinizovane gingive bila je značajno manja posle 3, 6, 9 i 12 meseci u odnosu na C grupu. Recesija gingive bila je značajno veća u C+M grupi u odnosu na C grupu posle 9 i 12 meseci. Zaključak. Razmatrajući stabilnost mekog i tvrdog periimplantnog tkiva, terapija zubnim implantima može biti uspešna prilikom ugradnje u estetskoj regiji gornje vilice.

Mechanical Properties of Direct and Indirect Composite Materials Used in Prosthodontics

AB  - This study was conducted to investigate composite materials using mechanical equipment corresponded to masticatory system composed of two antagonistic parts that simulates bimaxilar contact in occlusion and articulation. The aim of this study was to investigate restorative materials with different chemical properties and to find out which material showed the best mechanical properties. The experiment was conducted using 4 types × 20 specimens of direct resin composite materials and equal number of indirect resin composites (IRCs). The following composites were tested: Tetric (Ivoclar Vivadent), Kerr Herculite XRV (Kerr), Charisma (Heraeus Kulzer), Heliomolar (Ivoclar Vivadent), Artglass (Heraeus Kulzer), Targis/Vectris (Ivoclar Vivadent), Vita Zeta LC (Vita) and Kerr Herculite Lab (Kerr). The universal dimensions of all specimens were 5 mm × 5 mm with 2 mm thickness. This was achieved using silicone molds. A total of 160 specimens (80 direct composite specimens and 80 indirect composite specimens) were tested using tensile testing machine. Artglass showed minimal change in thickness (30.20 ± 23.0 μm) compared to others. The highest change in thickness was found in Heliomolar (86.2 ± 32.6 μm). In the group of direct composite materials, Tetric experienced the lowest thickness change (31.0 ± 8.8 μm), while Charisma showed thickness variation of 65.2 ± 27.3 μm. Among indirect composites, Vita Zeta LC showed the highest value of thickness variation (76.2 ± 31.3 μm). In this experimental study, indirect composite specimens showed superior mechanical performance compared to direct composite specimens highlighted Artglass as the material of choice viewed from mechanical aspect.

Analyzing strain in samples with all-ceramic systems using the digital image correlation technique

AB  - Introduction/Objective The study was conducted to identify the maximum strain generated in the samples composed of poly-methyl-methacrylate, Straumann® implants, and three types of ceramic systems. Methods Three types of experimental models were used, loaded by external load of 100 N, 300 N, and 500 N and analyzed using the digital image correlation method. The models were composed of yttriastabilized zirconia, e.max lithium disilicate, and Vita Enamic® hybrid ceramics, placed on the Straumann® cylindrical dental implant systems (4 × 10 mm) with straight abutments. Results Significant differences in strain values between samples with different crown material groups were detected (p = 0.000). This suggests that strain values were dependent on the type of crown material. Strain values were also affected by the region of interest (p = 0.000). Application of two-way ANOVA enabled testing of the interaction effect between two independent variables, crown material and region of interest, where a significant difference was also found (p = 0.046). This indicates that strain values were also influenced by different combinations of material type and region of interest. The highest strain values were found for Z (0.383 ± 0.015) in the apical region, and the lowest for E (0.303 ± 0.015) in the middle region. Conclusion The study shows maximum strain in the apical and marginal directions. When considered various all-ceramics, we noticed the minimum strain below Vita Enamics®, while the maximum strain was found in samples with yttria-stabilized zirconia crown.
AB  - Uvod/Cilj Studija je sprovedena da identifikuje maksimalnu deformaciju proizvedenu u uzorcima sastavljenim od polimetilmetakrilata, Štrauman® implantata i tri vrste keramičkih sistema. Metode Korišćene su tri vrste eksperimentalnih modela izloženih spoljašnjem opterećenju od 100 N, 300 N i 500 N i analiziranih uz pomoć metoda korelacije digitalnih slika. Modeli su bili sastavljeni od itrijum-cirkonije, e. maks. litijum disilikatne i hibridnih keramika Vita enamik®, postavljenih na cilindrične dentalne implantantne sisteme Štrauman® (4 × 10 mm) sa abatmentima pod pravim uglom. Rezultati Značajne razlike su otkrivene u vrednostima deformacija između uzoraka sa različitim keramičkim krunicama (p = 0,000). Ovo podrazumeva da su vrednosti deformacija zavisne od tipa keramičkog materijala. Vrednosti deformacija su zavisne i od regiona interesa (p = 0,000). Primena ANOVA testa je omogućila da se uoči interakcija između nezavisnih varijabli, meterijala keramičkih kruna i regiona od interesa, gde je takođe nađena statistički značajna razlika (p = 0,046). Ova činjenica ukazuje na to da vrednosti deformacija zavise od različite kombinacije tipa keramičkog materijala i regiona interesa. Najveće vrednosti deformacija su nađene na modelu Z (0,383 ± 0,015) u apikalnom regionu, dok su najmanje vrednosti deformacija nađene na modelu E (0,303 ± 0,015) u regionu srednje trećine. Zaključak Izveštaj je pokazao maksimalne deformacije u apikalnim i marginalnim pravcima. Kada se razmatraju različite vrste keramika, najmanje deformacije su primećene ispod kruna Vita enamik®, dok je najveća deformacija pronađena u uzorcima sa krunama itrijum-cirkonija.

AB  - Dental-implant overload can cause bone resorption. Load-transfer characteristics of dental implants are affected by their macro-design parameters. The goal of this study was to experimentally analyse the load-transfer characteristics of different dental implants, using polymethyl-methacrylate blocks. Three polymethyl-methacrylate blocks were created, with dimensions of (68 x 25 x 9) mm. Three dental implants, Nobel empty set3.5 mm x 15 mm, Strauman empty set4.1 mm x 10 mm and Strauman empty set4.8 mm x 12 mm. were placed in separate blocks. The samples were supported by a three-point-bending set-up and loaded with an axial force of 600 N. The 3D digital image correlation method was employed for strain and displacement measurements. The highest displacement and von Mises strain values were found for Strauman empty set4.1 mm x 10 mm (p  lt  0.05), 0.186 mm and 0.596 %, respectively. The sample of Nobel empty set3.5 mm x 15 mm showed the lowest strain values. The sample of Strauman empty set4.8 mm x 12 mm (p > 0.05) had similar strain values as Nobel empty set3.5 mm x 15 mm. The load transfer during axial loading was primarily affected by the size of the implant contact surface. The displacement and strain values in the implant vicinity may provide an insight into the effect of dental-implant design on the load transfer.

Effects of Prosthetic Therapy in the Treatment of Edentulous Patients with Obstructive Sleep Apnea

AB  - Background: This study is based on the assumption that the reduced vertical dimension of occlusion (VDO) in patients wearing complete dentures could be one of the causes of OSA appearance. The aim of the study was to examine the effect of prosthetic treatment which corrects the reduced VDO in edentulous patients with OSA. Methods: The study involved 20 edentulous patients with apnea/hypopnea index per hour of sleep (AHI/h) > 5.7  lt  32.5. The methodology of research required a multidisciplinary chronological protocol from collection of anamnestic data, intraoral examination, polysomnographic (PSG) registration during sleep, and diagnosis by using the magnetic resonance imaging (MRI) to fabrication of the complete dentures in accordance with the established clinical procedures. Volume analysis (in cubic millimeters) of the pharynx was performed as sum of the segmented areas multiplied by their thickness. Results: Prosthetic therapy in edentulous patients with OSA reduced the value of AHI/h. The average value of AHI/h based on the sample data before the treatment was 17.6 +/- 8.9 while three months after the treatment with complete dentures AHI/h amounted to 9.7 +/- 6.5 (p  lt  0.001). After three months from prosthetic rehabilitation, MRI showed a mean volume increase of 27% of pharynx in edentulous patients with OSA. Conclusion: The prosthetic treatment with complete dentures which corrects VDO provides adequate support to orofacial structures, increases the volume of the pharynx and reduces the symptoms of OSA in edentulous patients.

Unilateral complex partial denture performance evaluation: 5 Years follow up clinical study

AB  - Introduction Removable partial denture (RPD) is common treatment option for unilateral partially edentulous patients not indicated for implant therapy. Unilateral complex partial denture (UCPD) could be an alternative approach to RPD treatment, but there is lack of evidence about UCPD treatment outcomes during the long-term clinical performance. The aim of this study was to use periodontal, prosthodontic and participant satisfaction measures to evaluate the long-term clinical performance of UCPD. Material and methods This 5 year follow-up clinical study evaluated pocket probing depth (PPD) and vertical clinical attachment loss (CAL-V) of direct abutment (DA), indirect abutment (IA) and control teeth (CT). Also complications and failures of UCPD were analysed using questionnaire of participant satisfaction with UCPD (stability, comfort and manipulation). Results Evaluation of data showed that CAL-V and PPD significantly increased over time for DA, IA and CT (p lt 0.0001), but the tooth function (DA, IA and CT) did not significantly influence changes in PPD and CAL-V. The fracture of one abutment tooth and increase of the number of artificial teeth deformations (p=0.039) were observed after 5 years. Participant satisfaction with denture after 1 year and 5 years vs. 7 days was significantly improved. Conclusion Despite limitations of this clinical study and assuming regular oral maintenance with proper indication, UCPD might be considered as good treatment option for Kennedy II rehabilitation in patients not indicated for implant therapy or who cannot tolerate extensive RPD design.
AB  - Uvod Parcijalna skeletirana proteza (PSP) najčešća je terapijska opcija kod jednostrane krezubosti kada nije indikovana implantološka terapija. Jednostrana kompleksna parcijalna skeletirana proteza (JKPSP) predstavlja alternativu konvencionalnoj PSP, jedino što ne postoje evidentni podaci o uspešnosti terapije nakon duže kliničke upotrebe. Cilj ovog rada bio je da se na osnovu analize objektivnih pokazatelja, odnosno analize periodontalnih i protetskih parametara, kao i na osnovu subjektivnih ocena pacijenta, izvrši provera uspešnosti JKPSP retiniranim atečmenom tipa reze sa oprugom nakon petogodišnjeg praćenja. Materijal i metod Kod deset ispitanika oba pola izvršeno je praćenje gubitka vertikalnog pripoja gingive i dubine parodontalnih džepova, kod primarnog retencionog zuba (PRZ), sekundarnog retencionog zuba (SRZ), koji su bili u sastavu JKPSP i kontrolnog zuba (KZ) suprotne strane vilice. Takođe, analizirani su protetske komplikacije (fraktura zuba, gubitak veštačkih zuba) i zadovoljstvo pacijenata na osnovu popunjenih upitnika, gde su oni ocenjivali stabilnost, komfor i rukovanje JKPSP. Rezultati Nakon analize dobijnih podataka uočeno je da se vrednost dubine parodontalnih džepova i gubitak vertikalnog pripoja gingive kod PRZ statistički značajno razlikuju u odnosu na SRZ i KZ (p  lt  0,0001). Zabeležene protetske komplikacije u vidu frakture retencionog zuba i deformacije veštačkih akrilatnih zuba nisu bile statistički značajne posle pet godina praćenja. Zadovoljstvo pacijenata JKPSP posle prve godine i posle pet godina nošenja se značajno popravilo u odnosu na prvih sedam dana posle predaje. Zaključak Nezavisno od ograničenja ove kliničke studije, uzimajući u obzir adekvatnu indikaciju i pravilno održavanje higijene, JKPSP se može smatrati dobrom opcijom za rehabilitaciju krezubosti Kenedi II klase kod pacijenata kod kojih nije indikovana terapija implantatima, odnosno koji ne prihvataju prisustvo velike spojnice. Pacijenti su bili zadovoljni protezama, njihovim komforom i lakoćom rukovanja protezama. .

Stress and strain distribution in the lower jaw with shortened dental arch: A finite element method study

AB  - Introduction/Objective The absence of functional loading due to molar loss might cause changes to the microstructure of the bone. Therefore we investigated and visualized deformation and strain pattern distribution of the mandibule with full arch dentition (FDA) and shortened dental arches (SDA) during occlusion. Methods A 3D model of an adult cadaveric dentate mandible, without pathological and traumatic damages, was developed based on CT scan images, set to 0.7 mm slice thickness. The scanned slices were imported into software where the bone and teeth were identified and modelled separately based on image density thresholding. Using the software and based on the grey-scale analysis of the slices initial meshes for the cortical, cancellous bone and teeth were generated. Results Highest stress/strain values were registered in the structures adjacent to molars i.e. molar region of processus alveolaris assigned by blue color in FDA model. Similar to FDA, the SDA models exibited deformation with evident highest strain (9.33%) at the processus coronoideus and mandibular angle. The highest overall stress (5 MPa) and strain (6.67%) was found in the mandibular intercanine segment of the SDA model, considering the mandibular body. Conclusion Finite element method can be employed as a powerfull tool for visualization of the stress and strain of the loaded mandible models with full and shortened dental arches. It was registered that molar support loss caused higher stress and strain in SDAs compared to FDA model.
AB  - Uvod/Cilj Odsustvo funkcionalnog opterećenja usled gubitka molara može izazvati promene u mikrostrukturi kosti. Cilj ovog istraživanja je bio da se ispitaju i vizuelizuju naponi i deformacije u mandibulama sa intaktnim zubnim lukom (IZL) i skraćenim zubnim lukom (SZL) u toku okluzije. Metod 3D model kadaverske donje vilice odrasle osobe (procenjena vizuelnim pregledom bez patoloških i traumatskih oštećenja) razvijen je na osnovu KT slike, debljine sloja od 0,7 mm. Skenirani preseci su uneti u softver gde su identifikovana i modelirana tvrda tkiva kosti i zubi pojedinačno na osnovu segmentacije slike. Početne mreže za kortikalnu kost, spongioznu kost i zube su proizvedene korišćenjem softvera i na osnovu analize sive skale preseka. Rezultati Najveće vrednosti napona i deformacija su registrovane u strukturama donje vilice neposredno uz molare u tzv. molarnom regionu alveolarnog nastavka, što je označeno plavom bojom na IZL modelu. Slično IZL, i SZL modeli su pokazali najveće vrednosti deformacija (9,33%) u koronoidnom nastavku i mandibularnom uglu. Najveći napon (5 MPa) i deformacija (6,67%) pronađeni su u interkaninom regionu SZL modela, ukoliko se razmatra telo donje vilice. Zaključak Metodom konačnih elemenata su vizuelizovane slike napona i deformacija modela donje vilice sa punim i skraćenim zubnim lukovima. Uočeno je da gubitak molarne potpore izaziva jače napone i deformacije u SZL modelima u poređenju sa IZL modelom.

abstract = "Introduction/Objective The absence of functional loading due to molar loss might cause changes to the microstructure of the bone. Therefore we investigated and visualized deformation and strain pattern distribution of the mandibule with full arch dentition (FDA) and shortened dental arches (SDA) during occlusion. Methods A 3D model of an adult cadaveric dentate mandible, without pathological and traumatic damages, was developed based on CT scan images, set to 0.7 mm slice thickness. The scanned slices were imported into software where the bone and teeth were identified and modelled separately based on image density thresholding. Using the software and based on the grey-scale analysis of the slices initial meshes for the cortical, cancellous bone and teeth were generated. Results Highest stress/strain values were registered in the structures adjacent to molars i.e. molar region of processus alveolaris assigned by blue color in FDA model. Similar to FDA, the SDA models exibited deformation with evident highest strain (9.33%) at the processus coronoideus and mandibular angle. The highest overall stress (5 MPa) and strain (6.67%) was found in the mandibular intercanine segment of the SDA model, considering the mandibular body. Conclusion Finite element method can be employed as a powerfull tool for visualization of the stress and strain of the loaded mandible models with full and shortened dental arches. It was registered that molar support loss caused higher stress and strain in SDAs compared to FDA model., Uvod/Cilj Odsustvo funkcionalnog opterećenja usled gubitka molara može izazvati promene u mikrostrukturi kosti. Cilj ovog istraživanja je bio da se ispitaju i vizuelizuju naponi i deformacije u mandibulama sa intaktnim zubnim lukom (IZL) i skraćenim zubnim lukom (SZL) u toku okluzije. Metod 3D model kadaverske donje vilice odrasle osobe (procenjena vizuelnim pregledom bez patoloških i traumatskih oštećenja) razvijen je na osnovu KT slike, debljine sloja od 0,7 mm. Skenirani preseci su uneti u softver gde su identifikovana i modelirana tvrda tkiva kosti i zubi pojedinačno na osnovu segmentacije slike. Početne mreže za kortikalnu kost, spongioznu kost i zube su proizvedene korišćenjem softvera i na osnovu analize sive skale preseka. Rezultati Najveće vrednosti napona i deformacija su registrovane u strukturama donje vilice neposredno uz molare u tzv. molarnom regionu alveolarnog nastavka, što je označeno plavom bojom na IZL modelu. Slično IZL, i SZL modeli su pokazali najveće vrednosti deformacija (9,33%) u koronoidnom nastavku i mandibularnom uglu. Najveći napon (5 MPa) i deformacija (6,67%) pronađeni su u interkaninom regionu SZL modela, ukoliko se razmatra telo donje vilice. Zaključak Metodom konačnih elemenata su vizuelizovane slike napona i deformacija modela donje vilice sa punim i skraćenim zubnim lukovima. Uočeno je da gubitak molarne potpore izaziva jače napone i deformacije u SZL modelima u poređenju sa IZL modelom.",

Strain visualization of supporting tissues rehabilitated using two different types of removable partial dentures

The aim of this study was to analyze strain distribution of supporting tissues beneath two different types of removable partial dentures, commonly indicated in the conventional rehabilitation of partially edentulous patients. Methods This in vitro study included two groups of experimental models composed of the mandibles (Kenedy Class 1) and two types of removable partial dentures. These models were exposed to occlusal loading and the digital image correlation method was used for strain visualization and strain measurement. Results The highest strain was measured beneath the removable partial dentures, on the surfaces of bone adjacent to distal abutments and in the anatomical structure called the retromolar area. Strain values in the experimental models with clasp removable partial dentures ranged 0-10%. Strain values in the experimental models with attachment - removable partial dentures ranged 0-2.3%. Conclusion The findings showed that the attachment retaining removable partial dentures induced lower strain in the residual alveolar ridges. However, higher strain was detected in the marginal bone next to the abutment teeth.
AB  - Uvod/Cilj Savremene biomehaničke analize omogućavaju kompletnu vizuelizaciju deformacija od opterećivanja različitih zubnih nadoknada u protetskoj rehabilitaciji. Cilj ove studije je bio da analiza distribucije deformacija unutar potpornih tkiva ispod dva različita tipa najčešće korišćenih parcijalnih skeletiranih proteza. Metode In vitro studija je obuhvatila dve grupe eksperimentalnih modela donjih vilica (Kenedi 1 klasa krezubosti) i dva tipa parcijalnih skeletiranih proteza. Modeli su bili izloženi okluzalnim silama, a za prikaz i merenje deformacija je korišćena metoda digitalne korelacije slika. Rezultati Najveća deformacija je izmerena ispod parcijalnih proteza, na površinama kosti koja okružuje distalne zube nosače i u retromolarnoj regiji. Strain determination of self-adhesive resin cement using 3D digital image correlation method

AB  - Introduction/Objective In an attempt to simplify dental procedures, a new group of resin cements, self-adhesive resin cements (SARCs), have been introduced. Performance of SARCs can widely vary. One of the main reasons of adhesion failure is polymerization shrinkage. The aim of this study was to determine, evaluate, and measure strain field of self-adhesive dual cure resin cement during polymerization in self-cure mode using 3D digital image correlation (DIC) method. Methods The self-adhesive Maxcem Elite (Kerr, Orange, CA, USA) cement was tested in five cylindrical samples (5 mm in diameter and 2 mm in thickness) prepared by filling plastic ring-type molds. Digital images were recorded immediately after sample preparation. Results Non-uniform strain distribution was found in resin cement with higher strain values along the periphery (up to 15%) and lower strain values in central parts (around 4%) of each sample. Conclusion It can be concluded that DIC is a powerful tool for full-field strain measurements in material characterization.
AB  - Uvod/Cilj U nastojanju da se pojednostave stomatološke procedure uvedene su nove grupe kompozitnih cemenata - samoadhezivni, dvojnovezujući kompozitni cementi. Svojstva ovih cemenata mogu da variraju u velikoj meri. Jedan od glavnih razloga narušavanja adhezionih svojstava kompozitnih cemenata je polimerizacijska kontrakcija. Cilj ovog rada je da se odredi i izmeri deformaciono polje samoadhezivnog dvojnovezujućeg kompozitnog cementa tokom hemijske polimerizacije korišćenjem eksperimentalne tehnike - metode 3D korelacije digitalnih slika. Metode Samoadhezivni kompozitni cement Maxcem Elite (Kerr, Orange, SAD) ispitivan je na pet uzoraka prečnika 5 mm, debljine 2 mm, koji su pripremljeni punjenjem plastičnih kalupa prstenastog oblika. Digitalne slike su zabeležene neposredno posle pripreme uzoraka. Rezultati Neuniformno deformaciono polje je pokazalo veće vrednosti deformacija na periferiji uzoraka (do 15%) i manje vrednosti deformacija u centralnim delovima uzoraka (oko 4%). Zaključak Može se zaključiti da je 3D korelacija digitalnih slika precizna i pouzdana metoda za merenje deformacionih polja u oblasti karakterizacije materijala.

Experimental methodology for analysis of influence of dental implant design on load transfer

AB  - Deformations in the vicinity of dental implants are affected by their design and if threshold level of 0.3 % is surpassed, bone resorption could occur. The goal of this study is to present a novel experimental approch for the analysis of effect od dental implant geometry on the surrounding structure strain values. A bone block model, with dimensions of 68x25x9 mm, was made from polymethyl-methacrylate. 3D printed block mold also provided a fixture for vertically placed Strauman ø4.0x12 mm. The sample was loaded in a three-point bending setup. The axial force of 600 N was applied on the dental implant. The Digital Image Correlation method was used for strain and displacement measurement. The highest von Mises strain of 0.7 % is located in the area of implant neck. The maximum displacement value in loading direction was 0.466 mm. Surface strain and displacement are correlated with implant geometry. This experimental methodology can be utilized to estimate dental implant load transfer characteristics.
AB  - Dizajn dentalnih implanata utiče na vrednosti deformacija koje nastaju u njihovoj okolini, i ako se pređe granična vrednost od 0.3 %, može doći do resorpcije kosti. Cilj ove studije je da predstavi novi eksperimentalni pristup za analizu uticaja geometrije dentalnih implanata na deformacije okolne strukture. Model kosti, sa dimenzijama od 68h25h9 mm, je napravljen od polimetil-metakrilata. Kalup za model koji je napravljen pomoću tehnike 3D štampe, je takođe služio i kao fiksator položaja za vertikalno postavljeni implant Štrauman ø4.0x12 mm. Uzorak je opterećen na savijanje u tri tačke. Aksijalna sila od 600 N je primenjena na dentalni implant. Za merenje deformacija i pomeraja je korišćena metoda Digitalne korelacije slika. Najveće vrednosti Von Mizesovih deformacija od 0.7 % su izmerene u području vrata implanta. Maksimalne vrednosti pomeraja u pravcu opterećenja su iznosile 0.466 mm. Površinska deformacija i pomeraji su povezani sa geometrijom implanata. Ova eksperimentalna metodologija se može koristiti u cilju određivanja karakteristika prenosa opterećenja dentalnih.

Stress and strain analyses of removable partial denture abutment tooth in relation to the position of the minor connector

AB  - Introduction/objective: For optimum loading distribution, the angle formed by the occlusal rest and the vertical minor connector from which it originates should be less than 90°. The objective of the article was to visualize the optimum angle between the occlusal rest and the minor connector in terms of intensity and distribution of occlusal loads using finite element analysis. It was the intention, concerning biomechanical behavior, to document that the optimum angle between the occlusal rest and the minor connector should be less than 90°. Methods: Three different virtual models of partial edentulous Kennedy III class were created using the CATIA design computer program with different angles between the occlusal rest and the minor connector. Stress distribution after simulated occlusal loading was analyzed using the finite element method. Results: Comparing the results obtained for three models, the highest stress values were seen in model 3 (the angle between the occlusal rest and the small connector is greater than 90°) whether the load is applied in the middle or at the end of the saddle. Conclusion: Within limitations and on the basis of the study results, the minimum compressive stress was seen in model 1, where the angle between the occlusal rest and the minor connector was less than 90° whether the load is applied in the middle or at the end of the saddle. It is recommended that obtuse angle between the rest and the minor connector should be avoided due to potential hazardous stress concentration on abutment teeth.
AB  - Uvod/Cilj: Okluzalni naslon i mala spojnica treba da zaklapaju međusobni ugao manji od 90 stepeni, kako bi se obezbedilo najpovoljnije prenošenje opterećenja. Cilj rada je bio da se metodom konačnih elemenata prikaže ugao između okluzalnog naslona i male spojnice koji je najpovoljniji za prenošenje okluzalnog opterećenja. Namera je bila da se, posmatrano sa biomehaničkog aspekta, dokumentuje da je ugao manji od 90 stepeni između okluzalnog naslona i male spojnice najpovoljniji. Metod: Izrađena su tri različita virtuelna modela krezube vilice klase krezubosti Kenedi III u programu CATIA sa modelovanim različitim uglovima između okluzalnog naslona i male spojnice. Analiza distribucije napona i deformacija nakon simuliranog okluzalnog opterećenja izvršene su metodom konačnih elemenata. Rezultati: Posle simuliranog okluzalnog opterećenja sva tri modela najveći napon je uočen kod modela 3 (ugao između okluzalnog naslona i male spojnice veći od 90 ste- peni), bez obzira na to da li je opterećenje aplikovano na sredini ili na kraju sedla. Zaključak: U okviru ograničenja u istraživanju, najmanji kompresioni napon uočen je u modelu 1 (ugao između oklu- zalnog naslona i male spojnice manji od 90 stepeni) bez obzira na to da li je opterećenje aplikovano na sredini ili na kraju sedla. Preporučuje se da se tup ugao između okluzalnog naslona i male spojnice izbegava zbog mogućih štetnih koncentracija napona na retencionom zubu.

Does the Prevalence of Periodontal Pathogens Change in Elderly Edentulous Patients after Complete Denture Treatment?

AB  - Purpose: To determine if wearing complete dentures can cause changes in prevalence of some of the most common periodontal pathogens in elderly edentulous patients. The need for understanding the composition of oral microflora in edentulous patients has been recognized by some authors, but no studies have dealt with the changes that occur in periodontal pathogens' prevalence as a result of complete dentures. Materials and Methods: A total of 30 edentulous elderly (average age 71) patients participated in the study. Complete dentures were fabricated for each patient, and the residual alveolar ridges were swabbed before denture insertion. After a period of 6 months swabs were taken again. Identification of P. intermedia, A. actinomycetemcomitans, P. gingivalis, T. forsythia, T. denticola, and F. nucleatum was done by the polymerase chain reaction (PCR) method and primers specific for each microorganism. Results: A noticeable increase in the presence of periodontal pathogens was observed after 6 months of denture wearing; targeted bacteria were identified in 17 pre-insertion samples compared to 28 post-insertion samples. The McNemar test was used to compare the prevalence of periodontal pathogenic bacteria before and after dental treatment. p lt 0.05 indicated statistical significance. Three microorganisms showed a statistically significant difference between the first and second swabbing-A. actinomycetemcomitans (6.7% vs. 40.0%, p = 0.006), P. intermedia (30.0% vs. 73.3%, p = 0.004), and T. forsythia (6.7% vs. 30.0%, p = 0.004). There was also an increase in bacteria co-associations 6 months post-insertion of complete dentures. Conclusions: The results of the present study suggested that wearing complete dentures caused a considerable increase of periodontopathic bacteria prevalence in elderly patients. Better understanding of oral microflora and the impact dental treatment has on bacterial colonies is important in modern dentistry.

AB  - This study aimed to assess the effectiveness of various combinations of the implant-supported overdentures and removable partial dentures on nutritional status in a randomized sample of 150 older adults. The body mass index (BMI) and the Mini Nutritional Assessment (MNA) were recorded before and after prosthetic rehabilitation. There was statistically significant improvement in the MNA and BMI, measured before prosthetic rehabilitation, and after 6 and 12 months (P  lt  .001). The prosthetic rehabilitation of patients 65 years and older, using implant-supported overdentures and removable partial dentures, improved BMI and MNA scores used in the assessment of nutritional status.

AB  - Purpose: To assess the changes in oral health-related quality of life (OHRQoL) and satisfaction among older adults after receiving new complete dentures, and the association of age, gender, education, medical history, past prosthetic history (number of previous complete dentures and time since current complete dentures) and quality of existing complete dentures with patients' ratings of satisfaction with their complete dentures. Materials and Methods: The study comprised 114 community-dwelling adults >= 65 year of age, all wearing complete dentures in both jaws. The sample was selected from four senior day centres. Data were collected using clinical oral examinations and a self-administered questionnaire. Dentures were clinically evaluated for retention, stability, occlusion, articulation and vertical dimension. The questionnaire recorded data on sociodemographic information, previous denture history, oral satisfaction scale (OSS), and OHRQoL through the Oral Impacts on Daily Performance (OIDP) scale. Results: There was a significant improvement in OHRQoL after treatment (mean OIDP score: 2.7) compared to before treatment (mean OIDP score: 6.6). The largest changes were in relation to impacts on eating (33.3% to 15.9%) and smiling (17.5% to 4.5%). Six months after placement of the dentures, patient satisfaction improved compared to before treatment (p  lt  0.001). The results of this study demonstrated that satisfaction was correlated with age, gender, and past prosthetic history in the patients rehabilitated with complete dentures. Conclusions: The provision of new dentures among older adults resulted in significantly better satisfaction and OHRQoL than wearing existing dentures. There was no significant correlation between the clinical assessment of the dentures and patients' satisfaction with them.

AB  - Strain analysis in the vicinity of the dental implants were the subject of many research papers. In this study, new experimental method was employed for strain determination in the surrounding area of the axially loaded dental implant, which was embedded in the resin block. This studies can improve understanding of strain distribution, and its possible effects on the surrounding structure. Dental Implant Branemark with dimensions of 14 x 3.75 mm was immersed into rectangular block of polymethil methacrylate in vertical position. Implant was distanced 2 mm and 4 mm from the block sides, and they were named as the surface 1 and surface 2, respectively. Axial load, in the range of 0 - 500 N, was applied using Universal testing machine Tinius Olsen. Digital Image Correlation method was used for measuring deformation. Results were presented in the form of horizontal and vertical strains. Horizontal strains were mostly tensile, with maximum value of 0.3 % on the bottom of the surface 1. This values decreased in the upper area of the block side. Vertical compressive strain were 2-3 times higher on the surface 1, when compared to the surface 2, with maximum values of 0.7 % located on the bottom of the surface 1, and 0.3 % on the bottom of the surface 2. Resin block models could be used for determination of strain distribution under axially loaded implants. Greatest strain concentration were located under the implants apex, in the bottom area of the block. It was concluded that, in this region, strain values decreased linearly with increase of distance from the implants surface.

Association Between Dentition Status and Malnutrition Risk in Serbian Elders

AB  - Purpose: The aim of this study was to find the correlation between the number, type, and placement of symmetric functional tooth units and the nutritional status using body mass index (BMI) and Mini Nutritional Assessment (MNA) in patients with both maxillary and mandibular removable partial dentures. Materials and Methods: A randomized study was conducted at the Clinic for Prosthodontics, School of Dentistry, University of Belgrade, Serbia, followed by determination of number of functional tooth units. After insertion of the prostheses, the nutritional status was assessed, using BMI and MNA form. Nutritional status was reviewed 12 months after prosthetic treatment. Number of teeth and age were analyzed using analysis of variance and t test. For other variables such as BMI baseline, BMI follow-up, MNA baseline, MNA follow-up, change in BMI, and change in MNA, Kruskal-Wallis Test or Mann-Whitney Test were used. Results: The results on the MNA changes showed that patients with symmetric natural functional tooth units had significantly higher values (2.9 +/- 0.8) compared with patients without natural functional tooth units (1.9 +/- 0.6) or with asymmetric natural functional tooth units (1.6 +/- 0.4). Patients with asymmetric artificial functional tooth units had significantly lower change in MNA (1.9 +/- 0.4) than those with symmetric artificial functional tooth units (2.6 +/- 0.9). Conclusion: The findings of this study suggest that adequate rehabilitation with symmetric positioning of the functional tooth units can improve the nutritional status of partially edentulous patients.

AB  - The purpose of this study was to investigate the influence of the abutment angulation upon the strain distribution pattern for the vertical loading situation by means of the digital image correlation (DIC) method. In addition, to find the correlation between acrylic-layer thickness around implant body and surfaces strain induced by vertical loads. Two types of samples consisted of the Straumann (R) cylindrical dental implant system (4x10mm) with the SLActive (R) surface and the poly-methyl-methacrylate were used in this study. For strain analysis, the DIC system was used, manufacturer GOM. The optical deformation measurement system consists of special set of stereo cameras and lenses, and ARAMIS software (6.2.0, Braunschweig, Germany). Maximum von Mises strain was 0.30% in the sample with the straight abutment and 0.50% in the sample with the angled abutment. Minimum strain measured by Aramis was 0.01%, detected in the 6mm surface layer of the sample with straight abutment. According to results obtained by Aramis data processing, the 4mm surface layer indicated greater overall strain in apical direction with the strains of 0.18-0.50%, depending on the force intensity. Higher strain was noticed in the thinner surface layers. The angulated abutment induced higher strain in both surface layers than the straight abutment did.

AB  - In this paper, finite element model of edentulous lower jaw (mandible) is generated, as well as the model of support layer and complete denture using various software. The aim of this study was to produce 3D digital mandibular complete denture, mucosa, and supporting bone models using reverse engineering and evaluate effect of mucosa thickness and resiliency on stress distribution of complete denture. In this study we used a model of mandibular complete denture and 5 mm thickness of mucosa (which represent the resiliences soft). The influence of resilient mucous membranes layer thickness is analysed by assigning loads to simulate the force due to chewing on the lower right and left central incisor and lower right and left first molar. Based on the numerical results, we conclude that the maximum stress values were concentrated in cortical bone.

AB  - Purpose: This study was conducted to identify the prevalence of partially edentulous individuals in Serbia and to identify prosthodontic treatment used for their rehabilitation as well as to compare prosthodontic treatment concepts among Serbian elderly participants who answered closed-format questionnaires. Materials and Methods: The study was conducted at the Clinic for Dental Prosthetics, University of Belgrade, Military Medical Academy and the Department of Prosthodontics of Home Centre Belgrade; the study consisted of 120 participants. A questionnaire with closed-format questions regarding the influence of applied therapy concepts for participants with Kennedy Class I partial edentulism was created. Statistical software SPSS 11.5 was used for data analysis. Chi-square tests were performed, and p-values were calculated for each parameter. Results: A total of 1081 people were examined; 730 participants were partially edentulous in at least one jaw. Five hundred forty-one participants wore Kennedy Class I maxillary or mandibular removable partial dentures (RPDs). Ninety percent of the patients were restored with RPDs; 60% of participants were unrestored, had shortened dental arches, and requested alternative therapeutic solutions. Significantly lower needs for new treatments were identified in participants with fixed dental prostheses (7.5%). Conclusions: Kennedy Class I was the most common type of partial edentulism among Serbian elderly, usually treated with RPDs. Materials and Methods: The study was conducted at the Clinic for Dental Prosthetics, University of Belgrade, Military Medical Academy and the Department of Prosthodontics of Home Centre Belgrade; the study consisted of 120 participants. A questionnaire with closed-format questions regarding the influence of applied therapy concepts for participants with Kennedy Class I partial edentulism was created. Statistical software SPSS 11.5 was used for data analysis. Chi-square tests were performed, and p-values were calculated for each parameter. Results: A total of 1081 people were examined; 730 participants were partially edentulous in at least one jaw. Five hundred forty-one participants wore Kennedy Class I maxillary or mandibular removable partial dentures (RPDs). Ninety percent of the patients were restored with RPDs; 60% of participants were unrestored, had shortened dental arches, and requested alternative therapeutic solutions. Significantly lower needs for new treatments were identified in participants with fixed dental prostheses (7.5%). Conclusions: Kennedy Class I was the most common type of partial edentulism among Serbian elderly, usually treated with RPDs. Elderly participants with the applied shortened dental arch and RPD therapy concepts requested alternative therapeutic solutions more often than those rehabilitated with fixed partial dentures.",

Frequency and quality of root canal fillings in an adult Serbian population

AB  - Introduction Estimation of frequency and quality of root canal fillings is the way to evaluate endodontic treatment needs and success/failure rates of performed endodontic procedures. Objective The aim of this study was to determine the frequency and quality of root canal fillings and the frequency of apical periodontitis on endodontically treated teeth in a group of adult patients from Serbia. Methods In order to analyze the presence and quality of root canal fillings and the frequency of periapical radiolucencies on endodontically treated teeth, 3526 teeth were examined on orthopantomograms of 153 adult patients. Results Overall, 12.5% of examined teeth were root filled, and 51.8% of them had radiographic signs of apical periodontitis. The analysis of root fillings quality revealed the presence of more inadequate ones (55.9%). The frequency of apical periodontitis was significantly higher in teeth with inadequate than in those having adequate root canal obturation (72.2% and 25.9%, respectively). Conclusion The frequency of apical periodontitis on root-filled teeth in this group of patients was high, indicating a low success rate of performed endodotic procedures and high endodontic retreatment needs.
AB  - Uvod Utvrđivanje učestalosti i kvaliteta punjenja kanala zuba jeste način da se procene potrebe za endodontskim lečenjem i stepen uspeha primenjenih endodontskih procedura. Cilj rada Cilj ovog rada je bio da se odrede učestalost i kvalitet punjenja kanala korena zuba, kao i učestalost apeksnih parodontitisa endodontski lečenih zuba kod odraslih pacijenata u Srbiji. Metode rada Radi utvrđivanja postojanja i kvaliteta punjenja kanala korena i učestalosti rasvetljenja u periapeksu endodontski lečenih zuba, analizirano je 3.526 zuba na ortopantomogramima 153 odrasle osobe. Rezultati Ukupno je endodontski lečeno 12,5% pregledanih zuba, od kojih je 51,8% imalo radiografske znake apeksnih parodontitisa. Analiza kvaliteta punjenja kanala pokazala je da je bilo više neadekvatnih punjenja (55,9%). Učestalost apeksnih parodontitisa je bila značajno veća kod zuba s neodgovarajućim kanalnim opturacijama (72,2%) u odnosu na one s odgovarajućim punjenjem kanala korena (25,9%). Zaključak Kod ispitanih pacijenata utvrđena je visoka učestalost apeksnih parodontitisa zuba s punjenim kanalima korena, što ukazuje na slabu uspešnost primenjenih endodontskih zahvata, kao i na potrebu za ponovljenim endodontskim lečenjem.

AB  - This study analyzed the influence of platform switching on the stress/strain distribution of vertically loaded implants. Employing finite element analysis, the aim of this study was to determine the distribution of stresses and strains generated by a simulated vertical load on different implant models positioned in the molar region with respect to changes of implant design and biomechanics. Two different implant designs, one with and one without platform switching, with the same length (11.5 mm) and diameter of (3.80 mm) were analyzed. Both virtual implant-bone models were subjected to a load of 500 N. The results showed that the maximum stress/strain in implant-bone samples was located in the area of the implant neck, with stress/strain values decreasing in the apical direction. It was found that the implant design with platform switching developed lower stresses and strains than the implant design without platform switching.

Biomechanical interactions between bone and metal-ceramic bridges composed of different types of non-noble alloys under vertical loading conditions

AB  - The purpose of this study was to compare the mechanical properties of three metal-ceramic bridges of different types of dental alloys and to present and evaluate the possible biomechanical interactions between a marginal bone and metal-ceramic bridges during vertical loading. The research was done as an experimental study. A mandible with an intact anterior region was used. The preparation of the remaining teeth for receiving three types of porcelain-fused-to-metal (PFM) restorations was performed. Vita VMK 95 was used for all three metal-ceramic restorations. These three metal-ceramic bridges composed of different alloys, nickel and non-nickel, served as different models: the Niadur-nickelferous model, the Wiron 99-nickel model and the Wirobond C-cobalt-chrome model. The maximum compressive strain of 5% for all three virtual models is observed in the region of central incisors. The Niadur model has the lowest mean strain (2.62%) in comparison with the other two models. The mean strain of Wiron 99 is lower, by 0.10%, than the mean strain of the Wirobond model. Biomechanical behavior of the presented models caused by the vertical-loading conditions is explained as an interaction between the marginal bone and the metal-ceramic bridges. All of them, nickel and non-nickel models, indicate a similar strain (deformation) distribution; however, from the biomechanical perspective, Niadur is more favorable than the other two materials.

Treatment of obstructive sleep apnea patients using oral appliances: Our experiences

AB  - Background/Aim. Obstructive sleep apnea (OSA) is one of the most prevalent sleep disorders. It is recognized as a serious risk factor for car and workplace accidents due to daytime sleepiness, and factor for coronary heart diseases and stroke. The aim of this study was to examine the effectiveness of oral appliances for mandibular advance in treating mild to moderate OSA. Methods. A total of 15 patients were included in this study, all diagnosed with mild or moderate OSA. Oral appliances were custom made for each patient in protrusive position at 50% of maximum mandibular advancement. The patients were given instructions not to sleep on their backs and avoid alcohol consumption during the study as these are the factors that can contribute to symptoms progression. Results. Complete and partial treatment success was achieve in 14 of the patients. Apnea-hypopnea index values were significantly lower (p  lt  0.05) at the end of a 6-month observation period compared to those at the treatment beginning. A great improvement in symptoms was observed, with daytime sleepiness index values significantly reduced already within the first month of the treatment. Conclusion. Treatment of obstructive sleep apnea with oral appliances has proven successful. Patients were comfortable using oral appliances and were ready to wear them for prolonged period of time. Use of oral appliances is very common in the world and should not be discarded. They are also very comfortable, practical and affordable comparing to continuous positive airway pressure (CPAP) apparatus, not to mention surgery. Use of oral appliances is safe and very well tolerated, and ought to be offered to patients with OSA.
AB  - Uvod/Cilj. Opstruktivna apneja u snu je jedan od najčešćih poremećaja spavanja. Dokazano je da su bolesnici koji pate od ovog poremećaja skloniji saobraćajnim nezgodama i povredama na radu, kao i da su izloženi većem riziku od nastanka koronarne bolesti srca i moždanog udara. Cilj ovog istraživanja bio je da se utvrdi uspešnost terapije blage i umerene opstruktivne apneje u snu oralnim aparatima. Metode. Ukupno 15 bolesnika sa prethodno uspostavljenom dijagnozom blage ili umerene opstruktivne apneje u snu bilo je uključeno u istraživanje. Oralni aparati su individualno urađeni za svakog bolesnika, sa mandibulom u protruzionom položaju 50% od maksimalnog. Bolesnicima je preporučeno da izbegavaju spavanje na leđima i unošenje alkohola tokom trajanja studije, jer ovi faktori mogu uticati na pogoršanje simptoma. Rezultati. Potpuni i delimični uspeh terapije postignut je kod 14 bolesnika. Vrednosti apneja-hipopneja indeksa bile su statistički značajno niže (p  lt  0,05) od početnih vrednosti na kraju 6-mesečnog perioda posmatranja. Registrovano je i značajno poboljšanje simptoma, kao i značajno umanjenje vretnosti indeksa dnevne pospanosti već posle prvog meseca terapije. Zaključak. Oralni aparati su bili uspešni u lečenju opstruktivne apneje u snu. Bolesnici su iskazali veliku lagodnost i bili su spremni da nose oralne aparate i duži vremenski period. Upotreba oralnih aparata je uobičajena u svetu, i ne treba da bude zanemarena. Vrlo su praktični, lagodni za nošenje i pristupačniji u poređenju sa aparatom za kontinuirani pozitivni pritisak u disajnim putevima (CPAP), a pogotovo sa hirurškom intervencijom. Upotreba oralnih aparata je bezbedna i trebalo bi je ponuditi bolesnicima kao jedno od mogućih rešenja.

They are also very comfortable, practical and affordable comparing to continuous positive airway pressure (CPAP) apparatus, not to mention surgery. Use of oral appliances is safe and very well tolerated, and ought to be offered to patients with OSA., Uvod/Cilj. Opstruktivna apneja u snu je jedan od najčešćih poremećaja spavanja. Dokazano je da su bolesnici koji pate od ovog poremećaja skloniji saobraćajnim nezgodama i povredama na radu, kao i da su izloženi većem riziku od nastanka koronarne bolesti srca i moždanog udara. Cilj ovog istraživanja bio je da se utvrdi uspešnost terapije blage i umerene opstruktivne apneje u snu oralnim aparatima. Metode. Ukupno 15 bolesnika sa prethodno uspostavljenom dijagnozom blage ili umerene opstruktivne apneje u snu bilo je uključeno u istraživanje. Oralni aparati su individualno urađeni za svakog bolesnika, sa mandibulom u protruzionom položaju 50% od maksimalnog. Bolesnicima je preporučeno da izbegavaju spavanje na leđima i unošenje alkohola tokom trajanja studije, jer ovi faktori mogu uticati na pogoršanje simptoma. Rezultati. Potpuni i delimični uspeh terapije postignut je kod 14 bolesnika. Vrednosti apneja-hipopneja indeksa bile su statistički značajno niže (p  lt  0,05) od početnih vrednosti na kraju 6-mesečnog perioda posmatranja. Registrovano je i značajno poboljšanje simptoma, kao i značajno umanjenje vretnosti indeksa dnevne pospanosti već posle prvog meseca terapije. Zaključak. Oralni aparati su bili uspešni u lečenju opstruktivne apneje u snu. Bolesnici su iskazali veliku lagodnost i bili su spremni da nose oralne aparate i duži vremenski period. Upotreba oralnih aparata je uobičajena u svetu, i ne treba da bude zanemarena. Vrlo su praktični, lagodni za nošenje i pristupačniji u poređenju sa aparatom za kontinuirani pozitivni pritisak u disajnim putevima (CPAP), a pogotovo sa hirurškom intervencijom. Upotreba oralnih aparata je bezbedna i trebalo bi je ponuditi bolesnicima kao jedno od mogućih rešenja.",

A preliminary evaluation of normative and realistic needs for prosthetic treatment among frail elderly patients in Serbia

AB  - Background/purpose: Common to all frail elderly patients in Serbia is the fact that only little or no attention is given to their dental care. A normative need for prosthetic treatment exists for all partially edentulous and edentulous patients who lack adequate prosthetic devices. The realistic need for treatment means the opportunity to perform the treatment that will be beneficial for each patient individually. The goal of this study was to determine the normative and realistic needs for prosthetic treatment among frail elderly patients in Serbia. Materials and methods: The study involved 117 elderly and frail patients provided service at the Geriatrics Institute in Belgrade, and among families in suburban municipalities. We determined the patients' dental status, cognitive status, general health condition, and desire for treatment. Results: Results showed that 82.9% of patients had normative need for prosthetic treatment. Realistic need for prosthetic treatment was significantly lower-that is 19.7% to 28.2% of patients can be provided appropriate prosthetic treatment in order to improve their oral functionality and quality of life. Conclusion: Although realistic need for prosthetic treatment is significantly lower than normative need, treatment shouldn't be denied to patients who can receive it, especially when the treatment recommended is less invasive.
