Efficiency of root canal cleaning using a single instrument with reciprocating motion systems and full rotation systems: SEM study

AB  - Introduction Efficient cleaning and canal design present a basic precondition for successful endodontic treatment. The aim of this study was to evaluate, using SEM analysis, the efficacy of cleaning the apical third of the canal with one instrument in both reciprocating motion and full rotation systems. Materials and methods Thirty (30) single-rooted teeth (upper incisors), extracted due to periodontal problems, were used and selected in 2 groups. In the group 1, instrumentation was done with a single instrument UNICONE with reciprocal movements (MEDIN, Czech Republic), while in the group 2, a single instrument XPS endo SHAPER (XPS) (FKG, Dentaire, Swiss) with full rotation was used. 2% solution of NaOCl and 17% EDTA solution were used as irrigation agents in both groups. SEM analysis of the smear layer in the apical third was done at standard magnification of 1000x and presented on a scale of 1-5. Results showed mostly clean canal walls and no smear layer in the apical region in both tested groups. Canal walls were somewhat cleaner after using UNICONE with reciprocal movements (80%) compared to the canals instrumented with XPS ENDO SHAPER (76%), but this difference was not statistically significant. Conclusion Single-file NiTi systems did not remove smear layer completely, but provided efficient cleaning in the api-cal part of the canal. UNICONE file with reciprocal movements was somewhat more efficient in removing the smear layer compared to the full rotation XPS ENDO SHAPER file.
AB  - Uvod Efikasno čišćenje I oblikovanje kanala je osnovni preduslov za uspeh endodontskog tretmana. Cilj ovog rada je bio da se SEM analizom proveri efikasnost čišćenja apikalne trećine kanala jednim instrumentom kod sistema sa recipročnim pokretima I sistema sa punom rotacijom. Materijal I metode Korišćeno je 30 jednokorenih zuba (gornji sekutići) ekstrahovanih zbog parodontalnih problema I podeljenih u dve grupe. U prvoj grupi preparacija kanala je realizovana instrumentom sa recipročnim pokretima - UNICONE (MEDIN, Czech Republic), a u drugoj instrumentom sa punom rotacijom - XP-endo Shaper (XPS) (FKG, DENTAIRE, Swiss). Kao sredstvo za irigaciju u obe grupe korišćen je 2% rastvor NaOCl I 17% rastvor EDTA. SEM analiza razmaznog sloja apikalne trećine je urađena na standardnom uvećanju od 1000× I procenjivana prema skali od 1 do 5. Rezultati Istraživanje je ukazalo na uglavnom čiste zidove kanala I bez razmaznog sloja u apikalnoj regiji u obe testirane grupe. Zidovi kanala su bili nešto čistiji posle primene instrumenta sa recipročnim pokretima - UNICONE (80%) u odnosu na kanale druge grupe gde je korišćen instrument sa punom rotacijom - XPS (76%), ali bez statistički značajnih razlika. Zaključak NiTi sistemi sa samo jednom turpijom ne uklanjaju potpuno razmazni sloj, ali obezbeđuju efikasno čišćenje apikalnog dela kanala. Turpija sa recipročnim pokretima UNICONE je bila nešto efikasnija u uklanjanju razmaznog sloja u odnosu na XPS turpiju sa punom rotacijom.

The application of MTA as apical plug for root canal obturation: In vitro study

AB  - Introduction Prognosis of endodontically treated tooth is directly correlated to the quality of apical obturation. Modern concept of endodontics in particular way points out the quality of apical filling. The aim of this study was to assess the quality of root canal obturation with MTA apical plug using the method of gas (argon) penetration. Material and Methods Sixty-six freshly extracted single rooted (single canal) teeth were divided into the two experimental (30 teeth) and one control group (6). All canals were instrumented using hand and rotary files in step-back technique and copious irrigation of 1%NaOCl. In the first experimental group teeth were obturated using different sealers: Gutta Flow (Roeko), AH Plus (DeTrey), Acroseal (Septodont) and mono gutta-percha cone (10 canals each). In the second experimental group obturation involved 3 mm of MTA-Angelus apical plug while the remaining canal space was filled with the same three sealers as in the first group. The rate of gas permeability by Leak detector-Edwards LD 416 was measured in all teeth. Results The best seal was found in teeth obturated with Gutta Flow and MTA plug with average diffusion rate of 264.4 sec while the worst quality of obturation was found with Acroseal (178.5 sec-the highest gas permeability). All samples with MTA plug exhibited significantly lower leakage than the samples filled without MTA apical plug. Conclusion Root canals filled with MTA apical plug exhibited statistically significant lower gas permeability in comparison to the ones filled with sealer and guttapercha cones only.
AB  - Uvod Prognoza uspeha endodontski treatiranih zuba je u direktnoj korelaciji sa kvalitetom opturisanog endodontskog prostora. Savremena koncepcija endodontskog tretmana poseban akcenat stavlja na kvalitet preparacije i opturacije apeksnog dela kanala. Cilj ovog rada je bio da se metodom merenja gasne propustljivosti (argona) proveri kvalitet opturacije kanala korena zuba nakon primene mineral-trioksid agregata (MTA) kao apeksnog čepa. Materijal i metod U istraživanje je uključeno 66 jednokorenskih-jednokanalskih zuba razvrstanih u dve grupe (po 30 zuba), dok je šest zuba korišćeno kao kontrola. Preparacija svih kanala je urađena ručnim instrumentima i primenom step-back tehnike uz obilnu irigaciju 1% NaOCl. U prvoj grupi zuba je opturacija realizovana silerima uz pomoć gutaperka konusa Gutta Flow-Roeko (10), AH Plus-DeTrey (10) i Acroseal-Septodont (10). Drugu grupu su činili zubi kod kojih je u apeksnom delu kanala aplikovan MTA, a potom su kanali ispunjeni primenom tri silera kao u prvoj grupi. Svi zubi iz obe grupe su podvrgnuti metodi merenja brzine gasne propustljivosti pomoću aparata Argon Leak Detector, Edwards LD 416. Rezultati Najbolju hermetičnost su pokazali uzorci kanala ispunjeni primenom Gutta Flow+MTA sa prosečnom brzinom (264,4 sek.), a najslabiji kvalitet opturacije sa Acroseal silerom (najveća gasna propustljivost 178,5 sek.). Svi uzorci druge grupe sa MTA čepom su pokazali statistički značajno manju propustljivost nego oni u prvoj grupi sa silerom i gutaperkom bez MTA čepa. Zaključak Opturacija kanala sa postavljenim MTA apeksnim čepom pokazala je značajno manju propustljivost gasa u poređenju sa uzorcima ispunjenim endodontskim silerom i gutaperka konusima.

Traditional dentistry knowledge among Serbs in several Balkan countries

AB  - Background/Aim: There are still unrevealed treasures of traditional dental medicine, that is, the reason to investigate and present various ways in treatment of oral and orofacial tissues, as well as magic and religious elements involved in representative areas among Serbs. Materials and Methods: Information was collected from the elderly non-professional folk dentists and herbalists with the additional help of local physicians and dentists that was done through questionnaire and personal interviews. Results: Classified and prepared material consists of total 1038 inquiry sheets. The 41 data were averagely obtained by inquiry form, i.e. 41,984 information for the whole research. The most voluminous was the group of 64 recipes, including 39 for gums diseases and 25 for toothache, while only seven ones were mentioned for magic way of treatment. Among them, 18 prescriptions were of nonherbal origin. The study revealed 84 herbal original prescriptions, including 67 plant species (29 families) including local name, synonyms, and preparation mode. Traditional healers used predominantly herbal recipes to treat painful tooth, gum disease, blisters - herpetic ulcers/lips and mouth/, stomatitis/painful mouth, ptyalismus/, maxillary sinusitis, bad breath, teeth cleaning and bleaching. Very few methods of treatment appeared as inadequate (magical practice), whereas majority were noted as beneficial ones (herbal medicine). Still many people in distant nonurban areas use various plant recipes, especially as the first aid in oral disease healing. Conclusions: The significance of plants obtained from unpolluted areas, whose active ingredients have not yet been used in dental pharmaceutics, should be further investigated in the future.

AB  - Endodontic treatment of the teeth with severe root canal curvatures has become one of the greatest problems in clinical practice. Recently, new instruments and techniques have been introduced in the endodontic procedure. The aim of this study was, on the basis of clinical practice, to show the possibilities of the endodontic treatment in the teeth with severely curved root canals, after dependable decision about endodontic instruments and techniques. This article presents a complete endodontic procedure in two teeth with different curvature shape of the root canal in patients that were treated at the Department of Restorative Dentistry and Endodontics, School of Dentistry, Belgrade. Coronal to apical 'crown down' technique was used for the root canal preparation with hand NiTi ProTaper instruments with progressive multicone design. The main reasons for choosing manual technique were complexity, shape of presented curvatures and better tactile sensation control of canal instruments in order to avoid possible mistakes and complications. Clinical X-rays of the teeth with permanent obturation clearly showed that extremes of the root canal morphology could be successfully solved by the right choice of all factors during endodontic procedure.
AB  - Endodontsko lečenje zuba s jako zakrivljenim korenovima je značajan problem u praksi. Primena novih instrumenata i tehnika u lečenju ovakvih zuba poslednjih godina je postala sve izvesnija.Cilj ovog rada je bio da se na primerima iz kliničke prakse ukaže na mogućnost endodontskog lečenjazuba s veoma izraženim krivinama kanala korena nakon pravilnog izbora endodontskih instrumenata, odnosno tehnike instrumentacije. U ovom radu je prikazan celokupni endodontski postupak lečenja dva zuba pacijenata Klinike za bolesti zuba Stomatološkog fakulteta u Beogradu sa različitim oblicima krivine centralnog korenskog kanala. Prime-njena je krunično-apeksna 'crown-down' tehnika preparacije uz korišćenje ručnih nikl-titanijumskih instrumenata tipa ProTaper s progresivnom multikoničnošću. Osnovni razlozi za odabir ručne tehnike preparacije bili su složenost morfologije, oblik prikazanih krivina i bolja taktilno-senzorna kontrola kanalnih instrumenata; cilj je bio da se izbegnu moguće greške i komplikacije. Prikazani radiografski snimci zuba sa jasno opturiranim kanalnim sistemom pokazuju da su i ekstremi morfološkog izgleda kanala rešivi pravilnim odabirom svih činilaca endodontske procedure. .

The prevalence of oval root canals in the apical and middle third of mandibular molars and premolars

AB  - Introduction. Oval root canals are all groups of human teeth. In such canals it is a challenge to fulfill the necessary preconditions for a successful outcome of root canal therapy - complete cleaning, shaping and obturation. Aim. The aim of this study was to investigate the shape and diameter of root canals in the apical and middle third of human mandibular molars and premolars, to determine the prevalence and extent of long oval canals. Materials and methods. The study was carried out on 80 extracted human teeth, 40 molars and 40 premolars. The teeth were placed in a specially designed muffle mould and embedded in transparent acrylic resin. After the polymerization of the acrylic resin, the teeth were taken out of the mould and sectioned at levels 3 and 6 mm from the apex. The cross-sections were photographed under 30Х magnification using a digital camera. The long and short canal diameter were measured on the images and their ratio calculated. Results. In the apical third 22% of the premolars and 35% of the molars had oval root canal morphology. In the middle third 36% of premolars and 41 % of molars. Conclusion. Root canals of mandibular molars and premolars are often oval in their cross-section. They seem to be more frequently oval in their cross-section than previous studies have shown.
AB  - Uvod. Kanali korena gotovo svih grupa zuba su obično ovalnog oblika na poprečnom preseku. Ovakav morfološki oblik kanala predstavlja poseban izazov za potpuno instrumentaciju i opturaciju, a samim tim i za uspešan ishod terapije. Cilj. Cilj ovog rada bio je ispitivanje oblika i dijametra korenskih kanala humanih donjih molara i premolara u apeksnoj i srednjoj trećini kako bi se utvrdila prevalencija morfoloških oblika kanala. Materijal i metod. Istraživanje je sprovedena na 80 ekstrahovanih humanih donjih zuba (40 molara i 40 premolara). Zubi su postavljeni u posebno dizajniran kalup i zaliveni providnim samopolimerizujućim akrilatom Zatim su zubi sečeni mikrotomom na nivoima od 3 i 6 milimetara udaljenosti od apeksa. Poprečni preseci su fotografirani digitalnom kamerom pod uveličanjem od 30X. Na fotografijama zuba vršeno je merenje dužeg i kraćeg prečnika kanala korena i izračunavanje njihov odnos. Rezultati, U apeksnoj trećini korena 22% premolara i 35% molara imalo je ovalan oblik korenskog kanala. U srednjoj trećini ovakvu morfologiju pokazivalo je 36% premolara i 41%molara. Zaključak. Korenski kanali donjih molara i premolara su u velikom procentu ovalnog poprečnog preseka. Ovako čest morfološki oblik kanala identifikovan u ovim grupama zuba je značajan podatak koji može uticati na izbor tehnike preparacije.
