AB - Introduction/Objective. We aimed to investigate the effects of diabetes mellitus (DM) on rat dental pulp repair by measuring time-dependent changes in expressions of vascular endothelial growth factor (VEGF) and bone morphogenetic protein 2 (BMP 2) following direct pulp capping. Methods. Two groups, each of 20 Wistar rats, received either streptozotocin (for DM induction) or the same volume of sterile saline. A week later, the pulp of maxillary and mandibular right incisors in diabetic and non-diabetic groups were exposed and capped with calcium hydroxide in order to provoke reparative response. The levels of VEGF and BMP 2 were determined in the pulp tissue lysates one and seven days after the pulp capping, using enzyme-linked immunosorbent assays. Results. Diabetic state per se increased VEGF level, with a peak at first day after the pulp capping (19.3 ± 0.9 pg/mg, p < 0.001), but did not affect BMP 2 levels. Significant increase of BMP 2 expression was noticed on the seventh day in capped pulp, but only in diabetic rats (16.7 ± 1 pg/mg, p = 0.001). Positive correlation between VEGF and BMP 2 was found on the seventh day following capping, only in diabetic pulp (r = 0.905, p = 0.003). Conclusion. Diabetes-induced increase in VEGF expression reflects changes in the inflammatory phase of pulp repair in DM. Increase in BMP 2 expression suggest that stimulating effect of calcium hydroxide appears seven days after diabetic pulp capping.

AB - Anatomically, dental pulp is connective tissue and specific microcirculatory system with significant reparatory abilities intending to preserve pulp vitality. Various therapeutic approaches in the treatment of affected pulp may be compromised by various factors leading to treatment failure. Due to microcirculatory system disorders, treatment of affected dental pulp in patients with diabetes mellitus (DM) is additional challenge. The function and levels of growth factors could be altered in various diabetic tissues including dental pulp. Among them are growth factors important for reparative response of the pulp. There are experimental evidences that DM impede dental pulp reparation. Therefore, clinical procedures aiming to preserve vitality of diabetic dental pulp should be applied with caution. The aim of this paper is to present basic factors and parameters that affect reparative response of dental pulp in patients with DM.

AB - Introduction Estimation of frequency and quality of root canal fillings is the way to evaluate endodontic treatment needs and success/failure rates of performed endodontic procedures. Objective The aim of this study was to determine the frequency and quality of root canal fillings and the frequency of apical periodontitis on endodontically treated teeth in a group of adult patients from Serbia. Methods In order to analyze the presence and quality of root canal fillings and the frequency of periapical radiolucencies on endodontically treated teeth, 3526 teeth were examined on orthopantomograms of 153 adult patients. Results Overall, 12.5% of examined teeth were root filled, and 51.8% of them had radiographic signs of apical periodontitis. The analysis of root fillings quality revealed the presence of more inadequate ones (55.9%). The frequency of apical periodontitis was significantly higher in teeth with inadequate than in those having adequate root canal obturation (72.2% and 25.9%, respectively). Conclusion The frequency of apical periodontitis on root-filled teeth in this group of patients was high, indicating a low success rate of performed endodotic procedures and high endodontic retreatment needs.

AB - Statement of problem. Diabetes mellitus type 2 is associated with a variety of oral mucosal changes, including an altered level of salivary vascular endothelial growth factor. However, the authors identified no studies concerning denture stomatitis and salivary vascular endothelial growth factor in immediate denture wearers with diabetes mellitus type 2. Purpose. The purpose of this study was to investigate the incidence of and risk factors for denture stomatitis in immediate complete denture wearers with and without diabetes mellitus type 2 and to investigate the relationship between levels of salivary vascular endothelial growth factor and clinical forms of denture stomatitis in both groups. Material and methods. Individuals without diabetes (n=42) or with diabetes mellitus type 2 (n=36) who were candidates for complete immediate dentures were included in the study. After 1 year of wearing relined immediate dentures, participants were evaluated for denture stomatitis and potential contributing factors. Salivary vascular endothelial growth factor levels were measured with enzyme-linked immunosorbent assays. Data were analyzed with the chi(2) test or the Student t test where appropriate, as well as with binary logistic regression analysis. Results. The incidence of denture stomatitis was 61% in participants with diabetes mellitus type 2 and 38% in those without diabetes. Low denture stability and diabetes mellitus type 2 were risk factors for denture stomatitis. Salivary vascular endothelial growth factor concentrations at the beginning of the study were 557.6 +/- 94.7 pg/mL in participants with diabetes mellitus type 2 and 103.5 +/- 21.6 pg/mL in those without diabetes. In Newton Type I and Newton Type II denture stomatitis, vascular endothelial growth factor levels were 460.9 +/- 55.4 pg/mL and 1445.2 +/- 422.1 pg/mL in individuals with diabetes and 73.2 +/- 10.0 pg/mL and 306.5 +/- 22.6 pg/mL in those without diabetes. Conclusions. Perceived denture stability and diabetes mellitus type 2 are independent risk factors for the occurrence of denture stomatitis. Altered salivary vascular endothelial growth factor levels and denture stomatitis are more prevalent in denture wearers with diabetes mellitus type 2.

AB - Introduction. Postoperative sensitivity in restorative dentistry can be related to preparation trauma, dentin adhesives' ability to seal open dentinal tubules, deformation of restorations under occlusal stresses and microleakage. Objective. The study assessed possible reduction in postoperative sensitivity with low shrinkage compared to conventional composites using different bonding agents and the influence of the operator skill on the incidence of postoperative sensitivity. Methods. Nine hundred and sixty permanent premolars and molars with primary carious lesions from patients 21 to 40 years old were used. Cavities 2 to 3 mm deep and with margins in enamel were prepared by four operators. Two operators had five years (A and B) and two had over 20 years (C and D) of clinical experience. Teeth were divided into eight groups each contained 120 restorations: (1) Els®+James-2 (original formula), (2) Els®+James-2 (new formula), (3) Els®+Excite, (4) InTenSe®+James-2 (original formula), (5) InTenSe®+James-2 (new formula), (6) InTenSe®+Excite, (7) Tetric Ceram®+Excite, and (8) Point 4®+OptiBond Solo Plus. At 14 days postoperatively, two independent operators, who did not take part in the clinical procedure, assessed postoperative teeth sensitivity using special questionnaires. Data were analyzed using non-parametric chi-square, Mann-Whitney and ANOVA tests. Results. Group 8 showed significantly higher score than the other groups. Less postoperative sensitivity was reported with two low-shrinkage composites (groups 2, 3, and 5) but with no significant difference. There was no statistical difference between groups 1, 2, 3, 4, 5, 6 and 7. Operator A had the highest postoperative sensitivity score compared to the other three. Conclusion. Conventional composite material Point 4® with its bonding agent caused significantly more postoperative sensitivity than low shrinkage composites combined with different adhesives. Operator skill influenced the incidence of postoperative sensitivity.
Rezultati. Tip kompozitnog materijala s odgovarajućim adhezivnim sistemom i spretnost stomatologa utiču na učestalost pojave osetljivosti zuba posle restauracija srednje dubokih kaviteta II klase.

AB - Dental pulp is a connective tissue which makes embryologic, anatomic and functional entity with surrounding dentin referred to as the pulp-dentin complex. Due to specific localization within solid dentinal walls, as well as unique vasculature, pulp reactions significantly differ to reactions of other connective tissues. Commence, type and intensity of pulp reparative response depend on cellular events including upregulation of synthetic and secretory activity of odontoblasts, specific cytodifferentiation, and neovascularization at the injury site. The signaling molecules involved in induction and regulation of these events are growth factors including very important bone morphogenetic protein 2 (BMP-2) and vascular endothelial growth factor (VEGF). BMP-2 expression was found to be increased during tertiary dentinogenesis, and significant role of this growth factor in differentiation of cells involved in pulp reparative response was established. Neovascularization, regulated by VEGF activity, was found to be critical for pulp reparative reaction. Diabetes mellitus (DM) is wide spread disease causing metabolic alterations as a result of increased oxidative stress in tissues. Significant consequence of this pathogenic mechanism is alteration in tissue levels of growth factors, particularly VEGF, but also BMP-2. Considering the importance of BMP-2 and VEGF in processes of dentinal reparation, and the fact that DM alters the function of these growth factors in various tissues, the hypothesis of this study was that DM significantly alters the level and role of BMP-2 and VEGF in pulp reparative responses...

AB - Introduction: Vascular endothelial growth factor (VEGF) and bone morphogenetic protein 2 (BMP 2) are growth factors (GFs) identified within the dentine-pulp complex and involved into the cellular events connected to the pulp-healing response. It is well established that the expression of these GFs is increased in different tissues in diabetes mellitus. Because there are no data concerning the levels of VEGF and BMP 2 in human dental pulp, the aim of present study was to quantify VEGF and BMP 2 levels in intact dental pulp and dental pulp that underwent reactive dentinogenesis in healthy and diabetic human subjects. Methods: The study was conducted on 28 healthy and 28 subjects with controlled diabetes type II who underwent pulp extirpation as a part of prosthetic rehabilitation. Pulp were collected from intact teeth and teeth treated by indirect pulp capping. The levels of VEGF and BMP 2 were determined in the pulp tissue lysates with enzyme-linked immunosorbent assay. Results: The levels of VEGF and BMP 2 were significantly higher in intact teeth pulp of diabetic than in healthy subjects. The concentrations of these GFs were significantly lowered in teeth with indirect pulp capping both in healthy and diabetic persons. Furthermore, VEGF and BMP 2 levels were in strong positive correlation. Conclusions: Similar changes in the levels of VEGF and BMP 2 in intact and treated teeth of healthy and diabetic patients could be suggestive of associated roles of these GFs in responses of healthy and diabetic dental pulp. AB - The most important requirements for successful endodontic treatment are accurate diagnosis and good knowledge of canal morphology. A mandibular second premolars rarely have more than one root and one root canal. Data from the literature indicate low percentage of a mandibular premolars with three or more root canals. Unusual morphology of the root canal system is diagnostic and therapeutic challenge. It is very important to analyze preoperative radiograph and to explore thoroughly the entrance of the root canal. The aim of this study was to present two patients who had lower second premolars with three canals that required endodontic therapy.

AB - Feline dental resorptive lesions affect more than one third of all adult domestic cats and continue to be an enigma in the veterinary dental science although different theories about the pathogenesis of these lesions have been proposed. Recently, a hypothesis was introduced that local mechanical trauma could be an important factor in the initiation of feline dental resorptive lesions and that there is a correlation between the occurrence of resorptive lesions and occlusal trauma in cats. The aim of this study was to analyze stress distribution in feline tooth during occlusal loading in order to accept or reject the hypothesis that dental resorptive lesions in cats might be caused by occlusal trauma. A solid model of feline tooth had to be created in order to perform the investigation. The idea was to gain data for tooth displacement (deformation) and stress and strain distribution under loading generally for any feline tooth using the finite element method. The results of the study contribute to the theory that occlusal overload might be one of the causes in multifactorial resorptive lesions in cats. Succession of tensile and compressive stresses and tooth displacement during occlusal loading might be a contributive factor in the pathogenesis of feline resorptive lesions. However, further research is required to confirm this statement.
Naizmenično smenjivanje zateznog i kompresionog napona u strukturama zuba tokom mastikacije može imati uticaja u patogenezi dentalnih resorptivnih lezija u mačaka.

AB - Mechanical behavior of biological structures is a common subject of scientific research. The results of such investigations offer the precise insight into the biomechanical properties of biological structures and are usefull for predicting their behavior when subjected to loading. Although, such biomechanical investigations were conducted on experimental animals nowadays are very popular investigations concerning mathematical experimental models. The most common is the finite element method analysis. A digital model of a structure of interest has to be created for an investigation with the finite element method. Once the digital model is created with the use of computer technology numerous changes of elements and structures are possible, with different applications of the simulated load. The aim of this paper was to to present the development of our own three-dimensional tooth model created for finite element analyses of intact tooth behavior under functional loading. Also, the idea was to evaluate the possibility for using finite element analysis in veterinary biomechanical research. Our own 3D model was created using computer software according to available literature data, and facts gained from freshly extracted intact teeth and plaster models. It is necessary to emphasize that FEM is an effective tool that has been adapted from the engineering arena to biomechanic research and has the potential to contribute to the growing scientific basis of knowledge in veterinary dentistry.
Jednom napravljen matematički digitalni model omogućuje primenom računara beskonačan broj promena oblika pojedinih elemenata i struktura, ali takođe i bezbroj simulacija aplikacija sila. .

AB - Biomechanical experimental studies are often a subject of interest to many researchers. The results of such investigations give a full insight of complex structural behavior under loading, visualize potential material weaknesses and evaluate if mechanical factors play a substantial role in the pathogenesis of the lesions of the investigated biological structure. The use of FEM enables most of the researchers to abandon the experimental animal models and expand the investigation on computational models, analyzing stress and strain distribution as an indicator of structural biomechanical behavior. The aim of this paper was to present computational modeling of any (human or animal) intact tooth behavior under loading and evaluate the intact tooth stress and strain distribution pattern. For the investigation to be carried out a mathematical model of an intact tooth had to be created. The difference in stress concentration, when masticatory forces are acting lateraly comparing to the ones acting vertically, can be attributed to the basic mechanical principle of leverage which states that forces concentrate at or near the fulcrum. When subjected to lateral loading, tooth with its root acts as a simple lever that fulcrums near the cervical region. This paper offers the new approach in veterinary dental scientific research. Once when a mathematical model is made, it is possible by changing the parameters to investigate different clinical situations. With increasing popularity of veterinary dentistry, the FEM/computational analyses could became a useful tool in biomechanical veterinary dental research due to its noninvasiveness.
AB - Biomehanička eksperimentalna istraživanja pružaju potpuni uvid u kompleksno ponašanje bioloških struktura pod opterećenjem, vizualizuju potencijalna slaba mesta u samom materijalu i analiziraju da li mehaničko opterećenje igra ulogu u nastajanju pojedinih patoloških lezija.
