Facial profile esthetics change of class II malocclusion patients treated with the Herbst appliance as perceived by orthodontists and laypersons

AB  - Introduction/objective: The recognition of differences in individual assessment of facial attractiveness could be valuable assistance in planning the orthodontic treatment. The aim of this study was to compare facial profile attractiveness changes of patients treated with the Herbst appliance perceived by orthodontists and laypersons. Methods: The patient sample comprised 33 young Caucasian still-growing patients, aged 14–18 years, with skeletal class II malocclusion treated with the Herbst and multibracket appliances. Facial profile photographs before and after the treatment were shown to 54 orthodontists and 50 laypersons. In the esthetics oriented poll, the evaluators rated the change in facial appearance. Results: The attractiveness scores differed between the two rater groups (p  lt  0.001), with orthodontists being more generous, whereas there was no significant difference between female and male evaluators in both groups (p > 0.05). However, scores differed significantly in grading female and male patients (p  lt  0.001), so that female patients got higher scores; younger evaluators graded more critically between different age groups of the evaluators (p  lt  0.001), as well as between the patients with different initial severity of malocclusion (p  lt  0.001). Conclusion: The difference in attractiveness scores differed between two groups, with laypersons being more critical than orthodontists. Higher scores were given to female patients by both groups, as well as by the evaluators in the older age group.
AB  - Uvod/Cilj: Prepoznavanje razlika u individualnoj proce- ni facijalne atraktivnosti može biti od velike pomoći u planiranju ortodontske terapije. Cilj rada je bio da se uporedi promena atraktivnosti profila lica bolesnika tretiranih Herbstovim aparatom od strane ortodonata i laika. Metode: Uzorak se sastojao se od 33 ispitanika uzrasta 14–18 godina, koji su imali malokluziju II klase i lečeni Herbstovim aparatom u kombinaciji sa gornjim i donjim fiksnim aparatom. Njihove profilne fotografije pre i posle terapije pregledali su 54 ortodonta i 50 laika. Oni su ocenjivali, u vidu ankete, promenu facijalne atraktivnosti. Rezultati: Ocene facijalne privlačnosti razlikovale su se između dve grupe (p  lt  0,001), tako što su ortodonti dava- li veće ocene, dok nije bilo statistički značajne razlike između polova u obe grupe (p > 0,05). Međutim, rezultati su se značajno razlikovali pri ocenjivanju muških i ženskih bolesnika (p  lt  0,001), te su bolesnice dobile veće ocene. Takođe, rezultati su se značajno razlikovali između različitih starosnih grupa ocenjivača (p  lt  0,001), tako da je mlađa grupa davala niže ocene, te bila kritičnija u pro- ceni. Razlika je bila značajna i kod ocenjivanja bolesnika sa različitim stepenom izraženosti anomalije (p  lt  0,001). Zaključak: Razlika u ocenama facijalne privlačnosti postojala je između dve grupe, te su laici bili kritičniji u ocenjivanju u odnosu na ortodonte. Veće ocene dobile su bolesnice od strane obe grupe, a starija uzrasna grupa ocenjivala je većim ocenama sve bolesnike.

Trauma of the Frontal Region Is Influenced by the Volume of Frontal Sinuses. A Finite Element Study

AB  - Anatomy of frontal sinuses varies individually, from differences in volume and shape to a rare case when the sinuses are absent. However, there are scarce data related to influence of these variations on impact generated fracture pattern. Therefore, the aim of this study was to analyse the influence of frontal sinus volume on the stress distribution and fracture pattern in the frontal region. The study included four representative Finite Element models of the skull. Reference model was built on the basis of computed tomography scans of a human head with normally developed frontal sinuses. By modifying the reference model, three additional models were generated: a model without sinuses, with hypoplasic, and with hyperplasic sinuses. A 7.7 kN force was applied perpendicularly to the forehead of each model, in order to simulate a frontal impact. The results demonstrated that the distribution of impact stress in frontal region depends on the frontal sinus volume. The anterior sinus wall showed the highest fragility in case with hyperplasic sinuses, whereas posterior wall/inner plate showed more fragility in cases with hypoplasic and undeveloped sinuses. Well-developed frontal sinusesmight, through absorption of the impact energy by anterior wall, protect the posterior wall and intracranial contents.

Inadequate prosthetic rehabilitation caused by fibrous and bone hyperplasia of maxilla: Case report

AB  - Normal bone healing after tooth extraction includes the following steps: blood clot forming, granulation, bone forming and final bone reorganization. In clinical settings connective tissue infiltration of extraction socket can result in fibrous scar formation rather than bone healing. Local and systemic factors seem to be major contributors to the occurrence of erratic socket healing. The aim of this case report was to describe oral-surgery treatment of a patient with inadequate bone and soft supportive tissue for prosthetic rehabilitation. Surgical procedure and recovery are presented, including final complete denture rehabilitation.
AB  - Nakon vađenja zuba obično slede brza organizacija koaguluma, formiranje granulacionog tkiva, osteoida, a zatim i zrele lamelarne kosti. Međutim, klinički se neretko može sresti infiltracija ekstrakcione alveole vezivnim tkivom, uz stvaranje fibroznog ožiljka umesto novostvoren kosti. Za to su odgovorni brojni lokalni i sistemski faktori koji doprinose nepravilnom zarastanju ekstrakcione alveole. Cilj ovog rada je bio da se opiše oralnohirurško lečenje pacijenta s neodgovarajućim stanjem koštanog i mekog nosećeg tkiva gornje totalne proteze. Prikazani su hirurški postupak i postoperacioni tok, zaključno s konačnim odgovarajućim protetičkim zbrinjavanjem pacijenta, koji je doveo do pravilnog zarastanja rane i time zadovoljavajuće protetičke rehabilitacije.

AB  - Understanding of the occlusal load distribution through the mid-facial skeleton in natural dentition is essential because alterations in magnitude and/or direction of occlusal forces may cause remarkable changes in cortical and trabecular bone structure. Previous analyses by strain gauge technique, photoelastic and, more recently, finite element (FE) methods provided no direct evidence for occlusal load distribution through the cortical and trabecular bone compartments individually. Therefore, we developed an improved three-dimensional FE model of the human skull in order to clarify the distribution of occlusal forces through the cortical and trabecular bone during habitual masticatory activities. Particular focus was placed on the load transfer through the anterior and posterior maxilla. The results were presented in von Mises stress (VMS) and the maximum principal stress, and compared to the reported FE and strain gauge data. Our qualitative stress analysis indicates that occlusal forces distribute through the mid-facial skeleton along five vertical and two horizontal buttresses. We demonstrated that cortical bone has a priority in the transfer of occlusal load in the anterior maxilla, whereas both cortical and trabecular bone in the posterior maxilla are equally involved in performing this task Observed site dependence of the occlusal load distribution may help clinicians in creating strategies for implantology and orthodontic treatments. Additionally, the magnitude of VMS in our model was significantly lower in comparison to previous FE models composed only of cortical bone. This finding suggests that both cortical and trabecular bone should be modeled whenever stress will be quantitatively analyzed.

AB  - The aim of the present study was to investigate the influences of the presence and position of a lower third molar (M3) on the fragility of mandibular angle and condyle, using finite element analysis. From computed tomographic scans of a human mandible with normally erupted M3, two additional virtual models were generated: a mandibular model with partially impacted M3 and a model without M3. Two cases of impact were considered: a frontal and a lateral blow. The results are based on the chromatic analysis of the distributed von Mises and principal stresses, and calculation of their failure indices. In the frontal blow, the angle region showed the highest stress in the case with partially impacted M3, and the condylar region in the case without M3. Compressive stresses were dominant but caused no failure. Tensile stresses were recorded in the retromolar areas, but caused failure only in the case with partially impacted M3. In the lateral blow, the stress concentrated at the point of impact, in the ipsilateral and contralateral angle and condylar regions. The highest stresses were recorded in the case with partially impacted M3. Tensile stresses caused the failure on the ipsilateral side, whereas compressive stresses on the contralateral side.

AB  - In this study, we describe a new semi-quantitative method for measuring the extent of bleeding in pathohistological tissue samples. To test our novel method, we recruited 120 female patients in their first trimester of pregnancy and divided them into three groups of 40. Group I was the control group, in which no dilation was applied. Group II was an experimental group, in which dilation was performed using classical mechanical dilators. Group III was also an experimental group, in which dilation was performed using a hydraulic dilator. Tissue samples were taken from the patients' cervical canals using a Novak's probe via energetic single-step curettage prior to any dilation in Group I and after dilation in Groups II and III. After the tissue samples were prepared, light microscopy was used to obtain microphotographs at 100x magnification. The surfaces affected by bleeding were measured in the microphotographs using the Autodesk AutoCAD 2009 program and its "polylines" function. The lines were used to mark the area around the entire sample (marked A) and to create "polyline" areas around each bleeding area on the sample (marked B). The percentage of the total area affected by bleeding was calculated using the formula: N=Bt x 100 / At where N is the percentage (%) of the tissue sample surface affected by bleeding, At (A total) is the sum of the surfaces of all of the tissue samples and Bt (B total) is the sum of all the surfaces affected by bleeding in all of the tissue samples. This novel semi-quantitative method utilizes the Autodesk AutoCAD 2009 program, which is simple to use and widely available, thereby offering a new, objective and precise approach to estimate the extent of bleeding in tissue samples.

AB  - Purpose: The aim of this study was to investigate the relationship between surgical techniques and implant macro-design (self-tapping/non-self-tapping) for the optimization of implant stability in the low-density bone present in the posterior maxilla using resonance frequency analysis (RFA). Materials and Methods: A total of 102 implants were studied. Fifty-six self-tapping BlueSkyBredent (R) (Bredent GmbH &, Senden, Germany) and 56 non-self-tapping Standard Plus Straumann (R) (Institut Straumann AG (R), Waldenburg, Switzerland) were placed in the posterior segment of the maxilla. Implants of both types were placed in sites prepared with either lateral bone-condensing or with bone-drilling techniques. Implant stability measurements were performed using RFA immediately after implant placement and weekly during a 12-week follow-up period. Results: Both types of implants placed after bone condensing achieved significantly higher stability immediately after surgery, as well as during the entire 12-week observation period compared with those placed following bone drilling. After bone condensation, there were no significant differences in primary stability or in implant stability after the first week between both implant types. From 2 to 12 postoperative weeks, significantly higher stability was shown by self-tapping implants. After bone drilling, self-tapping implants achieved significantly higher stability than non-self-tapping implants during the entire follow-up period. Conclusions: The outcomes of the present study indicate that bone drilling is not an effective technique for improving implant stability and, following this technique, the use of self-tapping implants is highly recommended. Implant stability optimization in the soft bone can be achieved by lateral bone-condensing technique, regardless of implant macro-design.
